Lung cancer, also known as carcinoma of the lung or pulmonary carcinoma, is a malignant lung tumor characterized by uncontrolled cell growth in tissues of the lung. If left untreated, this growth can spread beyond the lung by process of metastasis into nearby tissue or other parts of the body. The vast majority (80–90%) of cases of lung cancer are due to long-term exposure to tobacco smoke. About 10–15% of cases occur in people who have never smoked. These cases are often caused by a combination of genetic factors and exposure to radon gas, asbestos, or other forms of air pollution, including second-hand smoke. Worldwide, lung cancer is the most common cancer among men in terms of both incidence and mortality, and among women has the third highest incidence, and is second after breast cancer in mortality. Learn the signs of lung cancer to catch the disease early and ensure a positive outcome. Below are some early signs of lung cancer according to healthline.com. No Warning Lung cancer may not produce any noticeable symptoms in the early stages. The deadliest form of cancer in the U.S., involving cells that originate in the bronchi or lung. The term includes four cell types: squamous cell (epidermoid) carcinoma, adenocarcinoma, large cell (anaplastic) cancer, and small cell (oat cell) cancer. INCIDENCE In 2013, the National Cancer Institute (NCI) estimated that more than 228,000 people would be diagnosed with lung cancer and that about 159,500 would die of the disease. It is the second most common cancer in both men and women. CAUSES The vast majority are caused by carcinogens in tobacco smoke, including secondhand smoke. Other risks include exposure to carcinogenic industrial and air pollutants (asbestos, uranium, arsenic, nickel, chromium, iron oxides, coal dust, and radioactive dusts), concentrations of radon gas, and familial susceptibility. SYMPTOMS AND SIGNS Symptoms may include persistent cough, chest pain, dyspnea, weight loss, or hemoptysis. Changes in normal respiratory patterns or hoarseness, as well as pneumonia, bronchitis, epigastric pain, symptoms of brain metastasis, arm or shoulder pain, or swelling of the upper body are other possible signs. DIAGNOSIS Diagnostic tests include a chest x-ray, computed tomography (CT) scan, cytological sputum analysis, and bronchoscopy. Other tests include needle biopsy, magnetic resonance imaging (MRI), thoracentesis, thoracotomy, mediastinoscopy, or pulmonary function tests. TREATMENT Treatment includes lung surgery, radiation therapy, and chemotherapy (including epidermal growth factor receptor blockers) often provided serially or in combination. PROGNOSIS People with localized lung cancer have an approx. 5-year survival of 50%. People with widely metastatic lung cancer have a 5-year survival of less than 5%. PATIENT CARE Staging determines the extent of the disease and aids in planning treatment and predicting the prognosis. Lung cancer is relatively difficult to cure but much easier to prevent. Children and adolescents should be discouraged from smoking tobacco products, and current smokers should be assisted in their efforts to quit, e.g., through referrals to local branches of the American Cancer Society (ACS), smoking-cessation programs, individual counseling, or group therapy. SCREENING AND PUBLIC HEALTH Chest x-rays do not show small, early cancers, but computed tomography (CT) scanning can be used to screen people who have a long history of smoking and evidence of emphysema. In this high-risk group, screening detects the disease in its early stages when it is most likely to be curable. Smokers over the age of 55 who undergo annual lung cancer screening with low-dose CT scans have improved survival relative to those who do not. However, the public health costs of mass screening are high compared with the cost of encouraging smokers to quit or teaching teenagers not to start smoking. We will be reviewing Epidemiology, Pathology Pathophysiology, Clinical Signs and Symptoms, Investigations and Treatment/Management of Lung Cancer. EPIDEMIOLOGY OF LUNG CANCER Adenocarcinoma is the most common right now and is the most common. It is the third most common cancer and is the most common cause of cancer death. RISK FACTORS OF LUNG CANCER Smoking, Asbestos, Radiation, Arsenic Chromium, Nickel. Genetics, Scar Formation (Wegeners, Fibrosis, Scleroderma) Air pollution, HIV. CLINICAL SIGNS AND SYMPTOMS OF LUNG CANCER Pulmonary Symptoms, most common is cough, Hemoptysis (which ca lead to asphyxia) Chest Pain (dull achy), Dyspnea (secondary to pneumonia, atelectasis, lymphatic tumor spread, pleural effusion and pneumothorax), hoarsenes (when laryngeal nerve involved), pleural effusion (serous, serosangiounous, bloody) which may be caused by metastasis into pleural fluid. Super Vena Cava Syndrome occurs when central tumor invades SVC which causes red, edematous face, prominent veins throughout the body and this is more common in small cell lung cancer. Pancoast tumor is a lung cancer located in the apex of the lungs. Pancoast tumor may lead to thoracic outlet syndrome (pain weakness in arm and hand). Horner Syndrome causes miosis, anhydrosis and ptosis (MAP) and this is more common in Non Small Cell Lung Cancer. Paraneoplastic syndromes are due to hypercalcemia (PTHrP) bone mets, calcitriol release, cytokine. SIADH causes increase water absorption leading to dilution hyponatremia, cerebral edema, anorexia, nausea vomiting. Resolves with chemo and treatment. Lambert Eaton Myasthenic Syndrome (LEMS) is a condition with auto-antibodies of the calcium channels in the Neuromuscular Junction. Proximal Muscle weakness and rising and standing. Hematologic derangements such as anemia, leukocytosis, thrombocytosis and hypercoagulability. Hypertrophic osteoarthropathy may lead to digital clubbing, periositis, Arthralgia, Cushings due to release of Ectopic ACTH. Metastasis symptoms occur when metastasis of lung cancer to liver, adrenal gland, Lung, Spine, Brain. Screen with PET-CT. INVESTIGATIONS FOR LUNG CANCER X-Ray may show a coin lesion. First step in management is to look at a previous x-ray and look for a sign of lung cancer on the old x-ray. Cytology of the sputum, bronchoscopy, and percutaneous. CT Scan and PET CT Small Cell Lung Cancer Oat cells look like small cells with little cytoplasm. All are of neuroendocrine origin (feyrter cells) on bronchus. Small Cell Lung Cancer is found central and has limited stage (Lung and 1 lymph node) Extended stage is spread beyond the lungs. Usually has early metastasis. May release ACTH, SIAD, Lambert eaton myasthenic syndrome. Adenocarcinoma Mucin and TTF-1 Positive type of Lung Cancer. Graded from low grade to high grade. Bronchoalverolar carcionma is in situ carcinoma and has better prognosis. Most common in Non-Smokers (BAC) Squamous Cell Lung Cancer Irritation of bronchus causing metaplasia, then dysplasia, then carcinoma in situ and finally lung cancer. Also associated with Keratin pearls and cavitation from central necrosis. Goes into hilar nodes and gets late metastasis. Asymptomatic and can be deteetd with sputum cytology. Large Cell Lung Cancer Lots of cytoplasm, unknown origin and is a waste basket column. Peripheral and has late metastasis and can use cancer. TREATMENT OF LUNG CANCER Small Cell Lung Cancer metastasizes early on and therefore surgery is not an option. Cisplatin, Carboplatin and vincristin is also used. Responds very well to chemotherapy. Prophylactic cranial irradiation (PCI) to prevent brain metastasis. Surgery is possible if the tumor is very small and no metastsis Non Small Cell Lung Cancer is not responsive to chemotherapy or radiotherapy so surgery is the only option.

Lung cancer is a very common illness and it is oftentimes seen in people who abuse cigarettes or other forms of tobacco. So when a person has a new cough, blood in their sputum, unexplained weight loss, or changed in their voice, they need to inform the patient of this occurrence and the reason for that is, is that lung cancer is a very difficult disease to screen for. Well, there may be data that certain patients can benefit from Annual CAT scan screening, the majority of patients will develop their cancer in between even a yearly screening and some new symptoms mandate an evaluation. The other thing that is important to remember is that the symptoms may not seem to be related to your lungs. You may suddenly experience a lack of appetite, unexplained weight loss, new pain or swelling in an area that you do not associate with your chest. So if you have a change in your physical health and are a smoker, in particular, you should seek an evaluation with a physician. David S. If you've noticed an uptick in your frequency of illnesses like cold, flu, bronchitis, or pneumonia, you should talk to your doctor about the possible causes of your depressed immune system. Dr. Kelley says that when cancer settles in lung tissue and bronchial tubes, the lungs become more susceptible to illness. 2. Unexplained weight loss. If you've lost interest in food or feel full quickly after beginning to eat, lung cancer may be taking a toll on your appetite.
