My horse is getting older (15) and stiffer in his hocks and stifles. He is currently getting a SmartPak for joints as well as the Devil's Claw Plus for anti-inflammatory. A lot of people, not experts, have told me that he will benefit from joint injections. While I agree that it may help him move better, I am hesitant about the cost/benefit ratio. My horse is mainly turned out in a dry lot pasture, ridden lightly in the arena or out on easy trails, 1 to 3 days per week. Is the cost of joint injections going to be practical for his workload? Is there anything else I can add to his routine that may help, but be less costly? Just looking for your two cents. 2. I was wondering if all horses can get swollen joints after exercise or if it's just Thoroughbreds. If so, is there any way to treat it other than cold packs and ice for horses that hate it, like mine? I've got a cob on light work that does a little galloping every second day or so but gets swollen fetlocks. Is this normal? If not, what should I do? 3. I ride an older horse (20) and he loves to jump still. He will jump the wood logs on the property sometimes. Are there any ways to keep his joints and bones in good condition, like a supplement or liniment? Since he is older, I worry he might pull something. We don't jump higher than 3 feet. 4. My 17-year-old Oldenburg gelding used to be a high-level hunter horse in his younger days (before I knew him). From mid-2016 to mid-2017, he was leased out by his owners and received horrific farrier work, where he had 3-degree front wedges on with no apparent purpose or need for them. When he came to my barn, he was about 10 percent sound. He is a million times better now and is perfectly suitable for my riding (very minimal, low jumping on good days), but he still has a little bit of pain in his right front on and off and comes out stiff some days. He has stifle issues as well, apparently due to compensating for the pain in his fronts. He is on previcox and methocarbamol, which work extremely well for him, as well as some joint and overall health supplements. My question is this: Do you have any suggestions on how to improve quality of life of semi-retired sport horses with chronic osteoarthritis? I have read a lot about high dose omega-3, for example, with the EPA and DHA. Thanks so much!!!! 5. My horse is an 11-year-old off the track Thoroughbred who has issues with his sacroiliac (SI) joint and an old injury in his left stifle. We are working to build his muscles and topline for strength and take the time to properly warm him up. What are some good exercises or healthy practices to help make him more comfortable during work? BONUS QUESTION!: What’s the difference between joint injections done in the muscle/vein and joint injections in the joint? What types of issues are better treated with what injection sites?

Osselets in Horses Brian S. Burks, DVM, Dipl. ABVP Board Certified in Equine Practice The fetlock, incorrectly called the ankle, as the human ankle is homologous to the hock, is the junction of the canon bone and the pastern. Specifically, the fetlock is made of the distal metacarpal/metatarsal bone, the proximal phalanx, and the proximal sesmoid bones. It is a high-motion joint under tremendous mechanical stress in performance horses. Such horses are prone to chip fractures and osteoarthritis of the fetlock joint. Any injury to this joint can begin the process of osteoarthritis. Osselets occur mainly in the front limbs, likely because more weight is carried on the forelimbs of the horse. The term ‘osselets’ refers to irritation of the front or side of the fetlock joint, with inflammation and joint effusion. In chronic cases, there is a bony lump at the cranial joint. Osselets are from micro-tearing of the joint capsule attachment, over time creating a firm, bony lump and joint effusion that is most evident on the sides of the joint. Acutely, they are very painful; however, over time they become less painful and may not bother the horse, being more of a cosmetic defect. Sometimes they are accompanied by bone chips and arthritis. Any horse with osselets should undergo a full lameness exam with radiographs to determine the extent of pathology. This disease is commonly bilateral, and the lameness can be difficult to detect. There may be a short, choppy gait, and pain with flexion of the joint. The horse may point one limb, if one is more sore or only one limb is involved. It may be called “green osselts” in early stages. As the arthritis worsens, so does the lameness. The joint capsule thickens and the periosteum that covers the bone becomes pulled and stretched, causing new bone growth. This is called exostosis, and can be palpated through the skin. Eventually this decreases the range of motion of the fetlock joint. Eventually, the periostitis leads to articular cartilage deterioration, and the attachment of the deep digital flexor tendon is damaged, leading to chronic lameness. The joint capsule continues to thicken, and the cartilage ulcerates, leading to loss of athletic ability. Conformation is important in the development of osselets; short, upright pasterns are the major fault leading to traumatic arthritis. There is less shock absorption, leaving concussion to travel up through the fetlock joint, causing repeated microtrauma, leading to arthritis. Other factors include foot imbalance, rugged terrain, and over-training, which causes muscle fatigue, causing stumbling during work. Osselets are most common in racehorses that are young and over-trained. Draft horses tend to have short, upright pasterns and may work on hard surfaces. Diagnosis is based upon clinical signs, nerve blocking patterns, and radiography. Pain during fetlock flexion, a choppy gait, and heat and swelling are common, especially with green osselets. The foot should be ruled out as a source of lameness by using a palmar digital nerve block. A higher nerve block or installation of carbocaine into the joint (intraarticular block). Radiographs can determine the amount of exostosis and rule out chip fractures. Later stages of disease will have narrowed joint spaces as the cartilage is destroyed. Cartilage can only be imagined radiographically as a space between two bones; it cannot itself be imaged by this modality. When diagnosed early and treatment implemented, many horses can resume normal work. The horse may, however, have a permanently enlarged fetlock. All work should be stopped anytime a horse becomes lame. Acute inflammation can be treated using ice therapy several times per day. After several days, warm, moist heat can be applied. Systemic and/or topical NSAID medications are often used for at least six weeks. Sometimes other treatments are necessary, including joint injections and polysulfated glycosaminoglycans (PSGAGs). Joints may be injected with steroids and sodium hyaluronate (HA). These medications can help decrease inflammation within the joint, but caution should be used with steroid injections as their over-use can lead to cartilaginous destruction, rather than protection by limiting inflammation. Polysulfated glycosaminoglycans (PSGAGs) are found in Adequan® and help to maintain or repair cartilage. They also reduce inflammation within the joint. In some cases, it can be injected into the joint, but there is a higher risk or joint sepsis with this product. Horses with green osselets should be stall rested, but they can swim during this time. Swimming provides exercise, toning muscles and building cardiovascular capacity while removing pressure from the limbs. After several weeks, careful exercise out of the pool may begin.
