This is my experience with Ziprasidone, generic for Geodon and akathisia. Akathisia is a type of dystonia or extrapyramidal symptom. It is terrible. I wanted to do a follow-up Geodon review. Geodon is the name brand of ziprasidone. I want to cover Geodon side effects aka ziprasidone side effects. I took it for my bipolar disorder since other bipolar medication didn't seem to help stop the shadow people or audio hallucinations. Watch more How to Understand Schizophrenia videos: http://www.howcast.com/videos/512799-Side-Effects-of-Antipsychotic-Drugs-Schizophrenia Anti-psychotic medications are very useful and have helped many people with schizophrenia to lead healthy and productive lives by taking care of some of the symptoms, including delusions, hallucinations, disorganized thinking, disorganized behavior, and negative symptoms, or lack of motivation that may come with schizophrenia. However, they don't come without side effects. I'm going to talk with you about four different categories of side effects that seem to be very common. One is what we call extrapyramidal symptoms. Those are motor or movement side effects of these medications. The older anti-psychotics tend to have more of these side effects. They include feeling stiff in your muscles, having tremor in your hands or in your body, feeling restless and unable to sit still, or having what we call tardive dyskynesia. That occurs over time, where you might have movements of your mouth or of your limbs that are repetitive and that appear after a period of treatment. Those are the extrapyramidal symptoms, or EPS. They're more common with the older medications. With the newer medications, the more common side effects tend to be what we call the metabolic side effects, or cardiometabolic side effects. Those include weight gain, an increased blood sugar or risk for diabetes, and an increased cholesterol. Initially, we were really excited about all of the new anti-psychotic medications. Then, when we saw how people gained weight and got more heart disease, partly related to the medications, we started to see that there was a big risk there, because really, it's heart disease and diabetes that tend to be the things that lead to early mortality in people with schizophrenia. So, that's the second category of side effects. There's also anticholinergic side effects. Those include dry mouth, feeling flushed or hot, having constipation. Those are things that tend to happen with the anticholinergic side effects. Different medications, both old and new, have more or less anticholinergic side effects. One last category that I'll mention is prolactin. Prolactin is a body hormone that you have. For some of our anti-psychotic medications, in particular Risperidone, is probably the biggest culprit, increase prolactin production. What happens, then, is that women especially may not have their periods, they may have difficulty conceiving a child. And, for some people, there's breast enlargement and even lactation, both men and women. That can be a very disruptive side effect, obviously, and needs to be addressed. If you have side effects with anti-psychotic medications, definitely, there's many choices of anti-psychotic medication, and so you should speak with your doctor or psychiatrist about it, because they can try different things to see what works best for you and causes the least burden of side effects to you. Many people have no side effects at all from anti-psychotic medications, thank goodness. If you have schizophrenia and you're thinking about medications, find a treatment provider you trust so you can work on finding the right medication for you.

A study was published in 2006 in JAMA [Journal of the American Medical Association] on these three drugs. The subjects were elderly people [in nursing homes], and over half of them, 54%, were dead within 12 days of starting this study. These drugs can cause nerve damage, weight gain, pounding heart, temporary paralysis. These drugs are not needed to sleep. There are much better ways to sleep. This was noted by Joan Mathews Larson, PhD in a speech she gave in September 2007. SKIP AHEAD: 1:01 – Antipsychotic Mechanism 1:58 – Antipsychotics and their Indications 3:30 – Neuroleptic Malignant Syndrome (Typical Antipsychotic Side Effect) 4:18 – Extrapyramidal Symptoms (Typical Antipsychotic Side Effect) 6:19 – Atypical Antipsychotics and their side effects 8:57 – Mood Stabilizers 12:58 – Anxiolytics and Benzodiazepines We will start with a quick review of some material from my previous video on psychosis. Symptoms of schizophrenia can be broken down into 2 categories, Positive and Negative Symptoms. Positive symptoms include behaviors or sensations that are not normally present. Examples include hallucinations, delusions, and catatonia. These symptoms are thought to be related to an excess of dopamine. I remember this by remembering that “doPamine has a P in it”. So P for Positive and P for Dopamine. Negative symptoms are the absence of normal behavior. Examples include a lack of initiative, diminished speech, disheveled appearance & flat affect. These symptoms are thought to be related to an excess of serotonin. As we will see antipsychotics affect dopamine and serotonin to varying levels. The indications for this class of drugs includes psychosis (mainly schizophrenia), Mania (mainly bipolar disorder), aggression and Tourette's disease. Typical Antipsychotics primarily block dopamine receptors in a non-specific manor. Therefore, these drugs work best for positive symptoms, and have little effect on negative symptoms. The non-specific mechanism of the drug also means there are lots of side effects. Some of these medications come in a slow release injectable form so they can be used in non-compliant and aggressive patients. There are a lot of high yield side effects so we will break them down one by one Neuroleptic Malignant Syndrome (or NMS) is a rare but potentially fatal adverse reaction of typical antipsychotics. It involves fever, altered mental status, rigidity and autonomic instability (such as tachycardia, hypertension, diaphoresis etc.). You may also see elevated myoglobin in blood or urine and elevated Creatine Kinase (CK). One of the ways I think about it is that it looks kinda sorta like Serotinin Syndrome that you can see with antidepressatns. If you see this you have to emergently stop the medication, provide supportive care and consider adding Dantrolene Extrapyramidal Symptoms (or EPS) are due to blockage of Nigrostriatal dopamine. It can present with a number of different symptoms. Akasthisia is a general sensation of restlessness Acute Dystonia is involuntary continuous muscle contractions often of the neck. Another common presentation of acute dystonia is Oculogyris Crisis when your eyes get locked looking upward and you have to lean over to see Dyskinesia (AKA Pseudoparkinsonism) presents like Parkinson’s Disease with symptoms like a pill rolling tremor, cogwheel rigidity & bradykinesia (or slow movement) Tardine Dyskinesia (or TD) is uncontrollable facial tics, grimacing & tongue movements As scary as these symptoms may look, they are generally not medical emergencies. In most cases you will continue to use the drug with perhaps a reduction in the dose or the addition of an anticholinergic mediation like Benzatropine or Diphenhydramine. Tardive Dyskinesia is the exception and requires cessation of the medication as it can be permanent. Usually you would switch a patient with TD to a 2nd gen antipsychotic. Hyperprolactinemia is a side effect due to Blockage of Tuberoinfundibular dopamine. It presents just like any other disease that increases prolactin.
