Antibiotics, Do you need them?: A guide to viral infections with Dr. Alan Christianson http://drchristianson.com/blog Do you need to take an antibiotic? Life comes at us fast, with ous schedules we can’t afford to be sick. We become desperate! Let’s get to the question. Our four vulnerable areas are ears, throat, lungs and sinuses. Multiple symptoms in those areas equal a virus. Let’s talk about the illusions of viral infections. Symptoms last from seven to ten days. It goes from not bad to awful in this time. In the latter stages you will start questioning if you need an antibiotic. Then you say, “Hey, I am done with this. This has gone on for too long, and I just need this gone!” This is day seven or eight. The next two days, you magically get better. An important fact is viruses are typically a 10 day issue. It often feels as if the antibiotic fixed the issue. Really, it did not, as the symptoms would have gone away in that time frame anyway. In some moments however antibiotics are need. Let’s take a look: The throat: Strep throat benefits from antibiotic treatment. In most cases, strep throat is just a bad, painful throat without a runny nose, ear pressure or a cough. The ears: Although it isn’t as common for adults, some get complicated ear infections. If they do, they will not commonly have a runny nose or a sore throat. The symptoms will be more localized in the ears, along with fever and some pain. The lungs: If you have a very productive cough, fever and fatigue (probably without a sore throat, a runny nose or ear pressure), you could have pneumonia. It does take a chest X-ray to confirm. Listening to the lungs is not perfect in most cases. If you are listening closely, you might notice I didn’t address one area: the sinuses. In many levels of my training, I was taught sinus infections take a long time to get rid of, so patients probably need two courses of antibiotics. Sinus infections take about three weeks to go away. Antibiotics or not sinuses will take three weeks. If you do take an antibiotic, your odds of having more sinuses infections that year are 50% higher than if you did not. So, if you do take an antibiotic for a sinus infection, it won’t go away faster, but you have a good chance of getting sick sooner than you would otherwise. I am not against antibiotics, but by very conventional guidelines, Antibiotics are not much help with a mixture of symptoms like bronchitis and flu. One note about the flu: There is a treatment, called Tamiflu, which is kind of like an antibiotic against influenza. The data says it isn’t helpful. Here’s a trick to treating viruses. Viruses shed themselves by sweating more than anything. When kids get sick, they get high fever, and then are done. As adults, we do not have that kind of vitality. Here’s how we do it: At bedtime, prepare some ginger tea. Fill the bathtub with water that is as hot as tolerable. Put extra blankets on your bed and wear sweat clothes. A silly amount of clothes. Jump in the tub for about fifteen minutes, while drinking some warm to hot ginger tea. Ginger is a diaphoretic warming you up internally. Once you have cooked yourself pretty good in the tub, wear the sweat clothes in bed with blankets, while you sleep. After a few hours of sweat, throw off the clothes and go back to sleep. you will awaken and will have kicked the virus! You will have have broken the cycle. This method is a stronger antibiotic for most, common, respiratory infections and has no side effects! Pneumonia is an infection of the lungs that can cause mild to severe illness in people of all ages. It can be caused by viruses, bacteria and fungi. The Centers for Disease Control and Prevention (CDC) reports it is the leading cause of death in children younger than 5 years of age worldwide. Globally, pneumonia kills nearly 1 million children in this age group each year. Pneumonia isn't just a public health issue in developing countries, each year in the United States about 1 million people have to seek hospital care due to pneumonia and approximately 50,000 people die from the condition. Interestingly, most of the people affected by pneumonia in the United States are adults according to the CDC. In the United States, common causes of viral pneumonia are influenza and respiratory syncytial virus (RSV), and a common cause of bacterial pneumonia is Streptococcus pneumoniae (pneumococcus). However, clinicians are not always able to determine which germ caused someone to get sick. Fortunately, many of these infections can often be prevented with vaccines and can usually be treated with antibiotics, antiviral drugs (such as Tamiflu) or specific drug therapies. When someone develops pneumonia in the community (not in a hospital), it's called community-acquired pneumonia. Pneumonia developed during or following a stay in a healthcare facility (like hospitals, long-term care facilities, and dialysis centers) is called healthcare-associated pneumonia, which includes hospital-acquired pneumonia and ventilator-associated pneumonia. Common signs of pneumonia include cough, fever and difficulty breathing. Certain people are more likely to become ill with pneumonia, including: • Adults 65 years or older • Children younger than 5 years old • People who have underlying medical conditions (like asthma, diabetes or heart disease) • People who smoke cigarettes We can all help prevent pneumonia by following good hygiene practices, such as washing hands regularly and disinfecting frequently touched surfaces. The World Health Organization and the CDC class Tamiflu as an essential drug and many countries have stockpiled the drug at great expense. However, in a recent article in the Public Library of Science, this premise is being challenged. Roche Pharmaceuticals has refused to release the scientific research related to this question. The public is paying for stock piling but has no data proving that Tamiflu prevents pneumonia or saves lives. The data we do have shows that Tamiflu shortens the course of the illness by 1/2 - 1 day. Highlights: - The latest updated Cochrane Review: Neuraminidase inhibitors for preventing and treating influenza in healthy adults and children, is based on full internal reports of 20 Tamiflu (oseltamivir) and 26 Relenza (zanamivir) trials. These trials involved more than 24,000 people and the findings challenge the historical assumption that neuraminidase inhibitors are effective in combating influenza. The evidence also suggests there are insufficient grounds to support the use of Tamiflu in preventing person-to-person spread of influenza. This raises further questions about the most effective way to support drug regulation and public health policy decision-making. Claims about the effectiveness of Tamiflu against complications were a key factor in decisions made by governments around the world to stockpile these drugs in case of a pandemic. The US has spent more than $1.3 billion buying a strategic reserve of antivirals, while in the UK the government has spent almost £424 million for a stockpile of about 40 million doses*. - Compared with a placebo, taking Tamiflu led to a quicker alleviation of influenza-like symptoms of just half a day (from 7 days to 6.3 days) in adults, but the effect in children was more uncertain. There was no evidence of a reduction in hospitalisations or serious influenza complications; confirmed pneumonia, bronchitis, sinusitis or ear infection in either adults or children. Tamiflu also increased the risk of nausea and vomiting in adults by around 4 percent and in children by 5 percent. There was a reported increased risk of psychiatric events of around 1 percent when Tamiflu was used to prevent influenza. I had the flu-B a month ago and I had medical care treated with tamiflu! But I had been no better for long days after that with the obstinate cough, bloody sputum and a gouty pain in my right foot! At last I coughed up pretty much blood during work, so I couldn't help but retire from my work and went to a big city hospital to be inspected! The doctor told me that it was strongly suspected of lung cancer which grew up to be 2&1/3 inches after CT scan inspection!! I got a little surprised but I was feeling calm with no shock and no fear at all! I was able to accept my death in some months, a year or... smoothly! I was living with my loved gospel music and with going to hitting golf balls with my loved gears thinking may have been my last shots in this world really though unhealthy! I'd had pulmonary camera inspection and PET inspection before I was noticed that it was not a malignancy! Calm as always! Bacterial pneumonia?! I may be a no Gods and Buddha debater basically, of course I don't make light of Gods and Buddha at all, I should thank Them now! " I'm gonna serve the Lord"! Speaking of "I'm gonna serve the Lord", I can remember the version of "Rev. Robert Blair & The Violinaires Of Detroit" in Checker, 1966! (KLTV) - It is the beginning of the peak flu months, and some recent information suggests the most popular flu drugs don't work. The information is not referring to flu vaccines; those, doctors said, are reliable. The study shows the actual drugs like Tamiflu and Relenza, used to treat the flu, are not all that effective. Thousands of Americans die from the flu each year and, according to a study by the Cochran Collaboration, an independent medical research group, the drugs to treat it aren't helping. 24,000 patients who used Tamiflu were reviewed. The drug did decrease symptoms, just slightly, by 17 hours - from seven days to 6.3 days. Tyler area pharmacist David Davis said, in his experience, the findings sound about right. “If you catch the flu and you are going to be sick for seven days maybe you're only sick for six days,” he explained. The study also found the drug didn't prevent pneumonia, a common secondary symptom to the flu, that often lands patients in the emergency room. “Somebody that catches the flu that would've ended up in the hospital anyway; the drug is probably not going to prevent that from the evidence that I have seen,” Davis said. The study also found no reduced risk of death. But Trinity Mother Frances pharmacist James Tyler disagreed. “They work best when given early on,” Tyler explained. He said if given at the first sign of flu Tamiflu and related drugs can help you feel better. “They would reduce the severity, so the body aches, the fever, the general feeling of just feeling terrible all over,” he said. But, if given too late he questioned whether those drugs would help at all. “I think it's not as clear. In general I would say probably not.” Roche, the makers of Tamiflu, issued this statement saying, “The company fundamentally disagrees with the overall conclusions,” and cited a study they funded last year, which “showed the drug was associated with a reduction in the risk of death by 19 percent.” The Centers for Disease control still lists anti-viral drugs like Tamiflu as the recommended treatment for flu. Still, medical experts said the flu vaccine is the best defense. So, if you have not gotten your flu vaccine yet, you are encouraged to go get it. We are told there is no serious shortage of any vaccine in the East Texas area, although the four strain vaccine recommended for elderly has been going fastest.

Flu season is just around the corner and health experts are encouraging everyone to get vaccinated, including pregnant mothers. Dr. Melissa Lee, a gynecologist at Lee Memorial Health System, says no matter what stage of pregnancy it’s important to get a flu vaccine. “They are considered a higher risk category so we actually recommend every pregnant female get the flu vaccine.” The flu shot will protect both mother and baby throughout the entire pregnancy. “Not only does it protect the mom during pregnancy but it also transfers some immunity to the baby during pregnancy, and if you’re breast feeding, also after pregnancy,” said Dr. Lee. The vaccine helps the pregnant mother develop antibodies, protecting them from anyone who may have the flu. But some mothers worry about what the vaccine does to the baby. Dr. Lee says the vaccine will protect the baby too. “Everybody is always concerned about the thimerosal and the additive that are in it. Actually, thimerosal has not been associated with any autism issues and that’s usually the big red flag that everybody gets concerned about,” said Dr. Lee. If a pregnant mother gets the flu, it puts her and the baby at risk. “The risk of complications during pregnancy with the flu is much higher because your immune system is a little bit more compromised and your respiratory system also isn’t as good at clearing things,” said Dr. Lee. A pregnant mother who tests positive for the flu is at risk for pneumonia, hospitalization, even preterm labor. “A lot of symptoms they have are fever, chills, coughing, runny nose,” said Dr. Lee. Fortunately, pregnant mothers can take Tamiflu during pregnancy to help get rid of the symptoms. CINCINNATI (Liz Bonis) -- Public health experts in both northern Kentucky and Cincinnati said the tri-state is following a national trend. The flu and several other viruses are now at epidemic proportions. That also raises the risk for hospitalizations and deaths so there are some things people need to know to try and get the right treatment. Dr. Scott Woods of Bethesda Family Practice said, "Just lots of viruses going around, several of them causing pneumonia. Viral pneumonia is quite common right now." Viruses that cause colds and other illnesses tend to come on gradually and tend to have general respiratory symptoms. The flu tends to strike suddenly and usually includes respiratory symptoms but also body aches and a high fever. It's hard at times to tell the difference though. To find out if people have the flu they can get a quick check at their doctor's office or a walk in-clinic. If people do have the flu an antiviral agent such as Tamiflu in the first few days may help shorten how long people are sick. It can also make symptoms less severe. If people don't have the flu, hydration and taking over-the-counter medicines to ease a few aches is generally recommended. Hand washing and avoiding people who are sick is the best defense. If people do get sick they can lower the risk they'll cause harm to others by staying away from babies under six months. Especially stay away from babies under three months because if they spike a fever they are going to get a sepsis workup according to Dr. Woods. It's also better if people avoid visiting people in the hospital or at home if they are older or have compromised immunity due to breathing problems or heart conditions. Flu: treatment, and prevention Treatments for flu As flu is caused by a virus, antibiotics cannot help, unless the flu has led to another illness caused by bacteria. Antivirals, such as oseltamivir (Tamiflu) and zanamivir (Relenza), may be prescribed in some circumstances. Painkillers can alleviate some of the symptoms, such as headache and body pains. Some painkillers, such as aspirin, should not be given to children under 12. Individuals with flu should: stay at home avoid contact with other people where possible keep warm and rest consume plenty of liquids avoid alcohol stop smoking eat if possible It is a good idea for people that live alone to tell a relative, friend, or neighbor that they have flu and make sure someone can check in on them. Should people with flu tell their doctor? A doctor should only be informed if: the individual is frail or elderly their temperature remains high after 4-5 days symptoms worsen the individual feels seriously ill they become short of breath and/or develop chest pain If worried, a phone call to the doctor may be a better solution than making an appointment. Flu risks and complications In the majority of cases, flu is not serious - it is just unpleasant. For some people, however, there can be severe complications. This is more likely in very young children, in the elderly, and for individuals with other longstanding illness that can undermine their immune system. The risk of experiencing severe flu complications is higher for certain people: adults over 65 babies or young children pregnant women individuals with heart or cardiovascular disease those with chest problems, such as asthma or bronchitis individuals with kidney disease people with diabetes people taking steroids individuals undergoing treatment for cancer those with longstanding diseases that reduce immune system function Some of the complications caused by influenza may include bacterial pneumonia, dehydration, and worsening of chronic medical conditions, such as congestive heart failure, asthma, or diabetes. Children may develop sinus problems and ear infections. Flu prevention Flu vaccination Flu vaccination is the best protection against flu. Over 200,000 people are hospitalized from flu complications each year, and about 36,000 people are estimated to die as a result of flu. It is estimated that, globally, 250,000-500,000 people die each year as a result of flu. In industrialized countries, the majority of deaths occur among people over the age of 65. A flu epidemic - where a large number of people in one country are infected - can last several weeks. Health experts and government agencies throughout the world say that the single best way to protect oneself from catching flu is to get vaccinated every year. There are two types of vaccinations, the flu shot and the nasal-spray flu vaccine. The flu shot is administered with a needle, usually in the arm - it is approved for anyone older than 6 months, including healthy people and those with chronic medical conditions. The nasal-spray flu vaccine is a vaccine made with live, weakened flu viruses that do not cause illness. Seasonal flu shot A flu vaccine will contain three influenza viruses: influenza (H3N2) virus influenza (H1N2) virus one B virus As viruses adapt and change, so do those contained within the vaccines - what is included in them is based on international surveillance and scientists' calculations about which virus types and strains will circulate in a given year. Protection begins about 2 weeks after receiving the vaccination. Seasonal flu vaccinations should start in September or as soon as the vaccine is on hand, and continue throughout the flu season, into January, and beyond. This is because the timing and duration of influenza seasons are never the same. Flu outbreaks usually peak at around January, but they can happen as early as October. Seasonal flu shots are not suitable for some people Certain individuals should check with their doctor before deciding to have the flu vaccine, including: Individuals with a severe allergy to chicken eggs. Individuals who have had a severe reaction to a flu vaccination in the past. Individuals who developed Guillain-Barré Syndrome within 6 weeks of receiving a flu vaccine. Children under 6 months old. Individuals experiencing a fever with a moderate-to-severe illness should wait until they recover before being vaccinated. Flu virus types Three types of flu viruses exist - influenza A, influenza B, and influenza C. Types A and B viruses cause seasonal epidemics that hit the United States and Europe virtually every winter. The type C influenza virus causes mild respiratory illness and is not responsible for outbreaks.

Is it safe for a pregnant woman to take Tamiflu to protect her from getting influenza?

You wanted to know if your friend should be taking Tamiflu, because her husband has been diagnosed with influenza, and she's 36 weeks along. And the best advice I have for your friend is to call her doctor and ask them if they think she should be taking Tamiflu or any other antiviral medications. Based on their knowledge of her circumstances, they'll be able to give her the best advice. Generally speaking, the current recommendation is that women who are in their 2nd and 3rd trimesters should get a form of antiviral medication to help fight the flu if they are exposed to someone who has influenza. In the 1st trimester, though, this medication should not be given. These types of medications are in pregnancy category C. And that means, basically in this situation, that they haven't been proven to be safe during pregnancy, because, ethically, we can't study their effects on a pregnant woman and her baby. But they have been given to pregnant women, and we haven't seen any adverse effects for a woman or her baby that was exposed in utero during the 2nd and 3rd trimesters. We do know that, in the 1st trimester, there can be problems. And if a pregnant woman gets the flu, it's more likely to be serious as the pregnancy progresses. So in the 3rd trimester, she's most at risk. In the 1st trimester, the baby is most at risk to getting medication. So again, that's why a woman in the 2nd or 3rd trimester should get medication, but not those that are in the 1st. So again, the doctor knows the most about her situation and will be able to give her the best advice. Really, the best way to prevent the spread of illness is good hand-washing. Encourage your friend to wash her hands frequently, and to have her husband wash his hands frequently, and to decontaminate commonly-touched surfaces in the home, and just keep her distance until he's all better. And if she's gotten a flu shot during her pregnancy (which pregnant women should get at any point in the pregnancy), that will also offer her protection. Is this swine flu virus contagious? CDC has determined that this swine influenza A (H1N1) virus is contagious and is spreading from human to human. However, at this time, it is not known how easily the virus spreads between people. What are the signs and symptoms of swine flu in people? The symptoms of swine flu in people are similar to the symptoms of regular human flu and include fever, cough, sore throat, body aches, headache, chills and fatigue. Some people have reported diarrhea and vomiting associated with swine flu. In the past, severe illness (pneumonia and respiratory failure) and deaths have been reported with swine flu infection in people. Like seasonal flu, swine flu may cause a worsening of underlying chronic medical conditions. How does swine flu spread? Spread of this swine influenza A (H1N1) virus is thought to be happening in the same way that seasonal flu spreads. Flu viruses are spread mainly from person to person through coughing or sneezing of people with influenza. Sometimes people may become infected by touching something with flu viruses on it and then touching their mouth or nose. Can I get swine influenza from eating or preparing pork? No. Swine influenza viruses are not spread by food. You cannot get swine influenza from eating pork or pork products. Eating properly handled and cooked pork products is safe. How can someone with the flu infect someone else? Infected people may be able to infect others beginning 1 day before symptoms develop and up to 7 or more days after becoming sick. That means that you may be able to pass on the flu to someone else before you know you are sick, as well as while you are sick. What should I do to keep from getting the flu? First and most important: wash your hands. Try to stay in good general health. Get plenty of sleep, be physically active, manage your stress, drink plenty of fluids, and eat nutritious food. Try not touch surfaces that may be contaminated with the flu virus. Avoid close contact with people who are sick. Are there medicines to treat swine flu? Yes. CDC recommends the use of oseltamivir or zanamivir (tamiflu) for the treatment and/or prevention of infection with these swine influenza viruses. Antiviral drugs are prescription medicines (pills, liquid or an inhaler) that fight against the flu by keeping flu viruses from reproducing in your body. If you get sick, antiviral drugs can make your illness milder and make you feel better faster. They may also prevent serious flu complications.
