Most women with well managed gestational diabetes will have healthy babies of few with obligations. The most common obligation of gestational diabetes on the baby is large babies, that are more than the average weight which is because of the high blood sugar in mother which is transferred to the baby and causing the baby to produce more insulin. This insulin converts the glucose of baby in to fat and causes the over growth of the baby. Very low blood sugar at the time of birth is another complication which is again because of higher level of insolence in baby changing glucose to fat. Apart from that free term birth is also and common complication which maybe natural because of high blood sugar on the mother or it may be induce to avoid complications. Sometimes because of the pre term baby have more chances of getting jaundice. Respective repose Syndrome or breathing problems at the time of birth are more common in gestational diabetes mother. This is a word of caution if untreated gestational diabetes there is a slight risk of babies death during delivery or just after birth. Later effects of gestational diabetes on the babies they have the higher chance of developing obesity in later in life and also they are more pro 2 type diabetes later in life.

Infertility may be due to male factors, female factors or in a rare number of cases, it might be totally unexplained. Both male and female factors contribute equally to infertility. Some of the most common male factors are abnormalities in the semen analysis, which may be abnormal number of sperms or abnormal movement of the sperms or the morphology of the sperms may be abnormal. All these abnormalities may occur due to medical conditions such as diabetes, cystic fibrosis or even hypertension, thyroid disorders. So these may as say lead to abnormal production of sperms or motility of sperms. Certain local conditions may also lead to abnormal semen analysis, such as or they may be total azoospermia which may be obstructive in nature, due to infections or it can also be due to lack of production of sperms due to genetic causes. So all these factors vaicocele contribute together to the male infertility. What are some of the factors that lead to female infertility? Female infertility may be due to abnormalities in the ovaries, in the uterus in the tubes or the cervix. So some of the ovarian abnormalities that may give rise to infertility may be due to polycystic ovarian syndrome, which is one of the most common causes for infertility nowadays. Apart from that there may be diminished ovarian reserve, where the number of eggs that are present in the ovary itself may be reduced leading to infertility and then there is the tubal cause. This is another very common factor. Where in the tubes may be blocked or swollen, either due to infection or surgeries or due to certain other factors which may lead to total blockage of the tubes and also infertility. The uterine causes may also lead to infertility. So the uterus may have fibroids or adenomyosis or the uterus may have endometrial abnormalities, which may lead to a thin endometrium or tuberculosis or several other infections which can again give rise to infertility. Cervix or the mouth of the uterus can also give rise to infertility. One of the most common causes may be cervicitis or infection or it may bee due to atrophic or a very narrow cervix or it can be atrophic or even pinhole also. So these are some of the factors that lead to female infertility. Apart from that we have around 10% of the cases where the cause may be unexplained. So it may be attributed neither to the male or to the female. So these are the causes of unexplained infertility. This may be due to immunological problems that are undetected at that time. These problems do not come to light is we do not have too many tests to detect them.

Absence of menses is normal in pregnant, breastfeeding and menopausal women, but pathological otherwise. Amenorrhea is not a disease on its own, but rather a symptom of a variety of underlying conditions. PRIMARY amenorrhea is when a woman has NEVER had her periods, while SECONDARY amenorrhea is when a woman has STOPPED having them. Menstruation is part of the menstrual cycle, the monthly events that occur within a woman’s body in preparation for the possibility of pregnancy. Each month, an egg is released from an ovary in a process called ovulation. At the same time, the lining of the uterus THICKENS, ready for pregnancy. If fertilization does NOT take place, the lining of the uterus is shed in menstrual bleeding and the cycle starts over. The menstrual cycle is under control of multiple hormones secreted by the hypothalamus, pituitary gland, and ovaries. Basically, the hypothalamus produces gonadotropin-releasing hormone, GnRH; the anterior pituitary secretes follicle-stimulating hormone, FSH, and luteinizing hormone, LH; while the ovaries produce estrogen and progesterone. These hormones are involved in a REGULATORY network that results in monthly cyclic changes responsible for follicular maturation and ovulation. Amenorrhea can be caused by ANATOMICAL or ENDOCRINE problems. Anatomical causes refer to abnormalities in the female reproductive system and include: - absent or underdeveloped female organs in some genetic disorders, such as MRKH syndrome - congenital defects that OBSTRUCT blood outflow - and destruction of the uterine cavity by previous infections or surgeries. Endocrine problems refer to structural or functional defects of the hypothalamus, pituitary gland and ovaries. A common cause in this category is the impaired function of the hypothalamus which occurs when the hypothalamic-pituitary-ovarian axis is SUPPRESSED due to an ENERGY DEFICIT. This can result from weight loss, eating disorders, excessive exercise, malabsorption syndromes, or emotional stress. The common feature is a REDUCED production of GnRH by the hypothalamus, which results in corresponding LOW levels of FSH and LH and subsequent impairment of follicular maturation and absence of ovulation. Other endocrine causes include: - Kallmann’s syndrome, a genetic disorder associated with congenital defects of the hypothalamus, causing GnRH deficiency. - Sheehan’s syndrome, a condition in which excessive blood loss during childbirth or chronic hypotension during pregnancy IMPAIRS PITUITARY functions. - Tumors, infections, trauma or autoimmune destruction of the pituitary gland. - Polycystic ovary syndrome, an endocrine disorder in which FSH deficiency disrupts follicle maturation. - Loss of normal ovarian function in conditions such as Turner's syndrome - Thyroid disorders Treatment is by addressing the underlying cause and can range from nutrition plans, hormonal therapy to surgical interventions.

Turmeric is the third best-selling spice after flax and wheat grass in the last few years. Curcumin, its main ingredient, is a popular food additive with a nice, mild aroma and yellow color. Hence, people usually think that its consumption has no side effects. however, this is not completely accurate. This is because we often consider that natural things cannot be toxic or dangerous. Nonetheless, to some extent, this spice isn’t toxic, but there are individuals who have complained about nausea, diarrhea, stomach ache, and dizziness after turmeric consumption. Namely, when combined with black pepper, curcumin can be increased by 2000% and only one dose of this may feel like 29 cups. That is, the blood levels increase and this mixture can change the DNA in vitro. Therefore, to avoid unwanted side effects, these groups of people should avoid turmeric: 1. Pregnant and breastfeeding women. Women who are pregnant or breastfeeding should consume turmeric cautiously or even better, avoid it completely. Namely, when consumed orally, turmeric can put their pregnancy at risk by stimulating a menstrual cycle or the uterus. Also, men who are trying to be fathers should avoid it as well, because turmeric is known to decrease the testosterone levels and sperm movement. 2. People with gallstones. If you are a person who has problems with gallstones, you should avoid turmeric because a recent study showed that this spice can cause serious problems and ache in the gallbladder. However, it’s good to know that turmeric can prevent gallbladder stones and protect the liver by stimulating the gallbladder functions. 3. People with biliary tract obstruction. They should avoid this spice or consume it very carefully. 4. People with kidney stones. If you consume turmeric on a daily basis, you are at a higher risk of developing kidney stones. Moreover, this spice has high levels of soluble oxalate which easily attaches to calcium. Calcium oxalate cannot be dissolved and this is the main reason for kidney stone problems. To be on the safe side, consume only 50 mg daily. 5. Diabetics. Turmeric has been associated with lowered blood sugar levels. Therefore, if you are a diabetic, consume turmeric moderately in order to keep your blood sugar levels balanced. 6. People who will undergo surgery. The placenta is the organ that connects the fetus, via the umbilical cord, to the uterine wall of the mother. This is where the exchange between maternal and fetal blood takes place. The placenta provides the fetus with oxygen and nutrients and takes away waste such as carbon dioxide. Normally, it attaches at the top or side of the uterus. At birth, it is delivered AFTER the baby, in the third stage of labor. Placenta previa, or sometimes referred to as LOW-lying placenta, is a pregnancy complication in which the placenta attaches to the LOWER end of the uterus, NEAR or COVERING the cervical opening. Placenta previa can be classified as complete or marginal. The main symptom is painless bleeding in the second half of pregnancy. As the lower part of the uterus gradually THINS in preparation for the onset of labor, placental attachment is disrupted resulting in bleeding. Risk factors for development of placenta previa include: - previous pregnancies - previous surgeries of the uterus - previous placenta previa - carrying more than one fetus - maternal age of 35 or older - smoking and cocaine use The location of the placenta is usually checked during a routine ultrasound mid-pregnancy, but a low-lying placenta at this point may NOT be a cause for concern. In most cases, the placenta grows toward the richer blood supply in the upper uterus as the uterus expands in the third trimester. Only about 10% of all low-lying placentas persist until delivery. A placenta that completely covers the cervix is more likely to stay that way than one that's bordering it. Placenta previa presents significant risks to both the mother and the baby. A cesarean delivery is usually indicated because the placenta may block the birth canal completely, or bleed profusely during labor as the cervix dilates. The mother may experience excessive blood loss and the baby may suffer from hypoxia as a result of INadequate blood supply. The goal of treatment is to manage bleeding to get as close to the due date as possible. In case of little or no bleeding, bed rest and pelvic rest are recommended. Heavy bleeding is an emergency and might require a blood transfusion. If delivery is necessary before 37 weeks, corticosteroids may be given to help the baby's lungs develop. An uncontrollable bleeding would require an emergency C-section even if the baby is premature.

We do believe that we have tools to diagnose effect of fertility on various organs and one of things which is being noticed nowadays is fertility problems in men is specifically associated with heart disease, with diabetes and also with renal failure and depression. These are the four associations which people have found and still research is going onto what causes it . Is it because of something which happens when the baby is in the mother’s uterus? Is it because of imbalance in the body between testosterone and estradiol which are two primary hormones which are necessary to maintain proper sperm production. We do believe that many of these men have some amount of imbalance between testosterone and estradiol ratio which causes diabetes. It is not all men with fertility problem has diabetes. You mentioned that you're 37 weeks along, and it sounds like there's some current concerns about how big your belly is. Now, at each prenatal appointment, you probably notice how your doctor measures your belly from the bottom of your pubic bone to the top of your uterus, and this is called the fundal height. Basically, it should match your gestational age in centimeters. So when your 37 weeks along, you should be measuring about 37 centimeters. So if a doctor notices that you're measuring either larger or smaller than what you should be, then they have to look into it. Of course there are factors such as how the mother carries a baby. A tall girl will likely measure just a little bit smaller, because there's more space in her abdomen for the baby to hide. And a shorter girl might measure a little bit bigger, because the only place the baby can go is out. There are other factors too, like the amount of fluid surrounding a baby, and of course, the size of the baby. So if a doctor notices that a woman's abdomen is either smaller or larger than it should be, then it's a good idea to send her for an ultrasound to find out what's going on. And if it's the actual size of the baby, like if the baby is measuring small for their gestational age, then we have to wonder if we're looking at bad placenta that's not feeding the baby so well. And if the baby is small enough, then usually the decision is made to get the baby out, because generally speaking, mothers are the best incubator for a baby, but sometimes they're not if there are things going on. An example of what might affect the growth of a baby and make them larger than normal is gestational diabetes. And so that's something that's commonly tracked if women have diabetes. What's the size of their baby? How big are they growing? But in your case, it sounds like you're measuring small. It is good to listen to your doctor's advice to get another ultrasound. They're not trying to be annoying or to bug you. They're just trying to look out for the well-being of you and your baby. So if they give you recommendations after the ultrasound that weren't in your original plan, I do suggest talking to them about your concerns, and ultimately listening to their advice, because again, they're looking out for your best interest and want you and your baby to be as healthy as possible. Women who have diabetes of any type during pregnancy (whether it be gestational, or type 1, or type 2 diabetes) can expect to be monitored more closely during their pregnancy. You're considered high risk, and for this, it warrants regular monitoring and testing, especially when you get further along. Near the beginning of the 3rd trimester, you can expect to have regular non-stress tests. This is where you come in to either the doctor's office or another clinical setting and they put monitors on your belly that watch the pattern of the baby's heart rate. It also monitors for contractions, and if we see any irregularities in that testing, then further investigation is warranted. You can also expect them to watch your blood sugars closely to watch your symptoms and also to track the size of your baby. Patients frequently ask questions about whether aspects of their lifestyle influence their tendency to miscarry. The short answer is, not that we realize in any scientifically credible way. There is some early burgeoning evidence that obesity could adversely impact fertility or early pregnancy maintenance. I want to emphasize that that science is not crystal clear with regard to recurrent miscarriage, but could well be a hot topic of investigation in the next several years. Another area of medical care that I am involved in and try my best to make inroads in is that of glucose intolerance in pregnancy. Most people recognize the term diabetes. I suspect the public, most people out there, are now aware that gestational diabetes, which is diabetes diagnosed during pregnancy, is of growing concern in the United States, and it's directly linked to the obesity crisis, overweight obesity crisis that most people are aware of, I hope. At any rate, the more overweight or obese a patient is, especially a somewhat older mother, the more likely she is to have glucose intolerance identified. Typically, patients will have testing, a challenge of glucose with a subsequent blood draw to identify whether they can handle a glucose bolus (an intake of glucose) well, or whether they, in fact, have glucose intolerance, that is diabetes. Patients that have been pregnant before know that that's usually done around 28 weeks or so of pregnancy, but increasingly with overweight or obese patients, we're doing such testing earlier in pregnancy so we don't miss the diagnosis early in pregnancy. I think that's most appropriate. What I would like to encourage women who are considering pregnancy to get involved in is proper weight management and activity. One can't say enough about the benefits of those things with regard to pregnancy and with regard to long-term health. Eating a reasonable diet, both in terms of nutritional status within the diet and in terms of total calories, is what we should be about in the United States. We are not there as yet. Being moderately active is incredibly important for long-term health and for the avoidance of glucose intolerance in pregnancy or the management of glucose intolerance in pregnancy. General recommendations would include 150 minutes of moderate activity per week, and I think even more moderate activity than that would be a good general idea. I like to remind patients that it doesn't have to be triathlon training type stuff. No, people won't do that. I wouldn't do that. We're really talking about figuring out a way to walk 4 or 5 miles a day, as a part of your day, 5 or 6 days a week. One might incorporate that into their travel to and from work, or travel to and from childcare issues. One might incorporate that into a stroll at lunch or a stroll in the evening. But the concept is to be sure you're active enough to develop cardio-pulmonary health and to maintain reasonable weight. And I'm here to tell you it's about 4 or 5 miles a day or some equivalent.

How to stop leg and foot cramps at night and why do legs cramp at night? Nocturnal leg cramps are one of the most painful problem you can experience – they occur in the middle of the night and the sharp pain can ruin your sleep, leaving you without energy and with sore leg muscles in the morning. Leg cramps are involuntary contractions of the calf muscles that usually occur overnight. They can also strike at the soles or other muscles. The cramps may last from a few seconds to a few minutes, and the intense pain is often difficult to bear. Nocturnal leg cramps affect everyone regardless of age or gender, although they’re more common in middle aged people. Although science is still unsure of the main cause, there are numerous factors that contribute towards it. Dehydration Dehydration is one of the biggest causes of nocturnal leg cramps. Proper hydration means proper body and mind function, including the contraction of muscles. If you’re not keeping properly hydrated during the day, you will experience cramps overnight. Without sufficient amounts of water, your muscles will be robbed of essential nutrients they need to work normally. Nutritional deficiencies Potassium, magnesium, calcium or sodium deficiency can definitely cause the dreaded leg cramps. All these nutrients play different vital roles in your muscles and help them contract – if the muscles are robbed of them, they will fail to work properly. Potassium works with sodium and chloride and generates electrical impulses in the nerves and muscles, while calcium generates proper muscle contractions and nerve impulses. Magnesium stabilizes the levels of adenosine triphosphate, so any of these mineral deficiencies will result in an increased risk of muscle cramps and other problems. Standing for too long Standing for prolonged periods or wearing improper footwear can overburden your muscles and increase the risk of nocturnal leg cramps. According to one study, the same factor raises the risk of varicose veins, so make sure to cut the time spent on your feet. Sitting with crossed legs or sitting for too long can also cause the condition. Pregnancy Cramps are pretty common during pregnancy, especially from the second trimester onward. They can range from mild to pretty painful and occur as a result of the increased pressure of the uterus on some nerves and reduced blood circulation in the legs. Hypothyroidism Low levels of thyroid hormones in the body are definitely a cause for concern and may lead to a number of health problems including nocturnal leg cramps. Hypothyroidism reduces the levels of calcium in the body, resulting in muscle pain, weakness and numbness as well. Low thyroid hormone levels can also cause fatigue and low metabolism, which can result in inflammation and muscle cramps. Diabetes Diabetes patients often experience nerve damage (diabetic neuropathy) in the legs which results in sharp pain and numbness. The high blood sugar levels can dehydrate your body and lead to muscle cramps – if you experience this symptom and you’re a diabetic, you should consult with your doctor immediately. A few tips that will help you prevent nocturnal leg cramps. In order to prevent the cramps from occurring, you need to stay hydrated at all times. Consuming sport drinks with electrolytes can also prevent the problem. Drinking too much coffee, or sodas can dehydrate your body and increase the risk of cramps. There are many causes for recurrent pregnancy loss. Some of them are as simple as maternal and paternal age. As the age increases the risk for recurrent pregnancy loss increases. We find more risk in women more than 35 years and in men more than 40 years. The history of previous miscarriages increases the risk of pregnancy losses. The risk can be as low as 19% in case of one pregnancy loss and it can increase upto 40% in case of 3 pregnancy losses. Antiphospholipid is the most important treatable cause of recurrent pregnancy loss and it includes anticardiolipin antibodies, lupus anticoagulant antibeta 2 antibody 1 antibodies. It is typically characterised by 3 or more pregnancy losses that happens less than 10 weeks of pregnancy or 1 or more pregnancy losses after 10 weeks of pregnancy in a morphologically normal foetus or preterm births less than 34 weeks which are owing to placental disease. In some cases we have genetic factors playing a role especially because of parental chromosomal derangement when one parents has a balanced reciprocal robertsonian translocation or if there are embryonal chromosomal abnormalities. Some women have anatomic causes that is seen in normal 5 to 10% of cases especially if there are malformations of the uterus like the arcuate uterus or the septate uterus and this can cause pregnancy losses in almost 30 to 52% of the cases. Some women have cervical weakness called as cervical incompetence. Cervix is the lower portion of the uterus and it is supposed to hold the pregnancy delayed. If somebody has a weakness they have recurrent pregnancy losses typically seen in the second trimester of the pregnancy. Other causes could be polyps, subuterine myomas and intrauterine adhesions. There are also some endocrinal factors like uncontrolled diabetes uncontrolled thyroid imbalances or even polycystic ovaries which can contribute to RPL. Certain immunological factors have been researched nowadays. We find that these women tend to have more of T cytokine activity and deficiency and the blocking of the natural antibodies. With the current increase in the unhealthy eating habits obesity has also come up as an important cause. Infections as a cause of RPL are not so many. Initially people has testing for toxoplasma, rubella, cytomegalo virus, and the herpes. But currently they are very clear that this will not contribute to recurrent pregnancy losses and hence testing for this is not required. However bacterial vaginosis should be treated. There are some conditions like inherited thrombophelias like factor 5 latent mutation or protein C deficiency, protein S deficiency or a prothrombin gene mutation which can cause multiple microthrombi in the usual placental circulation.
