In this lesson, we will discuss Legal Chain of Custody versus Medical Break Glass Policy. Forensic evidence, which includes forensic photos and video, must be sequestered away from health records in order to preserve the Legal Chain of Custody. To better understand the difference between Forensic Evidence and Electronic Medical Records, we will take a look at the retention-destruction life cycle for Electronic Medical Records. On average, most states have a 5 to 10 year retention policy for Electronic Medical Records. The four stages of the EMR/EHR life cycle are Creation of the EMR/EHR, Utilization of the EMR/EHR for diagnosis/treatment and follow up planning, Maintenance of the EMR/EHR for the duration of the required record retention, and finally, the destruction of the EMR/EHR once the retention requirement has expired. By comparison, the retention-destruction life cycle for forensic evidence can have very different retention requirements than what is required for EMR/EHRs. In the life cycle for retention/destruction of forensic evidence, once forensic evidence is gathered during a sexual assault exam, that evidence will be retained and maintained under a Legal Chain of Custody for an indefinite period of time. Access to the forensic evidence is only granted to authorized users and those users will document their access in the Legal Chain of Custody prior to accessing it. Forensic evidence may also need to be retained well past the 5 - 10 years required for most EMRs/EHRs. Forensic evidence that is kept within an EMR/EHR will be at risk of entering the Destruction cycle long before it is needed for use in court. Medical Break Glass policies grant immediate access to restricted EHR/EMR users through an EHR/EMR data distribution system that documents access AFTER the fact. A Legal Chain of Custody prevents access until access is pre-requested, pre-challenged, pre-justified and then pre-confirmed BEFORE access is granted to any forensic evidence collected during a forensic medical exam.

Cub Scouts walked through a Naturist Beach in San Diego Famous for their motto “be prepared”, 6 cubs and their chaperones were in for a surprise as they took part in a Saturday morning hike at Torrey Pines state park, near La Jolla in San Diego. Things had started well as they walked along the beach, and no different to similar expeditions that happen around the country. This all went wrong though, as they found themselves on a state owned stretch of Black’s beach- a favorite congregation point of a nude sunbathing group called Black’s Beach bares. They use this part of the beach because it’s relatively tricky to reach, and an area where the California Department of Parks and Recreation doesn’t strictly enforce the rules that require visitors to wear appropriate clothing. Needless to say the 9 and 10 year old boys got an eyeful, the parents were wildly unimpressed, and the group leaders were forced to immediately change their planned route. The story made the national news and drew a lot of criticism for the organizers. Despite complaints by the parents, there was no punishment for the pack leaders who were deemed to have observed proper protocol and taken adequate steps once the mistake had been realized. 7. Trip to a morgue where there saw their classmates brain School trips are designed to be educational and fun, but there are still some places you’d think students wouldn’t be taken to during the formative years of their lives. One of which being a morgue. It’s actually a more common trip than you’d think, but one class from the forensic science club of Port Richmond High School was in for a nasty surprise when they visited their local mortuary. Clearly interested in the subject, the students had been warned of the sights they were about to witness, but nothing could have prepared them for the brain that was floating in a glass jar with one of their former classmates name written on it. Jesse Shipley had died two months earlier when a car he was in collided with an SUV, with the cause of death being registered as blunt impact wounds to the head. Some of the kids on the trip had even been to his funeral, where his parents thought their son had been properly buried. They had no idea that their son's brain had been removed. When they learned about what had been seen on the trip they were granted an emergency court order to have the brain returned to them so they could carry out a second burial. Despite how awful this whole story seems, one of the doctors in the medical examiners office claimed that no wrong had been done, as he would usually wait for another 6 brains to be collected before the neuropathologic examiner was called in for their opinion. 6. Trip to a prison Another tough place for kids to be taken on a school trip is a prison, but the idea is that it will teach them about the consequences of their behavior and hopefully help them avoid ending up in jail when they are older. This lesson didn’t quite go according to plan on a 2014 trip to Altcourse prison in Liverpool in the UK, when one of the students ending up in court as a result. Following the suggestion by a local police constable, a group of 15-year old pupils with behavioral problems were taken to the prison, but things hardly went to plan. While touring around the chapel, there was a sudden lockdown that meant they had to stay where they were for 90 minutes. The teens, clearly agitated by what was going on, started verbally abusing the staff, and these problems failed to calm down when they were finally able to leave. Back in the parking lot one student punched another in the head, leaving him bleeding with a concussion and a massive lump. The attacking pupil was arrested and taken to court with a charge of assault. He admitted the offence and was sentenced to nine months of youth rehabilitation, luckily avoiding time in the lock-up that the trip was supposed to help them stay away from.

The average UK reading age is that of a 9- to 12-year-old child, so as writers we must remember to keep sentences short and avoid jargon. We will need to translate technical medical terms, replacing ‘pharmacovigilance’ with ‘drug safety’, and ‘adverse event’ with ‘side effect’, for example. However, we also need to avoid being patronising and dumbing down, and to bear in mind the target audience. Writing for an expert patient group, we must avoid telling them what they already know. Whatever the audience, it is important to have a clear plan, put the key information first, and keep the text personal. The sentence: ‘If you need more information, please ask your doctor, nurse or pharmacist’ is a lot more patient-friendly than: ‘More information can be provided by the prescribing healthcare professional’. Similarly, it is important to use correct, consistent, patient terminology. If you do not have the luxury of being able to talk directly with patients to find out what terms they routinely use, patient information leaflets and patient websites can be useful alternatives, as can patient associations. Layout is also critical in patient-focused materials. Avoid dense paragraphs of text, and try instead to incorporate white space, use headings as signposts, and tabulate information or use graphs. Consider the text formatting too: block capitals are aggressive, while italics slow down the reader and underlining can be mistaken for hyperlinks. Dr. Choi digs into the psychopaths brain, literally. He examines how their brain reacts and thinks compared to an average brain, hoping that we do our part to guide the psychopaths to a better life. Dr. Choi is an assistant professor in the public psychiatry division of Oregon Health & Science University, and director of the Forensic Evaluation Service at the Oregon State Hospital. He graduated from Stanford University with a bachelor of science in a self-designed major of computational neuroscience. He received his medical and doctorate degrees under the federally funded Medical Scientist Training Program at the University of California, San Diego. His doctorate in neurosciences was awarded for work on the molecular mechanisms of visual system development in the lab of Dennis O'Leary at the Salk Institute. Dr. Choi completed psychiatry residency at UCLA-Neuropsychiatric Institute, and a fellowship in forensic psychiatry at University of Pennsylvania. The Center for Law & Human Behavior (CLHB) at the University of Texas at El Paso under the direction of the Center for Borders, Trade and Immigration (CBTIR) has begun the 2016-2017 Homeland Security Symposium Series. The intention of the symposium series is to increase the amount of education and training on DHS related issues and relevant security subject matter provided to homeland security enterprise practitioners and first-responders. The symposiums are open to individuals affiliated with the Department of Homeland Security as well as local federal, state, local, and tribal agencies. All of the symposiums are centered on topics that are requested and pertinent to the CBTIR’s homeland security enterprise stakeholders. The Center for Law & Human Behavior plans to continue its 2016-2017 symposium series with future in-depth conferences on subjects that reflect the needs and requests of its Department of Homeland Security stakeholders. The aim is to increase the amount of opportunities for workforce development for members of the homeland security enterprise. Professor Susan Young (Director of Forensic R&D, WLMHT) opens the conference by providing an overview of the conference aims. She highlights how the conference is a platform for the dissemination of the high quality research conducted within WLMHT, as seen by the number of speakers presenting research conducted at Broadmoor Hospital. Dr Nick Broughton (Medical Director, WLMHT) reports on the importance of maintaining high standards of care and services offered by WLMHT, and the important role for R&D in achieving this aim. Dr Broughton highlights his vision of a culture across WLMHT which supports and encourages R&D, and ensures its accessibility to all clinicians, service users and their carers. This presentation was given at a one day conference held at Broadmoor Hospital, West London Mental Health Trust (WLMHT) in the UK on Thursday 27th November 2014, entitled “Broadcasts from Broadmoor: Current Issues in Forensic Mental Health”. The conference featured a line-up of internationally renowned guest speakers with a wide range of experience and expertise in forensic/clinical psychology and psychiatry. Speakers were tasked with presenting on topics they identified as contemporary issues in offending behaviour. Demographic changes, the increasing demand for care and funding constraints have heightened pressures on our health and social care systems. Professor Croisdale-Appleby traces the development of integrating health and social care in England over the past 20 years; from ‘Improving Health for Mrs Smith’ in Torbay to the work of the current Vanguards and Sustainability and Transformation Partnerships. He contrasts this experience with international examples and explores the challenges presented by integration, including leadership. He examines the need for new roles which will extend the capacity of health and care professionals to deliver the services that must be developed to serve emerging and future needs. Professor David Croisdale-Appleby is a career and academic polymath, with interests spanning medicine, health, social care, and science including forensic science. He holds many Ministerial appointments in health and education, and professorships in a variety of disciplines. He is Chair of The Science Council and Dementia UK, and at Health Education England he holds responsibilities for higher education, medical education, quality and the NHS Leadership Academy. He is the author of the Independent Review of Social Work Education, Re-visioning Social Work Education, published by the government in 2014 which forms the basis for the current reforms in social work education and training, and to which Keele University contributed. His work is focused on improving the lives of vulnerable and disadvantaged people, and he has chaired many organisations representing and promoting their interests.

Population genetics theory allows quantitative predictions of evolutionary processes, integrating mathematical and statistical concepts with fundamental biological principles of genetic inheritance and processes such as mutation and selection. Population genetics theory is thus critical to understanding many pressing issues in biology, such as the evolution of antibiotic resistance in pathogens, the formation of new species and the emergence of cooperative and altruistic behaviors. This school aims to expose students and researchers from diverse backgrounds to the basics and the forefront of current research in population genetics. In addition to evolutionary biology students, we welcome students of mathematics, statistics, medicine, and physics who are interested in evolutionary theory. To equip students to think about issues in population genetics, we will first conduct a brief refresher course in mathematics, statistics, and basic biology including evolution and genetics. The remainder of the school will introduce and develop an understanding of population genetics theory and its applications to interesting phenomena such as the evolution of sex and recombination, phenotypic plasticity, polyploidy, and human diversification.
