In addition, sores and cracks may occur spontaneously. Atrophic vaginitis is one possible cause of postmenopausal bleeding (PMB). Urinary symptoms include painful urination, blood in the urine, increased frequency of urination, incontinence, and increased likelihood and occurrence of infections. A large number of postmenopausal women (who are not using topical estrogen) have at least some degree of vaginal atrophy; however, many women do not actively ask that medical attention be paid to this, possibly because it is naturally caused, or because of the taboo that still exists surrounding aging and sexuality. The cause of vaginal atrophy is usually the normal decrease in estrogen as a result of menopause. Other causes of decreased estrogen levels are decreased ovarian functioning due to radiation therapy or chemotherapy, immune disorder, removal of the ovaries, entering the post-partum period, and lactation. Various medications can also cause or contribute to vaginal atrophy, including tamoxifen (Nolvadex), danazol (Danocrine), medroxyprogesterone acetate (Depo-Provera), leuprolide (Lupron), and nafarelin acetate (Synarel). Vaginal atrophy can also be idiopathic. Use of vaginally administered estrogens (including vaginal tablets or cream) is appropriate before the condition becomes severe. Regular sexual activity may be helpful. A water-soluble vaginal lubricant can be helpful in mild cases. Increasingly, vaginally administered estrogens based on low dose of estriol are used to stimulate the vaginal epithelium proliferation. There is growing evidence to support the use of both Fractional Erbium and Fractional CO2 laser therapy, both have proven to be an effective treatment strategy, especially for patients such as cancer survivors for whom vaginal estrogen is not always an option. The characteristic of both Erbium and CO2 laser wavelengths is that they are highly absorbed within water. It is the water within the sub mucosa that is targeted by the laser. The hypothesised mode of action for Erbium laser is that through selectively heating the submucosa a process of neocollagenesis and neo vascularisation occurs. This can lead to an improvement of the blood flow and overall health of the treated area. Treatments take approximately 20 minutes and can be performed within an outpatient setting.

Hi. I’m Steve Goldring, the Hormone Pharmacist and I’m here to talk with you about Dyspareunia and how Estriol can help with menopause intimacy issues. As I mentioned in my last video, menopause causes changes in vaginal tissue that make intimacy uncomfortable. One study showed 44% of menopausal women saying intimacy was painful, that’s called Dyspareunia. Even women who weren’t sexually active complained of vaginal dryness, itching and irritation after menopause. In my last video, I went over some FDA approved estradiol products that reverse vaginal menopause symptoms. Another option, vaginal Estriol, also called E3, is available in the U.S. from compounding pharmacies. Estriol is one of the weakest bio-identical estrogens, exactly the same as an estrogen your body makes. Estriol has been prescribed as Ovestin® oral tablets or vaginal cream to treat vaginal menopause symptoms in Europe, Australia and the U.K. For over 20 years. In the U.S., a compounding pharmacist can make up estriol prescribed by your doctor in a vaginal suppository, troche or cream. Because it’s not FDA approved, estriol is covered by only a few insurance companies. You’ll pay around $50 to $70 for a vaginal estriol prescription, but that’s at least $100 less than most of the commercially available vaginal products. Estriol cream or suppositories are inserted vaginally once a day at bedtime, every night for a couple of weeks. After that, the dose is cut back to 2-3 times weekly. This regimen helps to build up vaginal tissue and reverse vaginal atrophy. Less frequent estriol doses help maintain that healthy vaginal tissue. Since Estriol is a weaker estrogen, it’s not used as often by itself for hot flashes, but it can really help with vaginal issues. Talk with your doctor about the possibility of getting a prescription for estriol from a specialty compounding pharmacy. There are some promising clinical trials going on with vaginal DHEA that I’ll tell you about in my next video, Part 3 in this series on treatment of vaginal menopause symptoms. Vulvavaginal dryness is very common and most women will experience some degree of vulvavaginal dryness in their lifetime - probably up to 70% of post-menopausal women and up to 15% of premenopausal women. Vulvavaginal dryness is a condition whereby the external genitalia and the vagina become dry, thin, pale, fragile, and actually becomes vulnerable to trauma, infection and even bleeding. The most common cause of vulvavaginal dryness is low estrogen. At any stage of a woman's life. Estrogen, if you will, is the life force of the vagina and the vulva. It promotes the production of glycogen, which is a substance that the good bacteria, Lactobacilli, thrives on. And which in turn keeps the vagina in a healthy vaginal pH. It also helps promote the production of mucus, the increase blood flow, it makes it elastic. When the estrogen levels are low in a woman -- usually in the postmenopausal stage but it can happen at other stages of life -- all those good things become reverse. And what happens is, you see these nice little ridges here in the vagina, those are called rugae -- they flatten out and the vaginal becomes thin, the blood flow decreases and the vagina becomes pale, the tissues lose the glycogen that I spoke of, and the good bacteria aren't there, the pH goes up and women lose lubrication and moisture. The same with the vulva. The vulva has estrogen receptors as well. And the estrogen in the vulva keeps the skin -- like skin in your body -- it keeps it very healthy. It increases collagen in the skin, subcutaneous fat. And when the estrogen levels are low, these qualities are diminished so women notice that there is thinning of the tissues, hair loss, and just very ...vulnerable! The tissue becomes very fragile. The symptoms of vulvavaginal dryness and which eventually, with long-term low estrogen can lead to atrophy are varied but some women present to me just with vaginal itching. Or they're very dry and don't lubricate during sex. And this has a tremendous impact on female sexual function. In fact the leading cause for Female Arousal Disorder is vaginal atrophy. And it can impact one's self image, one's self-esteem and overall quality of life. But vulvovaginal dryness doesn't just affect older women. 15% of premenopausal or reproductive aged-women also suffer with vulvavaginal dryness. The important thing is that women address this problem. Over 60% of women are afraid to address it and many providers fail to address it as well. I've had patients come in and until someone asks I say, "How are you doing? How's your relationship? Are you sexually active?" And one patient recently said to me. "Oh Doc! I am dry as sand down there!" Which really says it all. Vulvavaginal dryness can make women feel old. What are other causes for vulvavaginal dryness? It isn't just menopause. Women can have low Estrogen at various stages of their life. They can be breast feeding, they could have just had a baby where all the hormones have gone way down after being pregnant, they can be on anti-breast cancer medications that are anti-estrogens. They can be on Chemotherapy. They could have been exposed to pelvic radiation for pelvic cancers. Certain medications can cause dryness -- even nonspecific antihistamines. There are some birth control pills that are only Progesterone pills or Progesterone birth control and they don't have estrogen and I've noticed over time, that women are on these a long time, they know that they have decreased lubrication and suffer with some vaginal dryness. There are even some lifestyle factors that come into play that can aggravate dryness: Smoking, excess alcohol, lack of sexual activity and even obesity can impact your vulvovaginal area. I encourage women to speak to their doctors or their gyn providers about vulvavaginal dryness and if long term left unchecked it becomes atrophy. There are numerous remedies for this. We have are over-the-counter lubricants that can aid women during sexual activity and help with lubrication. There're also moisturizers that they can use externally and then there's also a number of localized estrogen therapies, which work very effectively because if you're lacking estrogen and you give it back locally, in that area, it works wonders to restore vulvovaginal health. And these come in many, many forms. We have rings and creams and tablets...So please, this is not a condition that women should suffer in silence. I’m Steve Goldring, the Hormone Pharmacist and I’m here to talk with you about Dyspareunia and how the hormone DHEA can help with menopause intimacy issues. As I mentioned in my last video, the hormone estriol has been used for years in Europe, to help relieve vaginal atrophy caused by the loss of hormones at menopause. Canadian endocrinology researcher, Dr. Fernand LaBrie, is conducting clinical trials using DHEA in menopausal women with vaginal atrophy. DHEA is a hormone that’s converted within the tissues into estrogen or testosterone when they’re needed. Dr. La Brie has published groundbreaking research in “Intracrinology,” this study of how hormones work within the cells. His research shows that DHEA, when given as a vaginal suppository, dramatically improves vaginal health, reduces painful intimacy and helps vaginal lubrication. Women receiving vaginal DHEA showed no increases in the levels of DHEA or other sex hormones in their overall circulation. Patients enrolled in the trials reported very mild, infrequent side effects. DHEA is in Phase 3 Clinical Trials, which means drug manufacturers are hoping to have a vaginal DHEA product on the U.S. Market in the next couple of years, pending FDA approval. DHEA is currently classified as a nutritional supplement by the FDA. It can also be compounded by a specialized pharmacy into a vaginal suppository or a cream. Education http://perimenopausalbleeding.mymenopauseonline.com/ Many women arrive at their menopause transition years without knowing anything about what they might expect, or when or how the process might happen, and how long it might take. Very often a woman has not been informed in any way about this stage of life; at least in the US, it may often be the case that she has received no information from her physician, or from her older female family members, or from her social group. In the US, there appears to be a lingering taboo which hangs over this subject. As a result, a woman who happens to undergo a strong perimenopause with a large number of different effects, may become confused and anxious, fearing that something abnormal is happening to her. There is a strong need for more information and more education on this subject.  Other therapies * Lack of lubrication is a common problem during and after perimenopause. Vaginal moisturizers can help women with overall dryness, and lubricants can help with lubrication difficulties that may be present during intercourse. It is worth pointing out that moisturizers and lubricants are different products for different issues: some women feel unpleasantly dry all of the time apart from during sex, and they may do better with moisturizers all of the time. This applies both to women that have suffered bone loss but have not yet suffered fractures, and women that have suffered both bone loss and fractures.
