This is the tube that moves food from your throat to your stomach. After it is removed, the esophagus is rebuilt from part of your stomach or part of your large intestine. Most of the time, esophagectomy is done to treat cancer of the esophagus. Esophagectomy is a very complex operation that can take between 4 and 8 hours to perform. It is best done exclusively by doctors who specialise in upper gastrointestinal surgery. Anesthesia for an esophagectomy is also complex, owing to the problems with managing the patient's airway and lung function during the operation. Lung collapse is highly probable as well as losing function of diaphragm and possible injury of the spleen. Average mortality rates (deaths either in hospital or within 30 days of surgery) for the operation are around 10% in US hospitals. However, recognized major cancer hospitals typically report mortality rates under 5%. Major complications occur in 10-20% of patients, and some sort of complication (major and minor) occurs in 40%. Time in hospital is usually 1–2 weeks and recovery time 3–6 months. It is possible for the recovery time to take up to a year.

A new 3 years project funded through the MRCG-HRB Funding Scheme with support from Breakthrough Cancer Research. Oesophageal cancer (cancer of the food pipe) has low survival rates and a very poor response to treatment. Sadly, this cancer type is on the rise in Ireland and is linked with increasing obesity rates. Unlike many other cancer types, we are still only using treatments that have existed for decades – chemotherapy drugs with radiation treatment (CRT) to kill the cancer cells followed by surgery. However the tumours in a large percentage of patients (70%) do not respond. As most patients go through CRT for no benefit, whilst enduring significant side effects, it is really important for the majority of patients that we can make their tumours sensitive to treatment. Cancer cells do two things to prevent radiation from killing them, they (1) generate a lot of energy and (2) send out signals to trick your body, for example, they encourage blood vessels in your body to grow to the tumour so it can get more nutrients. This allows the tumour to survive instead of dying. We discovered a novel drug called CC8 that stops cancer cells doing these two things in oesophageal cancer cells and can increase response to treatment in resistant cells. In this grant, using patient tumour samples and in mouse studies, we will package our drug with tiny gold particles. This packaging will make our drug go to the tumour and target the powerhouse of the cells that provides the energy for the cancer cells to survive. We will see how well this new combination works in oesophageal cells in the lab, in patient tumour samples and in mice. This work will position our drug along a pathway where it could eventually be given to oesophageal patients alongside standard CRT to make tumours respond and result in better outcomes for these cancer patients with a dismal prognosis.

Research led by the University of Texas at Arlington reveals how zinc can target esophageal cancer cells and halt their growth, thereby bringing us closer to new prevention and treatment strategies for the disease. Zinc is a mineral present in a variety of foods - including oysters, red meat, poultry, beans, nuts, and whole grains - and it is also available as a dietary supplement. Zinc is one of the body's essential nutrients. Not only is it important for fetal and childhood development and sense of taste and smell, but the mineral also aids cell functioning, wound healing, and helps our immune system to stave off infection. Previous studies have identified zinc deficiency in patients with esophageal csancer, indicating that the mineral may also help to protect against the disease. However, the molecular mechanisms underlying zinc's potential anti-cancer effects have been a mystery - until now. Zinc, calcium, and esophageal cancer Study leader Zui Pan, of the College of Nursing and Health Innovation at the University of Texas at Arlington, and colleagues found that zinc targets and blocks a specific calcium channel in esophageal cancer cells, preventing them from proliferating, or growing and dividing. Pan and colleagues recently reported their results in The FASEB Journal. For their study, the researchers assessed how human esophageal cancer cell lines and healthy human cells respond to zinc. They found that in esophageal cancer cells, the mineral inhibits a calcium channel called Orai1. This dampens excitable calcium signaling - a known contributor to cancer cell growth - and prevents cancer cell proliferation. Additionally, the team found that zinc did not target the calcium channel in healthy cell lines. "Our study," explains Pan, "for the first time to our knowledge, reveals that zinc impedes overactive calcium signals in cancer cells, which is absent in normal cells, and thus zinc selectively inhibits cancer cell growth." "It now appears that zinc and calcium can have a cross talk, meaning that they can be linked." A new treatment for esophageal cancer? According to the American Cancer Society, around 16,940 new cases of esophageal cancer will be diagnosed in the United States this year, and there will be around 15,690 deaths from the disease. Although esophageal cancer survival rates have improved recently, the prognosis remains poor when compared with many other cancers; only 20 percent of patients will survive for at least 5 years after being diagnosed with esophageal cancer. Such statistics emphasize the need for new therapies to prevent and treat esophageal cancer, and Pan and colleagues believe that their findings suggest that zinc supplementation could be a feasible option. In future studies, the team plans to find out more about the link between zinc and calcium signaling, and how zinc supplementation could be used to combat esophageal cancer. In the meantime, the researchers believe that their findings highlight the importance of including zinc as part of a healthful diet. The National Institutes of Health (NIH) recommend that men and women should aim to get around 11 milligrams and 8 milligrams of zinc, respectively, every day. Researchers at King Abdullah University of Science and Technology (KAUST), Saudi Arabia, and collaborators are now exploring a potential therapeutic option for such tumors that is based on the tumor-specific effects of zinc oxide (ZnO) nanoparticles. Previous studies have shown that ZnO treatment can kill cancerous cells while leaving healthy cells intact. However, the mechanism is poorly defined; some data suggest that ZnO particles are ingested by and dissolve within tumor cells, while other findings suggest that cells absorb free zinc released when these particles dissolve in the acidic tumor environment. Patients with esophageal cancer who refuse surgery when it is recommended are less likely to survive long term than similar groups of patients who undergo an operation, according to research presented today at the 54th Annual Meeting of The Society of Thoracic Surgeons. “Although it may be tempting for patients to opt for nonsurgical treatment for cardiothoracic diseases in order to avoid the perceived pain and complications of surgery, this choice may come with a price,” said senior author Brendon M. Stiles, MD, of Weill Cornell Medicine in New York, NY. “In our study, we show that even if patients recommended for surgery elect to pursue other treatments, they do more poorly than if they had included surgery as part of their treatment.” Dr. Stiles and colleagues from Weill Cornell Medicine queried the National Cancer Database for patients with esophageal cancer from 2004 to 2014. The researchers identified 18,549 patients, including 708 patients who were recommended for surgery but declined. Within this group, instead of surgery, 41% (292) of patients were treated with definitive chemoradiation, 36% (256) with sequential chemotherapy/radiation, 8.2% (58) with radiation and/or chemotherapy alone, and 14% (102) received no treatment. Propensity matching was performed to compare patients who refused surgery to those treated with preoperative therapy followed by surgery (525 in each group). Median survival was significantly better in the neoadjuvant group with surgery than in patients who refused an operation, 32 months vs. 22 months, respectively. The authors based their calculations on the records of almost 346,000 adults age 35 and older who died of these 12 cancer types in 2011. They used data from several sources including the 2011 National Health Interview Survey and the Cancer Prevention Study II. They estimate almost 168,000 of the deaths were attributable to smoking. Lung cancer had the largest percentage of deaths attributable to smoking with 80.2%, followed by larynx (voice box) cancer with 76.6%. About half the deaths from oral cavity/throat cancer, esophagus cancer, and bladder cancer were attributable to smoking. Percentages for the other cancer types studied ranged from 23.6% for liver cancer to 9.7% for colon and rectum cancer. More stop-smoking efforts needed The percentage of Americans who smoke cigarettes has decreased from 23.2% in 2000 to 18.1% in 2012, but smoking continues to cause a significant number of deaths from cancer. In 2015, an estimated 171,000 people will die from cancer that was caused by smoking. Race, Lower Socioeconomic Status Linked With Worse Survival Following Esophageal Cancer Surgery Researchers call for changes to reduce health care disparities Phoenix (January 26, 2016) – Poor black patients undergoing surgery for esophageal cancer are at higher risk for death than white patients and patients with higher socioeconomic status, according to a scientific presentation at the 52nd Annual Meeting of The Society of Thoracic Surgeons. “Prior institutional experience suggested that outcomes of some cancers might be based on socioeconomic status as well as race, so we developed this study to determine if the relationship existed in patients with esophageal cancer on a national scale,” said Loretta Erhunmwunsee, MD who led the study while at Duke Health. Dr. Erhunmwunsee is now with City of Hope in Duarte, Calif. Dr. Erhunmwunsee and colleagues used the National Cancer Data Base to gather information on 16,807 patients, of whom 6,147 were treated with esophagectomy for esophageal cancer (stages I-III) between 2003 and 2011. Patients were grouped using median income by zip code. Black patients made up 5% (293) of those who underwent esophagectomy, and 40.3% (2,476) of patients were in the lowest two income quartiles. Before adjusting for outside factors (age, sex, other health factors, etc.), black patients had worse overall survival compared to white patients (median survival was 33 vs. 46 months, respectively), and each lower income grouping was associated with progressively worse survival. “These results show that poorer patients are at a higher risk of death following surgery,” said Dr. Erhunmwunsee. “This disparity likely is based on many factors—minority patients and patients with lower socioeconomic status often smoke more, have poor eating habits, exercise less, and are exposed to environmental hazards, all of which lead to worse health. And specifically, increased smoking and poorer eating habits can increase the risk of esophageal cancer.” After adjusting for outside factors, both black race and lower income group remained significantly associated with worse overall survival; however, there was no difference in survival rates among black and white patients in the highest two income groups. A significant difference still remained between black and white patients in the lowest income groups (median survival was 26 vs. 40 months, respectively), indicating a racial and economic disparity. “Prior studies have shown that ethnic minority and poor patients who have access to care may still get inappropriate treatment; however, even when they do receive adequate therapy (surgery), as in this study, disparities are prevalent,” said Dr. Erhunmwunsee. “These finding are important to patients because being aware of the factors that are linked to higher death rates can empower patients to be more active with their own health and medical care. We also hope that awareness of the problem among physicians and patients alike will lead to more public and professional focus on solving this disparity through strategies aimed at health policy, practices of health systems, and training of health care providers.”

Don’t rely on routine tests alone to protect you from cancer. It’s just as important to listen to your body and notice anything that’s different, odd, or unexplainable. Here are some signs that are commonly overlooked: 1. Wheezing or shortness of breath One of the first signs many lung cancer patients remember noticing is the inability to catch their breath. 2. Chronic cough or chest pain Several types of cancer, including leukemia and lung tumors, can cause symptoms that mimic a bad cough or bronchitis. Some lung cancer patients report chest pain that extends up into the shoulder or down the arm. 3. Frequent fevers or infections These can be signs of leukemia, a cancer of the blood cells that starts in the bone marrow. Leukemia causes the marrow to produce abnormal white blood cells, sapping your body’s infection-fighting capabilities. 4. Difficulty swallowing Trouble swallowing is most commonly associated with esophageal or throat cancer, and is sometimes one of the first signs of lung cancer, too. 5. Swollen lymph nodes or lumps on the neck, underarm, or groin Enlarged lymph nodes indicate changes in the lymphatic system, which can be a sign of cancer. 6. Excessive bruising or bleeding that doesn’t stop This symptom usually suggests something abnormal happening with the platelets and red blood cells, which can be a sign of leukemia. Over time, leukemia cells crowd out red blood cells and platelets, impairing your blood’s ability to carry oxygen and clot. 7. Weakness and fatigue Generalized fatigue and weakness is a symptom of so many different kinds of cancer that you’ll need to look at it in combination with other symptoms. But any time you feel exhausted without explanation and it doesn’t respond to getting more sleep, talk to your doctor. 8. Bloating or abdominal weight gain Women diagnosed with ovarian cancer overwhelmingly report unexplained abdominal bloating that came on fairly suddenly and continued on and off over a long period of time. 9. Feeling full and unable to eat This is another tip-off to ovarian cancer; women say they have no appetite and can’t eat, even when they haven’t eaten for some time. 10. Pelvic or abdominal pain Pain and cramping in the pelvis and abdomen can go hand in hand with the bloating that often signals ovarian cancer. Leukemia can also cause abdominal pain resulting from an enlarged spleen. 11. Rectal bleeding or blood in stool This is a common result of diagnosing colorectal cancer. Blood in the toilet alone is reason to call your doctor and schedule a colonoscopy. 12. Unexplained weight loss Weight loss is an early sign of colon and other digestive cancers; it’s also a sign of cancer that’s spread to the liver, affecting your appetite and the ability of your body to rid itself of wastes. 13. Upset stomach or stomachache Stomach cramps or frequent upset stomachs may indicate colorectal cancer. 14. A red, sore, or swollen breast These symptoms can indicate inflammatory breast cancer. Call your doctor about any unexplained changes to your breasts. 15. Nipple changes One of the most common changes women remember noticing before being diagnosed with breast cancer is a nipple that began to appear flattened, inverted, or turned sideways. 16. Unusually heavy or painful periods or bleeding between periods Many women report this as the tip-off to endometrial or uterine cancer. Ask for a transvaginal ultrasound if you suspect something more than routine heavy periods. 17. Swelling of facial features Some patients with lung cancer report noticing puffiness, swelling, or redness in the face. Small cell lung tumors commonly block blood vessels in the chest, preventing blood from flowing freely from your head and face. 18. A sore or skin lump that doesn’t heal, becomes crusty, or bleeds easily Familiarize yourself with the different types of skin cancer —melanoma, basal cell carcinoma, and squamous cell carcinoma — and be vigilant about checking skin all over your body for odd-looking growths or spots.
