Category A and B drugs are mostly safe in pregnancy and can be used without any problem. But C and D drugs are having a little deleterious effects on the fetus compared to the other A and B drugs and it had to be very carefully used when it comes to pregnancy. Only then the potential benefits the drugs oppose to the growing fetus and the mother. Category X drugs are definitely dangerous to the fetus and they have to be avoided as they are teratogenic in the terms of causing abnormalities in the development of various organs. Coming to antibiotics in early pregnancy, the week number 8 – 10 or 7 – 11 is mostly the stage of organogenesis where all the organs are developing in a beautiful way in the fetus. So all the drugs which are in this period can have effect on the growing fetus to various degrees. So we try and avoid any drugs as much as possible, but if it is inevitable, like infections or any condition which si posing threat to the mother’s health, it is advised to use drugs in the safe categories, that is the A, B and sometimes C category drugs. Antibiotics do not affect the fetal growth, especially in the early pregnancy. It can be harmful to the organogenesis to the development of the fetus. So growth parameters are not affected in the early pregnancy , But there are a group of drugs that can cause teratogenic effects , that is abnormal effects on the fetus, that is warfarin, phenytoin, lithium, isotretinoin etc. So these drugs are to be avoided in pregnancy as far as possible. There are many antibiotics which can be used safely in pregnancy like penicillins, erythromycins, cephalosporins, metronidazole, clindamycin and various other drugs. But of course only after your gynecologist says okay with these drugs.

When should you commit to getting urine? When can you wait? When should you forgo testing altogether?   When do I get urine? Symptoms – either typical dysuria, urgency, frequency in a verbal child, or non-descript abdominal pain or vomiting in a well appearing child. Fever – but first look for an obvious alternative source, especially viral signs or symptoms. No obvious source? Risk stratify before “just getting a urine”. In a low risk child, with obviously very vigilant parents, who is well appearing, you may choose not to test now, and ensure close follow up. Bag or cath? The short answer is: always cath, never bag. (Pros and cons in audio) What is the definition of a UTI? According to the current clinical practice guideline by the AAP, the standard definition of a urinary tract infection is the presence of BOTH pyuria AND at least 50 000 colonies per mL of a single uropathogen. Making the diagnosis in the ED: The presence of WBCs with a threshold of 5 or greater WBCs per HPF is required. What else goes into the urinalysis that may be helpful? Pearl: nitrites are poorly sensitive in children.  It takes 4 hours for nitrites to form, and most children this age do no hold their urine. Pearl: the enhanced urinalysis is the addition of a gram stain.  A positive gram stain has a LR+ of 87 in infants less than 60 days, according to a study by Dayan et al. in Pediatric Emergency Care. When can I just call it pyelonephritis? In an adult, we look for UTI plus evidence of focal upper tract involvement, like CVA tenderness to percussion or systemic signs like nausea, vomiting, or fever.  It is usually straightforward. It’s for this reason that the literature uses the term “febrile UTI” for children.  Fever is very sensitive, but not specific in children. The ill-appearing child has pyelonephritis.   The well-appearing child likely has a “febrile UTI”, without upper involvement.  However, undetected upper tract involvement may be made in retrospect via imaging, if done. How should I treat UTIs? For simple lower tract disease, treat for at least 7 days.  There is no evidence to support 7 versus 10 versus 14 days.  My advice: use 7-10 days as your range for simple febrile UTI in children. Pyelonephritis should be treated for a longer duration.  Treat pyelonephritis for 10-14 days. What should we give them? Sulfamethoxazole and trimethoprim (Bactrim) is falling out of favor, mostly because isolates in many communities are resistant.  There is an association of Stevens-Johnson Syndrome (SJS) with Bactrim use.  This may be confounded by its prior popularity; any antibiotic can cause SJS, but there are more case reports with Bactrim. Cephalexin (Keflex): 25 mg/kg dose, either BID or TID.  It is easy on the stomach, rarely interacts with other meds, has high efficacy against E. coli, and most importantly, cephalexin has good parenchymal penetration. Nitrofurantoin is often used in pregnant women, because the drug tends to concentrate locally in the urine.  However, blood and tissue concentrations are weak.  It may be ineffective if there is some sub-clinical upper tract involvement. Cefdinir is a 3rd generation cephalosporin available by mouth, given at 14 mg/kg in either one dose daily or divided BID, up to max of 600 mg.  This may be an option for an older child who has pyelonephritis, but is well enough to go home. Whom should we admit? The first thing to consider is age.  Any infant younger than 2 months should be admitted for a febrile UTI.  Their immune systems and physiologic reserve are just not sufficient to localize and fight off infections reliably. The truth is, for serious bacterial illness like pneumonia, UTI, or severe soft tissue infections, be careful with any infant less than 4-6 months of age. Of course, the unwell child – whatever his age – he should be admitted.  Think about poor feeding, irritability, dehydration – in that case, just go with your gut and call it pyelonephritis, and admit. What is the age cut-off for a urine culture? In adults, we think of urine culture only for high-risk populations, such as pregnant women, the immunocompromised, those with renal abnormalities, the neurologically impaired, or the critically ill, to name a few. In children, it’s a little simpler.  Do it for everyone. Who is everyone? Think of the urine rule of 10s: 10% of young febrile children will have a UTI 10% of UAs will show no evidence of pyuria Routine urine culture in all children with suspected or confirmed UTI up to about age 10 What do I do then with urine culture results? From a quality improvement and safety perspective, consider making this a regular assignment to a qualified clinician. Check once in 24-48 hours to find...

I have undergone 2 brain operations at Florida Hospital Orlando between 2012 and 2017. In June 2012, Neurosurgeon Dr. Melvin Fields performed a botched brain operation on me. Approx. 4 months after, I returned to Florida Hospital of Orlando with a severe infection in my head. The incision site had broken down from the infection and pus, blood, and CSF fluid was oozing out of the incision. This has been an ongoing reoccurring issue for the last five years continuously. I have sought help many times at Florida Hospital of Orlando when the pus and CSF are oozing from the incision site. Each time I was admitted and administered antibiotics from the ICU and then sent home within 3-5 days. Approximately 7 months ago, I was readmitted with a severe infection at the incision site. I was admitted immediately. I was prepped for an operation the following morning. The goal was to eradicate the germ that was inside my head by removing the graft on my skull and the mesh. Hopefully. Approx 3 months after this botched life and death operation, I had returned to the ER with a severe infection at the incision site Again.. With my insistence, prior to administering antibiotics, the ER doctor at Florida Hospital of Orlando sent the specimen for culture to the lab. A day and a half later, the PA came into my room in the ICU and asked me how I was doing. I asked her what kind of infection did I have. She stated Lets see. As she looked at the computer she noticed that Dr. Fields had not ordered the specimen to be cultured as usual. So she herself ordered the labwork to be done. The following day I was released from the ICU and sent home with 28 tablets of Keflex in 750 mg. Approximately, three to five days later, my daughter went down to Florida Hospital of Orlando, and got all the test results from the records department. Upon receiving these records we were shocked to find out that I had tested positive for rare gram cocci, and I had no graft on the rear of my skull or a mesh protecting it. This infection was less than 1 millimeter from my brain, and it could have killed me instantly. To this day, I have never received a call from Florida Hospital of Orlando or the so-called neurosurgeons, that I was being treated by, to find out how I was feeling and to inform me that I had tested positive for rare gram cocci. I have sent eight emails over a period of 3 months, and two hand delivered letters to the administration, and to the C.E.O. of Florida Hospital of Orlando, and to the board members begging and pleading for someone from this administration to supersede in this matter and I have never been contacted except from their Risk Management Officer. This is extremely sad that this kind of medical treatment is being allowed in 2017. I released this video to educate the people of what kind of unsterile, unsanitary conditions that you may be subjected to while getting an operation at Florida Hospital of Orlando, while you are lying unconscious in their operating room at the hands of a sloppy, dirty Neurosurgeon that doesn't scrub up and assumes that the rules and regulations of the board of medicine doesn't apply to him. Every day I suffer from severe, debilitating headaches, dizziness, and nausea along with the occasional CSF and pus oozing from my right side of my head and my ear, and my face becoming swollen from the collection of CSF, while Florida Hospital of Orlando and Neurosurgeon Dr. Melvin Field refuse to acknowledge it. They blackballed me and have left me Abandoned with an infection inside my head that I will live with the rest of my life. - SCOTT
