Full Title: Robotic partial nephrectomy in a kidney with multiple renal arteries - Adrenaline rush in a renal surgery! Abstract: Saurabh Patil, Ashwin Tamhankar, Surya Prakash Ojha, Puneet Ahluwalia, Gagan Gautam. Dept of Uro-Oncology, Max Institute of Cancer Care, Max Super Speciality Hospital, Saket, New Delhi. Introduction: Partial nephrectomy has become the standard of care as a nephron-sparing procedure in properly selected patients with a Stage T1 tumor. With the advent of minimally invasive surgery, robotic partial nephrectomy has become a better alternative to open surgery for small renal masses with the advantage of faster recovery. Though routinely performed, robotic partial nephrectomy is technically demanding and may prove challenging in certain situations like multiple renal vessels. We hereby present our experience of a robotic partial nephrectomy case for a small renal mass in a kidney with multiple renal arteries. Case: 44 years old gentleman presented with an incidentally detected right renal mass. His clinical examination was unremarkable and all routine laboratory parameters were within normal limits. On preoperative DTPA scan, glomerular filtration rate(GFR) of right kidney was 42.1 ml/min, with a global GFR of 79.4. Contrast CT of abdomen showed a well-defined heterogeneously enhancing mass of size 49x37x44mm arising from upper pole of right kidney. Metastatic workup was negative. We did robotic partial nephrectomy using da Vinci Xi system. Intraoperatively, we found multiple renal arterial vessels, which could be managed with loop tourniquet and bulldog clamps. Results: Total console was time 3 hours 30minutes and warm ischemia 29min 10 sec. Estimated blood loss was 200 ml. Post-operative recovery was uneventful with a total 2 days of hospital stay. Histo-pathological examination showed Clear cell RCC, Fuhrman’s grade 2, with all resection margins negative. Conclusion: RAPN represents a safe and effective nephron-sparing surgery technique even for a kidney with multiple renal vessels. Innovation in the form of using vessel loop to take control of multiple vessels can be utilised. Whenever possible, CT renal angiogram must be included in preop imaging workup. Renal cell carcinoma (RCC, also known as hypernephroma, Grawitz tumor, renal adenocarcinoma) is a kidney cancer that originates in the lining of the proximal convoluted tubule, a part of the very small tubes in the kidney that transport waste molecules from the blood to the urine. RCC is the most common type of kidney cancer in adults, responsible for approximately 90-95% of cases. It has been described as one of the deadliest of cancers affecting the genitourinary tract. Initial treatment is most commonly either partial or complete removal of the affected kidney(s) and remains the mainstay of curative treatment. Where the cancer has not metastasised (spread to other organs) or burrowed deeper into the tissues of the kidney the 5-year survival rate is 60-70%, but this is lowered considerably when the cancer has spread. It is relatively resistant to radiation therapy and chemotherapy, although some cases respond to targeted therapies such as sunitinib, temsirolimus, bevacizumab, interferon alfa and sorafenib which have improved the outlook for RCC. The body is remarkably good at hiding the symptoms and as a result people with RCC often have advanced disease by the time it is discovered. The initial symptoms of RCC often include: blood in the urine (occurring in 40% of affected persons at the time they first seek medical attention), flank pain (40%), a mass in the abdomen or flank (25%), weight loss (33%), fever (20%), high blood pressure (20%), night sweats and generally feeling unwell. RCC is also associated with a number of paraneoplastic syndromes (PNS) which are conditions caused by either the hormones produced by the tumour or by the body's attack on the tumour and are present in about 20% of those with RCC. These syndromes most commonly affect tissues which have not been invaded by the cancer. The most common PNSs seen in people with RCC are: anaemia (due to an underproduction of the hormone, erythropoietin), high blood calcium levels, polycythaemia (the opposite to anaemia, due to an overproduction of erythropoietin), thrombocytosis (too many platelets in the blood, leading to an increased tendency for blood clots and bleeds) and secondary amyloidosis. When RCC metastasises it most commonly spreads to the lymph nodes, lungs, liver, adrenal glands, brain or bones. This video is targeted to blind users. Dr. Berg does not to do the ketogenic diet without lots of vegetables. You need 7-10 cups of vegetables per day. This will keep the liver clean. The ketogenic diet can cause a massive amount of fat being released through the body and through the liver. So veggies are necessary. Also - do not use vegetables as a calculation into your 20-50 grams of carbs per day. Vegetables are extra grams that should be above the 20-50 grams of carbs per day. Hi guys, in this video we are going to answer the question. Does the ketogenic diet cause a fatty liver. You are probably already know that being an alcoholic can cause a fatty liver, consuming a refined carbs and sugar but did you know that going into a ketogenic diet can also cause a fatty liver? We are going to use guidance physiology on page 927 and this is what it says, effect of insulin lack(means low insulin or lacking too much insulin) on causing a fatty liver. Strange as it may seem, though insulin lack causes dissolution(means dissolving) of the fat stores in adipose tissue, it causes a great increase in the amount of stored triglycerides in the liver, leading to a very fatty liver. I never in any of my videos recommend just the straight ketogenic diet, I always recommended adding a bunch of vegetables to the ketogenic diet because let’s take a look at what a ketogenic diet is low carbs below 20 to 50g, it doesn’t specifically tell you what carbs you can consume, the piece of bread could be a 20g of carbs, an apple or berries so it doesn’t talk about the vegetable content. What you want to do is you would want to beat up your vegetables, you want to do at least 7 to 10 cups of vegetables not just from your potassium and your nutrients but to keep that liver flushed out. We are not gonna calculate vegetables into this calculation of 20 to 50 grams. It is unlimited vegetables and don’t add the vegetables into your calculations. Thanks for watching!
