The GOP healthcare bill keeps some of the most popular parts of Obamacare, like letting young adults stay on their parents' health insurance until age 26 and requiring insurance companies to cover people with pre-existing conditions. But the Republican bill gets rid of the key element that made Obamacare work: the individual mandate. Now that people aren't required to have insurance, healthy people could leave insurance pools en masse, leaving sick people who are more expensive to cover. Hypocrisy is a minor sin in politics, but still, it is remarkable how much of it there is to be found in this legislation. A core Republican complaint when Obamacare was passed was that the law delayed many of its provisions in order to reduce public outcry and manipulate the CBO’s score. The GOP bill is similarly aggressive with such tricks, delaying changes to the Medicaid expansion until 2020 and pushing Obamacare’s tax on expensive insurance plans out until 2025. Because Republicans aren’t even trying to win Democratic votes, they’re stuck designing a bill that can wiggle through the budget reconciliation process (another thing they complained about Democrats doing). That means they can’t make major changes to insurance markets like repealing Obamacare’s essential benefit standards or allowing insurance to be sold across state lines. That last part is particularly striking, given that it was one of President Trump’s five demands in his speech last week. I’ve always been skeptical about the savings Republicans could wrest by changing those regulations, but now they can’t get those savings at all — which means sacrificing a key part of their theory of cost control. This bill has a lot of problems, and more will come clear as experts study its language, the Congressional Budget Office release its estimates, and industry players make themselves heard. But the biggest problem this bill has is that it’s not clear why it exists. What does it make better? What is it even trying to achieve? Democrats wanted to cover more people and reduce long-term costs, and they had an argument for how their bill did both. As far as I can tell, Republicans have neither. At best, you can say this bill makes every obvious health care metric a bit worse, but at least it cuts taxes on rich people? Is that really a winning argument in American politics? In reality, what I think we’re seeing here is Republicans trying desperately to come up with something that would allow them to repeal and replace Obamacare. This is a compromise of a compromise of a compromise aimed at fulfilling that promise. But “repeal and replace” is a political slogan, not a policy goal. This is a lot of political pain to endure for a bill that won’t improve many peoples’ lives, but will badly hurt millions. Why Your Plan Was Cancelled: Health Insurance and the Affordable Care Act

There's a bizarre reason why millions of Americans saw their health plans cancelled in 2013 and, as explained in a new video featuring Robert Graboyes of the Mercatus Center at George Mason University, millions more will lose their plans in years to come. Insurance coverage for Americans will remain in permanent turmoil because the Affordable Care Act (ACA) requires that all plans fit within cookie-cutter designs called "metallic tiers." (The tiers—bronze, silver, gold, and platinum—refer to the percentage of medical expenses a particular plan pays.) The video also explains that families may have to switch plans repeatedly because, as circumstances change, a plan that fits within a tier one year may not fit in any tier in a later year. How Medicaid & Obamacare Hurt the Poor - and How to Fix Them

"Most physicians can't afford to accept Medicaid" patients, says Dr. Alieta Eck, a primary-care physician based in Piscataway, New Jersey. "If you're getting paid about $17 per visit, it won't be long before you can't pay your staff or pay your rent." Medicaid is the nation's health care system for the poor. It's funded jointly by the federal government and the states. Medicaid is either the first- or second-largest budget item in all 50 states and the program is slated for a massive expansion under President Obama's health-care reform law. Despite the program's huge and growing overall cost, reimbursements to medical providers are so low that many practices refuse to accept Medicaid patients, causing long waiting periods for treatment. Eck and her husband, Dr. John Eck, are the founders of Zarephath Health Center, a free health care clinic in Somerset, New Jersey, where they each volunteers six hours per week taking care of poor patients. While the Ecks don't accept Medicaid in their private practice, some of the patients that show up at their free clinic are Medicaid recipients who can't find a regular doctor. "The hardest thing for a Medicaid patient to do is get a doctor's appointment," says Avik Roy, who writes a health care blog at Forbes.com and is a senior fellow at the Manhattan Institute. One consequence is that Medicaid recipients show up at emergency rooms at nearly double the rate of the privately insured, often with accute problems that could have been addressed earlier in a doctor's office. They're also more likely than both the privately insured and the uninsured to have late-stage cancer at first diagnosis. After they've been diagnosed, it's also difficult for Medicaid patients to find qualified surgeons who will treat them. A University of Virginia study found that Medicaid patients were about twice as likely as the privately insured to die in the hospital after surgery. Even the uninsured were more likely to make it out of the hospital alive than Medicaid patients. Despite the program's failings, in 2014 Obamacare will add millions of new patients to the program's rolls. "All too often, people who claim to care for the poor say, 'I'm going to give you a card that says you have health insurance and my work is done,'" says Roy. "But the hard part is making sure that person gets treated." Obamacare was designed to expand Medicaid by about 17 million enrollees by 2021, but it likely won't meet that goal because the Supreme Court ruled that states don't have to participate in this component of the law in order to keep current levels of funding. So far, the governors of 19 states have come out against expanding Medicaid in their states. So what's the best way to provide quality health care to the poor without spending more money that we don't have? Roy says the federal government should take the same money it spends on Medicaid and block grant it to the states so they can experiment with health care plans in which the patient is in control. "Let them spend it on the doctor of their choice," says Roy. "Let people take the money and get the bureaucrats out of the way, and you'll find there's suddenly a lot more efficiency in the way people actually get health care." Eck believes charity care could be a big part of the solution, if only the government made it easier for doctors to volunteer their time. She has worked with state Sen. Robert Singer (R-N.J.), who has co-sponsored a bill in New Jersey that would allow the state to cover physicians for malpractice in their private practices as a way of compensating them for volunteering. The bill is currently awaiting consideration by the state senate's health care committee. "Every doctor I talk to says, 'I would do that in a heartbeat,'" says Eck. In the meantime, when Obamacare takes full effect next year, charitable clinics like Eck's will be more essential than ever to pick up the slack for a social safety net that's already not working. "I've been doing this for nine years," says Eck, "and I can honestly say that I come away feeling good that I was able to make a difference." About 5 minutes. The GOP Fix for Obamacare: Rich Get Tax Breaks While Millions of Others Lose Healthcare

http://democracynow.org - House Republicans have unveiled long-awaited legislation to repeal much of the Affordable Care Act—also known as Obamacare—including its expansion of the Medicaid program for the poor. The proposal includes a large tax break for insurance companies that pay their CEOs over $500,000 per year. It also defunds Planned Parenthood and eliminates abortion coverage. The Republican proposal does retain Obamacare’s requirement that insurers cover people with pre-existing conditions. However, it scraps the revenue-generating mechanism that makes this possible: the individual mandate, which required all Americans to sign up for health insurance or pay a fee. President Donald Trump and fellow Republicans have repeatedly promised to repeal and replace Obamacare, but their efforts have faced internal divisions as well as sustained outcry from constituents at town hall meetings across the country. For more, we speak with John McDonough, professor at the Harvard T.H. Chan School of Public Health. He is the former executive director of Health Care for All in Massachusetts, which played a key role in the passage of the 2006 Massachusetts health reform law. Do you qualify for Obamacare? Under the Affordable Care Act—Obamacare--if you want to enroll in a health plan outside of the annual Open Enrollment Period, you must have what's called a qualifying event -- which is sometimes also referred to as a qualifying life event. An example of a qualifying event would be: 1. Permanently moving to a new city or state 2. Certain changes in your income 3. Changes in your family due to marriage, divorce, birth of a child, or loss of a loved-one 4. Loss of existing health coverage because of a job change or job loss 5. Loss of eligibility for Medicaid or CHIP, 6. Or, expiration of your COBRA benefits If you have a qualifying event, in most cases that triggers a 60-day Special Enrollment Period when you can change your existing coverage or apply for a new plan. If you have a qualifying event that triggers a special enrollment period, eHealth recommends that you have documentation of your qualifying life event with you when you apply for new coverage -- whether you apply online or offline. In some instances, documentation forms may not be necessary, but it's better to have them in case your new insurance company needs them.
