Massachusetts, Oregon Successes Bolster Obama Health Care Reform

The Affordable Care Act is working. 2.5 million more young adults ages 19 to 26 now have health insurance. The shrinking of the Medicare "donut hole" allowed 3.6 million seniors to save $2.1 billion on their prescription drugs last year. And the ban on insurers refusing to cover pre-existing conditions is saving lives (even among those who opposed so-called "Obamacare"). And even though most of its provisions don't go into effect until 2014, the data from Oregon and Massachusetts strongly suggest the 30 million people who will gain coverage will be much healthier and more financially secure. In Massachusetts, the 2006 health care reform Governor Mitt Romney signed into law lowered the insured rate from 10 percent to a national low of two percent. Even with its individual mandate, "Romneycare" is extremely popular, enjoying a 3 to 1 margin of support from Bay State residents. Now, a new study by Charles J. Courtemanche and Daniela Zapata published by the National Bureau of Economic Research (NBR) shows that universal coverage in Massachusetts is indeed making people there healthier. As Ezra Klein of the Washington Post summed up their findings: The answer, which relies on self-reported health data, suggests they did. The authors document improvements in "physical health, mental health, functional limitations, joint disorders, body mass index, and moderate physical activity." The gains were greatest for "women, minorities, near-elderly adults, and those with incomes low enough to qualify for the law's subsidies." Importantly, the researchers concluded that "the general strategies for obtaining nearly universal coverage in both the Massachusetts and federal laws involved the same three-pronged approach of non-group insurance market reforms, subsidies, and mandates, suggesting that the health effects should be broadly similar." (Or MIT professor and architect of both laws Jonathan Gruber put it bluntly last year, "they're the same f--king bill.") It's no wonder Mitt Romney used to recommend his Massachusetts reform as a model for the nation. If the individual mandate is one of the highest profile (if contentious) aspects of the 2010 Patient Protection and Affordable Care Act, the expansion of Medicaid is among the most important in enabling 30 million currently uninsured Americans to get coverage. By extending Medicaid coverage to families earning up to 133 percent of the federal poverty level (FPL) and providing subsidies to those up to four times the FPL, starting in 2014 the Affordable Care Act passed by Democrats in Congress will bring insurance to millions more Americans. A March 2011 analysis by the Commonwealth Fund revealed that when fully implemented, the ACA will bring relief to "nearly all of the 52 million working-age adults who were without health insurance for a time in 2010." As it turns out, America's future is Oregon's present. That's the word from another study released last fall by the NBER. The same nonpartisan group that determines the official beginning and end of recessions, NBER found, as Harvard researcher and former member of President George W. Bush's Council of Economic Advisers Katherine Baicker put it, "Medicaid matters." The NBER study avoided the pitfalls of past studies by examining the case of Oregon. After Oregon in 2008 established a lottery to add 10,000 people to its limited Medicaid rolls, the NBER team interviewed 6,000 of the lucky ones and 6,000 of the 90,000 who lost out. The results were striking: We find that in this first year, the treatment group had substantively and statistically significantly higher health care utilization (including primary and preventive care as well as hospitalizations), lower out-of-pocket medical expenditures and medical debt (including fewer bills sent to collection), and better self-reported physical and mental health than the control group. The New York Times provided some of the details of the Medicaid success story: Those with Medicaid were 35 percent more likely to go to a clinic or see a doctor, 15 percent more likely to use prescription drugs and 30 percent more likely to be admitted to a hospital. Researchers were unable to detect a change in emergency room use. Women with insurance were 60 percent more likely to have mammograms, and those with insurance were 20 percent more likely to have their cholesterol checked. They were 70 percent more likely to have a particular clinic or office for medical care and 55 percent more likely to have a doctor whom they usually saw. The insured also felt better: the likelihood that they said their health was good or excellent increased by 25 percent, and they were 40 percent less likely to say that their health had worsened in the past year than those without insurance.

Every organ and even every cell in our body has circadian or 24 h clock. Circadian clocks turn on and off thousands of genes at the right time of the day or night. These rhythms work together to maintain healthy balance of brain chemicals, hormones, and nutrients. When our rhythms break down we are more likely to suffer from a wide range of diseases from depression to cancer. We can tune our daily habits of eating, sleeping or getting the right amount of light to sustain our circadian rhythms. Healthy rhythms nurture healthy body and healthy mind. Dr. Satchidananda Panda, a professor at the Salk Institute for Biological Studies in La Jolla, California. Satchin’s work deals specifically with the timing of food and it’s relationship with our biological clocks governed by circadian rhythm and also the circadian rhythm in general. Professor Panda explores the genes, molecules and cells that keep the whole body on the same circadian clock. A section of the hypothalamus called the suprachiasmatic nucleus (SCN) lies at the center of the body’s master clock and gets input directly from light sensors in the eyes, keeping the rest of the body on schedule. Panda discovered how these light sensors work, as well as how cellular timekeepers in other parts of the body function. He also uncovered a novel blue light sensor in the retina that measures ambient light level and sets the time to go to sleep and wake up every day. In the process of exploring how the liver’s daily cycles work, Panda found that mice which eat within a set amount of time (12 hours) resulted in slimmer, healthier mice than those who ate the same number of calories in a larger window of time, showing that when one eats may be as important as what one eats. If the benefits of this “12-hour diet” hold true in humans, it could have profound impacts on treating overeating disorders, diabetes and obesity. The circadian clock, he found, even mediates the immune system. Mice with a crucial circadian molecule missing had higher levels of inflammation in their bodies than other mice, suggesting that genes and molecules involved in the circadian clock could be drug targets for conditions linked to inflammation, such as infections or cancer. Plan Type - The types of health insurance plans offered through Maryland Health Connection are Health Maintenance Organizations (HMOs), Preferred Provider Organization (PPO), Point-of-Service (POS), Exclusive Provider Organization (EPO), Integrated Delivery System (IDS), and HSA-eligible plans. (If you are eligible for Medicaid or your child is eligible for MCHP Premium, you will receive information in the mail about choosing a managed care organization through HealthChoice, the state’s Medicaid Managed Care Program.) HMO is a type of health insurance plan that usually limits coverage to care from doctors who work for or contract with the HMO. An HMO generally won’t cover or has limited coverage for out-of-network care, except in an emergency. HMO members usually have a primary care doctor and must get referrals to see specialists. HMOs often provide integrated care and focus on prevention and wellness. In general, HMO plans are the least expensive plans that health insurance carriers offer. PPO is a type of health insurance plan where you have the choice of getting care from in-network or out-of-network providers. You’ll pay less if you use providers in the plan’s network. If you have a PPO plan, you can visit any primary care doctor or specialist without getting a referral, and you do not need to select a doctor to be your primary care physician to coordinate your care. Generally these are the most expensive plans a health insurance carrier offers. POS is a type of health insurance plan where you can get medical care from both in-network and out-of-network providers. You’ll pay less if you use in-network doctors, hospitals, and other health care providers. If you have a POS plan, you’ll select a primary doctor from a list of participating providers. To visit an out-of-network provider, you’ll need a referral and you may pay higher out-of-pocket costs. EPO is a type of health insurance plan where generally services are covered only if you use doctors, specialists or hospitals in the EPO’s network (except in an emergency). Members do not require referrals to see a specialist. IDS plans require that you select a primary care provider to coordinate your care, do not cover services from out-of-network providers (unless in emergencies), and require referrals for most specialist visits. HSA-eligible plans have specific cost-sharing rules on how they cover benefits. For the most part, except for zero-cost preventive care and prescription drugs, HSA-eligible plans have cost-sharing that begins paying for care only after you have met your deductible. Also, all HSA-eligible plans must have a deductible high enough to be considered a High-Deductible Health Plan (HDHP).

When I consider purchasing an individual health insurance plan for myself or my family, do I have any financial obligations beyond the monthly premium and annual deductible? Answers: It depends on the plan, but some plans have the following cost-sharing elements that you should be aware of. Co-Payments: Some plans include a co-payment, which is typically a specific flat fee you pay for each medical service, such as $30 for an office visit. After the co-payment is made, the insurance company typically pays the remainder of the covered medical charges. Deductibles: Some plans include a deductible, which typically refers to the amount of money you must pay each year before your health insurance plan starts to pay for covered medical expenses. Coinsurance: Some plans include coinsurance. Coinsurance is a cost sharing requirement that makes you responsible for paying a certain percentage of any costs. The insurance company pays the remaining percentage of the covered medical expenses after your insurance deductible is met. Out-of-pocket limit: Some plans include an out-of-pocket limit. Typically, the out-of-pocket limit is the maximum amount you will pay out of your own pocket for covered medical expenses in a given year. The out-of-pocket limit typically includes deductibles and coinsurance. But, out-of-pocket limits don't typically apply to co-payments. Lifetime maximum: Most plans include a lifetime maximum. Typically the lifetime maximum is the amount your insurance plan will pay for covered medical expenses in the course of your lifetime. Exclusions & Limitations: Most health insurance carriers disclose exclusions & limitations of their plans. It is always a good idea to know what benefits are limited and which services are excluded on your plan. You will be obligated to pay for 100% of services that are excluded on your policy. Beginning September 23, 2010, the Patient Protection and Affordable Care Act (health care reform) begins to phase out annual dollar limits. Starting on September 23, 2012, annual limits on health insurance plans must be at least $2 million. By 2014 no new health insurance plan will be permitted to have an annual dollar limit on most covered benefits. Some health insurance plans purchased before March 23, 2010 have what is called "grandfathered status." Health Insurance Plans with Grandfathered status are exempt from several changes required by health care reform including this phase out of annual limits on health coverage. If you purchased your health insurance policy after March 23, 2010 and you're due for a routine preventive care screening like a mammogram or colonoscopy, you may be able to receive that preventive care screening without making a co-payment. You can talk to your insurer or your licensed eHealthInsurance agent if you need help determining whether or not you qualify for a screening without a co-payment. There are five important changes that occurred with individual and family health insurance policies on September 23, 2010. Those changes are: 1. Added protection from rate increases: Insurance companies will need to publically disclose any rate increases and provide justification before raising your monthly premiums. 2. Added protection from having insurance canceled: An insurance company cannot cancel your policy except in cases of intentional misrepresentations or fraud. 3. Coverage for preventive care: Certain recommended preventive services, immunizations, and screenings will be covered with no cost sharing requirement. 4. No lifetime maximums on health coverage: No lifetime limits on the dollar value of those health benefits deemed to be essential by the Department of Health and Human Services. 5. No pre-existing condition exclusions for children: If you have children under the age of 19 with pre-existing medical conditions, their application for health insurance cannot be declined due to a pre-existing medical condition. In some states a child may need to wait for the state's open-enrollment period before their application will be approved.

Today we’re gonna look at the difference between these two plans plus more. The question we get asked the most is, What’s the difference? Quite a bit! In fact, Medicare knows you’ll have questions, so their solution is to mail you a book every year. Here’s their explanation of Medigap. “Medicare Supplement (Medigap) Insurance policies, can help pay some of the health care costs that Original Medicare doesn’t cover...If you have Original Medicare (A&B) and you buy a Medigap policy, Medicare will pay its share of the Medicare-approved amount for covered health care costs. Then, your Medigap policy pays its share.” Ok, so in plain English, what exactly does that mean? It means that Medicare does not offer you full coverage, in fact, Medicare only covers 80% of your bills, leaving you responsible to pay the remaining 20%. This 20% is known as the Coverage Gap. That’s where Medigap comes in. It can cover the entire 20% gap, ensuring you’re a 100% protected. Medigap bridges the gap in Original Medicare by helping to pay for medical expenses that fall under that 20% gap. In other words, Medicare will pay 80% of your medical bills and your Medigap insurance will pay the remaining 20%. It’s really that simple. Now let’s look at Medicare Advantage or Part C Medicare Advantage (Part C) is an optional type of Medicare health plan offered by private insurance companies and limited to a local network of Doctors, Hospitals, and Specialists. These companies must follow rules set by the government and the rules can change each year. –Source, Medicare.gov Ok, pause. You might be asking yourself, Jared, do you mean to tell me that Medicare Advantage has restrictions on how I receive medical care? Simply put, Yes! You see, unlike Medigap, which completes your coverage, if you join a Medicare Advantage plan you’re limiting your freedom of choice and allowing the insurance companies to manage ALL of your health care. They tell you where you can and can’t go, and how much it’s gonna cost you. With your Red, White & Blue Medicare card, you’re given access to any hospital, doctor or specialist in the entire U.S. who takes Medicare. Think of it like a golden ticket that’s accepted virtually everywhere in the U.S. If you join a Medicare Advantage plan you’re giving up that ticket and allowing the insurance company to limit your health care options. For example, if you’re diagnosed with a terminal illness and you’ve heard nothing but great things about MD Anderson Cancer Center in Texas, your Advantage plan may not allow you access to their facility or doctors, because they’re out of network. Remember, Medicare Advantage limits you to a local network of providers in your general area. So unless you live in Texas near MD Anderson, chances are, with Medicare Advantage, you won’t be covered. Many Medicare Advantage plans may require you to pay monthly premiums, annual deductibles, co-payments, and co-insurance. They can also charge different out-of-pocket costs up to $6,700 dollars each year. Most Medicare Advantage plans don’t offer coverage outside the US. And they have different rules for how you get services, like whether you need a referral to see a specialist or which doctor will accept your insurance. Hospitals, and specialists frequently leave networks, making it harder to get the care you’re entitled to. Also, it’s important to note that because no one can predict how much care they’ll need from year to year, and with all the out of pocket costs like copays and deductible, Medicare Advantage makes planning & budgeting your annual expenses nearly impossible. When comparing the two, Medigap plans offer the most protection and remain the most popular choice of coverage for people throughout the U.S. So don’t roll the dice, make sure you’re 100% covered with the freedom to choose how you receive care. If Medicare did not pay for any of your treatment, a process called "redetermination" can determine Medicare's proper reimbursement amount. You must be able to show that you did not use your Medicare benefits. You can demonstrate that you are eligible for a redetermination if: 1. Your Medicare coverage ended BEFORE you took the drug in question or your coverage began AFTER you settled your case; or 2. You have another source of primary insurance coverage and can show that you had that other primary insurance coverage the entire time, from the date you took the drug in question through the date you settled your case. If you fall into one of these two categories and are able to provide the documentation Medicare requires, the Medicare global reimbursement amount in your case will be reduced accordingly.

Fat mass decreased on the ketogenic diet by 2.2 kg (0.7 kg greater than the Western diet group). Meanwhile, policymakers navigating this new landscape are also working largely without the benefit of a solid foundation of scientific evidence on the drug’s risks and benefits. In fact, a new National Academy of Medicine report describes notable gaps in scientific data on the short- and long-term health effects of marijuana. What do we know about the health impacts of marijuana, and what do we still need to learn? San Bernardino County Local Assistance Center  and Shelter Operation Compound

San Bernardino County, in coordination with the American Red Cross, activated the County's Mass Care and Shelter Plan and opened a Shelter Operations Compound (SHOC) at the San Bernardino County Fairgrounds at 14800 7th Street in Victorville to serve county residents impacted by the Blue Cut Fire. On Friday, Aug. 19, 2016, the County opened a Local Assistance Center (LAC) at the fairgrounds for residents affected by the fire. The LAC will provide a one-stop location for a variety of services including but not limited to disaster assistance, property information, and insurance claims. The SHOC is providing food, shelter, health care and animal care to residents displaced by the fire. Cots, food, showers and restrooms are available for residents. Public information is available to provide evacuees up-to- date information about the Blue Cut incident. Evacuees who have been sheltering in recreational vehicles may park their RVs at the SHOC and receive SHOC services while continuing to shelter in their RVs. Utility hookups for RVs will most likely not be available, so RVs must be self-sufficient.   WHAT AGENCIES ARE PARTICIPATING IN THE SHOC/LAC? Various County departments have joined the American Red Cross to provide services, including County Animal Care and Control, which is operating an animal shelter at the site. Arrowhead Regional Medical Center and the County Public Health Department are operating a medical health unit for minor medical needs. Counselors from County Behavioral Health are available for those in need of counseling. Randal Howard "Rand" Paul (born January 7, 1963) is an American physician and politician from Kentucky. Born in Pittsburgh, Pennsylvania, Paul attended Baylor University and is a graduate of the Duke University School of Medicine.
