April 3, 2015 11:34 PM CBS New York http://newyork.cbslocal.com/2015/04/03/seen-at-11-is-the-antibiotic-cipro-a-prescription-for-danger/ Cipro is a common antibiotic that is prescribed for treating a number of bacterial infections, but some say it may be a prescription for danger. “I wanted to be a firefighter since I was a kid,” Chris Jones said. For 9 years Jones has been living that dream, but the last 5 months have been a nightmare. “My life flipped upside down,” he told CBS2’s Dick Brennan. Flipped upside down, he said, because of crippling side effects he experienced after taking the popular antibiotic Ciprofloxacin or Cipro as it is more commonly known. “I went on a walk with my dog after taking the medicine, and it felt like a bomb went off in my body,” he said. It’s pain that continued ever since, especially in his legs. “His story is so, to me, typical of many of the stories I’ve read about and looked through the FDA files on,” Dr. Charles Bennett said. Dr. Bennett is a professor of pharmacy who recently filed a petition asking the FDA to put broader warnings on Cipro. the medication is part of a class of antibiotics called quinolones, along with the drugs Levaquin and Avelox. “Doctors are not aware of the full range of toxicities that are associated with the quinolones,” Bennett said. Critics also charge that the drugs, which are so strong they can kill anthrax, are too freely prescribed for illnesses that could be treated with less powerful medications. “They haven’t done a good enough job warning the public about how dangerous this drug is,” Jones said. The FDA said it’s logged just 4,500 reports of serious side effects, an exceedingly small percentage considering 23-million prescriptions are filled each year. Drug makers already list side effects including the possibility for tendon and nerve damage on the labels. “I just don’t know if I’m going to get better or not, and no one can tell me,” Jones said. Jones is living with the pain and other side effects, which even the FDA acknowledged may be permanent in some people. “The hardest thing is not knowing,” Jones said. The FDA told CBS2 it’s continuing to monitor complaints. Bayer, the maker of Cipro, said in a statement that they encourage patients to speak with their healthcare providers regarding any symptoms that they may experience after taking a prescription product.

WSET-TV Mar 30, 2015 11:03 PM Lynchburg, VA - Popular antibiotics used to treat common infections may be causing permanent damage. Fluoroquinolones have been at the center of more than 2-thousand lawsuits in the past year. They are among the most commonly prescribed antibiotics. Levaquin and Cipro are the big ones, and are used to treat everything from anthrax to a simple sinus or urinary track infection. "The most common side effects are nausea, vomiting and diarrhea, which can happen with any antibiotics" said Stephanie Puckett, Director of Clinical Pharmacy Services at Centra. Yet unlike other antibiotics, Fluoroquinolones come with a special 'black box' warning. That's the FDA's strictest warning, alerting patients of the possible nerve and tendon damage the antibiotic can cause. "I would consider the Fluoroquinolones a very strong or potent class of antibiotics because they do cover a large variety of organisms" said Puckett. But patients around the country say these prescription drugs carry more serious side effects than are listed on the label, including psychological effects. They have banded together on the Facebook page "The Fluoroquinolone Wall of Pain", to push the FDA for stronger warnings. Group member Wayne Smith says he only took Levaquin for 3 days and suffered from "depression from hell, crying all day and night for no reason at all… Panic attacks, 24/7 anxiety that would not stop." Similarly Lana Kennedy says she was "in a state of extreme confusion & began experiencing severe panic attacks" after taking Cipro. Patients filed a citizen's petition asking the FDA to expand its warning on Fluoroquinolones. The new warning would include other reported side effects like damage to mitochondria in cells and psychological effects. The FDA says "they are considering the matters raised by the petition and giving it [their} careful attention."

Chino resident Chris Jones was taking a powerful antibiotic called ciproflaxacin, more commonly known by the brand name “Cipro.” It’s taken by millions of Americans every year. Cipro belongs to a class of antibiotics called fluoroquinolones, which are so strong they can kill anthrax. They’re used to kill a wide variety of bacteria responsible for many common infections. Jones’ ordeal started in October with a bout of groin pain, which he thought was a hernia. His doctor suspected it was an infection and prescribed him Cipro, but Jones took the generic. Two days after starting the medication, Jones called his doctor about soreness he was feeling in his legs. The doctor said to keep taking the medication. Jones went to a specialist and told him about disturbing side effects patients were reporting online. He said the new doctor didn’t believe ciproflaxacin caused those problems: “He told me to stay off the Internet, and he wrote me another prescription.” A day after starting his third bottle, Jones starting feeling excruciating pain from his hips down: “I went on a walk with my dog and it felt like an explosion went off in my body.” Jones says he hasn’t been the same ever since. And no one missed the “old Chris” more than his twin brother, Jeremy: “We used to go to the gym together, hiking together.” They used to work at the same fire station, but Chris hasn’t worn his uniform for five months. That’s because he says he can’t climb ladders, can’t lift gurneys, and he can’t even lift his own children. “I was poisoned,” Chris Jones said. Some experts say fluoroquinolones are prescribed too freely for illnesses that could be treated with less powerful medications. “Doctors are not aware of the full range of toxicities that are associated with the quinolones,” said Charles Bennett, a professor of pharmacy at the University of South Carolina. Bennett has filed a citizen’s petition asking the federal Drug and Administration for broader warning for fluoroquinolones. He says Chris Jones is far from alone: “His story is typical of many of the stories I’ve read about and looked through the FDA files on.” The FDA has logged more than 4,500 reports of debilitating side effects linked to fluoroquinolones. Drug makers and the FDA are open about many of these drugs’ dangers, including damage to the tendons and nerves, which are listed on the label. But with up to 23 million prescriptions of fluoroquinolones being filled in a year, Jones believes they haven’t done a sufficient job of warning the public of the medications’ dangers. Jones says the FDA acknowledges that some side effects could be permanent: “I just don’t know if I’m going to get better or not. And no one can tell me.” The FDA told CBS2 it is continuing to monitor complaints and that petitions to update warning labels remain under consideration.

CBS Iowa Updated: Fri, Apr 03 2015, 10:29 PM EDT Her career is in insurance, but Patti Ireland's passion is photography. For years, Ireland would travel across the country photographing wild birds in their habitat. But a photography trip two years ago, would be her last. A trip to the doctor changed everything. After an asthma problem, her doctor gave her prednisone, a drug that's also known to weaken the immune system. As an extra precaution, she says her doctor then gave her a strong antibiotic called levofloxin, brand name Levaquin. It was a pre-emptive strike on any potential illness, not because she had symptoms. She says, "48 hours after I was given the first pill, I woke up with tremors, which you can hear in my voice and see in my hands. Ringing in my ears that sounds like living in a tent full of cicadas." Ireland wondered if the Levaquin pill was causing it. "I called the doctor and she said, 'Oh, no, it must be the prednisone. Keep taking it.'" She stopped taking the prednisone but took one more Levaquin pill. That was it. "8 days after stopping it," she says "I was in a hotel room in Florida on a photographic trip and I woke up in the middle of the night feeling like there wre hot pokers in my body. I couldn't walk. I had trouble breathing. I was panicking." Now two years later, the tremors are constant. Basic tasks like getting a drink of water are a challenge. There are no more photography hikes into the woods and her physical and cognitive issues forced her to retire early from her career. Dr. Julie Buchkina, MD, did not prescribe the Levaquin, but now treats Ireland's symptoms. She says, "It appears the symptoms she has experienced since then, are permanent." She goes on to say, "Ever since she took the Levaquin, she has struggled with new health conditions that she previously didn't have." There is a black box warning right now on Levaquin, the strictest warning a drug can have. But it warns of tendon issues like tendon ruptures and tendonitis, not the nerve and cognitive damage Patti is living with. The year after Patti took the two pills, in 2013, the FDA recommended warnings on the label and they are now posted to include the potential for nerve damage called peripheral neuropathy. The same report also points out the long-term damage to mitochondria cells, problems that could lead to diseases like Alzheimer's, Parkinsons and ALS. Dr. Buchina says, "Currently there is no official warning on Levaquin to warn abou the risk of increased damage to mitochondria, but that is something the FDA is looking into." Ireland doesn't blame the doctor who prescribed it. She says it was given to her in good faith. She also cannot sue the drug maker, as it was a generic version of Levaquin. She now just wants changes including regulation of this powerful antibiotic, adding "These drugs need to be regulated. They should only be used as they were designed, which is in life threatening situations where an infection isn't responding to any class of antibiotic." KDKA-TV health editor Dr. Maria Simbra explains why some are calling for a closer look at a certain group of antibiotics. From time-to-time, Janice Olszewski’s mother, Mary, needs to take antibiotics for urinary tract infections. “She gets very weak, she gets sick. Sometimes, she’ll get disoriented,” Olszewski said. Her doctor prescribed a medicine called Levaquin. “He said it would help clear the infection, and we should get fairly quick results with it. It’s been a Godsend for us,” Olszewski said. “If it wouldn’t be for Levaquin, we’d have a hard time managing her health care. “I’d say 20-30 times a year, we give a prescription for levofloxacin,” Dr. Marc Itskowitz of Allegheny General Hospital’s Internal Medicine said. “We try to reserve it for patients who need a broader spectrum antibiotic, may have been exposed to other antibiotics and are still sick. It’s very effective in those patients.” Taking it by mouth works the same as taking it by IV. It also comes in an easy-to-swallow liquid. While it kept Janice’s mother out of the hospital, not everyone has had a good experience. Karen Paddock was taking out the garbage when she cut her leg on a broken dish in the trash bag. She got stitches, followed by infection of the skin – cellulitis. “At that point, they gave her the Levaquin antibiotics for the cellulitis,” Bob Paddock said. Six months later, she developed tendon troubles and couldn’t walk. “It was in 2003, and at that point, I’m not sure all the risks that are known today were known then,” Paddock said. After ten years of using a scooter around the house in chronic pain, at age 43, she committed suicide – four days shy of her 20th wedding anniversary. Her husband has strong hopes for all similar antibiotics. “That they either get taken off the market, or they get a whole lot harder for anybody to prescribe,” he said. Levaquin, generically known as levofloxacin, is in a class of drugs called fluoroquinolones. They work by attacking bacterial DNA, making the bacteria unable to multiply. Known risks include nerve problems and tendon rupture. Fluoroquinolones are not recommended for people taking steroids, people with a history of seizures, and people with certain heart rhythm abnormalities. Pharmaceutical watchdog Dr. Charles Bennett is trying to expand the warnings by bringing a citizen’s petition to the FDA. “I’ve been doing this work so long, that I think patients find me,” Dr. Bennett said. His focus is the rare complication of mitochondrial toxicity. You can think of mitochondria as your cells’ battery packs. Without them, your cells have no energy. He said if an event happens right after the medicine is given, that would be a reaction. He also has lab researchers who have been able to recreate symptoms in mice. “There’s a little bit of an art, rather than a science. There’s not a specific diagnostic test,” he said. He would like to expand the black box warning on Levaquin, or establish a process so that patients must sign that they understand the risks of the drug. “This would make Levaquin a little bit less attractive to use if it goes through as a black box warning,” Dr. Amesh Adalja, of the UPMC Center for Health Securit, said. This infectious disease specialist said the class of antibiotics does have a role to play, but shouldn’t be prescribed automatically. “Even if you’re prescribing penicillin, it’s something you have to do with each infection that you treat. You have to make sure is this the right bug that you’re treating, and is this the right drug that you’re using,” Dr. Adalja said. “For every patient that has been harmed by a medication, there are thousands that have been helped by it. And we don’t often hear from them,” Dr. Itskowitz said. “She’s had adverse reactions in the past to numerous drugs, so we’re always very alert to those types of reactions. And we felt the risks were minimal,” Olszewski said. Hearing from Olszewski, she’s thankful her mother had the option of Levaquin. “Within a couple of days, she starts to feel better,” she said.

Labels for these antibiotics already warn about the risks for tendonitis, tendon rupture, central nervous system effects, peripheral neuropathy, myasthenia gravis exacerbation, QT prolongation and torsades de pointes- these are severe heart rhythm abnormalities, phototoxicity, and hypersensitivity. The concern that has received the most attention is the risk of tendon rupture. Fluoroquinolones, including ciprofloxacin (Cipro) and levofloxacin (Levaquin), are often used to treat urinary infections. These antibiotics have been associated with increased risks of tendon ruptures. Antibiotic-associated diarrhea can occur when you take antibiotics. These medications can alter the normal bacteria in the G.I. system. In order to decrease your chance of having diarrhea, your doctor can keep the duration of antibiotic treatment as short as possible that still eliminates the infection. Antibiotics that have been linked to photosensitivity are a concern for anyone who plays outdoor sports or exercises outside. Tetracyclines, fluoroquinolones and sulfa drugs are believed to carry this risk. It is possible that you could develop a serious sunburn on body parts exposed to the sun or have an increased risk of skin cancer if you use these medications. Theoretically you should avoid these antibiotics or use sunscreen and wear fully protective clothing. By no means does this list of concerns represent medical advice. [12] Powerful Antibiotic Could Be A Prescription For Danger Pt. II (Fluoroquinolone Toxicity)
February 24, 2015 11:35 PM CBS Miami http://miami.cbslocal.com/2015/02/24/powerful-antibiotic-could-be-a-prescription-for-danger-pt-ii/ Sixty year old Joseph King of North Miami said his life changed and he believes could end prematurely because of what he considers to be a toxic poisoning by the antibiotic Levaquin. Levaquin is one of a class of antibiotics called Flouroquinolones coming under heightened scrutiny by some medical researchers and the public. CBS4 Chief Investigative Reporter Michele Gillen asked King about the impact he believes the drug has had on his life. “It has destroyed it, completely destroyed it. As you can see it has killed my teeth. It has destroyed my jaw. So I cannot have any oral surgery to repair it. It has dissolved my tendons and connective tissue. And my muscles have actually decayed and my prognosis is very dim,” shared King. Click here to watch Michele Gillen’s report Four years ago, King said he was prescribed Levaquin in an emergency room, when he was suffering from salmonella poisoning, or food poisoning. “I was put on it intravenously then put on a dose of it orally for the next 30 days,” said King. As CBS4 News recently reported, this class of antibiotics carries a black box warning for possible tendon injuries. Since that first report, the Investigative team has reviewed an internal FDA safety report on the potential negative impact this class of drugs has on nerve endings. On page 12, CBS4 found the FDA’s own revelation of how these drugs can essentially impact a user in ways similar to neuro-degenerative diseases such as “Parkinson’s , Alzheimer’s, and ALS.” If the drugs can affect the function of our cells, the mitochondria and ultimately the brain, Alan Redd, a biological anthropologist, who is writing about this class of drugs, told Gillen it is chilling. “They state explicitly in there that mitochondrial dysfunction can be associated with neurological diseases like Alzheimer’s, ALS and Parkinson’s,” insisted Redd. He continued in a Skype interview with Gillen from his home in Kansas where he teaches at the University of Kansas. He believes even a recently revised warning is not strong enough. “There is a big disconnect between the safety report and that warning. There is no warning in the new package about brain damage, about Alzheimer’s about Parkinson’s or ALS. So there is a disconnect about the internal document and what was released to the public. And I find that disturbing. I find that outrageous actually,” Redd reflected. Redd said he was damaged by taking this class of antibiotics five years ago. He said he, “suffered a number of side effects. They have not resolved completely. ” Meanwhile, Redd is pushing for more transparency from the FDA regarding its own findings and joining supporters of a citizens petition filed with the FDA last June. His message to the FDA is, “come clean and tell the public and physicians that this is a class of drugs as you noted in your document that has the potential to damage mitochondria, cause dysfunction and potentially increase your probability for neurological diseases.” King agreed, saying “not only is the public not aware of this, the doctors are not aware of it either.” King said these drugs can be life saving for many, especially in the cases of life-threatening illnesses but he fears that without change, more patients could find themselves walking the road he is. What is his goal now? “To make it each day,” said King.

Tropical Diseases: Emerging Concerns
In today's climate, tropical diseases are a hot topic. Statistics are indicating an increasing prevalence of vector-borne and other diseases characteristic of the tropics, together with the re-emergence of certain communicable tropical diseases some of which are proving resistant to conventional drug treatments. The prevalence of tropical diseases has increased in Australia in the past ten to twenty years, due partly to an increase in international travel. The future impact of climate change is another consideration. With the recent flood disasters, they have also become a more immediate threat to public health. All health professionals need to be alert to the increasing incidence and changing spread of tropical diseases, as well as their relevance to special population groups, such as Indigenous Australians and refugees. This program looks at three main areas of concern: diseases endemic to tropical Australia; diseases arriving into Australia from overseas; and emerging threats from tropical countries to the north of Australia, such as multidrug-resistant tuberculosis.
