Definition of a high blood pressure is the same for both men and women according to the latest American College of Cardiology Guidelines 2017, states that blood pressure of less than 120/80 is considered as normal. If the systolic blood pressure is more than 120 to 129, then we call it as an elevated blood pressure. Stage 1 hypertension is when the blood pressure is more than 120 to 139 systolic or 80 to 89 diastolic, we consider this as a stage 1 hypertension. In stage 2 hypertension the blood pressure is more than 140/90. Patients with blood pressure more than 140/90, requires a medical drugs or medical therapy. The patients with high elevated blood pressure and majority of patients with stage 1 hypertension, they can be managed with non-pharmacological method of reduction of the blood pressure. We will treat these group of patients with medications only when there is a high risk.

The only way to reduce the blood pressure directly is by giving medications majority of the times these oral medications will take time to reduce the blood pressure, we try to reduce the blood pressure immediately in a situation where there is a target organ damage, like if the patient has develop a stroke or heart failure or kidney failure or because of high blood pressure, then only we will try to reduce blood pressure suddenly, we need to use intraparentral medications, there are multiple methods like non pharmacological methods for reducing blood pressure which includes following a good healthy lifestyle, which includes a regular exercise 30 minutes a day for atleast 5 times a week can reduce the blood pressure by 4 to 5 mmHg. If the intake of salt is reduced the total sodium chloride intake is less than 6 grams or the total sodium less than 2.5 grams a day, that will reduce the blood pressure by another 4 to 5 mm Hg. Intake of dash diet means intake of more green leafy vegetables, fruits and low fatty diet and results in reduction of blood pressure marginally, he should also increase the intake of potassium in our food. Potassium intake also reduces the blood pressure marginally. Apart from that reduce the intake of alcohol, avoid smoking, all these will reduce the blood pressure to around 2 to 4 mm of mercury and keeping the weight at ideal for the age and the height is very important, if there is overweight, even if there is reduction of weight, it can reduce the blood pressure . By doing all this non pharmacological methods, person whose blood pressure is marginally elevated, we might manage only with no pharmacological method rather than using a medication.

High blood pressure can also present has heart attacks. So on lying down the patient can present with chest pain. So if there is an involvement of a high blood pressure, the first symptoms of a high blood pressure is loss of vision. Few patients might develop head heaviness or giddiness even though there is no target organ damage but majority of the patients with high blood pressure are asymptomatic. So we must identify if the blood pressure is high or not by regular checking of the blood pressure.

Pulsus paradoxus (PP), also paradoxic pulse or paradoxical pulse, is an abnormally large decrease in systolic blood pressure and pulse wave amplitude during inspiration. The normal fall in pressure is less than 10 mmHg or 10 torr. When the drop is more than 10mm Hg, it is referred to as pulsus paradoxus. Pulsus paradoxus has nothing to do with pulse rate or heart rate. The normal variation of blood pressure during breathing/respiration is a decline in blood pressure during inhalation and an increase during exhalation. Pulsus paradoxus is a sign that is indicative of several conditions, including cardiac tamponade, pericarditis, chronic sleep apnea, croup, and obstructive lung disease (e.g. asthma, COPD). The paradox in pulsus paradoxus is that, on clinical examination, one can detect beats on cardiac auscultation during inspiration that cannot be palpated at the radial pulse. It results from an accentuated decrease of the blood pressure, which leads to the (radial) pulse not being palpable and may be accompanied by an increase in the jugular venous pressure height (Kussmaul's sign). Treatment of the pulmonary embolism depends on the clinical presentation of the patient. When the patient is hemodynamically compromised, like blood pressure is less than 90 then we need to use the medications called as clot busters where we dissolve the clot which is formed in the pulmonary arteries so if the patient is hemodynamically stable, then we must use a parenteral anticoagulants like heparin, low molecular weight heparin or fondaparinux. So once the patients clinical condition gets better we shift the patients on oral anticoagulants or blood thinning medications. If the patients has an obstruction of only a small branch of the pulmonary artery, we might treat the patient on OPD basis, only on newer oral anticoagulant medications. Apart from this specific drug, we need to give some specific medication to support the lung. So if the patients oxygen content in the blood is less, we need to use an oxygen. Sometimes we need to use the ventilator to support the lung and once this period of acute treatment is completed, patient will be put on oral anticoagulant medication for minimum period of 3 months. In some patients we might put the patient on lifelong therapy of this oral anticoagulant to prevent the recurrence of pulmonary embolism and deep vein thrombosis.
