Hear Dr. Sengupta present the structure and function of the spine, and conditions like lumbar disc herniation (often called "sciatica"), lumbar stenosis and claudication pain, and lumbar degenerative spondylolisthesis. The discs act as a cushion for the spine and if they start to push on the nerves it causes pain. Sciatica is a symptom, not the disease (pain goes down the leg) -- the diagnosis might be something different. More than 80% of patients do get better on their own no matter what treatment you have, or no treatment at all. Pain medication is the first line of treatment along with exercises and activity modification. Never stay in bed or remain inactive and learn the right way to lift and carry things. Not all exercises are appropriate in the beginning; you may need help from a professional to find the right way to do an exercise. After you've tried those options, surgery may be the next best choice --a discectomy. Dr. Sengupta provides satisfaction data up to four years out from patients who had surgery versus those who did not. Spinal stenosis or degenerative disc disease is when there is not enough space between vertebrae. As you bend forward you open up space for the nerves. Most patients age 60 or over often have this problem but may not be experiencing any symptoms -- don't jump to an MRI too soon. Keep active, do exercises, use a cane if you need to. Also try acupuncture, massage, physical therapy or see a chiropractor. It is important to get going -- keep doing household activities, plus exercises. Swimming can be particularly helpful. If nothing else is working, surgery is the answer -- a laminectomy. In this procedure, a bone is taken out, leaving the nerves uncovered (which is perfectly safe). Dr. Sengupta also provides four and ten year data for patients who had surgery versus those who did not. With spondylolisthesis one bone slips on top of another. The pain can be chronic or acute; as you move it hurts. Nicotine does cause this issue so plan to quit as an early measure. The important messages overall are that just because you feel pain, it doesn't mean you are causing harm. Keep active and don't simply lay in bed. DR Nabil Ebraheim UTMC Toledo. The cartilage of the knee is complex and it is made of elastic comprehensive structure. The normal articular cartilage is called hyaline cartilage; it provides a smooth, gliding surface to help the motion of the joint. There is about 2cc of the synovial fluid inside the knee that helps in the motion and lubrication of the joint. Between the hyaline cartilage which is called the articular cartilage you can see the meniscus, the lateral and the medial, the meniscus is a shock absorbing cartilage or cushion between the articular cartilages. The hyaline cartilage has 4 layers: 1- The superficial layer. 2- The middle layer. 3- The deep layer. 4- The calcified layer After the calcified layer you find the bone. These cartilage cells are supposed to live forever. Good cartilage cells are sterile; they can’t make more cartilage if these cartilage cells are destroyed. If the cartilage is subjected to excessive wear, trauma, injury, overuse, excessive weight or improper alignment, then the cartilage will wear away leaving the bone to rub against bone. The cartilage doesn’t have the ability to heal itself by hyaline cartilage, but sometimes it can heal itself by inferior type of cartilage called fibrocartilage, especially if the area that needs to be repaired is small. What is the treatment of the arthritis of the knee? 1- Medication: people respond differently to medications, the doctor select the type, dosage and duration of the treatment, the medication has to be safe and effective. Ask the doctor about the side effect of the medication. There are several medications for example: NSAIDS. 2- Losing Weight: it’s advisable to lose about 5% of the body weight, especially if the BMI was above 25. 3- Low Impact Physical Therapy: such activities like swimming or cycling, adds less stress on the knees, life style modification to protect the knee will slow the progress of the arthritis, there is a strong evidence that physical therapy will help the patient. The physiotherapy will decrease the pain, will improve the function, increase the strength, the range of motion; physiotherapy should be individualized with program that meets the patient’s needs, the life style, and expectations. 4- The physician will also use intra-articular injection of steroids; viscosupplementation, or hyaluronic acid, injection will relieve the patient’s pain and disability. 5- Other injection methods: - Gene therapy - PRP - Growth factor - Stem cells 6- Other treatment options: - Acupuncture - Massage - Glucosamine - Chondroitin sulfate - Valgus directing brace - Wedges in the foot - Arthroscopic debridement and lavage 7- Assistance devices: - Cane - Shock absorbing shoe - Shoe inserts - Knee sleeves - Support brace 8- Arthroscopy shouldn’t be done in arthritis unless there is loose body or meniscal tear causing mechanical symptoms of recurring lacking, catching, swelling and pain. There are some guide lines from the American Academy of Orthopaedic Surgeon AAOS: these guidelines are suggestive, and the treatment of the arthritis patient should be individualized and based on: - The doctor’s clinical judgment. - The patient’s clinical situation. - The evidence tested and published. It is a combination of all of these factors. If it works for you, it may not work for your neighbor. So basically arthritis can be minimal, and anything can help the arthritis ion this situation. But if the arthritis is moderate then the physician has many options for treatment and none of those options are predictable in the result. When the arthritis is severe, total knee is the most predictable option for treatment of arthritis. You have to reduce the pain and improve the function of the knee. More Details About This Video: Sometimes you just can't do exercises on land because they are either too painful, or you just might not be strong enough to do them on land. Aquatic therapy helps you get the exercise you need in a low impact environment. You can really concentrate on your technique because you don't have to worry about falling. After you do a warm-up in the pool of gait exercises, then you can work on some standing exercises. It is really important to make sure you have good posture when you are in the pool. It is really easy to move your upper body a lot, but you want to try to keep your upper body straight and upright. Squeeze in your abdominals to help keep that upright position. Using ankle weights can be very beneficial. Not only do they give you some extra resistance, but they also keep your feet on the ground while doing the exercises. Hold onto the side or a bar to help keep your upright posture. Start off with a hip flexion/extension (front to back) exercise. Lock out your knee and try to keep it straight the whole time. Pulling your toes up or into dorsiflexion will help keep your knee straight. Bring your foot up as high as you can without leaning back, and then bring it back as far as you can without leaning forward. Then still keeping the knee locked out, perform hip abduction/adduction (side to side). Try not to turn your foot out when kicking out to the side. This will change the muscle you are trying to work. Start off with 10 on each side, and then work your way up. Next is a hamstring curl. You are only bending your knee here. Try to keep your hip in a neutral position. If you bend your hip up, you are working different muscles. If you have to hold your hip down, that is fine, but it should stay in alignment with the other hip. Then you are going to do a Rockette kick. Bend your knee up into a march position, then kick your leg straight out. If that is too much of a stretch, don't bring your knee quite as high. Make sure to do all these exercises on both sides. When you are standing on the leg, and moving the other, you are using your muscles as stabilizers so they are being worked in different ways. The final two will be done with your feet about shoulder width apart, and again, make sure your feet are pointed forward. For the heel/toe raises, make sure to go as high up on your toes as you can to work the calf muscles. Then roll back onto the heels and pull your toes up to work the anterior tibialis muscle. Try not to stick your bottom back, just pull your toes up. Lastly, is a squat. Keep your feet flat on the ground the whole time. Stick your bottom back like you are going to sit in a chair to make sure your knees do not go in front of your toes. Bring your chest slightly forward, and keep your back straight. If these become easy and you can do 20-25 without difficulty, try doing them without holding on to the side. If that is still easy, try holding on, but increase your speed. Make smaller, but quicker movements, and this will add resistance in the water. If you can't hold your posture while doing these, then you are not quite ready yet! Judy Bryan, age 55, came to Premier Rehab two and a half months after having a lumbar spinal fusion. She had suffered from scoliosis since early childhood, but the increasing pain had finally convinced her to look into surgery, something Judy was very wary of doing. Judy ended up having lumbar spinal fusion from T9 to sacrum, totaling nearly half of her back. The buoyancy and resistance of the pool allowed Brad Stucky, Physical Therapist, to work with Judy to reeducate her muscles without the pain inhibition that prevents movement and causes muscle guarding. More Details About This Video: One of the best ways to get your walking or gait pattern back after an injury is aquatic gait exercises. Walking in the water not only takes away some of gravity, but it also allows you to concentrate on getting your correct pattern back. It is a great way to get a workout without a lot of impact on your joints. These gait exercises are a good warm-up before you do an aquatic therapy workout. It is really important to make sure you have good posture when you are in the pool. It is really easy to move your upper body a lot, but you want to try to keep your upper body straight and upright. Squeeze in your abdominals to help keep that upright position. Using ankle weights can be very beneficial. Not only do they give you some extra resistance, but they also keep your feet on the ground while doing the exercises. To start off, simply walk forward. Concentrate on a heel to toe walking. When we walk on land, our heels hit first and we roll onto and push off with our toes. Exaggerate this movement while you walk while keeping that upright posture. Then reverse it and go backwards. Now your toes are going to hit first and roll back onto your heels. Try not to lean back when you are doing this, and keep your body upright. Next, turn sideways. When you step, it is important to keep your toes pointed forward. If you turn your foot out when you step, you are changing the muscles being worked. Finally, march by bringing your knee up toward the surface of the water getting a bend in your knee and hip. Really try to drive your knee upward to get a good stretch. Again, don't lean forward, try to keep your body upright. Depending on how fast you go, these should take you between 5-10 minutes and is used as a warm-up to your exercises. I'd like to spend a moment now talking about an exercise called the swimmer. The swimmer is a great exercise used in the clinic, using physical therapy when we're trying to rehab a back patient. It's a little less strenuous on the back than some other ones, so I often introduce the exercise in the middle of their care as they're beginning their rehab strengthening component of their care. Hopefully the pain has been under control now and they're ready to start challenging their spine with more difficult exercises. So the key to this exercise is going to be you go face down now, and what you're looking to do is to try to lift up your leg and your opposite arm at the same time. Why don't you go ahead and do that? Good. You're going to hold that for a few seconds and the muscles that are being strengthened with this exercise are going to be the back and a little bit of your core as well as your hips and as well as your scapular and arm. Now switch. This is a little less strenuous on the back in that you are already flat, you're already supported. You're not trying to hold your balance like you would in an exercise like the bird dog. You're already flat and supported so your core can actually turn off a little bit and it's mostly just a back strengthener. Other side, just one more time. With this exercise you can actually do it with doing repetitions in each or you can actually hold it for an extended period of time with a goal being about 30 seconds to a minute. Switch one more time. The key to this exercise is going to be to really make sure that you're focusing at the knee to some degree. Make sure the knees are being straight. If the knee is bending a little during the exercise, it's going to cause the hamstring muscle to kick in and a little bit less of your gluten back where you really want to focus your attention. And that's the swimmer.
