R.A.P.N. in a kidney with multiple renal arteries... Drs. Patil / Gautam

Full Title: Robotic partial nephrectomy in a kidney with multiple renal arteries - Adrenaline rush in a renal surgery! Abstract: Saurabh Patil, Ashwin Tamhankar, Surya Prakash Ojha, Puneet Ahluwalia, Gagan Gautam. Dept of Uro-Oncology, Max Institute of Cancer Care, Max Super Speciality Hospital, Saket, New Delhi. Introduction: Partial nephrectomy has become the standard of care as a nephron-sparing procedure in properly selected patients with a Stage T1 tumor. With the advent of minimally invasive surgery, robotic partial nephrectomy has become a better alternative to open surgery for small renal masses with the advantage of faster recovery. Though routinely performed, robotic partial nephrectomy is technically demanding and may prove challenging in certain situations like multiple renal vessels. We hereby present our experience of a robotic partial nephrectomy case for a small renal mass in a kidney with multiple renal arteries. Case: 44 years old gentleman presented with an incidentally detected right renal mass. His clinical examination was unremarkable and all routine laboratory parameters were within normal limits. On preoperative DTPA scan, glomerular filtration rate(GFR) of right kidney was 42.1 ml/min, with a global GFR of 79.4. Contrast CT of abdomen showed a well-defined heterogeneously enhancing mass of size 49x37x44mm arising from upper pole of right kidney. Metastatic workup was negative. We did robotic partial nephrectomy using da Vinci Xi system. Intraoperatively, we found multiple renal arterial vessels, which could be managed with loop tourniquet and bulldog clamps. Results: Total console was time 3 hours 30minutes and warm ischemia 29min 10 sec. Estimated blood loss was 200 ml. Post-operative recovery was uneventful with a total 2 days of hospital stay. Histo-pathological examination showed Clear cell RCC, Fuhrman’s grade 2, with all resection margins negative. Conclusion: RAPN represents a safe and effective nephron-sparing surgery technique even for a kidney with multiple renal vessels. Innovation in the form of using vessel loop to take control of multiple vessels can be utilised. Whenever possible, CT renal angiogram must be included in preop imaging workup. This was a Finalist in the Robotic Surgeons Council of India KS Robotic Surgery Awards for May, 2018. Renal cell carcinoma

Renal cell carcinoma (RCC, also known as hypernephroma, Grawitz tumor, renal adenocarcinoma) is a kidney cancer that originates in the lining of the proximal convoluted tubule, a part of the very small tubes in the kidney that transport waste molecules from the blood to the urine. RCC is the most common type of kidney cancer in adults, responsible for approximately 90-95% of cases. It has been described as one of the deadliest of cancers affecting the genitourinary tract. Initial treatment is most commonly either partial or complete removal of the affected kidney(s) and remains the mainstay of curative treatment. Where the cancer has not metastasised (spread to other organs) or burrowed deeper into the tissues of the kidney the 5-year survival rate is 60-70%, but this is lowered considerably when the cancer has spread. It is relatively resistant to radiation therapy and chemotherapy, although some cases respond to targeted therapies such as sunitinib, temsirolimus, bevacizumab, interferon alfa and sorafenib which have improved the outlook for RCC. The body is remarkably good at hiding the symptoms and as a result people with RCC often have advanced disease by the time it is discovered. The initial symptoms of RCC often include: blood in the urine (occurring in 40% of affected persons at the time they first seek medical attention), flank pain (40%), a mass in the abdomen or flank (25%), weight loss (33%), fever (20%), high blood pressure (20%), night sweats and generally feeling unwell. RCC is also associated with a number of paraneoplastic syndromes (PNS) which are conditions caused by either the hormones produced by the tumour or by the body's attack on the tumour and are present in about 20% of those with RCC. These syndromes most commonly affect tissues which have not been invaded by the cancer. The most common PNSs seen in people with RCC are: anaemia (due to an underproduction of the hormone, erythropoietin), high blood calcium levels, polycythaemia (the opposite to anaemia, due to an overproduction of erythropoietin), thrombocytosis (too many platelets in the blood, leading to an increased tendency for blood clots and bleeds) and secondary amyloidosis. When RCC metastasises it most commonly spreads to the lymph nodes, lungs, liver, adrenal glands, brain or bones. This video is targeted to blind users. Dr. Berg does not to do the ketogenic diet without lots of vegetables. You need 7-10 cups of vegetables per day. This will keep the liver clean. The ketogenic diet can cause a massive amount of fat being released through the body and through the liver. So veggies are necessary. Also - do not use vegetables as a calculation into your 20-50 grams of carbs per day. Vegetables are extra grams that should be above the 20-50 grams of carbs per day. Hi guys, in this video we are going to answer the question. Does the ketogenic diet cause a fatty liver. You are probably already know that being an alcoholic can cause a fatty liver, consuming a refined carbs and sugar but did you know that going into a ketogenic diet can also cause a fatty liver? We are going to use guidance physiology on page 927 and this is what it says, effect of insulin lack(means low insulin or lacking too much insulin) on causing a fatty liver. Strange as it may seem, though insulin lack causes dissolution(means dissolving) of the fat stores in adipose tissue, it causes a great increase in the amount of stored triglycerides in the liver, leading to a very fatty liver. I never in any of my videos recommend just the straight ketogenic diet, I always recommended adding a bunch of vegetables to the ketogenic diet because let’s take a look at what a ketogenic diet is low carbs below 20 to 50g, it doesn’t specifically tell you what carbs you can consume, the piece of bread could be a 20g of carbs, an apple or berries so it doesn’t talk about the vegetable content. What you want to do is you would want to beat up your vegetables, you want to do at least 7 to 10 cups of vegetables not just from your potassium and your nutrients but to keep that liver flushed out. We are not gonna calculate vegetables into this calculation of 20 to 50 grams. It is unlimited vegetables and don’t add the vegetables into your calculations. Thanks for watching! The epidemic of chronic disease and understanding epigenetics | Kent Thornburg | TEDxPortland

16:49
When will we decide to conquer chronic disease? The moment we realize the future’s not only in our genes but also in our hands with the choices we make regarding food and nutrition. We now know how we grow before we are born, matters. A woman who is pregnant with a daughter is nourishing the egg that may someday become her grandchild. We must remove the vulnerability for developing chronic disease before it starts. Are Organic Foods More Nutritious?

There are appear to be no consistent differences in the level of vitamins and minerals in organic versus conventionally grown produce, but organic fruits and vegetables have more phenolic phytonutrients. This is the first of a 5-part video series summarizing the best available science comparing the nutritional content, pesticide risk, heavy metal toxicity, and food poisoning risk of organic versus conventionally raised foods, including a video on practical tips for making your own DIY fruit and veggie wash. Here’s a list of the next four queued up: • Are Organic Foods Safer? More Than an Apple a Day: Preventing Our Most Common Diseases

01:02:03
This is Dr. Greger's 2013 live presentation. Dr. Greger has scoured the world's scholarly literature on clinical nutrition and developed this brand-new live presentation on the latest in cutting-edge research on how a healthy diet can affect some of our most common medical conditions. Subscribe to Dr. Greger’s free nutrition newsletter at https://www.nutritionfacts.org/subscribe and get a free recipe from his new HOW NOT TO DIE COOKBOOK. (All proceeds Dr. Greger receives from the sales of his books, DVDs, and speaking engagements go to support the 501c3 nonprofit that runs NutritionFacts.org.) In my annual nutrition review last year, Uprooting the Leading Causes of Death (http://nutritionfacts.org/video/uprooting-the-leading-causes-of-death/), I explored the role diet may play in preventing, arresting, and even reversing our top 15 killers. Actually, if you recall, the top 16. Since side effects from prescription drugs kill an estimated 100,000 Americans a year, the sixth leading cause of death may actually be doctors. And that's just from adverse drug reactions. Add in medical mistakes (which the Institute of Medicine estimates kills at least 44,000 Americans) and that brings "health"care up to our country's third leading cause of death. Throw in hospital-acquired infections, and we're talking maybe 187,000 Americans dead every year (and millions injured) by medical care. The best way to avoid the adverse effects of medical and surgical tests and treatments is not to avoid doctors, but to avoid getting sick in the first place. This year I thought I'd run through the top dozen reasons people visit their doctors to highlight some of the latest research in hopes of moving my colleagues and me lower down the list of common killers. So you can more easily navigate through the menu of diseases I cover, this presentation is also available on DVD through my website (http://www.drgreger.org/DVDs) or Amazon (http://www.amazon.com/gp/node/index.html/?ie=UTF8&camp=1789&creative=390957&linkCode=ur2&me=A1QZTUS5K8MU75&tag=httpwwwdrgreg-20). Health benefits of Turmeric Tea and How to make it.

06:07
Isn't it amazing how the health of our family members can trigger us into action! So was the case with this Turmeric tea recipe. My father was recently diagnosed with osteoarthritis, a condition that was causing him a great deal of pain and for a man who was exceptionally active, the pain began to slow him down. Pain killlers weren't helping, in fact the side effects from the pain killers were worst than the arthritis! So we set about finding him a diet that he could have that would help with his condition and this tea ticked all the boxes. A natural anti-inflammatory and natural pain killer, my Dad is LOVING this tea as its tastes like chai and he uses it as a replacement for coffee and black tea. With many many other health benefits, this tea is an absolute must have for your medicine cupboard. Ingredients: 3 heaped teaspoons turmeric 1 heaped teaspoon ground cloves 2 teaspoons ground cardamon 2 teaspoons ground ginger 1.5 Lts of 3.5 pints of filtered water Honey to taste nut milk optional ------------- SUBSCRIBE here and let's connect over there too: Website: http://www.theinternetchef.biz/ Google+: http://goo.gl/G1XbR Twitter: https://twitter.com/bridget_cooks Facebook: https://www.facebook.com/TheInternetChef -~-~~-~~~-~~-~- WATCH NOW! Calling All Cars: Body on the Promenade Deck / The Missing Guns / The Man with Iron Pipes

01:28:24
The radio show Calling All Cars hired LAPD radio dispacher Jesse Rosenquist to be the voice of the dispatcher. Rosenquist was already famous because home radios could tune into early police radio frequencies. As the first police radio dispatcher presented to the public ear, his was the voice that actors went to when called upon for a radio dispatcher role. The iconic television series Dragnet, with LAPD Detective Joe Friday as the primary character, was the first major media representation of the department. Real LAPD operations inspired Jack Webb to create the series and close cooperation with department officers let him make it as realistic as possible, including authentic police equipment and sound recording on-site at the police station. Due to Dragnet's popularity, LAPD Chief Parker "became, after J. Edgar Hoover, the most well known and respected law enforcement official in the nation". In the 1960s, when the LAPD under Chief Thomas Reddin expanded its community relations division and began efforts to reach out to the African-American community, Dragnet followed suit with more emphasis on internal affairs and community policing than solving crimes, the show's previous mainstay. Several prominent representations of the LAPD and its officers in television and film include Adam-12, Blue Streak, Blue Thunder, Boomtown, The Closer, Colors, Crash, Columbo, Dark Blue, Die Hard, End of Watch, Heat, Hollywood Homicide, Hunter, Internal Affairs, Jackie Brown, L.A. Confidential, Lakeview Terrace, Law & Order: Los Angeles, Life, Numb3rs, The Shield, Southland, Speed, Street Kings, SWAT, Training Day and the Lethal Weapon, Rush Hour and Terminator film series. The LAPD is also featured in the video games Midnight Club II, Midnight Club: Los Angeles, L.A. Noire and Call of Juarez: The Cartel. The LAPD has also been the subject of numerous novels. Elizabeth Linington used the department as her backdrop in three different series written under three different names, perhaps the most popular being those novel featuring Det. Lt. Luis Mendoza, who was introduced in the Edgar-nominated Case Pending. Joseph Wambaugh, the son of a Pittsburgh policeman, spent fourteen years in the department, using his background to write novels with authentic fictional depictions of life in the LAPD. Wambaugh also created the Emmy-winning TV anthology series Police Story. Wambaugh was also a major influence on James Ellroy, who wrote several novels about the Department set during the 1940s and 1950s, the most famous of which are probably The Black Dahlia, fictionalizing the LAPD's most famous "cold case", and L.A. Confidential, which was made into a film of the same name. Both the novel and the film chronicled mass-murder and corruption inside and outside the force during the Parker era. Critic Roger Ebert indicates that the film's characters (from the 1950s) "represent the choices ahead for the LAPD": assisting Hollywood limelight, aggressive policing with relaxed ethics, and a "straight arrow" approach. http://en.wikipedia.org/wiki/LAPD

Flaxseed Oil has been linked to cancer and taking the concentrated oil is not recommended by Dr. Bowring. The conversion of flaxseed oil into the important omega 3 fats, DHA and EPA is very low and therefore it is always better to supplement DHA and EPA from a high quality fish oil supplement like VitaFish Oil. My Friend Irma: Psycholo / Newspaper Column / Dictation System

01:26:06
My Friend Irma, created by writer-director-producer Cy Howard, is a top-rated, long-run radio situation comedy, so popular in the late 1940s that its success escalated to films, television, a comic strip and a comic book, while Howard scored with another radio comedy hit, Life with Luigi. Marie Wilson portrayed the title character, Irma Peterson, on radio, in two films and a television series. The radio series was broadcast from April 11, 1947 to August 23, 1954. Dependable, level-headed Jane Stacy (Cathy Lewis, Diana Lynn) began each weekly radio program by narrating a misadventure of her innocent, bewildered roommate, Irma, a dim-bulb stenographer from Minnesota. The two central characters were in their mid-twenties. Irma had her 25th birthday in one episode; she was born on May 5. After the two met in the first episode, they lived together in an apartment rented from their Irish landlady, Mrs. O'Reilly (Jane Morgan, Gloria Gordon). Irma's boyfriend Al (John Brown) was a deadbeat, barely on the right side of the law, who had not held a job in years. Only someone like Irma could love Al, whose nickname for Irma was "Chicken". Al had many crazy get-rich-quick schemes, which never worked. Al planned to marry Irma at some future date so she could support him. Professor Kropotkin (Hans Conried), the Russian violinist at the Princess Burlesque theater, lived upstairs. He greeted Jane and Irma with remarks like, "My two little bunnies with one being an Easter bunny and the other being Bugs Bunny." The Professor insulted Mrs. O'Reilly, complained about his room and reluctantly became O'Reilly's love interest in an effort to make her forget his back rent. Irma worked for the lawyer, Mr. Clyde (Alan Reed). She had such an odd filing system that once when Clyde fired her, he had to hire her back again because he couldn't find anything. Useless at dictation, Irma mangled whatever Clyde dictated. Asked how long she had been with Clyde, Irma said, "When I first went to work with him he had curly black hair, then it got grey, and now it's snow white. I guess I've been with him about six months." Irma became less bright as the program evolved. She also developed a tendency to whine or cry whenever something went wrong, which was at least once every show. Jane had a romantic inclination for her boss, millionaire Richard Rhinelander (Leif Erickson), but he had no real interest in her. Another actor in the show was Bea Benaderet. http://en.wikipedia.org/wiki/My_Friend_Irma_%28radio-TV%29 Katherine Elisabeth Wilson (August 19, 1916 -- November 23, 1972), better known by her stage name, Marie Wilson, was an American radio, film, and television actress. She may be best remembered as the title character in My Friend Irma. Born in Anaheim, California, Wilson began her career in New York City as a dancer on the Broadway stage. She gained national prominence with My Friend Irma on radio, television and film. The show made her a star but typecast her almost interminably as the quintessential dumb blonde, which she played in numerous comedies and in Ken Murray's famous Hollywood "Blackouts". During World War II, she was a volunteer performer at the Hollywood Canteen. She was also a popular wartime pin-up. Wilson's performance in Satan Met a Lady, the second film adaptation of Dashiell Hammett's detective novel The Maltese Falcon, is a virtual template for Marilyn Monroe's later onscreen persona. Wilson appeared in more than 40 films and was a guest on The Ed Sullivan Show on four occasions. She was a television performer during the 1960s, working until her untimely death. Wilson's talents have been recognized with three stars on the Hollywood Walk of Fame: for radio at 6301 Hollywood Boulevard, for television at 6765 Hollywood Boulevard and for movies at 6601 Hollywood Boulevard. Wilson married four times: Nick Grinde (early 1930s), LA golf pro Bob Stevens (1938--39), Allan Nixon (1942--50) and Robert Fallon (1951--72). She died of cancer in 1972 at age 56 and was interred in the Columbarium of Remembrance at Forest Lawn Cemetery in Hollywood Hills. http://en.wikipedia.org/wiki/Marie_Wilson_%28American_actress%29
