Oh, boy. I've got Stage IV prostate cancer. But the question wasn't 'What is it that I have?' The question was 'What am I going to do about it?' At Weill Cornell we have a very strong program in genitourinary cancers. That includes prostate cancer, bladder cancer, kidney cancer, and testicular cancer. We also have really across the board top line medical specialties that's all here within one roof. It's very important to be connected to a hospital, such as New York Presbyterian. We have a great faculty that's at our disposal. It's a multi-disciplinary team. Not only the medical oncologists, but we also work very closely with our radiation oncology colleagues, our urology surgeons, from pathologists to radiologists. So whenever we see a new patient, we present the patient at a tumor board. And at that tumor board there's the whole gamut of physicians involved in care. What makes us unique is our dedication to research. And when I say research, not only laboratory research within the cancer center, but clinical research. We have our own clinical trials. UK medics travel to Gaza to improve breast cancer care

October is Breast Cancer Awareness Month, an annual worldwide campaign to raise understanding of the importance of breast cancer treatment, education and research. Across the occupied Palestinian territory, Medical Aid for Palestinians (MAP) is working to improve breast cancer care and supports multi-disciplinary teams of UK specialist oncologists, surgeons, nurses, and radiologists to make regular visits to Gaza and the West Bank. Last month, a MAP-supported breast cancer mission travelled to Gaza. The team comprised of Dr Ashwini Sharma, Consultant Radiologist, and Dr Gerard O’Hare, Oncology Clinical Nurse Specialist. Based at three hospitals, Al Shifa, Rantisi and the Gaza European, they delivered training to local health workers caring for women affected by the disease, teaching them new techniques to improve outcomes and quality of life for breast cancer patients.

Bahrain Specialist Hospital hosted the 1st BSH ONCOLOGY UPDATE 2015 on February 7th 2015 at The Gulf Hotel Convention Centre between 9 am and 3.30 pm. The event was organized by The Bahrain Gastroenterology and Hepatology Centre, as well as the department of Oncology, Bahrain Specialist Hospital. The title of this year’s conference was “Colorectal Cancer Symposium”. The conference was chaired by Dr. Najah R Zayyani, Consultant in Internal Medicine and Gastroenterology, who heads The Bahrain Gastroenterology and Hepatology Centre. Dr. Prasan Durga, Consultant Medical Oncologist at Bahrain Specialist Hospital, was the organizing secretary. Colorectal cancer is one of the most common curable, and often preventable cancers in the world. The key to achieving a high cure rate is early and effective screening to detect early cancers. In the Middle East region, various peculiar factors such as strong family history and high prevalence of conditions such as inflammatory bowel disease lead to a high incidence of colorectal cancer. The conference was aimed to discuss the various aspects of screening, early detection and effective treatment of this highly curable cancer, but often fatally neglected cancer. Various eminent speakers from within and outside of Kingdom of Bahrain participated in the discussions, which was well attended by over 150 delegates from healthcare authorities from Ministry of Health, Bahrain Cancer Society, doctors from various hospitals in government and private sector in the Kingdom, pharma and allied health specialties, and national and international media . The American Academy of Continuing Medical Education had accredited the conference to provide 5 CME hours of credit. The conference was supported by Rashid Al Zayani Charity Fund, started in the memory of late Mr. Rashid Al Zayani, who had a strong desire and motivation to establish oncology service at Bahrain Specialist Hospital. The BSH ONCOLOGY UPDATE will henceforth be an annual event in his memory, focusing on different cancers every year.

Thyroid Cancer Treatment Study-Mayo Clinic

The drug pazopanib may help revolutionize the care of patients with metastatic, rapidly progressive differentiated thyroid cancers, say researchers at Mayo Clinic who are publishing findings of a phase II clinical trial in Lancet Oncology. The researchers studied 37 patients with the most aggressive form of this cancer — developing in less than 5 percent of patients with differentiated thyroid cancer — and found that about  half (18) patients had a long-lasting response to pazopanib. Of that group, 12 are still alive without disease progression. The median progression-free survival time was 11.7 months, with an overall survival rate of 81 percent at one year. The researchers say that, to their knowledge, these findings represent by far the highest response rate yet reported in such aggressive cases of differentiated thyroid cancer. They caution however, that this drug is not meant to be used in slow growing differentiated thyroid cancers and that they cannot assess the survival advantage pazopanib offers to the patients studied.  Determining survival benefit would require a randomized clinical trial testing the agent, which inhibits all three vascular endothelial growth factor (VEGF) receptors, compared to other treatments or a placebo. "In this group of patients, we would have expected the cancer to have progressed in everyone within six months, but instead the median time to progression was almost a year in response to pazopanib therapy," says Keith Bible, M.D., Ph.D., a medical oncologist and researcher who led the multicenter clinical trial, funded by the National Cancer Institute. Most of the patients treated were enrolled at Mayo Clinic campuses in Minnesota and Florida. But as encouraging as this response is, it does not come without the potential for significant side effects, Dr. Bible says. The drug dose used in 16 patients had to be lowered because side effects were judged by oncologists to become potentially threatening or debilitating, and two patients experienced significant bleeding. Further, although two patients died in association with preexisting disease while enrolled in the study, the agent could have contributed in some way, he says. "Further studies of pazopanib in advanced thyroid cancer remain ongoing at Mayo Clinic and associated cancer centers to continue to learn more about how best to use the drug in these cancers," Dr. Bible says. "Such clinical trials may also provide patients access to this drug, which otherwise may be unobtainable due to cost, given that it is not yet approved for use in thyroid cancers."  This research builds upon a preliminary Mayo Clinic study that was reported in 2009. Plans are now underway for a larger phase III clinical trial with aggressive differentiated thyroid cancer patients that will be centered in Europe, with some sites to be opened in the U.S.  The authors declare no conflicts of interest. The manufacturer of pazopanib (GlaxoSmithKline) did not provide funding or other material support to the researchers and did not have access to the data.

Video abstract of review paper "Clinical epidemiology of epithelial ovarian cancer in the UK" published in the open access journal International Journal of Women's Health by Doufekas and Olaitan. Abstract Epithelial ovarian cancer is the fifth commonest cancer among women and the leading cause of gynecological cancer death in the UK. Most women present with advanced disease, mainly because the nonspecific nature of the symptoms lead to diagnostic delays. Recent data have shown a fall in ovarian cancer mortality rates in the UK, but rates are still higher when compared to other European countries or the USA. In addition, surgeons in the UK achieve on average lower optimal surgical cytoreduction rates in patients with advanced ovarian cancer. Despite a wealth of information on epidemiological risk factors, the pathogenesis of epithelial ovarian cancer remains largely unknown. This review presents the most recent data on incidence, mortality, and survival for epithelial ovarian cancer in the UK. Time trends, trends by age, international comparisons, and regional variation in incidence, survival, and mortality are presented within the context of a major reorganization of cancer services that took place in the UK over 10 years ago. Centralization of cancer services has meant that women with ovarian cancer receive treatment in specialist Cancer Centers. New MRI scan redefining myeloma treatment

Rodney Benjamin suffers from myeloma - a type of bone marrow cancer that affects plasma cells in the blood and can spread across bones throughout the body. It is a largely incurable disease but can be controlled if monitored constantly. Benjamin was one of 26 myeloma patients who took part in a pilot study at the Royal Marsden Hospital in the UK to test a new scanning procedure. Researchers refer to it as a whole-body, diffusion-weighted MRI because it uses a measurement called the Apparent Diffusion Coefficient (ADC) to record how restricted water movement is within tissues. Doctors are not only confident it could help with their choice of treatment but that the scans could also significantly reduce patients' reliance on bone marrow biopsies. Professor Nandita deSouza of the Institute of Cancer Research at Royal Marsden Hospital says a bone biopsy is very painful: "Multiple bone biopsies are not something a patient would want to undergo. Also bone biopsies are limited in that you take a biopsy from a particular area of the bone and you cannot biopsy the whole skeleton. Having a scanning technique that's sensitive, images disease within the marrow and looks at the whole skeleton at the same time is something that's very desirable." According to the study, after scanning patients before and after treatment doctors were able to correctly identify whether they had responded to treatment in 86% of cases. Such success rates enable clinicians to make better-informed decisions, respond quickly, and change drugs therapies if they are not working. Rodney Benjamin, who was diagnosed with the disease 10 years ago, is holding on for the next development in the treatment of myeloma. Gene test finds which breast cancer patients can skip chemo

UNDATED (AP) -- Many women with early-stage breast cancer can skip chemotherapy without hurting their odds of beating the disease - good news from a major study that shows the value of a gene-activity test to gauge each patient's risk. The test accurately identified a group of women whose cancers are so likely to respond to hormone-blocking drugs that adding chemo would do little if any good while exposing them to side effects and other health risks. In the study, women who skipped chemo based on the test had less than a 1 percent chance of cancer recurring far away, such as the liver or lungs, within the next five years. "You can't do better than that," said the study leader, Dr. Joseph Sparano of Montefiore Medical Center in New York. An independent expert, Dr. Clifford Hudis of New York's Memorial Sloan Kettering Cancer Center, agreed. "There is really no chance that chemotherapy could make that number better," he said. Using the gene test "lets us focus our chemotherapy more on the higher risk patients who do benefit" and spare others the ordeal. The study was sponsored by the National Cancer Institute. Results were published online Monday by the New England Journal of Medicine and discussed at the European Cancer Congress in Vienna. The study involved the most common type of breast cancer - early stage, without spread to lymph nodes; hormone-positive, meaning the tumor's growth is fueled by estrogen or progesterone; and not the type that the drug Herceptin targets. Each year, more than 100,000 women in the United States alone are diagnosed with this. The usual treatment is surgery followed by years of a hormone-blocking drug. But many women also are urged to have chemo, to help kill any stray cancer cells that may have spread beyond the breast and could seed a new cancer later. Doctors know that most of these women don't need chemo but there are no great ways to tell who can safely skip it. A California company, Genomic Health Inc., has sold a test called Oncotype DX since 2004 to help gauge this risk. The test measures the activity of genes that control cell growth, and others that indicate a likely response to hormone therapy treatment. Past studies have looked at how women classified as low, intermediate or high risk by the test have fared. The new study is the first to assign women treatments based on their scores and track recurrence rates. Of the 10,253 women in the study, 16 percent were classified as low risk, 67 percent as intermediate and 17 percent as high risk for recurrence by the test. The high-risk group was given chemotherapy and hormone-blocking drugs. Women in the middle group were randomly assigned to get hormone therapy alone or to add chemo. Results on these groups are not yet ready - the study is continuing. But independent monitors recommended the results on the low-risk group be released, because it was clear that adding chemo would not improve their fate. After five years, about 99 percent had not relapsed, and 98 percent were alive. About 94 percent were free of any invasive cancer, including new cancers at other sites or in the opposite breast. "These patients who had low risk scores by Oncotype did extraordinarily well at five years," said Dr. Hope Rugo, a breast cancer specialist at the University of California, San Francisco, with no role in the study. "There is no chance that for these patients, that chemotherapy would have any benefit." Dr. Karen Beckerman, a New York City obstetrician diagnosed with breast cancer in 2011, said she was advised to have chemo but feared complications. A doctor suggested the gene test and she scored very low for recurrence risk. "I was convinced that there was no indication for chemotherapy. I was thrilled not to have to have it," and has been fine since then, she said. Mary Lou Smith, a breast cancer survivor and advocate who helped design the trial for ECOG, the Eastern Cooperative Oncology Group, which ran it, said she thought women "would be thrilled" to skip chemo. "Patients love the idea of a test" to help reduce uncertainty about treatment, she said. "I've had chemotherapy. It's not pretty." The test costs ,175, which Medicare and many insurers cover. Others besides Oncotype DX also are on the market, and Hudis said he hopes the new study will encourage more, to compete on price and accuracy.
