Vitamin B12 Deficiency is Common After Bariatric Surgery

Taking daily vitamins is a habit that you will have to keep for the rest of your life as vitamin deficiencies are one of the most common side effects of bariatric procedures. Yet even with daily vitamin intake, some patients still develop low levels of vitamins and minerals. The most common is Vitamin B12 deficiency, followed by low levels of Vitamin B9, commonly known as folate. Patients undergoing more complex procedures such as Roux-en-Y gastric bypass and Bilopancreatic Diversion with Duodenal Switch are more apt to develop B12 deficiencies because of the physiological condition of malabsorption of those procedures that reroute the small intestine, bypassing the initial sections where most food is absorbed into the body. However, even procedures that are purely restrictive such as gastric sleeve, gastric plication and vertical banded gastroplasty and other surgeries that make the stomach smaller appear to have a slight effect on Vitamin B12 and a more extensive one on folate as they impact absorption. The procedures appear to interfere with the body’s ability to absorb these nutrients as B12 is normally broken down in the acidic environment of the stomach and is subsequently absorbed in the duodenum section of the small intestine. Researchers aren’t exactly sure why some patients develop deficiencies and others don’t, although the older you are, the more likely you will develop the deficiency. That’s why you need to make sure that your vitamin and mineral levels are frequently monitored to ensure that you have sufficient nutrients in your body to avoid problems. On top of that, Vitamin B12 deficiency can be sneaky with problems that show up suddenly. What bariatric patients can do to avoid deficiency problems is establish a foundation of proper eating. The best sources of Vitamin B12 are meat, eggs, poultry dairy products and other foods from animals. In other words, follow the guidelines for eating protein first in your meals. Vegans are at higher risk of developing this deficiency if they don’t eat grains fortified with the vitamin and are conscientious about taking a vitamin supplement. Vegetarians can try kelp and fermented foods such as tempeh and miso in addition to dairy products and fortified breakfast cereals to consume sufficient amounts of B12. Often the first signs of Vitamin B12 deficiency include fatigue or weakness. Other symptoms can develop slowly and can include: • Numbness or tingling in the hands, legs and feet • Difficulty walking and balance problems • Anemia • Jaundice • Swollen, inflamed tongue • Cognitive difficulties or memory loss • Paranoia or hallucinations Initial signs are often subtle and can take several years to fully manifest themselves. That’s why it’s important to maintain regular monitoring to ensure that your body has sufficient levels. Vitamin B12 is essential for cell metabolism, blood formation, and proper operation of the brain and nervous system and for making protein available to various parts of the body. If you notice any of these signs and haven’t had a blood test recently, ask your doctor to check your levels to head off severe problems.

High levels of insulin is the cause of weight gain. There is a hormone in the small intestine called GIP or gastric inhibitory polypeptide that gets triggered every time you eat by sending the signal to the pancreas to increase the insulin. Gastric Bypass is the process of cutting the part of the stomach by taking the large part and bypass it thereby taking out the GIP and it lowers your insulin and that causes you to lose weight and lower diabetes BUT it comes with a lot of side effects. I wanna explain the real reason why Gastric Bypass for weight loss and diabetes. Now that being said, I am not recommending Gastric Bypass. Ok? There's other ways of losing weight and improving this situation right here. But here is why it works, you got that stomach right and you got the small intestine right here. Here is the pancreas. What causes weight gain is levels of high insulin. What prevents you from losing weight is high levels of insulin. There is a hormone in the small intestine called GIP or gastric inhibitory polypeptide you don't need to know that , the name. But its just GIP. This hormone gets triggered every time you eat it sends a signal to the pancreas telling the pancreas to increase the insulin. When you do Gastric Bypass, they are cutting the part of the stomach and getting this whole are right here and just bypassing it so this is no longer involved thereby taking this whole part of the picture and lowering your insulin. So that's why of course you are gonna lose weight and you are gonna improve your diabetes. But it comes with a package and has a lot of side effects. And I think it is completely unnecessary because all you needed to do is to cut down the carbohydrates, the thing that stimulate this (insulin), do healthy ketosis and intermittent fasting so you are not eating so frequently. Because everytime you eat, you trigger this hormone. In that way you are gonna avoid getting this procedure. But if you wanna know why it works? that's why. Thanks for watching! We're always here for you before, during, and after your weight loss surgery procedure--because it's the after that's the most important part. Today, we're going to talk about B12 deficiency, and B12 deficiency is actually more prevalent than we originally thought. Folks over the age of 60, young women on birth control, folks that have had weight loss surgery in the past are all more at risk for B12 deficiency. The older population loses the ability to produce enough gastric acids and enzymes in their stomach to break down foods properly. One of the factors in this gastric acid is a product called intrinsic factor, which is actually a protein. And the role of intrinsic factor is that it cleaves on to the B12, whether it's from food or from the supplement, and helps that B12 travel through your digestive system to the end of your small intestine, where it's actually absorbed and metabolized. Without intrinsic factor, you won't be able to absorb the B12 your body needs. Another population that's at risk from B12 malabsorption would be folks that have had bariatric surgery procedures. Again, whether you've had gastric bypass, the band, or the sleeve, you've limited your ability to have enough gastric acid and enzymes in your new stomach--or your new pouch--to properly have enough intrinsic factor. One way to side-step this is to take the more active version of B12, which is Methylcobalamin. Methylcobalamin has the active component in it that, if you have less intrinsic factor or none at all, you're still guaranteed to absorb, hopefully, all or some of it in your small intestine. Some symptoms of B12 deficiency are low energy, fatigue, anemia, mental fatigue, mood changes, and sleep difficulties. The best ways to take your B12 is to make sure that your B12 supplement is indeed Methylcobalamin. That's the more active version that we spoke about earlier. The Importance of B12 | Post Surgery Nutrition | Blossom Bariatrics | Las Vegas Hi guys, Michelle Phelan here, nutritionist at Blossom Bariatrics and today I'm going to share with you some amazing tips and some awesome finds at the grocery store. Today I'd really like to talk a little bit about vitamin B12, the signs of B12 deficiency, and excellent sources of B12. Signs of B12 deficiency are fatigue, constipation, depression, low sperm count, and low libido, weakness, anemia, and asthma. So all of these can mimic other things going on in your body, so it's important to get your levels tested. So something to also keep in mind post surgery is that the nature of perhaps a sleeve surgery, or gastric bypass is that there's a protein in your stomach, produced in your stomach by the cells in your stomach called intrinsic factor and intrinsic factor, after surgery, particularly with sleeve patients, is available much less so. So you have less intrinsic factor available and this is the little guy who likes to hang on to that B12 whether from supplements or food and help that B12 travel safely through the different PH's in your digestive system where it can be disrobed and methylated, or become active at the bottom of your small intestine. So it's important again to make sure you have enough digestive juices, hydrochloric acid, and intrinsic factor in your stomach. Now if this is not the case, which happens in many patients post surgery with the sleeve procedure, it's important to take a high quality B12 supplement. Now many of you know that I really promote methylcobalamin B12 post surgery because it's methylated. It's already active, it's ready to go. Whether you have enough intrinsic factor or not. So play it safe. Make sure that you're B complex has the methylcobalamin B12 version of B12 in it. It's super important. Don't get anything else if you're a post-surgery bariatric patient. Something else I'd like to add is that there's some things that can deplete your body of B12 much more quickly. So if you are on things like Metformin, diabetes medication, birth control, depression medication, or Nexium, like acid reflux medication, these things can inhibit the absorption of B12 as well. In fact, although Nexium and these antacids help short term with the pain of acid reflux and acid indigestion long term, they inhibit your stomach's ability to create enough hydrochloric acid to properly break down food and it also inhibits your stomach's ability to make intrinsic factors. So you could be at a huge disadvantage to getting B12 if you're taking these medications. So it's always beneficial to supplement with methylcobalamin B12. Make sure that the B12 is in addition to your B complex. B12 is one of the eight B vitamins. They all work together best as a family. Okay, top B12 containing foods, so beef liver, I know right? But that is the top way to get B12 naturally. Sardines, I know, another ugh right? But again, the second best way to get B12 from food. Grass-fed organic beef, so there's another option as well. Tuna has B12 in it, cottage cheese, lamb, salmon, all of these things have high amounts of B12. But also keep in mind that although these foods are on the very much eat these foods list, as a post-surgery bariatric patient, if you like them, as a patient post surgery you're not able to eat a lot of them, maybe like one or two ounces. So you still need to supplement. If you really like beef liver, I still recommend you supplement with methylcobalamin B12. Thanks for watching. Patients classically present with megaloblastic anemia, atrophic glossitis, neurological changes characterized by symmetrical lower extremity neuropathy that is greater than upper extremity neuropathy with paresthesias and ataxia that may progress to weakness and spasticity, cerebellar ataxia, and personality changes. Concomitant folate deficiency is often present and may mimic many symptoms of B12 deficiency. Vitamin B12, or cobalamin, is a water-soluble vitamin found in meat, fish, and dairy products and, once combined with intrinsic factor in the stomach, is absorbed in the ileum. Patients at risk for B12 deficiency include those with a lack of intrinsic factor (pernicious anemia) or antibodies to intrinsic factor, patients with malabsorption syndromes, especially those who have had a gastrectomy, gastric bypass, or chronic pancreatitis, or those with infections such as Helicobacter pylori or tapeworm infections. In addition, strict vegans, alcoholics, and older adults are particularly vulnerable to B12 deficiency.

More and more and a very useful and effective tool for treating morbid obesity is bariatric is weight loss surgery. This is referring to things such as the laparoscopic banding procedure, or the lap band, or gastric bypass surgery. Now this has been found to be a very effective tool at treating morbid obesity but it is very important that a pregnancy is planned appropriately around that. Most experts would recommend that a patient wait until approximately a year after that surgery to attempt to conceive. The goal for that is to hopefully that they have achieved their weight loss and that they have been able to maintain that weight loss as well as achieve a normal nutritional status so that it's safe to achieve a pregnancy. During the pregnancy it's important for your physician to know that you've had that procedure done, sure it does slightly increase risks for problems such as having low iron or certain vitamin deficiency, especially if you haven't been very good about taking the vitamin supplements that are recommended after surgery. Most women who've had gastric bypass surgery are actually at a lower risk during their pregnancy than they would be if they were pregnant when they were suffering from morbid obesity. It's something that can be managed well. It's very important if you've had gastric bypass surgery that the routine testing that we have for gestational diabetes not be performed in those patients. I'm referring to the Glucola or the one hour glucose test. That is a 50 gram glucose challenge that we give to patients and if you were to make a patient who's had gastric bypass surgery drink that they would feel very ill. So, there are alternate ways to test for that during pregnancy and that's why it's one of the reasons why it's important to let your physician know about your complete medical history.

10 Causes Of Vitamin B12 Deficiency Are You At Risk

10 Causes Of Vitamin B12 Deficiency: Are You At Risk? Nearly two-fifths of the US population has a marginal amount of vitamin B12 (cobalamin) in their body, putting them at a high risk of a deficiency. You may think that the quick fix to a vitamin B12 deficiency is to eat foods that are rich in B12, but the solution may not be as simple. A B12 deficiency can be caused not only by inadequate amounts of the vitamin in the diet but also by poor absorption in the gut. Here’s a roundup of all the factors that may cause (or increase your risk of) vitamin B12 deficiency. 1. Veganism Or Vegetarianism. Since most sources of vitamin B12 are animal products, like meat or milk, vegans need to be extra-vigilant about vitamin B12 deficiency. In fact, it is so common among vegans that some people believe the escalation in the number of vitamin B12 deficiency cases is because of the popularity of veganism. When 40 research studies were analyzed on the prevalence of the deficiency among people, higher deficiency prevalence was found in vegans than in vegetarians. However, all vegetarians should be periodically screened for the deficiency. Do note that since the liver stores this vitamin, if you go vegan or stop eating vitamin B12 foods, your B12 stores can last for 3 to 5 years.3 The symptoms of B12 deficiency will start showing up after that period. While there are many vegetarian products like spirulina (blue-green algae) that are marketed as good sources of vitamin B12, some researchers have found them to be bogus. They have also found that the process of cooking and storing can, in fact, destroy the vitamin. 2. Heavy Drinking. Functional vitamin B12 deficiency is associated with alcohol abuse. This deficiency is more prominent in heavy drinkers and those with an alcoholic liver disease. A functional deficiency means that while the amount of B12 found in blood is within the normal range, B12 is not metabolized. 3. Long-Term Intake Of Antacids. If you have gastrointestinal disorders like heartburn, peptic ulcer, or acid reflux, you may be prescribed antacids. Antacids neutralize the effect of stomach acids. But your body needs gastric acid to activate certain enzyme reactions to absorb vitamin B12. Hence, using antacids on a long-term basis may result in the malabsorption of vitamin B12. 4. Weight Loss Surgery. Vitamin B12 deficiency is seen very commonly among people who have had a bariatric surgery. There are 2 types of bariatric surgery – gastric bypass where a portion of the stomach is made into a small pouch and attached to the distal segment of the small intestine; and restrictive techniques like gastric binding where a restriction is created in the stomach to limit the food intake of the patient. A gastric bypass surgery decreases the secretion of stomach acids and a protein called IF (intrinsic factor) both of which are important for B12 absorption. Gastric bypass surgery often leads to malabsorption of nutrients, one of them being vitamin B12. In fact, a study noted that 30% of the participants had a B12 deficiency even after 3 years of the gastric bypass surgery. This is because the stomach secretes a protein called the intrinsic factor (IF) which must bind to vitamin B12 to make it absorbable in the small intestine. Gastric bypass decreases the production of IF as only a small portion of the stomach is available to secrete the protein. This, in turn, leads to poor absorption of B12. 5. Atrophic Or Autoimmune Gastritis. Gastritis is the inflammation or erosion of the stomach lining and it can be of 2 types – atrophic or autoimmune. Both conditions destroy the cells of the stomach lining and reduce the production of hydrochloric acid, thereby affecting the absorption of vitamin B12 from food.8 You might have to take B12 supplements to mitigate the deficiency since the gastritis diet may exclude meat and milk products, except yogurt. 6. Pernicious Anemia. 7. Malabsorption Of B12 In The Small Intestine 8. Breastfeeding By A B12 Deficient Mother. 9. Age Above 50 Years. 10. Genetic Factors. Dietary Guidelines after Bariatric Surgery

In order to continue losing weight or maintaining weight, one must follow a program that one’s bariatric surgeon’s office recommends. Following a strict regimented dietary plan is often hard in the beginning, but slowly it becomes an unconscious routine. Some important aspects to be followed by gastric sleeve, lap band and gastric bypass patients: • Eat meals in small proportions. • Choose a diet low in calories, fats and avoid sweets. • Make a note of daily calorie intake and protein intake. • Chew your food slowly and in small quantities to make it easy to digest. • Avoid raw vegetables, rice, and fresh fruits. • Ground meat is preferred over large pieces of steak or pork as they are better tolerated. Your new formed stomach after gastric sleeve, gastric bypass or lap band cannot take large pieces of meat. • Avoid sugar. • Avoid carbonated drinks, soda and champagne. Do not drink out of a straw. This could curtail you swallowing air, which can escape into your pouch and therefore cause discomfort. Whether you have had a lap band, gastric sleeve or a gastric bypass, filling the pouch with air can cause a lot of pain and even stretch the walls of the pouch. Concentrate on limiting your daily calories to 300-600 calories per day for the first few months. Fluids Drink about a cup of water or a light soup between meals ( six to eight cups a day preferably.) Avoid caffeine. Avoid alcohol at all costs. As it will be absorbed faster into your system and can alter mood and can even act as a sedative. Protein Muscle loss is a common factor experienced when losing weight after bariatric surgery. High protein foods like eggs, fish, seafood poultry, soymilk, tofu cottage cheese, yogurt and all milk products have to be added into your diet. Supplements and Multi Vitamins The supplements and Multi Vitamins recommended by your bariatric surgeon have to be taken on a daily basis. They need to be crushed or cut into bit sizes. Remember that after a gastric sleeve, lap band or gastric bypass surgery, it can be difficult to pass through your new-formed anatomy. Keeping in contact with your doctor’s office is key to the ongoing success of weight loss. Your success story won’t be complete without following the guidelines recommended. We're always here for you before, during, and after your weight loss surgery procedure--because it's the after that's the most important part. Today, we're going to talk about the dangers of dehydration. Dehydration is a huge concern for post-surgery bariatric patients; 37% of bariatric patients are readmitted to the hospital for dehydration. Something to keep in mind is that 64 ounces is just 8 cups. There's about 12 hours in our day--that's about a quarter cup every fifteen minutes. I think we can all certainly sip a quarter cup of water or fluid every 15 minutes. You know, I get this a lot--sometimes water makes people nauseous or you're so tired of water. There's some ways to get around that. Play with temperatures. Sometimes, after surgery, things that are too hot or too cold can hurt your tender stomach while it's healing. However, once it's healed, try playing around with ice-cold water or room-temperature or warm water. Sometimes warm water is really soothing to your tummy while it's healing. If water is still making you nauseous, play with some flavors. Try throwing in lemons into your water or into the pitcher of water in the fridge, try strawberries, cucumbers, even mint leaves. 11 Signs And Symptoms Of Vitamin B12 Deficiency Though vitamin B12 deficiency is fairly common across the world, it’s often difficult to diagnose this deficiency just through symptoms. This is because vitamin B12 deficiency manifests in a widely diverse range of symptoms, from dizziness to OCD, all of which could be caused by another ailment or deficiency. Moreover, although B12 is a water-soluble vitamin, your liver could store it, unlike other vitamins, in amounts that could last you a good 3–5 years even after you stop consuming B12-rich foods.1 As a result, your B12 deficiency symptoms might show up much later than when the deficiency started. High levels of folic acid from supplements or fortified food could also mask your B12 deficiency. While a deficiency could occur due to the lack of B12 foods in the diet, as in the case of vegetarians or vegans, more insidiously, it could also occur when your body cannot absorb it effectively. So if you have any of the following risk factors, it might be wise to keep an eye out for the symptoms and get your B12 levels checked periodically. -Vegetarianism -Veganism -Alcoholism -Stomach or intestinal disorders -Use of antacids and proton pump inhibitors for a long time -Stomach reduction surgery -Pernicious anemia -Aging -Genetic conditions -As vitamin B12 is involved in so many functions in the body, -from producing DNA, RNA, and red blood cells to producing -energy, a vitamin B12 deficiency can manifest in a wide -variety of symptoms. Here’s a roundup. 1. Dizziness If you often feel a “head rush” when you get up after sitting for long periods of time, or while climbing up a flight of stairs, you might have a vitamin B12 deficiency. When your body has low levels of the vitamin, you may lose your balance, especially when you move too fast. If you’re feeling dizzy too often, you might want to make a trip to the doctor and take a blood test to detect your B12 levels. Dizziness or lightheadedness is a classic symptom of low blood pressure, and low blood pressure is a common fallout of vitamin B12 deficiency. 2. Numbness Or Pins And Needles It’s common to experience the sensation of numbness or “pins and needles” if you sit or stand in a certain position for too long. However, if you feel these even when you’re not compressing or exerting pressure on any body part, it could indicate a vitamin B12 deficiency. A vitamin B12 deficiency does not just lower your blood pressure but it also reduces the production of healthy RBCs in your blood, thus reducing oxygen supply to other organs. This is what causes the numbness. The pins and needles sensation could also be caused by demyelination of nerve cells – certain nerve cells are covered with a protective sheath called the myelin sheath which speeds up nerve signal transmission. Lack of vitamin B12 leads to the degeneration of the myelin sheath. 3. Pale Or Yellow Skin Pale and yellowish skin is one of the most common signs of a B12 deficiency. You may even notice a slight yellow tinge in the white of your eyes, not unlike in jaundice. When the B12 levels in your body dip, red blood cells become fragile and start breaking down into bilirubin. While skin paleness shows that there’s a drop in the levels of functional RBCs, the yellowness can be attributed to the increased levels of bilirubin – which is the same compound that makes the skin of jaundice patients go yellow. 4. Skin Lesions, Vitiligo, And Sore Tongue Any change in the B12 content in the body can lead to skin problems like vitiligo, dermatitis, hyperpigmentation, or acne. Skin lesions, especially those not responding to any other treatment, are most likely from a B12 deficiency. Your hair and nail could change, too. Glossitis or inflammation of the tongue is another symptom of vitamin B12 deficiency. In this condition, the tongue becomes smooth and loses its taste buds, which may make food intake difficult and further reduce your nutrient intake. 5. Poor Vision 6. Muscle Weakness 7. There are basically five essential tips for all bariatric patients. The first one is pay attention to what your eating. Sit down at meal times. Turn off all distractions, including your TV and your computer. The second one is chew. Chew your food really well. If you have to chew twenty times, please do it. That will help you slow down and pay attention to what your eating. The third tip is to eat within an hour of feet to the floor. By that I mean eat within an hour of getting out of bed. You're going to stoke that furnace, fuel your body and start your meal, and start your day with protein. Tip number four, don't buy the stuff you don't want to eat. Stay away from the starchy processed foods, the sugary foods, the starchy carbohydrates. These foods can promote and do promote weight gain all over again. And tip number five, make sure to have ample high protein snacks available at all times, Greek yogurt, cottage cheese, mozzarella cheese sticks, almonds, cashews, nut butters, carrot sticks, raw vegetables.
