Your heart gets a lot of attention from poets, songwriters, and storytellers, but today Hank's gonna tell you how it really works. The heart’s ventricles, atria, and valves create a pump that maintains both high and low pressure to circulate blood from the heart to the body through your arteries, and bring it back to the heart through your veins. You'll also learn what your blood pressure measurements mean when we talk about systolic and diastolic blood pressure. Quick Tip: Blood / Fluid Loss

01:42
750 ml: 15% of blood volume There is typically no change in vital signs and fluid resuscitation is not usually necessary. 1500: 15-30% of total blood volume. A patient is often tachycardia with a narrowing of the difference between the systolic and diastolic blood pressures. Skin may start to look pale and be cool to the touch. 2000 ml : 30-40% of circulating blood volume. The patient's blood pressure drops, the heart rate increases, peripheral hypoperfusion (shock), such as capillary refill worsens, and the mental status worsens. 3000 ml: The limit of the body's compensation is reached and aggressive resuscitation is required to prevent death

My Vasovagal Syncope Episode During Blood Work

21:46
This time was not a success, but the time before was much better. Here's a link to that video for a happy ending: https://www.youtube.com/watch?v=cG88GLHHDRU Vasovagal syncope (vay-zoh-VAY-gul SING-kuh-pee) occurs when you faint because your body overreacts to certain triggers, such as the sight of blood or extreme emotional distress. It may also be called neurocardiogenic syncope. The vasovagal syncope trigger causes your heart rate and blood pressure to drop suddenly. That leads to reduced blood flow to your brain, causing you to briefly lose consciousness. Vasovagal syncope is usually harmless and requires no treatment. But it's possible you may injure yourself during a vasovagal syncope episode. Your doctor may recommend tests to rule out more serious causes of fainting, such as heart disorders. Symptoms Before you faint due to vasovagal syncope, you may experience some of the following: Pale skin Lightheadedness Tunnel vision — your field of vision narrows so that you see only what's in front of you Nausea Feeling warm A cold, clammy sweat Yawning Blurred vision During a vasovagal syncope episode, bystanders may notice: Jerky, abnormal movements A slow, weak pulse Dilated pupils Recovery after a vasovagal episode generally begins in less than a minute. However, if you stand up too soon after fainting — within about 15 to 30 minutes — you're at risk of fainting again. When to see a doctor Fainting can be a sign of a more serious condition, such as a heart or brain disorder. You may want to consult your doctor after a fainting spell, especially if you never had one before. Request an Appointment at Mayo Clinic Causes Vasovagal syncope occurs when the part of your nervous system that regulates heart rate and blood pressure malfunctions in response to a trigger, such as the sight of blood. Your heart rate slows, and the blood vessels in your legs widen (dilate.) This allows blood to pool in your legs, which lowers your blood pressure. Combined, the drop in blood pressure and slowed heart rate quickly reduce blood flow to your brain, and you faint. Sometimes there is no classical vasovagal syncope trigger, but common triggers include: Standing for long periods of time Heat exposure Seeing blood Having blood drawn Fear of bodily injury Straining Prevention You may not always be able to avoid a vasovagal syncope episode. If you feel like you might faint, lie down and lift your legs. This allows gravity to keep blood flowing to your brain. If you can't lie down, sit down and put your head between your knees until you feel better. Barrow County EMS firestation 1, Statham, GA Failure to Act - MARC LOFTON ARREST

24:47
KELLY PICKENS DIED OCTOBER 12, 2017. SHE WAS THE DRIVING FORCE IN THE PURSUIT FOR JUSTICE AGAINST MARC LOFTON, POLICE CHIEF ALLAN JOHNSTON, OFFICER JOHNNY WOOD, MAYOR ROBERT BRIDGES, AND MEMBERS OF CITY COUNCIL (PARTICULARLY PERRY BARTON) WHO RELENTLESSLY TRIED TO BULLY HER AND SLANDER HER NAME TO COVER THEIR GUILT AND WRONGDOINGS, AND CRIMES. HER LAWSUITS WILL GO ON AND HER FIGHT WILL BE CARRIED ON. Update: All four of the EMTs in the video have now been disciplined by Director Skinner. John Skinner, EMS Director Fire Station Headquarters 222 Pleasant Hill Church Road, NE Winder, GA 30680   This an official complaint and demand for discipline of 4 of your employees: Tommy Crowe, Mike Garner, TJ Gower, and Matt Austin.  The Barrow County EMS has illegally withheld their names for nearly 2 years.  This information was requested by open records request in 2015, and again last week.  The names and information were finally forwarded to me by your law firm on Friday.   A video of this incident can be seen at the following link, and you may obtain a copy of the full body cam video from Statham PD.    As you are probably aware, these 4 EMTs were involved in an incident with me in 2015.   I was falsely arrested for DUI-drugs less safe Aug 1, 2015. I was not intoxicated. My charges were later dismissed.  The officer who arrested me says he suspected me of taking drugs because my hands were shaking, my face was flushed and I was sweating.  I had driven less than 1/4 mile to a gas station to get a bottle of cold water after helping a friend load some furniture.  During his interrogation and after, I was begging for water.  He arrested me (no struggle or anything), put me in handcuffs and in the back of a hot car.  I told him I was overheating and having trouble breathing so he called Barrow County EMS to check me out. The fire station is only about 1/2 mile away in Statham, GA.   He opened the car door to let me sit sideways with my legs out the door. A few moments later, I passed out, fell out of the car and hit the ground face first. Just before this, the EMT truck pulled up, and they saw me fall out.  This is all on video.   Two of them pick me up off the ground and put me back into the hot car. They NEVER took my blood pressure, or pulse, or did any kind of real examination at all.  They never checked my head, after they witnessed me hit the ground head first when I passed out. They refused to give me water that I continued to beg for. I was having a heat stroke and tetany.   They asked what was wrong with me.  They asked Why are you rolling around like that on the floor? (parking lot).  I told them I have lyme disease.  I also have heart problems (but did not say that). One of them asked after I said lyme disease But do you have any REAL problems?  They asked about the medications I took.  I listed each medication.  They are anxiety medications.  It went down hill from there.  They started laughing and joking about me being "crazy."  They are on camera pantomiming slapping me.  One of them repeatedly says "She has acute incarceritus."    Tommy Crowe stands over me YELLING "YOU AREN'T GETTING ANY WATER!!!"  While they all have their backs to me, the video shows me faint a second time.  Eventually they take me to the fire station to draw my blood for the DUI arrest. Even at the fire station, they did not take my blood pressure or anything.  I was in jail for the next few hours until I was bailed out. I didn't get home until late that night, and I did not go to the hospital.  I was exhausted and just wanted to be in my own bed.   I have a history of heart problems, as well as degenerative disc disorder,  lyme disease and anxiety.  They neither assessed or took precautions with me regarding any of these issues. It was their duty to asses me medically, rather than standing around passing judgement and assuming I was fine. I wasn't.   As paid employees on duty with Barrow County EMS, each of these men had a duty to act.  They failed in their duty, individually and collectively, miserably.   I expect an internal investigation to be done and included in the permanent record of each of these men:   Tommy Crowe, Mike Garner, TJ Gower, and Matt Austin

How to Treat Someone Having a Stroke | First Aid Training

01:35
Watch more How to Give First Aid videos: http://www.howcast.com/videos/504458-How-to-Treat-Someone-Having-a-Stroke-First-Aid-Training A stroke is very serious and needs to be treated right away. You'll commonly see this in elderly people, but you can see it in all ages, in all walks of life. You want to diagnose it quickly, and you want to get help quickly. So what is a stroke? A stroke is a stoppage of blood to the brain. It can either be through an obstruction, which is like a blood clot in the brain, or it can be through hemorrhaging. Something is bleeding out, and you're not getting enough blood to the brain. So what will you see? You'll see numbness. You'll see tingling. You'll see vision disturbances. You'll see trouble speaking, trouble understanding speech, trouble walking, dizziness, confusion. Sometimes you'll see a sudden, severe headache that comes out of nowhere. What are some of the signs that a person displays? Here's what you can do. You can have the person smile. You can have the person stick out their tongue. You can have the person raise their arms. You can have the person speak to you. With a stroke, you're going to see one side crooked. Their tongue will be crooked. Their smile will be crooked. Sometimes they can't even raise one side of their face. They'll have trouble walking and maintaining balance. You want to get help right away. The faster you get medication, the faster you get treatment, the better the outcome of a stroke is. Call 911. Tell them, "I think my friend, I think my family member, I think someone's having a stroke." With a stroke, every minute counts. Every minute that you're in the house, every minute that you're in transit to the hospital, and every minute from when you get to the hospital to when you get to the stroke unit. The faster you can get them treatment, the better their recovery will be.

Thankfully we were still on land and the crew saw me carrying her. Right away the Disney crew took care of the situation! They gave us a wheelchair and called the paramedics. We found out Nellie's blood pressure was extremely low which never happens to her and they stayed with her for a while. We kept Nellie in a wheelchair throughout most of the day until she wanted to get up and walk a bit. She still regretted going on some of the rides and we're just glad she's doing better now! Drowning (Pathophysiology and Management) for USMLE

15:56
Drowning Pathophysiology and Management for USMLE made simple. Handwritten lecture presentation for medical students. Drowning is defined as submersion or immersion in liquid leading to respiratory impairment. EPIDEMIOLOGY of DROWNING Less than 5 years old due to neglect or decrease supervision. The 15-25 year old age group is more common in river, lakes and beaches. Risk factors are individuals who can't swim well or risky behavior, alcohol, drugs, trauma, stroke, MI and seizures while swimming. PATHOPHYSIOLOGY OF DROWNING Firstly , patients begin to panic and lose normal breathing pattern to maintain perfusion. Patient initially holds breath until they have air hunger due to desire to breath. Finally the drowning patient aspirates the water and there is a reflex laryngospasm and finally leads to hypoxemia. Salt water drowning has higher osmality and higher amounts of solutes. High electrolyte concentration will draw water into alveoli from alveolar capillaries. Patients drowning in saltwater can have pulmonary edema. Fresh Water drowning will not draw water into alveoli, rather fluid will be drawn into capillaries causing and increase in intravascular volume causing hemodilituion. Dirty Water drowning can lead to aspiration of bacteria and eventually lead to pnuemonia. Also particulate matter can cause plugging in lungs. Most water goes to stomach and this is important because later the water can be asiprated. Pulmonary effects include washing out of surfactant leading to atelectasis (alveolar collapse). Since air can't entire there is subsequent V/Q mismatch and shunting. Auscultation there is wheeze crackles and shortness of breath. Neurologic effects are caused by hypoxemia, leading to ischemia and neuronal damage. Cerebral edema may also lead to high intracranial pressure. Cardiovascular effects of drowning are arrythmias (tachycardia/bradycardia) and atrial fibrillation. Renal effects of drowning include ATN. MANAGEMENT OF DROWNING Acute and prehospital stage consists of CPR, ventilation, supplemental oxygen and intubation (if apneic). In Emergency room management of a drowning patient includes looking at Airway (intubation) and ABG PaO2 less than 60mmHg and PaCO2 greater than 50mmHg. Breathing: Supplmental Oxygen to maintain above 94%. For cirulation just monitor blood pressure, cardiac telemotry and check oxygen saturation and end tidal Co2. Disabilities you need to watch for neurological symptoms by finding GCS score, elevate head to decrease intracranial pressure and consider diuretic. If seizure make sure to get non sedative and convulsant. Exposure of drowning patients and unclothing and rewarming is important. During healing it is important to rewarm to prevent vasoconstriction which leads to diuresis and hypokalemia and hypotension. PROGNOSIS OF DROWNING If submersion, resusication greater than 25 minutes. Apnea, pH less than 7.1, Age greater than 14, and GCS less than 5. This summarizes the discussion on pathophysiology and management of drowning for the USMLE

Exercise is a great way to help lower blood pressure. Exercise leads to an increase in the stroke volume, which is the amount of blood pumped with each heart beat. When stroke volume increases, the heart becomes stronger and does not have to work so hard in order to pump blood around the body to where it is needed. This reduces the risk of developing coronary heart disease and having a heart attack.

UPMC 20-Second Shout-Out for Trauma

04:59
This video was provided by UPMC Prehospital Care to provide an overview of a new handoff procedure being implemented. UPMC Trauma Services and Emergency Medicine at UPMC Presbyterian, Mercy and Children's have implemented a new program called "the 20 Second Shout Out." Essentially, what they have identified a specific time during the patient transfer process that EMS will be asked to give critical patient information. UPMC envisions when your crews enter the trauma room, a senior member/leader of the ED/trauma team will assess the patient's ABCs while remaining on your EMS stretcher. Once this is completed and any life threatening problems are corrected, the patient will be transferred to the hospital bed. The ED/trauma team leader will then ask the hospital staff to be quiet so that EMS can give their 20 Second Shout Out (report). This report consists of (6) critical elements designed to aid the resuscitation team in the initial minutes of care. A poster listing the following (6) elements will be prominently displayed in the trauma rooms at each facility your crews to reference when giving their report: The 20 Second Shout Out 1. Age, Gender, Mechanism of Injury, Time of Event 2. Prehospital Vital Signs: Heart Rate, Blood Pressure, O2 Sat, Respiratory Rate 3. Injuries 4. Prehospital Interventions 5. Changes in Patient Status: Any LOC, Hypotension? 6. Past Medical History, Allergies, Meds, Blood Thinners? UPMC asks that all additional patient related information be conveyed to hospital staff after the "Shout Out" and in most circumstances outside of the trauma room (i.e., family information, police involvement, registration info, social worker interaction, etc.)

Temperature Rising Can Be Deadly for Seniors with Chronic Conditions

01:21
Small rises in summer temperatures may shorten life expectancy for seniors with chronic medical conditions, according to Harvard University School of Public Health researchers. They studied Medicare data over almost 20 years to follow the long-term health of 3.7 million chronically ill people over 65. Each 1° Celsius increase in summer temperatures increased the death rate for elderly with chronic conditions between 2.8% and 4.0%, depending on the condition. The mortality risk increased the most for those with diabetes. "The increasing age of the population, the increasing prevalence of chronic conditions such as diabetes, and possible increases in temperature fluctuations due to climate change, means that this public health problem is likely to grow in importance in the future," warned epidemiologist Joel Schwartz, Ph.D. Help your local seniors. Visit them at least twice a day and watch for signs of heat exhaustion or heat stroke. Seniors may not feel thirsty even when their bodies are low on fluids. Encourage them to drink plenty of cool water, juice or sports drinks and to avoid beverages with caffeine or alcohol. Alcohol, in particular, increases dehydration. Take them to air-conditioned places if their residence is hot. They can also shower or bathe in cool water. Electric fans may provide comfort, but when the temperature is in the high 90s, fans will not prevent heat-related illness. Seniors' bodies do not adjust as well as those of young people to sudden changes in temperature. They are more likely to have a condition that upsets the body's normal response to heat and are more likely to take medicines that make it harder for the body to control its temperature and to sweat. People respond to heat exhaustion in different ways, but the CDC reports that common symptoms include: Heavy sweating Paleness Muscle cramps Tiredness Weakness Dizziness Headache Nausea or vomiting Fainting Skin: may be cool and moist Pulse rate: fast and weak Breathing: fast and shallow PLEASE SUPPORT OUR ADVERTISERS BY CLICKING ON THEIR ADS. Medical Edge: Six steps for stroke stopping

02:15
CINCINNATI (Liz Bonis) -- This is the perfect time of year to stock up on a few foods that can help stop a stroke. Hard not to notice that a lot of the end of summer produce is picture perfect this time of year and when people eat those foods they feel a lot better. But those foods also work against an injury to the brain known as a stroke. Sophia and Julia Madison said they do a lot of things regularly for lifelong, stroke-stopping success. They do hot yoga, running, and swimming. Yoga and other activities do help lower stroke risk by lowering stress. A restorative yoga class that Local 12 News had a chance to check out was aimed at just that. Mary Geibel of Mercy Health in Anderson said, This is a practice of being still, and that can be harried for some people. There are six steps that a report in Environmental Nutrition said could make a big difference in stopping a stroke; most of them of have to do with what people do with their diet. First up, people should step up the potassium in their diet by getting at least five servings each day of some end of summer seasonal foods including watermelon, cantaloupe, red bell peppers, orange bell peppers and all the local melons. Second, go nuts! Eat at least a quarter cup per day. Third, gelato or other dairy should be eaten each day. The calcium in milk, cheese and yogurt helps keep blood pressure down. Fourth, eat fresh salads and other whole foods instead of processed foods to lower salt or sodium intake. Finally limit alcohol intake and up exercise, not just for stress reduction but also calorie burning. This keeps weight down along with a plant based diet which keeps people strong on defense against a stroke.
