Gestational Diabetes is one of the most common complications of pregnancy, it is defined as altered blood sugars which is developed during pregnancy and they usually correct itself after the delivery. Prevention of GDM is very important to avoid future maternal and fettle adverse outcomes. Generally the high risk patients are those who had GDM in previous pregnancies, elderly pregnant women, PCOS, obese and people with strong family history. Usually in a planned pregnancy, pre pregnancy control few steps can be taken to avoid GDM in future that usually maintaining an ideal BMI by regular exercise, diet and losing weight. During pregnancy usually the GDM develops in second or third trimester though it can also start from the beginning itself. The fine tuning of diet and exercise is required to avoid GDM. It can be ideally done by maintaining ideal weight during pregnancy that is no weight gain about 6 months of pregnancy or total weight gain till 9 months about 4-5 kg is enough. Also high risk patient should go for regular exercises after consulting the doctor, 30 minutes walk per day on most of the days of a week, antenatal exercises as advised by the doctor, they can go for a swim which is good exercise during the pregnancy. Apart from that the diet should be controlled from the very beginning, no extra sugar, white sugar should be completely avoided, rice, high fiber diet should be taken from the beginning as it absorbs extra sugar from the body, it should be low carbohydrate and low fat diet. With this tuning probably the Gestational Diabetes can be put at hold, also regular checkups, good follow up with doctor, sugars should be checked at the first visit at about 6 months and almost every month at high risk case.

Let us have a brief look at what really causes diabetes in pregnant woman: Pregnant woman do not really need to worry about their blood sugar levels rising because it usually reaches normal levels during the course of time. One major cause is the fact that the placenta produces more insulin than normally required by the body, and results in hormones getting blocked. Gestational diabetes occurs in women generally during the last semester of the pregnancy. A healthy diet for gestational diabetes is also very important to be followed. Pregnancy is one such phase in a women’s life which she always looks forward to. Risk factors for gestational diabetes: 1. Obesity: Women who are overweight are under a greater risk of getting gestational diabetes. Therefore it is important to keep a check on your weight during pregnancy. 2. High blood pressure: Pregnancy tends to increase the blood pressure levels in women. In order to avoid such a situation, it is always advisable to do some yoga or meditation to keep the mind calm. Some basic precautions to be taken during pregnancy are: Healthy diet plan:  It is important to follow a similar diet as a diabetic patient. Once the baby is born, the mother can go back to her normal eating habits since all the sugar parameters come back to normal levels. Food to be consumed by the patient is as follows- low fat skimmed milk, drumsticks, spinach, fenugreek etc.  Food that should be avoided is- Potatoes, beetroots, bananas, mangoes, grapes and custard apples. Exercising:  This is another way to maintain normal sugar levels in the body. Exercising like taking small walks or yoga can help you control the glucose levels in the body. A check on your weight: weight management is quite a task for many mothers to be! Controlling your weight can go a long way in keeping your sugar levels well under control. It can be the happiest time of a woman’s life---but doctors say that joy can be overshadowed by a common medical condition. “It’s very common, unfortunately. As a society we are getting sicker, we are getting obese, and diabetes is on the rise,” said Dr. Ravi Chokshi, an OB/GYN with Lee Health. Diabetes becomes even more concerning when it’s developed during pregnancy. “There are two types of diabetes during pregnancy that we worry about. One is you’re a diabetic before you became pregnant, and that is a very serious problem that we try to get control of. The other is you’re not a diabetic. You had normal blood testing before. What happens in pregnancy is the placenta that the baby relies on releases hormones that increase your blood sugar,” said Dr. Chokshi. If the mother becomes diabetic during pregnancy, both the mother and baby are at risk for health concerns. “It actually affects the development of the baby, and it can cause heart defects and brain defects if the sugars aren’t brought under control quickly,” he said. These concerns are why doctors are screening women for diabetes throughout their pregnancy. “We are actually instituting a policy where almost every patient is going to get some blood sugar test at the start of pregnancy to make sure we’re not missing diabetics who didn’t know they were diabetic,” said Dr. Chokshi. If a woman is diabetic or becomes diabetic during pregnancy, doctors will help them with diet and medication, if necessary to get their blood sugar under control. “If you have diabetes during pregnancy and we don’t treat it adequately, you’re at a two to three time increased the risk for C-section; you’re at a three to four time increased risk for birth injury,” he explained. The goal is to catch diabetes early during pregnancy to prevent health concerns for both the mother and baby—both during and after delivery. What do I need to know about gestational diabetes?
All pregnant women should be screened for gestational diabetes around 28 weeks gestation. And it's important to test for this because if a woman has gestational diabetes or if she had previously diagnosed type 1 or type 2 diabetes, if uncontrolled, it can cause complications like preeclampsia, blood sugar problems for the baby after delivery, stillbirth, and an abnormally large baby, which has complications associated with that too. 5% to 18% of women develop gestational diabetes, and it all goes back to insulin resistance. Insulin is the key that unlocks your body's cells so that sugar can go inside and make the cells work. And your body naturally becomes a little more insulin-resistant during pregnancy so that there's enough sugar circulating in your blood so the baby gets what it needs. Normally, mothers can compensate for this by producing more insulin, but mothers who have gestational diabetes aren't able to make more insulin, and so blood sugar levels rise. So testing, like I said, for this is done are 28 weeks, and what will happen is the doctor will have you drink a very sugary drink. It contains 50 grams of glucose. And then they'll test your blood sugar an hour later and see how your body responded. If you get a value of 130 to 140, then you may have gestational diabetes, and the doctor will want to do a follow-up test. If the value you get on your first test is 200 or higher, then you just have gestational diabetes, but they may still want to do follow-up testing. And in both cases, it's doing a 3 hour glucose tolerance test. This is done when you're fasting, and then you drink double the amount of sugar. It's a 100 gram glucose sugar drink. And then they test your blood sugar every hour for 3 hours and see what it is. And then, based on those values, they determine if you truly have gestational diabetes or not. If the diagnosis is made, then there's a couple of different ways to manage it. And it's super important to keep your blood sugars under control so that you don't have the complications I discussed before. The management plan is going to be dependent on how severe your case is. And for a lot of women, it just involves some diet changes and exercise. For other women, it involves taking oral medication. And for about 15% of women with gestational diabetes, it has to be controlled with insulin. And you may start out just needing to make a few diet changes, and then as the pregnancy progresses, so do your body's needs, and so do your baby's needs, and so that may change the plan as well. So you could start out being diet-controlled, and then you may end up with oral medication or with insulin. So this speaks of the importance of consistent monitoring. A diabetic educator will meet with you and talk to you about your treatment plan and also tell you how often you should be monitoring your blood sugars. They'll take a peek at your values each week and, based on that, determine if additional changes in your treatment plan are necessary. Ellen Seely, MD, Director of Clinical Research in the Division of Endocrinology, Diabetes and Hypertension at Brigham and Women’s Hospital discusses the health risks associated with gestational diabetes mellitus (GDM) during and after pregnancy. Dr. Seely also describes Balance After Baby, a web-based program designed to educate women on eating healthfully and increasing their physical activity, with the goal of helping them lose their pregnancy weight within a year of giving birth. GDM is a type of diabetes that occurs only in pregnancy. It comes on in the second half of pregnancy and it goes away after delivery. In the US it’s estimated that five percent of all pregnancies are complicated by GDM. The rates of GDM are higher in Hispanic and non-white populations, ranging from ten to 20 percent of all pregnancies. Obesity is one of the main risk factors for GDM. In the United States, most health centers screen all women for gestational diabetes because obesity is becoming so common in the overall population. Gestational diabetes can impact the health of the mother and the fetus, as well as the newborn. Women with GDM have an increased risk of preeclampsia, which is a type of high blood pressure that develops during pregnancy. Preeclampsia is a serious condition that can lead to early delivery. Women who have gestational diabetes may also have larger babies, increasing their risk of cesarean section. While GDM goes away after pregnancy, the health risks persist. Research has found that 50 to 70 percent of women who have gestational diabetes may develop Type 2 diabetes later in life. Research has also found that babies born to mothers with GDM, have a higher birth weight and may become obese during adolescence, increasing their risk of developing diabetes and other conditions such as high blood pressure. The Pregnancy and Diabetes Program at Brigham and Women’s Hospital offers multidisciplinary care to women with GDM. The program includes a high risk obstetrician, two diabetes specialists, a nutritionist, and a nurse practitioner who is certified in diabetes education. The first approach to treatment is to control blood glucose levels with healthy eating and physical activity. Lifestyle changes, healthy eating and physical activity, are effective in controlling blood glucose levels about 75 percent of women with GDM. If the lifestyle changes don't work, insulin therapy is started to control a mother’s blood sugar for the rest of her pregnancy. At 28 weeks pregnant I was diagnosed with gestational diabetes. I am now 32 weeks and having to inject myself with insulin. Here's my story and how I am dealing with it. Update: I injected myself every night with the same dosage and at the same time every night until my water broke August 5th at 5:30 a.m. As soon as my placenta was delivered the gestational diabetes went away and I was back to the way it was before. I even had a burger and 🍟 after giving birth with a regular coke (a craving but not regular habit). Preventing Gestational Diabetes
From doctor’s appointments to blood tests, during pregnancy doctors make sure mothers are screened for anything that could cause health concerns to the mother and baby. “There is undiagnosed diabetes in the general population that gets picked up in pregnancy,” said Dr. Ravi Chokshi, an OB/GYN for Lee Health. But the problem can be treated with diet changes and medication, if necessary. “There’s a misconception that I’m pregnant; I’m allowed to eat as much as I want. I‘m eating for two, and that’s not really true. The American of Congress of OBGYN actually recommends you go up by 300 calories by your second and third trimester. Most of us already overeat by 200 and 300 calories a day,” he explained. Pregnant mothers are screened early to make sure their blood sugars are normal. “The recommendations are we screen at least everybody with a blood sugar very early in pregnancy. After that, we screen everybody after 24 weeks. The reason for that is the further pregnancy gets, the placenta gets bigger, and the hormones cause increased blood sugars,” said Dr. Chokshi. If a mother is diabetic during pregnancy, her blood sugars are uncontrolled—that blood sugar goes to the baby, causing the baby to create more insulin. “Babies are not only bigger; they grow differently. Their bellies are bigger than their heads. Their shoulders are bigger than their heads,” he said. Mothers who are diabetic during pregnancy are also at a higher risk of needing a C-section and developing diabetes later on in life. “If we can get your blood sugars in good control, studies show that you are going to have the same kind of pregnancy that non-diabetic moms have,” said Dr. Chokshi. Eating a healthy diet, avoiding processed foods, and eliminating sugar can help pregnant mothers keep their blood sugar under control. If I have gestational diabetes, how might it affect my baby, and will he be bigger than average?
Moms with diabetes, whether it be Type I, Type II, or gestational diabetes are more likely to cook large babies. It's because if your blood sugars aren't well controlled during your pregnancy you have a large amount of blood sugar circulating and this is getting to the baby as well. So they can grow extra large from having that extra sugar. It causes complications after delivery because their body is normal; they are producing insulin which is like a key that unlocks cells to put the blood sugar into the cell. Their body's response to this large amount of sugar is to produce a lot of insulin. This is great and what their body should do while they're inside of you. But once they're born and not getting all of that sugar, they still have a high level of insulin to compensate for that and it causes their blood sugar to rapidly drop. A lot of babies born to mothers with diabetes struggle with their own blood sugar after delivery. So the people who are taking care of your newborn will likely be in tune with this and be watching your newborn for signs of complications. But as for how big your baby is likely to be, who knows? It's hard to say. The chances of your baby being large are reduced by you controlling your blood sugars well during the pregnancy. So if your doctor has recommended a certain diet make sure you follow that and if your doctor has recommended medication to control your blood sugars, make sure you're taking that as prescribed and checking your blood sugars as frequently as your doctor has suggested as well. It's likely that in your third trimester you'll go to see some specialists that will watch you and the baby for signs of complications from diabetes. Usually your doctor will have you go to a specialist called a perinatologist to make sure that your baby is growing well and that the pregnancy overall is going well. They'll probably also have you talk with diabetic educators that will have you keep a log of your blood sugars and have you come in and discuss any complications or issues you're having with them. Your doctor is also likely to have you get frequent non-stress tests where you come in and they put your baby on the monitor, meaning they watch your baby's heart rate and your contraction pattern, for a short period of time to make sure your baby has enough oxygen and is doing well. Gestational diabetes is a condition where a pregnant lady will have high blood sugars between 24 to 28 weeks of gestation. When they have this problem it is better to consult a gynecologist and she get the consent from the concerned doctor to reduce the blood sugar levels and to reduce the maternal and t eh fetal complications. So a woman who has Gestational diabetes better to meet a dietician to get a diet schedule, how to control her diet and how to control her blood sugar by managing a proper diet during a routine food habits. That is a way to control diabetes during pregnancy. Second is blood glucose levels are equally important to maintain the pregnancy also. She cannot cut down completely the sugar intake which leads to lot of problems to the mother and the ongoing pregnancy. To avoid these complication, she has to have proper balanced diet where the carbohydrates is very minimal and carbohydrates should be the required quantity and it should be useful for the ongoing pregnancy. So you should have a diet counseling that is very essential. Along with this the pregnancy will be having its complications, you should reduce the blood sugar levels. The main pathophysiology in diabetic patients is there is a gland called as pancreas, which produces insulin which controls the blood sugar levels. Naturally the insulin will be quite low in those conditions where we need to supplement the insulin to control the blood sugar levels. So periodically she will have to undergo the periodic checkup of the blood sugar levels. She can do it at home every alternate days if possible so that if there are any variations in the blood sugar level, she can immediately consult a doctor and bring to down to normal. So if there is high blood sugar levels, it may lead to fetal complications like the baby may grow more bigger. That we call it as macrosomia of the fetal growth. So to avoid those conditions, maintain a proper blood sugar levels, maintain a proper diet schedule, and along with that follow proper physical activities like pregnancy like stretching exercises of 15 to 20 minutes walk everyday which keeps the body system perfect, It will burn some amount of calories also. We advise such women to take a frequent small mean which contains less amount of carbohydrates and with due care the process of pregnancy will go comfortably without having any maternal or any fetal complications. So pregnancy is a condition where he woman has to enjoy the situation and we have to go accordingly followed by the advise given by the concerned consultant.

And being healthy is a priority in this family. But daughter Angie fell into a slump at the beginning of her third pregnancy. She stopped exercising, began eating too much junk food and was three weeks larger than she should have been. Then her doctor told her she was at risk for gestational diabetes and he suggested medication. At 29 years old Angie didn't want that to be her solution. Her mother Therese Musyznski took action and dragged her out for a power walk, threw in some push ups and squats and got her) excited about fitness again. Plus she sent her the TurboFire workout DVDs, which Angie loved. In fact her kids would watch and cheer her on! At her next doctors appointment Angie had actually lost 12 pounds, her sugar levels were back to normal and she was right on track with her pregnancy. She felt amazing. learned from her mother that taking care of your body is a big responsibility and something you spend a lifetime doing. She is thankful to have such a positive role model.

Gestational Diabetes is one of the most common conditions that can happen during pregnancy. But what happens when you have it, and how can diet and exercise help? What Happens in Gestational Diabetes? is the second in a series of videos from the HRB Mother and Baby Clinical Trial Network Ireland, explaining what happens in some common pregnancy and newborn health problems, and how research is trying to answer the questions we still have. Our second Curious Parents video looks at Gestational Diabetes, a condition that affects up to 12% of pregnancies in Ireland. When you have gestational diabetes, your pancreas is not able to produce enough insulin to help convert sugar into energy. As a result, too much sugar is left in the blood, and left unchecked, this can have serious short term and long term effects for mum and baby. Dropping baby weight between pregnancies can decrease gestational diabetes risk
CINCINNATI (WKRC) - There is new research that could make a difference if you are having a baby. If you are pregnant, or planning on having a baby sometime soon, the new guidelines say to lose the baby weight between pregnancies. A report released Wednesday from the Cleveland Clinic says one in seven births are impacted by gestational diabetes. That's a a condition that causes high blood sugar during pregnancy. Now, recent research shows not shedding enough weight between pregnancies can increase a woman's risk of developing gestational diabetes. The study looked at more than 24,000 mothers and found that women who gained as little as six pounds between pregnancies have an increased risk. The risk is even greater if the mother had a normal weight or body mass index with her first pregnancy. The authors of this report say breastfeeding and extra exercise are the best ways to drop the weight in-between babies.
