Renal cell carcinoma (RCC, also known as hypernephroma, Grawitz tumor, renal adenocarcinoma) is a kidney cancer that originates in the lining of the proximal convoluted tubule, a part of the very small tubes in the kidney that transport waste molecules from the blood to the urine. RCC is the most common type of kidney cancer in adults, responsible for approximately 90-95% of cases. It has been described as one of the deadliest of cancers affecting the genitourinary tract. Initial treatment is most commonly either partial or complete removal of the affected kidney(s) and remains the mainstay of curative treatment. Where the cancer has not metastasised (spread to other organs) or burrowed deeper into the tissues of the kidney the 5-year survival rate is 60-70%, but this is lowered considerably when the cancer has spread. It is relatively resistant to radiation therapy and chemotherapy, although some cases respond to targeted therapies such as sunitinib, temsirolimus, bevacizumab, interferon alfa and sorafenib which have improved the outlook for RCC. The body is remarkably good at hiding the symptoms and as a result people with RCC often have advanced disease by the time it is discovered. The initial symptoms of RCC often include: blood in the urine (occurring in 40% of affected persons at the time they first seek medical attention), flank pain (40%), a mass in the abdomen or flank (25%), weight loss (33%), fever (20%), high blood pressure (20%), night sweats and generally feeling unwell. RCC is also associated with a number of paraneoplastic syndromes (PNS) which are conditions caused by either the hormones produced by the tumour or by the body's attack on the tumour and are present in about 20% of those with RCC. These syndromes most commonly affect tissues which have not been invaded by the cancer. The most common PNSs seen in people with RCC are: anaemia (due to an underproduction of the hormone, erythropoietin), high blood calcium levels, polycythaemia (the opposite to anaemia, due to an overproduction of erythropoietin), thrombocytosis (too many platelets in the blood, leading to an increased tendency for blood clots and bleeds) and secondary amyloidosis. When RCC metastasises it most commonly spreads to the lymph nodes, lungs, liver, adrenal glands, brain or bones. This video is targeted to blind users. Full Title: Robotic partial nephrectomy in a kidney with multiple renal arteries - Adrenaline rush in a renal surgery! Abstract: Saurabh Patil, Ashwin Tamhankar, Surya Prakash Ojha, Puneet Ahluwalia, Gagan Gautam. Dept of Uro-Oncology, Max Institute of Cancer Care, Max Super Speciality Hospital, Saket, New Delhi. Introduction: Partial nephrectomy has become the standard of care as a nephron-sparing procedure in properly selected patients with a Stage T1 tumor. With the advent of minimally invasive surgery, robotic partial nephrectomy has become a better alternative to open surgery for small renal masses with the advantage of faster recovery. Though routinely performed, robotic partial nephrectomy is technically demanding and may prove challenging in certain situations like multiple renal vessels. We hereby present our experience of a robotic partial nephrectomy case for a small renal mass in a kidney with multiple renal arteries. Case: 44 years old gentleman presented with an incidentally detected right renal mass. His clinical examination was unremarkable and all routine laboratory parameters were within normal limits. On preoperative DTPA scan, glomerular filtration rate(GFR) of right kidney was 42.1 ml/min, with a global GFR of 79.4. Contrast CT of abdomen showed a well-defined heterogeneously enhancing mass of size 49x37x44mm arising from upper pole of right kidney. Metastatic workup was negative. We did robotic partial nephrectomy using da Vinci Xi system. Intraoperatively, we found multiple renal arterial vessels, which could be managed with loop tourniquet and bulldog clamps. Results: Total console was time 3 hours 30minutes and warm ischemia 29min 10 sec. Estimated blood loss was 200 ml. Post-operative recovery was uneventful with a total 2 days of hospital stay. Histo-pathological examination showed Clear cell RCC, Fuhrman’s grade 2, with all resection margins negative. Conclusion: RAPN represents a safe and effective nephron-sparing surgery technique even for a kidney with multiple renal vessels. Innovation in the form of using vessel loop to take control of multiple vessels can be utilised. Whenever possible, CT renal angiogram must be included in preop imaging workup. Immunotherapy is a type of cancer treatment that uses the patient's immune system to treat their cancer. By producing proteins, cancer can sometimes avoid detection by the immune system. Because the immune system can't detect the cancer, it can't fight it, so that how the immunotherapy treatment helps. It can help boost or restore the immune system, so that it is better able to detect and destroy the cancer. In this video, oncologist Dr. Stephen Lemon explains how immunotherapy works, as well as what types of side effects patients may experience. Stephen Lemon MD is a medical oncologist, who practices at the Overlake Hospital Cancer Treatment Center in Bellevue, WA. ------------------------------------------------------------------------------------------------------------------------------------------------------------- What is immunotherapy? And how does it work? (video transcript) As a cancer treatment, immunotherapy uses the patient’s immune system to treat cancer. So here is how it works. Our immune system consists of blood cells, the lymph system and certain organs like the liver and spleen that keep us healthy by fighting off infections. Normally we would fight cancer with our immune system, which makes antibodies like it does for a virus or bacterial infection. But sometimes cancer conceals itself from our immune system by producing proteins that hide it. And because our immune system can’t find the cancer, it can’t adequately fight it. And that’s where immunotherapy treatment comes in. Using materials created by our bodies, or within a laboratory, the immunotherapy treatment helps boost or restore our immune system’s function, so that it can better detect and destroy the cancer. What types of cancers can immunotherapy treat? Immunotherapy can be used to be treat many types of cancers. Such as Non small cell lung cancer Kidney Melanoma skin cancer Bladder Head and neck cancers Hodgkins lymphoma Stomach cancer And other cancer types as well. What are some existing FDA approved immunotherapy treatment drugs? There are currently a number of different immunotherapy treatments such as Keytruda, Tecentriq & Opdivo and others. Sometimes these treatments are used alone or with chemotherapy. So, How is immunotherapy given? And for how long? Most immunotherapy treatments are given intravenously on 2 or 3 week cycles. Immunotherapy is generally given for as long as there is clinical benefit and there are no serious side effects. So what are the side effects from immunotherapy? Just like any cancer treatment, immunotherapy treatment may cause some common, as well as less common, side effects. Some common side effects of of the treatment are: feeling tired pain in muscles, bones, and joints decreased appetite nausea cough constipation shortness of breath diarrhea rash Less common side effects from immunotherapy are: Inflammation of the lung, intestine, kidney, liver, and other organ systems Allergic reaction Immunotherapy should also not be taken by women if they are pregnant. As a cancer treatment, immunotherapy offers a lot of promise. And recent research, and ongoing studies, are continuing to provide us with more breakthrough immunotherapy treatment options. Between traditional chemotherapy, targeted therapy and now groundbreaking immunotherapy, we are heading towards a cure for cancer.

It serves many important bodily functions, such as eliminating food and bodily waste and removing toxins from the intestines, lymph system, blood and more. The colon is also essential when it comes to proper digestion, which is key to our overall health. There are many health conditions related to colon health which we want to avoid, such as irritable bowel syndrome, celiac disease and colonic polyps. Fortunately, there are foods we can consume to increase the health of our colon and its proper functioning. Lets now discuss these foods for colon health. 1. First, fruits such as raspberries or blueberries are important for the overall health of our colon. One reason is because they are loaded with important fibre that can help flush our digestive system and promote bowel movement. Fibre is incredibly important for colon health. Raspberries, in just a one cup serving, contain about 8 grams of fibre. Along with this, they are also a great source of important vitamins, minerals and nutrients, as well as antioxidants and phytochemicals. Blueberries are especially well known for being an antioxidant powerhouse. 2. Dairy products such as yogurt and milk are also great for the overall health of our colon. This is due to the high amounts of vitamin D these products contain. Vitamin D may be able to help reduce the risk of colorectal cancer as some studies have indicated. Yogurt is especially helpful for the health of our colon since it contains healthy active bacterial cultures. These healthy bacteria help to maintain proper balance between good and bad bacteria in our bodies and support gastrointestinal health. If that wasn’t enough, dairy products also contain good amounts of calcium, which can also lower the risk of colon-related diseases and ailments. 3. If we want to improve the health of our colon, then salmon can be very beneficial for this goal. This is because salmon is a great source of healthy omega-3 fatty acids. Research suggests that when omega-3 fatty acids are consumed regularly, this may help to reduce inflammation through out the entire body, and perhaps more importantly, improve the overall function and health of colon cells. As well, salmon contains many different nutrients, as well as protein and antioxidants to support colon health. 4. Dark colored vegetables, such as broccoli and artichokes are also great for colon health for a number of different reasons. These vegetables are great sources of antioxidants and phytochemical as well as different vitamins, minerals and nutrients. Broccoli is a great source of vitamin C, a potent antioxidant. Important for overall colon health, these vegetables are packed healthy fibre. Artichokes are especially high in dietary fibre, with just one heart containing about 10 grams of fibre. These vegetables are more than just nutritious as they can help with digestion, promote bowel movements, help with the elimination of toxins in the body and may reduce the risk of colon cancer. 5. Lastly, for improved colon health, beans such as black and kidney beans are incredibly helpful in this regard. They are an excellent lean protein that can be consumed as an alternative to fatty red meats. They also contain excellent amounts of vitamins, minerals and nutrients, especially magnesium, iron and calcium. They are surprisingly high in antioxidants which can reduce inflammation through out the entire body as well as eliminate free radicals and reduce oxidative stress, benefiting our colon. My answer is that it really depends on you. I will show you the pros and cons of rice and go over what I believe is the healthiest rice today. There is a debate today about if you should consume rice or not: ⁃ The paleo diet community says to stay grain-free and therefore that rice is not healthy ⁃ Chinese medicine followers and vegans think rice can be an incredible superfood I believe there is validity to both but it depends on the person and the preparation of the rice. Rice to stay away from: Refined White Rice (This is not jasmine or basmati rice but the sticky white rice from fast food or Chinese restaurants.) In this video the presenter John G. Clark M.D goes through the causes of diabetes. The best way to defeat a lifestyle disease such as diabetes you have to stop what is causing it and take up habits that promote good health. I would also recommend another natural remedies expert, Agatha Thrash MD , she too promotes the use of diet in treating diabetes. Dr Agatha Thrash of Uchee Pines Institute says, "for over 70 years, traditional treatment for diabetes was a high fat, low carbohydrate diet with insulin by injection, or pills by mouth. Justification for prescribing the high fat diet was that it keeps the blood sugar from rising too much after a meal, and it prevents too much sugar from spilling into the urine. But the disadvantages of fatty diets far outweigh the advantages. This type of diet does not reduce blood sugar, nor the insulin requirement to handle the excess sugar. In fact, it tends to make the body less sensitive to insulin and induces resistance to it. Elevation of blood fats leads to hardening of the arteries. It promotes the accumulation of ketone bodies in the body tissue and fluids, and accelerates aging." Other diets that have been used to control diabetes are high protein, high carbohydrate and high fiber diets. Protein diets seem to prevent a significant rise in blood sugar, but are "impractical, monotonous, expensive, promote kidney and liver failure and hardening of the arteries, and are usually high in fat," hence not recommended. Refined carbohydrate diets (sugar, white flour, white rice, etc.) are rapidly absorbed, elevating the blood sugar after meals, as well as increasing triglycerides. Such a diet is a detriment in treating diabetes, and is no treatment at all. However, when complex carbohydrates--whole grain bread and cereals, brown rice, bran, fruit, vegetables and no refined sugar--compose most of the meal, the disadvantages of a high carbohydrate diet disappear. The body better utilizes the carbohydrate and glucose in the food. When mildly diabetic persons switch from 45% refined carbohydrates to 85% complex carbohydrates, their glucose tolerance test improves. Most Americans on a high meat diet eat between 14 and 20 grams of plant fiber every day. The ideal vegetarian diet provides 65-70 grams. Insulin must hook up on one side with sugar (glucose), and the other side must slide into insulin "docking sites" (receptors) on cells. For sugar to be properly utilized, the docking sites must be filled with insulin. Here is another advantage of a high fiber diet--the fiber increases the number of docking sites. Obese individuals have fewer insulin receptors, hence fewer sites for sugar-hooked insulin to slide into. Fasting for several days, until the blood glucose returns to normal, multiplies insulin docking sites. This usually takes three to five days, and should be done only in type II (or "adult onset") diabetics. Type I diabetics should never fast. If the person is obese, fasting for a day or two a week, non-consecutively, can be very helpful for diabetic control. In addition to decreasing the rapid rise of blood sugar after a meal and increasing the number of insulin docking sites on cells, a high fiber diet lowers blood fats, helping to carry cholesterol out of the body. It keeps the blood sugar at a lower level than a fiber-free meal. Triglycerides (blood fats) and cholesterol are also decreased, thus lessening the risk of coronary heart attack. Exercise along with the diet is important and cannot be overemphasized. Exercise enhances the sensitivity of the tissues to insulin, increasing the number of insulin receptors. It helps decrease body fat, thus making people more sensitive to insulin. Dr Agatha Thrash further states that, "We have found over the years that a total vegetarian diet, high in fiber and the unrefined carbohydrates, low in fats; coupled with a regular exercise program; and weight control is the very best to control diabetes and to prevent the serious complications of this disease. By far the majority of people stay on the program. They enjoy the food, for it is palatable, practical and attractive. The whole family can benefit from eating this food. Lectins are a type of protein found in nearly all animals and plants. They bind to the sugar molecules (polysaccharides) which cover the surface of most cells in the human body. They are thought to have evolved as a natural defense in plants, mainly as a toxin which deters animals from eating the plants. Furthermore, plants use lectins for cell organization, to communicate with their environment, and as reserve proteins. Benefits of Lectins Lectins also have various health benefits. For instance, they may have some anti-cancer potential via changes to the immune system and minimizing the growth of tumors, according to a review article issued in "Critical Reviews in Food Science and Nutrition" in 2005. Additionally, the human body uses lectins to achieve many basic functions, including inflammatory modulation, cell to cell adherence, and programmed cell death. 13 Healthy Foods High In Lectins You Should Eat Regularly: #1 Red Kidney Beans - they contain 20,000–70,000 hau. However, once they are thoroughly cooked, they contain only 200–400 hau, that is considered a safe level.
