Deficiencies in complaint and Recall Handling cause warning letter. Due to deficiencies in the investigation of the scope and root causes of reported complaints, the U.S. Food and Drug Administration (FDA) has issued a warning letter to an Indian drug manufacturer. The FDA inspection already took place in December of 2016. Because of the non-satisfactory reply in regards to the inspection findings by the manufacturer, the FDA issued the warning letter in August 2017. The FDA inspectors noticed that investigations into process deviations and out-of-specification (OOS) laboratory results were insufficient, and did not include scientifically-supported conclusions. According to FDA the manufacturer´s response to this observation was inadequate because, amongst others, it failed to fully investigate the scope and root causes of a reported complaint. The FDA now expects the manufacturer to retrospectively review all complaint, manufacturing, and laboratory investigations associated with each product that is produced for the U.S. market. Furthermore, a plan to ensure that all future investigations are thorough, scientifically sound, and result in appropriate and effective CAPA (corrective actions and preventive actions) should be provided. Another major deficiency was that the firm failed to follow the company´s own written procedures regarding the recall of failing products (for example: after receiving a complaint sample the defect has been confirmed, but a product recall as directed by company´s own procedures regarding recalls of defective products has not been initiated in time). Therefore, the FDA demands to provide a list and summary explanation for all other instances in which product(s) distributed within the last five years failed to meet established specifications, but for which actions prescribed in the firm´s Quality System Manual and recall procedure have not been taken. Additionally, the planned CAPA for each such instance should be provided including an explanation of CAPAs for ensuring to follow own procedures regarding product quality and recalls. In addition the FDA observed deficiencies in cleaning operations. The authority demands sampling procedures and analytical methods used to test residues found during the inspection, including validation protocols and validation reports. Furthermore, a plan is required to ensure that personnel responsible for cleaning, verifying equipment cleanliness, and maintaining equipment are appropriately trained and capable of performing their assigned duties. An overall assessment of the adequacy of your cleaning program for all equipment, with special emphasis on difficult-to-clean parts should be provided. Another deficiency was found regarding Process Controls. The FDA observed that the firm "does not have an adequate ongoing program for monitoring process control to ensure stable manufacturing operations and consistent drug quality". FDA’s guidance document, Process Validation: General Principles and Practices, for general principles and approaches that FDA considers appropriate elements of process validation, should be followed.

Various of medicine production line STORYLINE China launched a nationwide drug recall system on Wednesday, shifting responsibility to companies to recall harmful drugs, a day after US and Chinese officials signed an agreement on the safety of drugs and medical devices. The recall plan will place Chinese-made drugs and imported drugs in three classes according to their possible danger to people's health, and each will have to be recalled within different timeframes, ranging from 24 hours for the most serious drugs, to 72 hours for those deemed less life-threatening. As the global pharmaceutical industry increasingly uses drugs either made or containing ingredients sourced in China, China is working hard to restore confidence in its manufacturing capabilities. This is especially true after a series of products ranging from toys to toothpaste to dog food were found to contain poisonous products earlier in the summer. The new recall rule puts the onus on companies to have systems in place to recall bad drugs within the set time period and take responsibility to alert the government of any noticeable bad reactions on patients that are caused by their drugs. Announcing the recalls State Food and Drug Administration Deputy Director Yan Jiangying told reporters the government hoped to cooperate with drug companies to establish an effective system. Companies are also encouraged to carry out voluntary recalls, and may be excused punishment if they do this. Drug distributors and medical institutes must also notify authorities of any safety risk to a particular drug. Unlike in the US where drug recalls by companies are voluntary, the Chinese government can also order a compulsory recall of bad drugs. A fine three times the total value of the recalled drugs will also be levied on the company who did not voluntarily recall the drug. The recall doesn't deal with counterfeit drugs, however, as these are covered under a different law. Under the agreement made with the US on Tuesday, China's State Food and Drug Administration will require firms manufacturing specific drugs and medical devices to register with that agency. (CNN) -- A positive test for salmonella has caused Kroger to recall four of its seasonings. The grocery store chain is recalling Kroger Ground Cinnamon, Kroger Garlic Powder, Kroger Coarse Ground Black Pepper and Kroger Bac'n Buds. A sample of Kroger Garlic Powder, from a South Carolina store, tested positive for salmonella contamination in a Food and Drug Administration test. There have been no reports of illnesses associated with the seasonings, but Kroger said in a statement that it is recalling them this week as a precaution. Kroger has removed the products from its shelves and asks that customers who have bought the seasonings not eat them and return them to a store for a full refund or replacement.
