Inflammatory breast cancer is an uncommon but aggressive form of breast cancer, and many women don't recognize the warning signs. Dr. Beth Overmoyer of Dana-Farber discusses the symptoms of IBC and what you should do if you discover them. Transcription: Narrator: Yvette Hudgins is getting chemotherapy to treat her breast cancer. She has a rare form on the disease called Inflammatory Breast Cancer, or IBC. Hudgins: I just thought it was just inflammation. I would never, ever have thought in a million years it was breast cancer. Narrator: Like most patients who get IBC, the mother of three never heard of it, or knew the warning signs. Hudgins: I thought at first, you know, maybe it was just a little change. And then, it just progressed, so I made an appointment with my doctor. Narrator: Yvette’s doctor is Beth Overmoyer, an IBC expert at Dana-Farber Cancer Institute in Boston. She says when most people check for breast cancer, they’re expecting lumps, but IBC can look like this: what many of us would think was just a rash or bruise. Overmoyer: Inflammatory Breast Cancer occurs one to five percent of all breast cancers, so a very small number of women. And so, because it is uncommon, you don’t necessarily jump to that as the first diagnosis. Narrator: The most common symptoms of Inflammatory Breast Cancer include redness, warmth, and swelling, skin that appears discolored or dimpled, like an orange peel, and itching and pain in the breast.IBC is very aggressive. Studies show the five year survival rate is between twenty-five and fifty percent, so Dr. Overmoyer says it’s important to watch for signs. Overmoyer: If you notice some change, just call your primary care physician or your healthcare provider and have somebody check it out. Reassurance never hurt anybody. Narrator: Yvette agrees. She caught her cancer early. Now, she and her fiancé are planning a cruise. She knows she’s one of the lucky ones. Hudgins: I love cruises, so yeah. Next year, I’m taking a ten [day], because I’m not going to be able to go this year. So next year I’m taking a ten day. I’ve never been on a ten day, but we’re going on a ten day next year. It’ll be my celebration cruise. Narrator: At Dana-Farber Cancer Institute in Boston, I’m Ann Dore, reporting.

I was diagnosed with Stage IIIB triple negative inflammatory breast cancer (IBC) on January 13th of 2016. Before then, I hadn't even heard of it. After 8 rounds of chemo, a double mastectomy and 33 radiation treatments, I'm still alive and kicking! For more info, visit www.theibcnetwork.com * Symptoms of IBC. Below I've posted some of the symptoms of IBC. What symptoms did I have? I experienced rapid growth in my affected breast, dimpling skin, some redness, hard swelling, orange peel skin, and an inverted nipple. All this happened with a matter of weeks after having a mammogram and ultrasound. * I was diagnosed by mammogram, ultrasound,PET scan and biopsy. * Rapid, unusual increase in the size of one breast * Dimpling or ridging of breast skin * Redness of the breast: Redness involving part or all of the breast is a hallmark of inflammatory breast cancer. Sometimes the redness comes and goes. * Swelling of the breast: Part of or all of the breast may be swollen, enlarged, and hard. * Warmth: The breast may feel warm. * Orange-peel appearance: Your breast may swell and start to look like the peel of a navel orange (this is called “peau d’orange”). * Other skin changes: The skin of the breast might look pink or bruised, or you may have what looks like ridges, welts, or hives on your breast. * Swelling of lymph nodes: The lymph nodes under your arm or above the collarbone may be swollen. * Flattening or inversion of the nipple: The nipple may go flat or turn inward. * Aching or burning: Your breast may ache or feel tender. And remember, only 10% of women diagnosed with IBC present with a lump. * IBC accounts for 1-5 % of all breast cancers diagnosed. * Inflammatory breast cancer progresses rapidly, often in a matter of weeks or months. Inflammatory breast cancer is either stage III or IV at diagnosis, depending on whether cancer cells have spread only to nearby lymph nodes or to other tissues as well. It is not commonly picked up by a mammogram. * It is more common and diagnosed at younger ages in African American women than in white women. The median age at diagnosis in African American women is 54 years, compared with a median age of 58 years in white women * Diagnosing IBC is difficult. This is because IBC is not easy to see on a mammogram & often missed due to the lack of a defined lump and diffuse tumor clumps. It's described as being web-like or looking like cotton candy. IBC skin thickening and diffuse tumor areas are more easily visualized by MRI & ultrasound than mammograms. It is possible to have a completely negative mammogram and still have IBC so do not rely on this mode of imaging alone. Only one third of women with IBC have palpable lumps. IBC can be spread out over the entire breast very quickly.

A new lump or thickening in your breast or armpit could be a sign of cancer. Get to know the size, shape and feel of your breasts. See your doctor if you notice any changes or anything that is unusual for you. Breast pain is very common and it’s not normally due to cancer. You might get pain in one or both breasts for a while, which goes after a time. Skin changes include puckering, dimpling, a rash, or redness of the skin. The skin on your breast might look like orange peel. Or the skin might feel a different texture than usual. One nipple might turn in or sink into the breast. It might look or feel different to usual. Fluid leaking from a nipple in a woman who isn't pregnant or breast feeding can be a sign of breast cancer. Although any of these symptoms can be caused by things other than breast cancer, if you have them, they should be reported to a health care provider so that he or she can find the cause. Step 1: Do a monthly self-exam Start performing a monthly self-exam as soon as your breasts are fully developed. Checking yourself regularly is important -- you need to know what your breasts feel like normally so you can recognize any changes. Examine yourself several days after your period ends, when your breasts are least likely to be swollen and tender. If you're no longer having periods, choose a day that's easy to remember, such as the first or last day of the month. Keep in mind that it's not uncommon for breasts to feel lumpy due to benign fibrocystic breast disease, cysts, scar tissue, infections, and other causes that have nothing to do with cancer. Tip For instructions on how to do a breast self-exam properly, go to "Breastcancer.org":http://www.breastcancer.org/. Step 2: Have lumps checked Know what you're feeling for: a lump that feels different from your breast's normal lumpiness, like discovering a pebble in your oatmeal. Though many lumps are benign, anything that feels new or odd should be checked by your doctor -- even if you've recently had a clean mammogram. Check for lumps in your armpits, too. Tip Cancerous lumps are more likely to be hard, painless, and unmovable. Step 3: Beware of dimpled skin Look for visible changes, like dimpled, puckered, thickened, reddened, or scaly breast skin, or a flattening or indentation on the breast. All are potential breast cancer symptoms that should be evaluated. Step 4: Note nipple changes Recognize the nipple changes that can indicate breast cancer -- pain; redness; scaliness; itching; skin thickening; the nipple turning inward; or discharge other than breast milk. Step 5: Have pain and swelling evaluated See your doctor about swelling in all or part of your breast, or breast pain. Though swelling and soreness are usually no cause for concern, these symptoms can be signs of a rare but aggressive form of the disease known as inflammatory breast cancer. Step 6: Get regular check-ups Have your doctor perform a breast examination at your yearly check-up, and begin annual mammograms at age 40. If you have a family history of the disease, tell your doctor: they may suggest that you start having mammograms at an earlier age. Knowing the signs of breast cancer -- and being proactive about knowing how to recognize them early -- is the best way to protect yourself. Did You Know? 6 Symptoms of Breast Cancer That Aren’t a Lump Lumps get most of the attention when you think about the symptoms of breast cancer. You’ve probably heard that you should check your breasts regularly and be on the lookout for new or unusual bumps you can't remember being there before. If you do find one, don't panic—some women's breasts happen to be lumpy without it being a sign of cancer. But if it's a new lump, feels different from other lumps, or you just want some reassurance, it’s a good idea to get it looked at by a doctor. But there are other breast cancer signs you should know, too. “It’s not uncommon for breast cancer to present itself as something other than a lump,” Jack Jacoub, M.D., a medical oncologist and medical director of MemorialCare Cancer Institute at Orange Coast Medical Center in Fountain Valley, Calif., tells SELF, estimating that anywhere from 10 to 20 percent of breast cancers he's seen don't involve one. While the most common symptom of breast cancer is still a new bump or mass, according to the American Cancer Society, here are a few others that should be on your radar, too. 1. Skin dimpling. Tumors can be deep in your breast and cause inflammation around them that tethers to the ligaments and skin, Dennis Holmes, M.D., a breast cancer surgeon and researcher and interim director of the Margie Petersen Breast Center at John Wayne Cancer Institute at Providence Saint John’s Health Center in Santa Monica, Calif., tells SELF. When this happens, part of your skin can be pulled in, creating a dimpled effect. This tends to be more obvious when your arms are raised, Dr. Holmes says, so make sure you also elevate your arms when you’re inspecting your breasts and bring this change up with your doctor. 2. Nipple retraction. Most women’s nipples stick out, but it’s possible to have inverted nipples, where your nipple is pulled into the breast. That’s no biggie from a medical standpoint. What is concerning, though, is if your nipple used to stick out and starts to get pulled inward. Nipple retraction can be caused by a tumor that’s located in the center of your breast, says Dr. Holmes. “It involves the milk ducts and causes them to shorten, pulling in the nipple, ” he explains. Like dimpling, this is more obvious when your arms are raised and is more than enough reason to check in with your doctor. 3. Nipple discharge. It’s important to take a few things into account with this one, says Dr. Jacoub. If you’re pregnant or breastfeeding, it’s normal to have some nipple discharge. But if you’re not and you have bloody or clear discharge from your nipples, even when you're not squeezing them, it’s important to get it checked out, John Kiluk, M.D., F.A.C.S., a breast cancer surgeon at Moffitt Cancer Center, tells SELF. Keep in mind, though, that nipple discharge isn't automatically a sign that you have cancer. Noncancerous tumors in the breast, called papillomas, can cause a bloody discharge, according to the Mayo Clinic, and birth control, breast infections, and having fibrocystic (i.e., lumpy) breasts can also cause discharge. In any case, a medical professional can help you determine the cause and figure out the best course of treatment if necessary. 4. Breast asymmetry. It’s pretty likely that your boobs aren’t a perfectly-matched set, but if you start to see that one is suddenly becoming bigger than the other or its shape is changing somehow, it’s time to call your doctor. “The most important thing is noticing a change,” Dr. Kiluk says. A ductal or lobular breast cancer can cause asymmetry in your breasts, although weight gain and loss can as well. Bottom line: You won’t know what’s going on until you get it checked out. 5. Redness or a rash. Your boobs are regularly subjected to things that can irritate them, like your bra, lotions, and soaps. But if you notice a redness or freckle-like rash on your breast that feels warm to the touch and isn’t going away, you should get it checked out. Again, it could just be the soap you found in your partner's shower or the new detergent you switched to. However, in rare cases, it could be a sign of inflammatory breast cancer, a rare and aggressive form of breast cancer, Dr. Holmes says. Worth noting: People with nipple piercings can develop the skin infection cellulitis, which has similar symptoms, Dr. Jacoubs says. Cellulitis requires a doctor’s care, too, so if you're dealing with strange nipple symptoms, you might as well make an appointment. Johnston:This is the first time I’ve swung a club this year. So, watch this one, right up into the trees. Dore:Michael’s time and energy is now spent fighting cancer, and it’s a type of the disease most of us associate with women. Michael has breast cancer. Johnston:I was a hospital corpsman in the Navy for six years. I knew these things could happen. Did I really expect it to happen to me? No. Dore:Dr. Beth Overmoyer is Michael’s doctor. She’s a breast cancer expert at Dana-Farber Cancer Institute in Boston. She says for many men, a diagnosis of breast cancer can be embarrassing. Overmoyer:Suddenly, a man is faced with not only having cancer, but having cancer where the predominant representative color is pink. So we need to really overcome that, because male breast cancer is treated very effectively and it is the same cancer as that in a woman. Dore:And, like women, Dr. Overmoyer says it’s important for men to know the warning signs. The most common symptoms of male breast cancer include a lump or swelling of the breast, skin dimpling or puckering, nipple discharge or retraction, and scaling or redness around the chest. Undergoing a mastectomy and chemotherapy to treat the cancer has meant a tough couple of months for Michael and his family. But, he caught his cancer early and wants warn other men: They need to check for breast cancer, too. Johnston:The people I work with, a lot of them are doing self-exams right away. They never knew they had to do that. They said that they were examining other parts of their body, and said now we’ve got to add this to the list. Dore:Michael says helping others helps him feel good, and staying healthy helps at home. Johnston:I do have a good attitude, and I’ve assured my wife that I’m not going anywhere, so I have to stick to my word, now, on this one.
