Vohra Wound Physicians: Healing Wounds, Saving Lives Understand Wound Care: Wound Measurement Demonstration Summary- This is a demonstration of a wound care physician exhibiting the correct measurement of a wound. Japa Volchok, DO explains how to accurately and consistently measure wounds. Volchok discusses the importance of proper measurement and documentation in the wound healing process. This demonstration is performed by a trained wound care physician for educational purposes only and should not be tried at home. Understand Wound Care: Wound Measurement Demonstration Commentary: Japa Volchok, DO In this demonstration we will be exhibiting the correct measurement of a wound. For wound measurement you will want to have a disposable paper or similar measuring device as well as a cotton-tipped applicator. The measuring device is either a paper or plastic disposable measuring tape. It is generally marked in centimeter markings with sub-markings in millimeters. A cotton-tipped applicator is useful for measuring depth and checking for any undermining. This model demonstrates a wound. By convention, the superior aspect of the wound would also usually coincide with the head. The inferior aspect would be the foot. We measure length from a head to foot direction. Width is measured from left to right or right to left. Depth is measured at the maximum location of depth in the wound. The left to right or right to left convention does not matter in terms of the documentation. It is important however, that length and width are correctly measured and documented. As the convention from head to feet for length and left to right for width are important in being able to reproduce measurements between measurers. In this particular instance, we would start off by measuring the wound in the length dimension. Where there is the maximum length of this wound of approximately 5.7 centimeters. The width would then be measured at its maximum point of width which is 2.5 centimeters. The depth we then would determine by probing the wound with our cotton-tipped applicator using the cotton end. We would probe all areas of the wound to determine where the most depth was located. It appears to be approximately in the middle of the wound. We would then slide our finger down the cotton-tipped applicator to where it is flush with the intact skin. Pinch the applicator at that point and withdraw from the wound. This can then be laid over your measuring device and the depth determined. In this particular instance, it appears to be 1.8 or 1.9 centimeters. Once the length, width and depth of the wound have been measured, it is important to then record any evidence of undermining and what the dimensions of that undermining are. In determining undermining, gently probe the wound with your cotton-tipped applicator circumferentially around the wound. You can see that over on this side of the wound we are starting to see some undermining. It appears at this location, the cotton-tipped applicator slips deeper under the wound then it does at any other point. This would be the maximum area of undermining. As you can see, by convention, the superior aspect of the wound or the head aspect of the wound would be 12 o'clock. If we then progress clockwise around a clock face, the area of maximum undermining is approximately 10 o'clock. Similar to how we measured the depth of the wound, you would want to slide your finger down the cotton-tipped applicator. Pinch it with your nail. Then, bring it up to your measuring device. In this particular instance, it appears that the undermining is approximately 1.7 centimeters. As you will recall from the last image, the undermining is located at 10 o'clock. The Tissue Viability team of East Coast Community Healthcare provides tissue viability advice and support for the prevention and management of pressure ulcers, skin tear, and education across Norfolk and Waveney. The team identified unwarranted variation with a high level of requests for same day visits for residents in care homes with reported skin tears; leading to increased demand on the district nursing service. Additionally it was often discovered that a skin tear had not occurred and there was a frequent use of inappropriate dressings or inappropriate referral to hospital. The district nursing team discussed these issues with the Tissue Viability team, and agreed a need to improve the recognition and prevention of skin tear in relevant care homes as a catalyst for the change of practice. Evidence and research on skin tears was reviewed and a training package and a pathway for their care was developed; ensuring safety was central and a clear referral process to other services when necessary. Three care homes were identified to pilot the skin tear pathway for a 3 month period; including a review of the pathway and training package. The care of 26 residents was audited and the outcomes identified successful application of the correct wound care dressings, completed healing rates, and a decrease in the number of same day visits requested inappropriately. The education package and treatment pathway is now being rolled out to other care homes and district nursing teams in Great Yarmouth and Waveney, including Health Care Assistants and Assistant Practitioners who provide care to patients at home. There has also been a request from a neighbouring Clinical Commissioning Group for a similar programme. If the cut is more than one-fourth inch long or deep, you're gonna want to talk to your doctor or seek medical treatment; you may need stitches for that. So what do you do? Get the person away from any kind of danger; you're gonna want to clean the wound. Start by cleaning your own hands Whenever there is a break in the skin there is a risk for an infection so if your hands are clean you won't be spreading infection to this open wound. So you clean it with soap and water; if there's any kind of dirt, or grit, or particles, or pebbles, you're gonna want to get those out. Usually just with a wash cloth; rubbing with get it out. It doesn't feel that great but it will get all that stuff out. You don't want to leave it in there because that could lead to infection as well. If you have squeezers, you can wash them in alcohol and try to pluck all that stuff out. Once everything is clean and there's no more grit or dirt or sticks or anything like that you're gonna want to put some antibiotic ointment on there and you're gonna want to wanna cover it up with a bandage. If the bleeding soaks through, just put another pad or another piece of gauze or another clean cloth on top. Don't peek through and see what's going on; just keep it on top of there and hold it for a good 10 minutes. If the bleeding goes more than 10 minutes or starts to spurt, you need to call for help right away. You're gonna want to change the bandage once a day or whenever it gets wet or it gets dirty just to take sure that no infection, nothing can get in there to start an infection. If you haven't had a tetnus shot in a while or you're not sure when you've had your last tetnus shot, you're gonna want to talk to your doctor about getting a booster. Once the wound has been covered and you've been watching it everyday, if you see any signs of infection like puss, redness, swelling, or people getting the chills or fever or anything that's out of the ordinary you're gonna want to talk to your doctor and see what you can do about that.

It took myself and producer Fiona Lamptey 2 years, 217 Kickstarter backers and a whole lot of fake blood to pull off. SFX legend Colin Arthur (NeverEnding Story, 2001: A Space Odyssey) helped us achieve the impossible, allowing SFX Supervisor Jen Cardno and her team set a new standard for anatomical practical effects. Actor Sebastian Armesto (Anonymous, Pirates of the Caribbean) spent as long as 8hrs in makeup each day to bring this story to life. There are approximately 39 million people in the United States over the age of 65 and this number is projected to grow to 86.7 million by 2050.1 This age group has specific challenges as related to wound care, one of which is skin tears. The skin is the largest organ of the body and has many important functions. It protects from the environment, helps control body temperature, assists in controlling fluid and electrolyte imbalance, and contains nerve receptors. Skin changes are among the most visible signs of aging and also among the most problematic for the elderly. With aging, the outer skin layer thins and changes in the connective tissue reduce the skins' strength and elasticity. This increases the risk for a skin tear, which is a traumatic wound resulting from the separation of the skin layers. Rubbing, pulling, or even the simple act of removing a band aid may result in a skin tear. Tears can be problematic as aging skin repairs itself more slowly and wound healing may be much slower. This can contribute to pressure ulcer and infections. Diabetes, cardiac conditions, poor nutritional status, and lowered immunity can also affect healing.
