Dr. Mary Frances Pilcher, a dermatologist on the medical staff of Lee Health, says it can be difficult for patients to tell the difference. “It’s very hard to tell sun spots from melanoma, so that is the reason it’s important to come get checked.” Skin cancer can appear as spots that are raised, flat, white, or black. Doctors say if the spots become bigger, painful, or itch, it’s time to see your doctor. “It’s very important to also remember that you hear about melanoma, the brown black spots, but non melanoma skin cancer is far more common and those can actually look like pimples or bug bites,” said Dr. Pilcher. After age 30, patients should not develop new moles or spots that don’t go away. “Anything that’s been there, that’s new, that hasn’t gone away after about a month should be checked,” said Dr. Pilcher. Skin cancer is any kind of tumor that originates in the skin---and there are many different types. “Basal cell skin cancer typically does not spread. They can be locally aggressive. Squamous cell skin cancer, which is the second most common type of skin cancer, can spread to other parts of the body. It typically goes to the lymph nodes and lungs, and other places as well. Melanoma can go anywhere and it can be deadly,” said Dr. Pilcher. But if caught and diagnosed early, doctors say skin cancer is curable. “The risk is if you leave them too long. Most skin cancers that are the non-melanoma type can be cured simply by cutting them out. Melanoma if caught early can also be cured by cutting it out. It’s when they have sat there for a long time and they have had time to spread,” said Dr. Pilcher. About one in five people in the United States will develop some type of skin cancer in their life. DVA skin cancer management  There are many types of skin cancers including BCCs or basal cell cancers, SCC or squamous cell cancers as well as melanomas. Todays review is on Bowen disease or Intraepidermal Squamous Cell Carcinoma. This is a common non-invasive cancer of the skin affecting the epidermal layers of skin. IECs most frequently occur on sun-exposed areas and present as red, scaly and persistent patches. IECs may arise de-novo, or from lesions of solar keratosis. If left untreated, Bowen disease may progress to an invasive Squamous Cell Carcinoma. This is one of the most common skin cancers I see in DVA or Department of Defence patients in Australia, and is due to the high UV radiation we have in cities like Brisbane.  Clinical Features Bowen disease most frequently presents as a persistent red scaly patch that slow grows over time. The most commonly affect areas include the legs, arms, and face- areas of sun exposure. Bowen disease or IECs can also affect the lips, oral mucosa, or even genital areas. As Bowen Disease is asymptomatic there is often a delay in the diagnosis, as it may mimic conditions such as dermatitis, psoriasis, tinea, and sun-spots.  Associations Most cases of Bowen disease arise from sun-damaged skin, including areas of actinic keratosis. The most common associations include- UV radiation. This plays a major contributing factor in the development of IECs. Immunosuppression. Patients on certain immunosuppressive medications are more likely to develop IECs.  Arsenic exposure. This was a common cause of Bowen Disease in the past, as arsenic was frequently used in medicines during the 60s and 70s including Bells Asthma Mixture, sheep dip, and as pesticides. Arsenic induced IECs-Bowen disease is a rare event today, as occupational safety has increased. A time lag of 10-20 years is seen between exposure and cancer.  Human Papilloma virus infections. Other associations include genetic diseases, radiotherapy and skin lesions such as porokeratosis, and solar keratosis. Investigations Bowen’s Disease can be suspected clinically, however in the majority of cases, a biopsy should be performed to confirm the diagnosis.  Treatment  There are multiple ways to manage Bowens/IECs. Treatment modalities will depend on the location and size of the cancer, the number of lesions, the degree of functional impairment.  Each modality has both advantages and disadvantages.  Cryotherapy or liquid nitrogen  Liquid nitrogen can be an excellent method of treating IECs. Most cases will require a double –freeze cycle of cryotherapy. This treatment is not recommended for IECs below the knee, as poor healing may occur.  Creams  Imiquimod, or 5-fluorouracil are effective in the treatment of Bowen disease. Clinical studies have shown cure rates approaching 80-90%. Treatment appears to be most successful when these creams are applied for 6-16 weeks.   Curettage and Electrocautery When small patches of IECs are present they may be treated via curettage and cautery. A local anaesthetic is given prior to the procedure to prevent pain and discomfort.  Photodynamic therapy  Photodynamic therapy or PDT can be an excellent non-surgical method of treating skin cancers such as Bowen’s/ IEC’s. PDT uses  a photosensitive cream called 5-aminolevulinic acid or methyl aminolevulinate, that is applied to the skin cancer. The cancerous cells of Bowens/ IEC take up this chemical over a period of 3 hours. A low-level laser light then activates the chemical, and destroys the skin cancer cells. Patients will require two treatments spaced a week or two apart. DVA or the Department of Veteran Affairs in Brisbane, covers PDT for both sunspots on the face and also IECs on all areas. PDT can also be used for multiple or large IECs. The benefit of PDT is that it selectively targets neoplastic cells without affecting the surrounding epidermis. This results in improved cosmetic outcomes, and faster healing times.  Surgery  Surgery is the ideal method of treating IECs in certain areas including recurrent IEC or ones that dwell in hair follicles. Surgery has the highest cure rate for Bowens/ IECs. For the majority of patients, especially DVA patients in Australia, non surgical methods, including the use of photodynamic therapy, is the future. Dr Davin Lim, Dermatologist. Basal cell carcinoma is the most common of all skin cancers, and for most people, it is little more than a nuisance. "They have found different spots, mostly on my arms and on my back, from being out in the sun when I was younger," says Bob Davies. Davies has had several close encounters with basal cell. But he's always come away with a clean bill of health. Although it rarely metastasizes, the cancerous lesions can spread. "Although they have a very small risk of metastasizing, which means moving to another part of the body, they can get invasive. And they can get deeper and bigger," says Dr. Lowell Hart, oncologist and hematologist on Lee Memorial Health System's medical staff. The standard in care is to have a skin cancer treated by a dermatologist. Someone with an aggressive basal cell likely faces additional treatments; a more traditional protocol used to destroy cancerous tumors. "Sometimes radiation therapy can be of help because they tend to be sensitive to radiation. So the main stage of treatment for the early ones is surgical removal and sometimes the radiation therapy," says Dr. Hart. Basal cell also tends to be more disfiguring and problematic if it's left untreated for many years. Basal cell often appears as a waxy bump and is most common on the face, neck and arms. "People are living longer and the longer you live, you've had more time for these mutations and cancers to develop," says Dr. Hart. The take away Davies learned is by dealing with skin abnormalities when they are a small inconvenience, they're less likely to turn into a real pain. "It hurts a little tiny bit, like getting a needle. But it's very quick and very fast and it's the safe thing to do," says Davies. There are several factors to consider before choosing the best treatment modality. Options depend on the location, histologic type and patient concerns. Nodular and superficial BCCs are the least aggressive and can be completely removed by electrodesiccation and curettage or by simple surgical excision. Superficial BCC can be treated with every other day application of imiquimod cream. This immune modulating agent induces an intense inflammatory reaction and the tumor clears without scarring. The histologic helps to determine therapy. The micronodular, infiltrative, and morpheaform BCCs have a higher incidence of positive tumor margins after excision and have the greatest recurrence rate. Clinically, BCCs with these patterns have poorly defined borders and are not apparent during physical examination. They subtly extend into surrounding tissue and are easily missed by blind treatment techniques such as surgical excision. These tumors need more aggressive treatment with wide excision or microscopically controlled surgery. Tumor size is important consideration. Electrodesiccation and curettage afford excellent results for small (less than 2 cm) nodular BCCs located on the forehead and cheeks. Nodular BCCs on the forehead and cheek that are larger and have well-defined margins should be excised and closed; electrosurgery for large tumors may result in large, unsightly scars. The margins of sclerosing BCCs cannot be determined by inspection, and either excision or, preferably, Moh’s micrographic surgery should be performed. The location of the tumor must also be considered when making a decision about treatment. Tumors about the nose, eye, and ear require special consideration. Lesions of the nose greater than 1 cm, lesions of the margin of the eyelid and the vermilion border of the lip, lesions involving cartilage, and sclerosing epitheliomas respond poorly to electrodesiccation and curettage. BCCs of the medial canthus are particularly dangerous. The skin rests close to bone and cartilage, and tumor cells initially invade and proceed to migrate undetected along periosteum or perichondrium. Healing occurs over inadequately treated tumors, and deep invasion and lateral extension can remain undetected, resulting in a tumor of massive proportions. Extension to the eye and brain is possible.

It often begins after age 50 and can occur in normal skin or in a burned or injured area. One of the best ways to prevent skin cancer is to avoid the sun’s strongest rays, basically, from 10 a.m. to 4 p.m., You should also use a sunscreen daily that has a sun protection factor (SPF) of 15 or higher. Other preventative methods include wearing sunglasses and wide-brimmed hats. Children should be protected with sunscreen and kept out of the midday sun as well. I usuually post wildlife videos, but I was recently diagnosed with Melanoma skin cancer stage one. I wanted to document the stages of the disease and treatment and hopefully the revocery. This was on the back of my arm. I hope this might help educate people about the disease and get to understand you must use sunscreen when out in the sun. I will continue to post my wildlife videos, but I thought this was important to get the message out about the horrible disease. Mine was caught early and I only have an ugly scar, others were not so lucky and lost their life.

In the U.S., the risk for melanoma is now 1 in 58. This is up from 1 in 1500 in 1930. If melanoma is diagnosed early, the chance for cure approaches 100%; however, for widespread metastatic melanoma, survival is only 6 to 9 months. What you don't know can harm you or prove fatal. Avoid being a statistic and learn the warning signs and causes of this potentially curable form of skin cancer.Key points Typically Appear in sun exposed areas of skin tend to be less than 6 mm in diameter, have an orderly, homogenous surface with a symmetrical, sharply demarcated border, round or oval shape, and have a light to dark brown color. Are categorized as junctional, compound, or intradermal based upon their location in the epidermis or dermis Begin to appear within the first 6 months of life, most are present by the age of 30, and slowly regress with age. Most Nevi are benign, however atypical MN or a large number of nevi should be examined and followed by a dermatologist Common Acquired Melanocytic Nevi, or moles, are clusters of melanocytes that develop primarily due to genetic factors. However, there is some evidence that they may be created by a stimulus, such as ultraviolet radiation, that causes a proliferation of melanocytes. Melanocytes are pigment (melanin) producing cells and are formed from neural crest cells that migrate to the skin, central nervous system, eyes, and ears during normal embryogenesis. Melanocytic Nevi are categorized by their location in the skin as junctional nevus (epidermis), intradermal nevus (dermis), or compound nevus (both dermis and epidermis). Melanocytes begin in the basal layer of the epidermis coming together to form junctional nevi. Over time melanocytes may migrate down through the epidermis-dermis junction to the dermis forming compound and intradermal nevi respectively. Generally, Common Acquired Melanocytic Nevi first appear as brown papules or macules, smaller than 6mm in diameter. As we age, more nevi become visible and their appearance begins to change so that the nevi lose their pigmentation, and become more elevated and dome-shaped. Melanocytic nevi are generally light to dark brown, but may range in color from skin colored to jet black. Melanocytic Nevi appear in both men and women. Moles are more prevalent in light skinned people then dark skinned people. Darker skinned people tend to have MN appear on the palms, soles of their feet, and underneath their fingernails, while Light skinned people are more likely to have moles appear on their trunks and extremities. Melanocytic nevi first appear in early childhood and usually stop appearing by early adulthood. For this reason, new moles in middle or late adulthood should carry a higher suspicion for Melanoma. Most adults will have 10-30 moles on their body. Differential Diagnosis · Atypical Mole(Dysplastic nevus) · Melanoma, Malignant Melanoma · Congenital Nevi · Given their different appearances, Junctional, Intradermal, and Compound Nevi have their own differential diagnoses. Diagnosis Key points Clinical presentation is the most important aspect of diagnosis Histologic examination confirms diagnosis Diagnosis is primarily made by history and clinical presentation. When the clinical picture is murky, a complete excisional biopsy and histopathologic evaluation of the mole is made to rule out Melanoma. The histologic findings of small, uniform, symmetrical, well circumscribed nests of melanocytes are reassuring for melanocytic nevi. Treatment Key points In most cases moles do not require treatment Suspicious moles should be removed and examined Prevent skin damage from sun exposure Melanocytic Nevi are, by definition, benign and most moles remain benign throughout a person's lifetime. Therefore, most moles will never need to be treated. However, suspicion that a mole may be a Melanoma, a change in the size, shape, or pigmentation of the mole, chronic irritation, or cosmetic concerns are reasons that the melanocytic nevi may be removed via excisional biopsy. When this is performed, if possible, the entire lesion should be removed and undergo histologic evaluation to rule out malignancy. It is important for individuals who have a significant number of nevi to have counseling about the dangers of excessive sun exposure, skin protection, and to undergo periodic total skin inspections by a dermatologist.

So this episode is going to be about a trick for doing better self skin cancer exams. Generally speaking, in most medical issues, symmetry is a reliable indicator that something isn't dangerous. So, in doing a self skin exam, if you find a mole or growth that is symmetric, then it is probably normal and not cancerous or precancerous. And just to remind you, a growth is symmetrical if you can fold it down the middle and the two halves are identical. Just like when I fold this circle in half. Yet in all of medicine there is only one situation that I know of where symmetry actually is a bad sign and usually indicates that something is abnormal and in fact at very least, precancerous. That situation occurs when two moles occur symmetrically to each other on your body. So imagine a line drawn down the center of your body, which of course goes right through your belly button. If you have two moles on your stomach, both about equal distance to the left and right of your bellybutton, then there is a very high probability that both of those moles are precancerous and each have a 20 to 25% life time probability of becoming melanomas. They should be shown to a dermatologist and probably be removed. The bottom line is that moles which are symmetric to each other with respect to the midline of your body are usually very dangerous, precancerous moles which if not removed have an unacceptably high probability of becoming deadly melanomas, a cancer that sadly claims the life of one American every hour. This is a truly rare medical example of symmetry being very dangerous, so if you come across it, show it to your doctor.
