Dizziness While Exercising – All About Postural Blood Pressure

https://www.drlam.com/blog/dizziness-while-exercising-all-about-postural-blood-pressure/22264/ This is Dr. Lam from DrLam.com and the Adrenal Fatigue Center. Today we’re going to talk about a common problem that some people experience when they’re doing, for example, exercises that the head is not in the right position, leading to dizziness. Now, dizziness can be due to a variety of factors. Most commonly, when it is related to postural change, we call it postural blood pressure problems. Now, the body’s blood pressure is regulated in many ways through an automatic mechanism. When the body’s head is below a certain level, in order to get more blood to the brain, the body’s blood vessels automatically constrict and that is intended to bring more blood to the brain so you don’t faint. Now, when this mechanism is defective or not functioning well, then problems can arise. For example, you can feel dizzy. You can feel the room spinning. You can feel even nauseated. You can have headaches and in severe cases, you can faint. The first thing you have to do if you have this type of experience is to get yourself back to a situation where your head is level or above your heart so that your body has the ability to regain its composure. The medical workup including cardiac as well as neurological workup will include checking out your heart, making sure that your carotid blood vessels are okay, there’s no blockage. Make sure your heart is not having any arrhythmias that are causing the heart rate to be dysregulated during these situations. Make sure that your ears and your nose are okay so that you don’t have any postural changes that can affect your balance. Metabolically, you’re going to make sure that your sugar is not low beyond a certain point. If everything checks out, then chances are you’ll be told that this is posture related. There’s really not much you can do if that’s the case, if everything works out. In the case of adrenal fatigue, we do see some postural changes and oftentimes, this can be improved with increasing the vascular volume such as with more fluids as well as more salt. Regulating and normalizing blood pressure often can also be beneficial and helpful. As far as exercise, you have to moderate yourself. Oftentimes, when the body is in a state of adrenal fatigue, the fight or flight response can be activated and when this happens, dizziness can occur as well. There are many, many reasons and you want to make sure that there’s no conventional medical reasons that you cannot check out. If everything turns out to be normal, do look into adrenal fatigue because it is very common. The good news is that, oftentimes, these are spontaneously healing signs. As your body gets stronger, if it is indeed related to adrenal issues, then oftentimes it gets better by itself. Taking medications can have its place if the situation is severe and you don’t want to be in a fainting situation, which can be dangerous to yourself and others. Regulating blood pressure, etc. can be considered with conventional medication. The problem with this is that if you overregulate, you can be a little more dizzy and a little more calm and sometimes walk around like a zombie because you can be sedated in order to avoid these problems. At the same time, if you don’t look at the underlying problem and resolve it, then these masking medications can actually be detrimental and negative to your overall health over time because your problem underneath is really not resolved. If it’s a matter of simple insufficient salt or adrenal weakness, then there are simple ways you can do, from lifestyle, nutrition, and supplementation that can help with the adrenals and then the postural blood pressure changes can resolve. These are the first steps you should take. However, especially if you are older and you have a previous history of heart disease, then you need to see a doctor first and make sure there’s nothing else going on. How To Lower Blood Pressure Quickly

03:28
http://www.control-your-blood-pressure.com Hypertension can wreck havoc on many body organs. Its effects on the heart are especially damaging. High blood pressure and heart disease are closely connected. Moderate to high blood pressure significantly increases the load on the left side of the heart. Arterioles are often diseased or constricted, which increases resistance to blood flow. When this happens, the heart has to work much more to pump blood into the arterial system including the aorta. When any muscle is overused, it tends to increase in size and bulk. The heart muscles react the same way. An enlarged heart is a sign of trouble. Heart enlargement can be detected in several ways. A chest x-ray can reveal it. So can an electrocardiogram or even a physical examination. In many high blood pressure patients, the walls of the left ventricle thicken. This increases the workload on the heart. Eventually, the heart falters and the left side of the heart no longer pumps blood adequately. When this happens, major organs and tissues of the body are denied sufficient blood supply. The affected person becomes lethargic and weak. Because of the higher pressure in the left ventricle, blood from the lungs cannot drain into the top compartment of the heart (the atrium). As a result, the lung tissues get congested which brings on bouts of breathlessness and coughs. The patient may have a dry cough or even frothy phlegm with blood stains in it. This is a very scary experience to the sufferer and onlookers. In an acute attack, it seems to the patient that he or she is choking to death. Breathlessness due to heart problems may be first noticed during prolonged physical exertion. If the condition has progressed further, breathlessness may occur even while resting. This is a serious situation and needs to be tacked immediately. If this damage worsens, the right side of the heart will be affected as well. When pressure builds up in the right atrium, it will be difficult for veins to drain blood into it. When the heart's condition deteriorates to this stage, the external jugular veins may become enlarged and more prominent. These veins are anyway close to the skin and may be visible even in healthy adults, but stress on the heart can increase their protrusion. There are several other symptoms of right side heart failure. They include an enlarged liver, swollen ankles and feet, loss of appetite, swollen abdomen and lower urine flow. High blood pressure and heart disease frequently have a cause and effect relationship. Making necessary lifestyle changes can help combat both.

http://tinyurl.com/qge9zu3 Hypotension or low blood pressure is pressure that is lower than normal blood pressure. Regular blood vessels pressure for the human entire body is 120/80 mmHg. If your bp is 90/60 mmHg or reduce you may have reduced blood pressure. Hypotension happens because of low blood flow through arteries and veins that can lead to less oxygen becoming passed to essential organs. This really is due to weakness, flabby and over dilated blood vessels. In situation of less oxygen becoming passed to brain, heart, eye and kidney, they are able to get harm and in some significant situations it can permanently damaged essential organs. A few of the reasons that can cause hypotension are pregnancy, diabetes, the hormonal problems, hypothyroidism, hyperthyroidism, and low blood sugar. Some over-the-counter medications can also produce this issue. The overdose of higher bp drugs, abnormal center beats or rhythms, disrupting functioning of heart, heat stroke and liver illness might also trigger hypotension. Some other factors that can cause sudden drops in blood vessels stress are loss of blood vessels, low body temperature, higher entire body temperature, sepsis, a severe blood infection, heart muscle disease causing heart failure, serious dehydration, due to reaction from medication and also the serious allergic response (anaphylaxis). The main symptoms of low blood pressure are fainting, fatigue, dizziness, nausea, vertigo, transient blurring of vision, over exertion, mental disturbances, severe confusion and breathlessness. But with the normalization of the bp, these symptoms will subside gradually. Drink lots of water as dehydration minimizes the blood volume which causes the drop in stress. Consume one glass of water per hour to keep your entire body hydrated. Salt can also be great for hypotension patient. You are able to improve your salt intake, but this improve in salt may vary from person to individual. Carry a walk after you take your meals. This assists in bringing the blood stress degree to regular. Soak ten little raisins in water overnight. Chew every raisin gradually before swallowing it. Carry on doing this for a month. This really is very useful in normalizing reduced blood pressure. Another useful house remedy is almonds. Soak 5 almonds in drinking water overnight. Grind them to create a smooth paste and mix it in glass of milk. Boil the milk and drink it cozy. Groundnuts have most of the needed nutrients and protein. Roasted groundnuts are more nourishing. Boil dates in sweetened milk. Take this milk warm along with the boiled dates, two or three times. Sugarcane juice is also a great energy booster and recommended for reduced bp. Wheatgrass is believed to rejuvenate and revitalize the energy levels. Extract the juice and consume it immediately whilst nevertheless fresh for best result. Carry 2 to 4 grams of guggul twice a day together with hot milk, preferably within the winter months. http://www.lowbloodpressuretreatment.chineselivings.com

10 Warning Signs Your Heart Isn't Working Properly

09:25
How to Prevent Heart Disease. About 610,000 people die from heart disease each year in the U.S. alone. There are tons of factors that can cause a person to develop heart problems, but most of them come down to poor lifestyle choices. Your body gives you warning signs early on so that you can catch an illness in its early stages or even prevent it altogether and save your life. Here are 10 of the most common red flags that indicate bad heart health. #heartproblems #healthyheart #bodysigns A cough that won't quit 1:38 A persistent cough can be a symptom of all kinds of health issues, and cardiovascular disease is one of them. For instance, a chronic cough that produces a pinkish bloody liquid is very common in people with heart failure. Despite what it sounds like, heart failure doesn't mean that the heart completely stops beating. It just means that a person’s heart isn’t pumping blood as well as it should be. As a result, the body doesn't get enough oxygen since it’s carried to all the tissues by the blood. Nausea and loss of appetite 3:07 A lot of heart disease patients show a lack of appetite, nausea, or both, even if they've had just a couple bites of food. The reason for that is built-up fluid around the liver and intestines that interferes with proper digestion. These symptoms are usually accompanied by abdominal pain, upset stomach, or feeling bloated. In fact, abdominal cramps that come and go in short spurts are extremely common right before a heart attack. Your skin becomes paler or takes on a bluish tint 5:28 Being naturally pale doesn’t necessarily mean you have a heart condition. But sudden paleness can be a result of reduced blood flow due to a heart that’s not pumping blood as effectively as it should be. Shock, which can be caused by any condition that prevents the blood in your body from circulating properly, is the main reason why people who are suffering from a heart attack or heart failure turn pale all of a sudden. Chronic or sudden fatigue 7:02 Fatigue can be one of the main symptoms indicating an impending heart attack. Women are more likely to experience heart-related fatigue than men, with 70% of female heart attack patients reporting that they’d felt extremely tired before their episode. Physical or mental activity isn't the reason for such exhaustion, and it increases by the end of the day if it’s due to a failing heart. It’s really hard for this symptom to go unnoticed. Sometimes it's exhausting just to perform even the simplest tasks, like making the bed or taking a shower. Music: The 126ers - Darkness of My Sun Density & Time - City of Jewels https://www.youtube.com/audiolibrary/music TIMESTAMPS 10. Pain that spreads to the arm 1:01 9. A cough that won't quit 1:38 8. Swollen legs, ankles, and feet 2:25 7. Nausea and loss of appetite 3:07 6. Extreme levels of anxiety 3:47 5. Lightheadedness or fainting 4:26 4. Your skin becomes paler or takes on a bluish tint 5:28 3. Skin rashes or unusual spots 6:20 2. Chronic or sudden fatigue 7:02 1. Increased perspiration 7:45 SUMMARY -If their heart isn’t working properly, a lot of men feel pain in their left arm, while women experience the same pain in either or both arms. -A persistent cough can be a symptom of all kinds of health issues, and cardiovascular disease is one of them. -When your heart isn’t pumping enough blood through the body, fluid from your blood vessels leaks into surrounding tissues. Your lower extremities are the most affected simply due to gravity. -A lot of heart disease patients show a lack of appetite, nausea, or both, even if they've had just a couple bites of food. -People who suffer from extreme anxiety from a very early age or most of their life are more prone to developing coronary heart disease. -Feeling lightheaded and experiencing temporary loss of consciousness, also known in the scientific community as “syncope”, is super common among heart patients. -Sudden paleness can be a result of reduced blood flow due to a heart that’s not pumping blood as effectively as it should be. -People with eczema were found to have a 48% chance of suffering from high blood pressure and a 29% possibility of having high cholesterol. -Sometimes it's exhausting just to perform even the simplest tasks, like making the bed or taking a shower before a heart attack. -People experience flu-like symptoms, have clammy skin, and sweat regardless of the air temperature or physical exertion. How to breathe when lifting weights

03:12
http://www.scoobysworkshop.com In this video I talk about the right way to breathe when lifting weights. There is a lot of conflicting information out there and I tell it like I see it. I'm not a doctor, but I think that high blood pressure can cause: hernias (a drag but not life threatening), burst blood vessels in the eyes (again a drag but not life threatening). The problem is that if you have any weak blood vessels, the high blood pressure can burst them and if its in your brain you die instantly. Any M.D's out there who can correct me and lend some expertise to this???? What are the dangers of momentary high blood pressure caused by holding the breath under extreme exertion? I didnt mention any specific medical problems in this video because that is beyond my area of expertise. I will add them via annotations when I hear from some medical professionals. http://www.scoobysworkshop.com Nutrition is key to being able to add muscle! I cover nutrition on my website, if thats not detailed enough for you I have reviewed and recommended some excellent books: http://scoobysworkshop.com/goodfitnessbooks.htm

Can Any Patient Suffering From High Blood Pressure Play Outdoor Sports? There is one organ which causes high blood pressure (and it's not what you think!) and there are specific exercises you can use to target and fix your blood pressure easily. No diet, lifestyle changes or medication required: http://hibloodpressure.stream/ Answers from: Liang-Hai Sie Rajnikant Prajapati Jaydeep Shetty Rajnikant Prajapati Certainly if it is well controlled, no problem. Physical exertion causes your systolic blood pressure to rise, your diastolic blood pressure would mostly be stable, thus widening the difference pulse pressure: Regular physical exercise has been shown to lower one's blood pressure, so beneficial for one's health, if done for at least 150 minutes a week, see Exercise for Hypertension: A Prescription Update Integrating Existing Recommendations with Emerging Research. After acute exercise there is a post exercise hypotension for the duration of around 24 hours after the exercise, so the recommendation is to exercise 4 - 7 days but preferably every day in the week, for at least 150 minutes per week: One can do the daily exercise in one daily session, or divided in multiple sessions a day as long as a session is at least 10 minutes in duration. It seems that both moderate to submaximal aerobic and dynamic resistance exercise is effective. What Is A Healthy Heart Rate - What Affects Heart Rate - What Is Maximum Heart Rate

02:23
In this video we look at what is a healthy heart rate, what affects heart rate, and how to take your pulse. We also look at normal heart rate as well as exercise and heart rate. Transcript notes What is heart rate? What is a healthy heart rate? What is a normal resting heart rate? Heart rate is the number of times your heart beats in a minute. It is measured in bpm or beats per minute. A healthy, Normal resting heart rate is usually between 60 and 80 beats per minute. The resting heart rate of a woman is usually about 10 beats more per minute than that of a man in the same age range, and elite athletes can have a resting heart rate near 40 beats per minute. This is because Individuals that are fit have a larger stroke volume of the heart. Stoke volume is the amount of blood ejected from the heart per beat. So, the heart of someone who is really fit does not need to beat as many times to have the same output of blood as an unfit person. Another thing that has a major effect on heart rate is stress. During stressful situations the body releases the hormone adrenaline which causes an increase in heart rate, which is beneficial when you are in danger, but unhealthy in everyday situations. When you are sitting and resting, your heart beat will be lower, and as you start to move around, your heart beat will increase. When you exercise your heart rate will vary, depending on the intensity of the exercise, your age, the position your body is in, standing vs. lying down, your current fitness level, the type of exercise you are performing, and the temperature and humidity that you are in. Medications can also have a dramatic effect on heart rate. As we get older, our maximum attainable heart rate decreases. There is a simple equation that is used to estimate a person’s heart rate max. 220 – your age, so for instance, for a 35 year old person, the equation would be 220 – 35 = 185, so their max heart rate should be somewhere around 185 beats per minute. That would be at full physical exertion, like sprinting all out. There is some margin for error of estimating heart rate max, so this number will give you a general idea. There are many training devices that will gauge your heart rate throughout your day and exercise sessions, or you can do it the old fashioned way put the tips of your fingers on the blood vessel on the thumb side of your wrist. Use a timer on your phone and count the number of beats for 10 seconds, and then multiply that number by 6. For example I counted 11 beats, so 11 times 6 is 66. That means my resting heart rate is at 66. And that be the basics on heart rates.

Exercises For Improving Blood Circulation In Legs

06:27
See how to dramatically and naturally, improve your blood flow and circulation in 30 days or less: ►► http://DrSam.co/yt/LegCirculationExercises-BF =============================================== Best Exercises For Increasing Blood Flow & Circulation To Your Legs & Extremities =============================================== I completely understand because my mom has had similar legs pains, but no varicose veins. Actually, my dad has the varicose veins in the family. In today’s video, I’ll quickly discuss 3 best exercises for increasing blood flow and improving circulation to your legs and your entire body. You can do them anywhere and anytime, regardless of your age or fitness level. And it will not cost you anything! Primary Cause Of Heart Attacks & Strokes Poor blood flow and circulation is one of the primary causes of a heart attack or stroke. This is because it can lead to: -Hypertension and high blood pressure -Hardening and narrowing of your arteries -Higher cholesterol and plaque build up Warning Signs Some of what “warning signs” that you may have circulation problems include Frequent pain and cramping in your legs Tingling or numbness in your feet Tired, aching or weak legs and feet Slow-healing wounds Thickened toenails Varicose and “spider” veins Why Circulation Is So Important Now, improving circulation to your legs is very important because you have to pump the blood uphill to your heart… going against gravity! When you walk, your foot and leg muscles are contracting and relaxing and basically acting like a “second heart”, to continually pump blood back up through your legs and to your heart. Unfortunately, if you are sedentary for prolonged periods of time … like sitting for too long, on a long flight or bedridden… your “second heart” is inactive and blood can gather in your veins and around your ankles and this can lead to pain in the lower legs… and worst case scenario it can lead to a life-threatening blood clot. The good news is that by performing regular leg exercises, you can promote healthy blood flow from you legs to your heart and thus, help prevent blood clots and leg pain. Exercises Walking I think walking is one of the best exercises you can do and almost anyone can do it, any where, any time of the day and it doesn’t cost you anything. Ideally you would walk outside in fresh air if possible. First thing morning and/or late evening are good times but, whenever you can is a better than nothing. To start, all you need is about 15 minutes, 3x weekly. Ideally you would work up to daily walks of about 30 minutes. And if you have pain or trouble walking, even 3 minutes is a good start. Try your best to walk just a little bit every single day. Rebounder A rebounder is basically a mini trampoline. I like this because it is very low impact and super easy on your joints. It also helps clean your lymph nodes, which helps prevent many cancers and illnesses. I put mine in front of the TV while watching a favorite show and do it for about 10-20 minutes daily. You don’t need much at all. By the way, you’re not trying to jump high on it and try to touch the ceiling. You only need to go up a few inches. Just enough for your toes to be off the roubound. It’s just a light up and down motion. Again, almost anyone can do it, any time and anyplace. You can even travel with it. Body Weight Squats This last exercise is great for building and strengthening your entire lower body … From your ankles to your leg muscles and all the way to your lower back and stomach. You can simply use your own bodyweight and do sets of about 15-20 repetitions. Of course, if you’re first starting out, simply doing even 2-3 comfortable repetitions is a great start. Go down as far your comfortable and without pain. Over time, as you get stronger and more flexible, you can do more repetitions and go deeper. Ideally, you would do maybe 1 set of 15 or more repetition, 2-3x daily. Maybe in the morning, noon and night time. More often and throughout the day helps keep the blood flow regular and continuous. And if you don’t have good coordination, simply hold onto a wall or something sturdy as you squat down. Summary Poor blood flow is bad for your health, if affects your entire body. The best exercises for improved circulation, especially to your legs is: -Daily walking -Using a rebounder -Bodyweight squats Click here: ►http://drsam.co/yt/legcirculationexercises-bf ================================================================================ Thank you for watching. How to tell if the palpitations you are getting are serious or not

11:40
York cardiologist, Dr Sanjay Gupta discusses palpitations and how to work out if the palpitation represent a heart rhythm problem by tapping out the different beats. www.yorkcardiologist.co.uk. Hello, my name is Sanjay Gupta and I’m a cardiologist in York. Today I thought I’d do a little video on the subject of palpitation. Palpitations are really common symptoms that I see in my clinic and majority of palpitations are not serious. The big worry when people experience palpitation is whether they may have a heart rhythm disturbance. Even if they have a heart rhythm disturbance, most heart rhythm disturbances are not serious. So, that’s a little bit of reassurance. The first thing that I would like to talk about is what is a palpitation? I suppose palpitation is a rareness of one’s heartbeat to the extent that it feels abnormal. So, the hart could be beating normally but us sensed abnormally. Now, there are many normal causes of palpitations, so to speak. So, for example, if you were walking down a dark alley and someone jumps out suddenly, then that would cause palpitation; that would cause the heart to beat very fast and that would be felt as a palpitation. But that doesn’t signify a problem with the heart itself; the heart is just doing what is meant to do, it is beating fast in response to the adrenaline that is circulating around the body. Similarly, anxiety and stress can cause palpitation; caffeine can cause palpitation, people who drink a lot of caffeine; alcohol can cause palpitation; exercise can, obviously, cause palpitation. So, these are the plan that of sort of normal things that can cause palpitations which don’t signify anything to be worried about.

Best Exercise to Lower Blood Pressure

04:52
Best Exercise to Lower Blood Pressure It seems as though many Americans are living a life that leads to high blood pressure or hypertension. As people age, the situation gets worse. Nearly half of all older Americans have hypertension. This disease makes people five times more prone to strokes, three times more likely to have a heart attack, and two to three times more likely to experience a heart failure. The problem with this disease is that nearly one third of the folks who have hypertension do not know it because they never feel any direct pain. But overtime the force of that pressure damages the inside surface of your blood vessels. However, according to experts, hypertension is not predestined. Reducing salt intake, adopting a desirable dietary pattern losing weight and exercising can all help prevent hypertension. Obviously, quitting bad habits and eating a low fat diet will help, but the most significant part that you can do is to exercise. And just as exercise strengthens and improves limb muscles, it also enhances the health of the heart muscles. Heart and Exercise The exercise stimulates the development of new connections between the impaired and the nearly normal blood vessels, so people who exercise had a better blood supply to all the muscle tissue of the heart. The human heart basically, supply blood to an area of the heart damaged in a “myocardial infarction.” A heart attack is a condition, in which, the myocardium or the heart muscle does not get enough oxygen and other nutrients and so it begins to die. For this reason and after a series of careful considerations, some researchers have observed that exercise can stimulate the development of these life saving detours in the heart. One study further showed that moderate exercise several times a week is more effective in building up these auxiliary pathways than extremely vigorous exercise done twice as often. Such information has led some people to think of exercise as a panacea for heart disorders, a fail-safe protection against hypertension or death. That is not so. Even marathon runners that have suffered hypertension, and exercise cannot overcome combination of other risk factor. What Causes Hypertension? Sometimes abnormalities of the kidney are responsible. There is also a study wherein the researchers identified more common contributing factors such as heredity, obesity, and lack of physical activity. And so, what can be done to lower blood pressure and avoid the risk of developing hypertension? Again, exercise seems to be just what the doctor might order. If you think that is what he will do, then, try to contemplate on this list and find some ways how you can incorporate these things into your lifestyle and start to live a life free from the possibilities of developing hypertension. But before you start following the systematic instructions, it would be better to review them first before getting into action. 1. See your doctor Check with your doctor before beginning an exercise program. If you make any significant changes in your level of physical activity — particularly if those changes could make large and sudden demands on your circulatory system — check with your doctors again. 2. Take it slow Start at a low, comfortable level of exertion and progress gradually. The program is designed in two stages to allow for a progressive increase in activity. 3. Know your limit Determine your safety limit for exertion. Use some clues such as sleep problems or fatigue the day after a workout to check on whether you are overdoing it. Once identified, stay within it. Over-exercising is both dangerous and unnecessary. 4. Exercise regularly You need to work out a minimum of three times a week and a maximum of five times a week to get the most benefit. Once you are in peak condition, a single workout a week can maintain the muscular benefits. However, cardiovascular fitness requires more frequent activity. 5. Exercise at a rate within your capacity The optimum benefits for older exercisers are produced by exercise at 40% to 60% of capacity. Indeed, weight loss through exercise is an excellent starting point if you wan tot prevent hypertension. Experts say that being overweight is linked to an increased risk of developing hypertension, and losing weight decreases the risk. Tips to know if the Chest Pain is Heart related or something else - Dr. Anantharaman Ramakrishnan

02:55
Many of you may wonder when you have a chest pain when the pain is because of cardiac reasons or it is because of some other condition. So, in fact the only way to lively differentiate between cardiac chest pain and non cardiac chest pain is to look at cause of cardiac necrosis that is troponin I or T along with the electrocardiography. Even electrocardiograms sometimes miss cardiac injury where as troponin I or T does not, so if the pain has been there for more than 4 to 6 hours this will tell us the difference. The simple clinical tests tell you f this is a simple pain and not really a cardiac space, so sometimes the musculoskeletal pain the pain will increase this moment either when you move your shoulder or your body from side to side that could be increase in the chest pain or relief of the chest pain so that will indicate the musculoskeletal cause. Whenever you swallow food or if the pain arises soon after eating this could indicate intestinal cause for chest pain. If you take a deep breadth and there is a chest pain that could be a pleural cause for the chest pain. These are some ways to differentiate between cardiac and non cardiac chest pain. Cardiac is more like a heaviness or comprehensive feeling in the center of the chest which is an anginal pain brought on by exertion and relieved by rest. If it is sudden and excruciating in nature then probably the patient is having a heart attack. In any case if you have sudden chest severe pain then get it evaluated in an emergency route where the chances of survival are maximum instead of going to a small clinic.
When should you have a Stress Test?

01:35
Stress can take a toll on our bodies. In fact, many health reports encourage us to take time to relax to lower stress. But health experts say during a stress test they want to put stress on your body, particularly your heart. Jennifer Moss, a cardiology advanced provider, says during a stress test patients walk on a treadmill as health experts monitor their heart. “A stress test is something we use to assess for coronary artery disease, so blockages in the arteries that would increase blood and oxygen flow to the heart.” If a patient has a pervious history of heart problems, has diabetes, or has high cholesterol, they’re considered high risk for coronary artery disease. “Generally the reason we order a stress test is a patient is having symptoms such as chest pain or shortness of breath on exertion,” said Moss. The most common stress test is on a treadmill. The faster you go, the more stress you put on your heart. Using an EKG and a blood pressure cuff, health experts monitor how the patient’s heart responds to that stress. “We want to know if when the heart is being stressed and worked out and that muscle is working, is the blood and oxygen still getting to the heart. We look at the EKG for any signs that says no that’s not the case,” said Moss. For patients who are unable to use the treadmill, a chemical stress test is used. “We kind of do the same thing with medication to make the heart stressed then we take images to look at the heart,” said Moss. If patients experience chest pain, shortness of breath, or are unable to complete the test, doctors may recommend further testing to examine the heart. How To Lower Blood Pressure Immediately

04:13
http://www.control-your-blood-pressure.com Hypertension can wreck havoc on many body organs.

What causes a slower heart rate while going to sleep? - Dr. Sreekanth B Shetty

01:01
Heart rate is not constant. It is varying with physical activity. So when we are quite as in when we are sleeping, the heart rate tends to go down normally, that is what we call the increased vagal tone and a reduced sympathetic tone. Sympathetic tone is what leads to a reduced heart rate and blood pressure, when we are agitated or when we are anxious or when we do physical exertion. So it is normal phenomenon for the heart rate to go down during sleep.
Myocardial Infarction and Angina for USMLE Step 1 and USMLE Step 2

Handwritten Video lecture on Myocardial Infarction/Angina for the USMLE Step 2. Will be covering Pathogensis, Clinical Signs and Symptoms, ECG and Diagnosis as well as Management Coronary Artery Disease occurs when there is an imbalance between oxygen supply of the heart with the oxygen demands of the heart. ETIOLOGY/CAUSES Decreased Oxygen supply - Atherosclerosis, Prinzmetal Angina, Arterial Thrombi, Coronary Emboli, Aortitis, Dissection Increased Oxygen Demand - Hypertrophy, Catecholamines, Tachycardia RISK FACTORS Modifiable - LDL and HDL, Hypertension, Diabetes, Obesity, Smoking Non-Modifiable - Family History, Male, Greater than 64 years old CLINICAL SIGNS AND SYMPTOMS Pain - tightness, squeezing, knot in chest, heavy weight, toothache and jaw ache, lump in the throat. Exacerbated with exertion and recumbency, sleeping. Percipitated by heavy meal, cold, sexual activity. Radiation to epigastric, shoulders down to fingers, lower jaw, subscapular space in the back. Relieved with nitroglycerin, rest. Usually occurs in the early morning due to increase sympathetic acitivity and platelet aggregation Angina Equivalent - dyspnea, nausea, fatigue. Usually seen in elderly and diabetic. PHYSICAL EXAMINATION Tachycardia and hypertensive due to pain. Auscultation may show S3/S4 or paradoxical S2 split. Levine's Sign will show clenched fist on the chest. ECG Findings Subendocardial ischemia shows ST Depression or T wave inversion. Transmural Infarct shows ST Elevation or hyperacute T wave. Pathological Q wave develops along with R wave depression. Usually takes a few hours to develop. After a few days there is a much deeper Q wave and t wave inversion. CARDIAC BIOMARKERS Troponin T and Troponin I are more specific than CKMB. Troponin peaks at 12 hours and last for weeks. CKMB rises in a 2-3 hours and goes down in 2-3 days. STRESS ECG Patient walks on treadmills while being monitored by ECG until reach 85 percent of maximum heart rate. Positive if symptoamtic, ECG changes, drop in blood pressure, ventricular tachycardia. ECHO Shows wall function and may show akinesia or hypokinesia. Used for patients with chronic angina, prior MI, pathologic Q wave and Heart Failure Electron Beam CT Calculate Calcium to calculate aggetson score which is prognostic Coronary Angiography Detects which arteries is occluded. MANAGEMENT First 10 minutes - Check for hemodynamic stability. Ask for recent bleeding neurological condition, and aortic dissection. Use of cocaine or Ergotamine (prinzmetal angina). Determine if RV or LV Infarction. Initial treatment consists of aspirin, clopidogrel, B-blocker, ACE Inhibitor, Statin, and Morphine. Nitroglycerin. Oxygen is only given if saturation is less than 94 percent. CLASSIFICATION STABLE ANGINA - Pain only at rest. Patient should be stress ECG or Echo. If positive than perform angiography. Minimize risk factors by decreasing LDL, increasing HDL, statins, beta-blockers ACUTE CORONARY SYNDROME - Constant Pain UNSTABLE ANGINA - No elevation of cardiac markers and No ST Elevation. Anti-coagulate, B-Blockers, Nitroglycerin. TIMI SCORE - Age greater than 65, Markers, ECG Changes, Risk Factors, Ischemia in 24 hours, CAD, Aspirin in last 7 days. Score of 0-4 is low risk and can be treated medically. 5-7 shoudl undergo cathetrization. NSTEMI - No ST elevation, but positive cardiac biomarkers STEMI These patients require PCI within 90 minutes. Especially if high risk. If symptoms greater than 24 hours, than fibrinolysis is ineffective because clot is too strong. FIBRINOLYSIS - not preffered due to high bleeding risk. Use aleteplate, tenecteplase. Only indicated if PCI is unavailable. If fibrinolysis doesn't work than must do rescue PCI. CORONARY ARTERY BYPASS GRAFT - Done after complicated PCI, Left main Artery involvement or three vessels involved. Severely LV ejection fraction. Or complications such as septal rupture, papillary muscle rupture and aneurysm. NON-ACUTE MANAGEMENT Continue anti-platelets - aspirin, clopidogrel Nitrates - prophylactic Anti-coagulants - risk of emboli Increase ambulation

Incline whole body mass (muscle) doesn't expand pulse sums, however fat does, particularly when it can be about your midriff. Huge numbers of our employments request us to sit throughout the day at work then we continue to sit significantly more when we get house. With time, this prompts to bodyweight accomplish, which makes your heart work harder to pump blood by method for the framework. Inertia normally adds to bigger heart rates. In spite of the fact that salt admission alone isn't consequently negative, it realizes significantly more h2o maintenance, which raises circulatory strain. A direct admission of salt, especially ocean salt or Celtic salt, for sound people is fine. Most Americans tend not to get sufficient potassium. It manages the measure of sodium in our cells and removes the rest to our kidneys. Bring down scopes of potassium can bring about a development of sodium while in the body. Basically in light of the fact that getting 3 or significantly more mixed beverages consistently copies and people's possibility of making higher pulse, it's imperative to confine the measure of liquor admission. Devouring a major entirety will sooner or later place heaps of strain to the heart, liver, pancreas, and mind. Eating a great deal of prepared and greasy dinners can bring about hypertension as well. By adjusting an eating arrangement loaded with vegetables and natural product and entire grain items, angle, nuts, magnesium, and potassium, weight may be cut back again down. How DeLauer Builds a Workout Program (Joe Rogan Inspired)

Time and time again, people try to piecemeal different workout programs together. Okay, they'll go on the internet and they'll find bits and pieces of Jim Stoppani's approach or another person's approach, and they try to just like hybridize them all together. What I want to do is I want to give you the basic outline and the understanding of what I do, and then you can take it and run with it. A lot of this came to life simply because of that video that Joe Rogan did with Firas Zahabi on his podcast, and I thought that it really made a lot of sense, and it reflected a lot of what I do personally, so I wanted to break this down, sort of expand upon what Joe Rogan and Firas Zahabi talked about, and give you the tools that you need. Okay, so first off, let's go back and break down a little bit about what Joe Rogan and Firas Zahabi talked about. Firas Zahabi talked about his training methodology, what he uses with his MMA clients, and what have you. For the most part, he talked about rating of perceived exertion. He talked about how if you reduce your intensity with your training, you have more opportunities to train said body part. To break it down simply, he said rather than training to failure, train sub-maximally. Train like five or six reps instead of 10, almost to failure but not all the way to failure. Then, hit with more frequency, basically giving you more opportunities to hit said body part or hit said focus group. Basically, by training with a little less intensity, you are able to be more consistent. The 80% rule implies that on average, I would say that delayed onset muscle soreness kicks in above this, okay? Above 80% is where we tend to find ourselves getting sore and unable to train that group the next day. If I trained my chest at 83%, I will probably be more sore on Tuesday, the next day. If I were to train at 80% or less, I'm more than likely able to still train that body part the next day. Now, here's the thing. That only truly applies concretely for intensity. Let me explain these variables for a second. We have to break our training down into four variables always, okay? We have volume, how much you're training, how much overall workload you're putting into the muscle in a given day, right? We have intensity, how hard you're training, what your actual intensity in as far as like how heavy you're going, frequency, how often you are hitting a body part, and duration, how long is your workout, usually more applicable to endurance or HIIT style training. Okay, this is something that I've worked on for years. This is full body training with emphasis training, so basically, you're doing a full body focus with specific emphasis on different body parts. This is going to be, again, be applied for body composition or performance, which I'll make sense of in one second. Here's an example. Monday. Go into the gym, and you do a full body workout following this 80% rule, meaning you're training at six or seven reps out of 10, so not going to failure, full body, but you pick one of the above variables for one body part to put extra emphasis on. Example, Monday, walk into the gym. I train my full body, but I decide I want to have focus on my legs today. What I will do is I will pick one variable that I want to add into my workout for my legs. Let's just say it's intensity. What that would look like is my full body routine is normal, and then I train my legs, and I'm going to go above 80% with my intensity on Monday. I'm not blasting my legs. I'm not going in and just hitting my legs. I'm not even putting a ton of focus on my legs. I'm putting a slight emphasis. I think there's a difference between focus and emphasis, a little bit of just an extra bit of attention to the legs, okay? I can expand on this more, but I need some feedback from you, so if you liked this and you want me to expand on a specific component of it, I need you to put it down on the comments section so that i can review it and do another video on it. Again, this is a more advanced technique. I'm happy to do a simpler breakdown for someone that's maybe just getting started as well. As always, make sure you keep it locked in here on my channel. I know this is a little bit of a different realm from what I'm normally talking about, but I hope you enjoyed it. I'll see you in the next clip.

Valvular diseases of the left heart

This is a brief overview of aortic stenosis, mitral stenosis, aortic regurgitation (aka aortic insufficiency), and mitral regurgitation (mitral insufficiency). I created this presentation with Google Slides. Image were created or taken from Wikimedia Commons I created this video with the YouTube Video Editor. Valvular diseases of the left heart Aortic and mitral valve stenosis and regurgitation (insufficiency) Failure of aortic valve to open (during systole) Etiology: Congenital, such as bicuspid aortic valve (least common); rheumatic fever; age-related calcification (most common, similar mechanism to coronary atherosclerosis) Demographic: 60+ y/o, males Physical exam: Murmur: Crescendo-decrescendo, harsh, high-pitch in right upper border (peak is later with worse murmur) Paradoxical S2 split Parvus et tardus carotid pulse Might hear rales in lungs ECG: LVH Clinical presentation: angina increased O2 demand due to increased tension (Laplace's law T=PV/(2h), increased afterload) decreased O2 supply bc worse coronary perfusion with diastolic dysfunction dyspnea - pressure backup in LV, then LA, then pulmonary circulation, which causes pulmonary edema syncope - with exertion → low cardiac output, exercise leads to vasodilation of muscular vasculature → both decrease MAP → less blood to brain Treatment: Aortic valve replacement if area is 1 cm^2, percutaneous balloon valvuloplasty (temporary palliative treatment) Aortic valve regurgitation Failure of aortic valve to close (during diastole) Etiology: aortic root (aortic aneurysm, dissection, syphilis); leaflets of valve (congenital, IE, rheumatic fever) Physical exam: Murmur: diastolic decrescendo high-pitched murmur, best heard at 4th left intercostal space Widened pulse pressure diastolic pressure decreases (LV dilates with volume overload, LV compliance increases) systolic pressure increases (increased volume with each beat) Large, hypertrophic, global heart on CXR Clinical presentation: angina - increased demand from increased LV size; decreased supply from decreased diastolic filling of coronary arteries (due to low diastolic pressure) heart failure - LV dilation and subsequent remodeling decreases systolic function Treatment: If LV function is normal, give vasodilators to lower SVR, promote blood going forward If LV function is impaired (EF 50%), aortic valve replacement sx Mitral valve stenosis Failure of mitral valve to open (during diastole) Valve area less than 2 sq cm (normally 4-6 sq cm) Etiology: Most commonly caused by rheumatic fever; can also be congenital, infective endocarditis, calcific disease Physical exam: Murmur: opening snap (chordae tendinae tense) followed by diastolic decrescendo rumble, perhaps increase in intensity at end with atrial contraction, heard best at apex Might hear rales in lungs ECG: RVH and LAH Clinical presentation: dyspnea - high LA pressure transmitted to lungs, which causes pulmonary edema hemoptysis - high pressure in lungs compensated by collateral channels from pul veins to bronchial veins; collaterals rupture Right heart failure → JVP, leg edema, ascites, large liver afib - high pressure in LA causes enlargement, disrupts conduction system stagnant blood can embolize, cause clots → stroke Treatment: Diuretics for pulmonary and systemic congestions; percutaneous balloon mitral valvuloplasty, mitral commissurotomy, and Failure of mitral valve to close (during systole) Etiology: Primary: Acute MR can be caused by papillary muscle rupture (inferior MI from RCA block); disorder of valves (ie, mitral prolapse) Secondary: result of underlying myocardial problem Physical exam: Murmur: Holosystolic high-pitched murmur during systole, heard best at apex Might hear rales in lungs ECG: acute inferoposterior MI if papillary muscle is ruptured Clinical presentation: Acute MR causes dyspnea - sudden increase if LA pressure from backflow of blood, which is transmitted to lungs → pulmonary edema Chronic MR Low CO (LA dilates → more backflow) → weakness, fatigue Right heart failure (result of left heart failure) → leg edema, ascites LV contractile dysfunction (lower forward output) → dyspnea Treatment: Primary: surgical correction of papillary muscle or mitral repair/replacement, diuretics for pul edema, vasodilator to promote forward CO Secondary: treat underlying myocardial problem, treat LV contractile dysfunction
