Health Reform for Young Adults to Age 26

The new health reform law offers several benefits for young adults. This three-minute video outlines these benefits, such as allowing young adults to remain on their parents' health plan until they turn 26. Featuring Sara Collins, vice president for the Affordable Health Insurance Program at The Commonwealth Fund. This video is part of a series produced by the non-partisan Alliance for Health Reform in Washington, DC (allhealth.org). Our aim is to explain simply and in concrete terms the major provisions of the health reform law (the Patient Protection and Affordable Care Act of 2010). The series is supported by the Robert Wood Johnson Foundation. To suggest questions that you would like to have answered in this series, please send an email to BillErwin@allhealth.org SARA COLLINS: The reason young adults are at such high risk of being uninsured is they lose coverage at two key transition points -- both involving graduation. And the results are really consequential. A lot of young adults do have pre-existing conditions. They tend to be a healthier age group overall, obviously more healthy than older adults. But conditions like asthma, diabetes can make it really difficult for them to get affordable health insurance in the individual insurance market. And we also find that young adults who are uninsured have much higher rates of not getting needed care, because of the cost. Also having medical bill problems, not being able to pay their student loans. So going without coverage really does put them at risk, financial risk. Young adults, starting in September (2010), were able to come onto their parents' policies, if their parents had an employer-based policy that included dependent coverage. Young adults, whether they're married, whether they're not financially dependent, whether they're living in a different state, not living at home -- so all young adults up to the age of 26. The major provisions (of the health reform law) start in 2014 -- expansion of Medicaid to 133 percent of poverty, that's $14,000 a year for an individual, about $29,000 for a family of four. So this is a huge improvement in the coverage of young adults -- young adults who lost their (Medicaid) coverage when they turned 19, as children, are going to be able to stay on Medicaid if their family income allows them to do that. So a major improvement in coverage. About 7 million young adults who are currently uninsured would be eligible for Medicaid under the new expansion. For young adults who are in families with slightly higher incomes -- those earning between 133 percent of poverty and 400 percent of poverty, that's about $88,000 for a family of four, $44,000 for an individual -- they will be eligible to purchase a private health plan through these new state health insurance exchanges, with subsidized premiums and also cost-sharing subsidies to offset their out-of-pocket costs. Because of the loss of coverage at graduation, this law really is in many ways a graduation gift for young adults. So instead of graduating and losing their coverage, they're graduating and they'll have lots of options for insurance coverage to protect them, protect their families from catastrophic medical costs. USEFUL TERMS PREMIUM - The cost of health plan coverage, not including any required deductibles or copayments. The cost of the premium may be shared between employers or government purchasers and individuals. COST SHARING - Any out-of-pocket payment the patient makes for a portion of the costs of covered services. Deductibles, coinsurance, copayments and balance bills are types of cost sharing. Health Insurance Marketplaces in 2014: Behind the Numbers Friday, July 11, 2014 With the first open enrollment period for health insurance marketplaces now completed, an estimated 8 million people have enrolled in new private health insurance plans, with millions more newly enrolled in Medicaid. This briefing will look behind the enrollment numbers to take a detailed look at the demographics of marketplace enrollees, initial consumer experiences with health plans and lessons for next year's open enrollment period. What have we learned from the first open enrollment experience and what will it mean for 2015? How did the enrollment experience vary from state to state? How many previously uninsured Americans accessed coverage through the Marketplaces? What do people think of their new coverage? Has access to affordable coverage and care improved? Are ACA's procedures and safeguards for preventing inaccuracies in the enrollment and premium subsidy process working effectively? A panel of experts addressed these and related questions. Arnold Epstein, deputy assistant secretary, Office of Health Policy, ASPE, provided a federal perspective on enrollment in the new health insurance marketplaces. Linda Sheppard, special counsel and director of Health Policy and Analysis for the Kansas Insurance Department, discussed factors affecting the enrollment experience in their federally facilitated marketplace. Richard Onizuka, chief executive officer, Washington Health Benefit Exchange, offered lessons learned on Medicaid and private coverage enrollment through a state-based marketplace. Timothy Jost, professor, Washington and Lee University School of Law, and consumer representative to the National Association of Insurance Commissioners (NAIC) talked about implementation from a broad consumer perspective across a variety of states. Sub Headline: Understanding Your Plan Choice to Make Smarter Health Plan Decisions Synopsis: The health insurance industry is chock full of acronyms very few consumers actually understand. Too often, consumers are too focused on bottom line costs while not fully understanding network terminology, like PPO, HMO or HSA. This creates another obstacle for health insurance consumers—they don’t understand terminology or have enough information to differentiate plans and networks, so they purchase plans that don’t adequately meet their needs. Content: PPO, HMO, HSA…what does ANY of this actually mean? We all know the law requires us to have health insurance, but too often we, as consumers, make some of our most important decisions based on bottom-dollar costs without taking into consideration some of the most important factors. Often, we’re looking at whether or not a plan includes copays for services such as doctor visits and prescriptions, or how high the deductible is, while overlooking a vitally important piece of the puzzle—the network a plan utilizes. Typically, the lowest-cost plans utilize “HMO” networks. HMO stands for “Health Maintenance Organization.” Generally speaking, HMO’s will offer more restricted access to the healthcare system. Quite often, you’ll find many of the top-rated hospitals in your area do not accept HMO plans, or accept very few. Your list of “in-network” physicians will also generally be significantly smaller than you typically see with PPO plans (which we’ll touch on here as well). With an HMO, you must designate a “gatekeeper” physician to be your point of contact for all your healthcare needs. If you require medical attention for something beyond the scope of your gatekeeper’s area of expertise, you must get their referral in order to see a specialist or any other healthcare provider. HMO plans, as a rule of thumb, do not provide any coverage for services provided by a healthcare provider outside of the HMO network. The importance of this fact cannot be understated. PPO’s are inherently different. PPO stands for “Preferred Provider Organization” and generally does provide coverage for services received by providers outside of the PPO network. While the cost of services provided by a provider outside of the PPO network is often much higher, your access to hospitals, doctors and other facilities is typically much broader, giving someone who is insured under a PPO more freedom to choose their providers. These are the two types of network arrangements we see in the health insurance market, and then, from them, we have different plan types, such as HSA Qualified High Deductible Health Plans, or “HDHP’s”. These HSA qualified HDHP’s can utilize either an HMO network or a PPO network. In order to be deemed as a “qualified” plan, the health insurance plan cannot not have copays for doctor visits, prescriptions or anything else prior to the deductible being met. In other words, if you go to the doctor for an illness, and the cost of whatever that doctor did to treat you was $200, then $200 is what you’ll have to pay. However, the benefit of a qualified HDHP with an HSA is the HSA itself. HSA stands for “Health Savings Account.” Think of it as a bank account you’re allowed to have in conjunction with a qualified HDHP, in which you, your employer or both of you may contribute money to, up to an IRS-specified cap. Any amount you personally contribute to this account (in other words, not your employer) becomes tax deductible on your personal income tax return the following year, and more importantly, as long as you use this money for qualified medical expenses, you never pay taxes on the money you contribute to the HSA.
