Hi, I'm Dr. Jared Nielsen. I'd like to talk today about the importance of a vitamin called thiamine or B1. B1 is very, very important for liver function, especially for people who have a history of alcoholism. It's quickly depleted in the system and is essential for that person to be repleted, if you will, their B1 vitamin. There are a lot of people that don't think, "I am really an alcoholic or I don't really have a drinking problem." What's interesting is they'll start to manifest some of the symptomatology of B1 deficiency, peripheral neuropathies, sensitivity or hyper‑sensitivity in the legs and feet. These are very commonly overlapped with diabetics. Diabetics also would get that type of symptomatology as the liver becomes congested or a fatty infiltrated liver. When we look at blood labs, a lot patients will actually say, when we see cholesterol levels going up and triglyceride levels matching or even exceeding that they don't drink alcohol. What we see a lot of times is that they're chronic soda pop abusers. Without naming specifically different brands or types of soda drinks, what we find is people who have a daily soda pop actually will start to also have fatty infiltrated livers and their thiamine levels can start to go down. See Text below for more information about Niacin and its effects on Lipids overall. Niacin (or Nicotinic Acid) is a form of vitamin B3. Niacin is one of the essential nutrients for the human body. Niacin deficiency can cause pellagra, while insufficient niacin in the diet can cause nausea, skin and mouth lesions, anemia, headaches, and tiredness. A recommended daily allowance of niacin would be 2–12 mg/day for children, 14 mg/day for women, 16 mg/day for men, and 18 mg/day for pregnant or breast-feeding women. Niacin in much higher doses than those recommended as daily dietary allowance is a potent agent against Dyslipidaemia. In doses as high as 1.500 - 2.000mg is proven to reduce LDL (bad) cholesterol by 20%, increase HDL (good) Cholesterol by 30%, and also reduce Triglycerides by almost 40%. It is also proven to reduce the atherogenic fraction of LDL: Lp(a), and apolipoprotein Apo-B (which is the carrier of lipids in LDL-C). Two large studies have been conducted with these doses of Niacin with patients under treatment with Statins. (AIM-HIGH & HPS2-THRIVE). These studies concluded that there is no added cardiovascular benefit by concomitant use of high doses of Niacin along with statin treatment, when LDL-C levels are already very low. It seems though that Niacin treatment is effective in patients not treated with statins. Niacin in such high doses can cause Flushing. Another form of vitamin B3 is Nicotinamide, but only Niacin has been found to have this strong effect on dyslipidaemia.

HDL is not cholesterol and neither is LDL. “High” and “low” refer to the relative proportion of protein to fat in the bundle. HDL stands for High Density Lipoprotein. LDL stands for Low Density Lipoprotein. (There are three other lipoproteins; chylomicrons, VLDL and IDL) Cholesterol and fat are not water-soluble so they need to be carried around the body in something to do their vital work. The carriers of such substances are called lipoproteins. (4) Lipoproteins: Lipoproteins travel round the bloodstream and act as transporters. So LDL and HDL are carriers of cholesterol, as well as triglycerides and phospholipids and proteins. LDL is the carrier of fresh cholesterol and HDL is the carrier of recycled cholesterol. VLDL, LDL, and HDL: The liver is central to the regulation of cholesterol levels in the body. Not only does it synthesize cholesterol for export to other cells, but it also removes cholesterol from the body by converting it to bile salts and putting it into the bile where it can be eliminated in the feces. Chylomicrons and VLDL deliver TAG to cells in the body. Two types of lipoproteins are triglyceride-rich: the chylomicrons and VLDL. Chylomicrons are synthesized by enterocytes from lipids absorbed in the small intestine. VLDL is synthesized in the liver. The function of these lipoproteins is to deliver energy-rich triacylglycerol (TAG) to cells in the body (pink pathway). TAG is stripped from chylomicrons and VLDL through the action of lipoprotein lipase, an enzyme that is found on the surface of endothelial cells. HDL is involved in reverse cholesterol transport. Excess cholesterol is eliminated from the body via the liver, which secretes cholesterol in bile or converts it to bile salts. The liver removes LDL and other lipoproteins from the circulation by receptor-mediated endocytosis. It travels in the circulation where it gathers cholesterol to form mature HDL, which then returns the cholesterol to the liver via various pathways. LDL delivers cholesterol to cells in the body. As VLDL particles are stripped of triacylglycerol, they become more dense. These particles are remodeled at the liver and transformed into LDL. The function of LDL is to deliver cholesterol to cells, where it is used in membranes, or for the synthesis of steroid hormones. Cells take up cholesterol by receptor-mediated endocytosis. LDL binds to a specific LDL receptor and is internalized in an endocytic vesicle. Receptors are recycled to the cell surface, while hydrolysis in an endolysosome releases cholesterol for use in the cell. LDL is what we call a carrier protein, and one of its important jobs is to carry a fundamentally important chemical to every cell in the body. This chemical is a critical component of cell membranes, serves as a brain antioxidant, and is the raw material from which your body manufactures vitamin D, cortisol, estrogen, progesterone, and testosterone. Study - The British Medical Journal (BMJ): The research team — comprised of experts from seven different countries — evaluated data collected from 19 studies on a total of 68,094 older adults. The team was seeking to determine if LDL cholesterol is associated with death in the older adults. According to the cholesterol hypothesis, it should directly relate. Major events in cholesterol metabolism include: - Dietary cholesterol ABSORBED in the intestine and carried in chylomicron to the liver. - The liver PACKAGES its cholesterol pool - a combination of endogenous and dietary - together with triglycerides, another type of lipid, into particles of VERY-LOW-density lipoprotein, VLDL. - VLDL travels in bloodstream to other organs. Maybe your doctor's telling you that you're doing something terribly wrong. Maybe he's telling you you need to get off that ketogenic diet because your cholesterol levels are high. Maybe you're just scared. Maybe you're just concerned as to why they're higher, or maybe you're trying to be proactive and trying to understand why they might get higher. In this video, I'm going to help you understand that higher levels of cholesterol on a ketogenic diet could very well mean a very, very good and rather cool thing. You see, we have to remember that we're on a ketogenic diet. Everything is surrounding fats. We're eating a lot of fats, we're metabolizing fats, we're going to have more lipids and we're going to have more cholesterols in the bloodstream, plain and simple. We have to remember that. What's interesting is that higher levels of cholesterol on a ketogenic diet are being linked and associated with weight loss. Believe it or not, once weight loss starts and fatty acid metabolism really kicks in, we see an increase in cholesterol. So it usually means that you're having an influx of weight loss occurring at that very point in time. You see, as fat starts to break down, cholesterol goes up, and it has to do with the fact that triglycerides are mobilized and they end up getting broken down into fuel but the cholesterol just kind of hangs out. The cholesterol doesn't really get burned for fuel. It's not metabolized like that. It's not energy. So what it does, it ends up sitting in the bloodstream, waiting to get recycled by the liver. It's not doing anything good or doing anything bad. It's just elevated and it's usually indicative of the fact that your body is using a little bit more fat for fuel at that very point in time. So you will find it's a temporary increase and once the weight loss starts to plateau a little bit and starts to slow down, your cholesterol levels will probably start to come down. You've probably seen an increase in both your HDL and your LDL, and it just has to do with the fact that both of them are mobilized. On that note, we have to understand HDL and LDL and the relationship with ketosis, and once you understand this, everything's going to make a lot more sense and I can almost guarantee you're going to come out of this video overall feeling a lot less stressed out about this situation. First off, HDL and LDL, they're not even cholesterols. They're transport vehicles. They deliver cholesterols. So you've got high density lipoprotein and low density lipoprotein. Low density lipoprotein, the "bad cholesterol" delivers nutrients. It delivers cholesterol, delivers fat and fat-soluble nutrients to the cell. Very important, but for some reason it gets a bad rap. Then we have HDL. HDL's job is to go around and pick up waste particles or used LDL, particles of LDL, and take them back to the liver for recycling. So somewhere along the lines, messages got going to confused. For some reason we think that LDL is bad because it's delivering fat, even though it's totally important. For some reason we think that purely recycling that fat is the only good thing that should be occurring. Though the question of whether it's calcium alone or vitamin D, or perhaps the combination of both, is responsible for improving cholesterol levels has been debated for many years, a recent study made by the NAMS board of trustees hoped to study the effects of Vitamin D, andby doing so, settle these questions once and for all. The women within the WHI trial were made to take either a supplement containing 1,000 mg of calcium and 400 IU of vitamin D3 or a placebo. This analysis looked at the relationship between taking supplements and levels of vitamin D and cholesterol in some 600 of the women who had both their cholesterol levels and their vitamin D levels measured. It was found out that the women who took the calcium and vitamin D supplements were twice as likely to have vitamin D levels of at least 30 ng/mL, which are the normal vitamin D levels according to the Institute of Medicine. This was as opposed to the women who took the placebo. Supplement users were also found to have lower levels of low-density lipoprotein, otherwise known as the "bad" cholesterol, that were between 4 and 5 points lower than those who took the placebo. In addition to these findings, the researchers discovered that among the supplement users, those with higher blood levels of vitamin D had higher levels of high-density lipoprotein, which is the "good" cholesterol, and lower levels of triglycerides, although for the triglycerides to be lower, blood levels of vitamin D had to reach a threshold of about 15 ng/mL and lower.
