Finasteride, the active ingredient in the brand names Propecia and Proscar, is a medication used to treat and slow down male pattern baldness. It works by blocking the chemical DHT which causes hair follicles to shrink and die. DHT initiates the start of puberty in boys and causes the genitals to develop and the growth of pubic and body hair.more importantly, it causes the prostate to grow during puberty. Finasteride inhibits the conversion of testosterone into DHT, and may cause abnormalities in the external genitalia of a male fetus if finasteride is exposed. First off, finasteride is NOT approved for use in women to treat hair loss. A study was done on pregnant monkeys who were given large doses of finasteride by mouth, and had resulted in male offspring with abnormalities of the sex organs. Pregnant rats were also given finasteride and the results showed that it increased the risk of pre-term birth and impaired cognitive functioning in newborns. The defect causes the opening of the urinary tract to appear on the underside of the penis when it should actually be at the tip. Due to the complications of genital detects in male babies, women are warned against touching broken or crushed tablets of Propecia, as there is the remote possiblity that they may absorb some of the active ingredient into their system. To guard against this issue, the tablets have a coating on them. However, some people do cut up 5mg of Proscar into 1.25 mg tablets, so taking caution when doing this around pregnant women is highly recommended. If you do cut Proscar or Propecia tablets, make sure you don’t use the same cup as your girlfriend/wife as the residue can come into contact. Although highly unlikely, theoretically, there is a risk of birth defect in a male baby’s sex organs in a couple has intercourse during the stage of pregnancy when the sex organs are developing as traces of finasteride can be found in small amounts in semen. After birth of the baby, finasteride should not be used if the baby is breastfed as finasteride can also be absorbed by the mother and passed onto the baby through breastfeeding. Finasteride, the generic brand for Propecia and Prosper, is an FDA approved medication used in the US to deal with male pattern baldness (MPB). Finasteride prevents the conversion of testosterone into dihydrotestosterone (DHT) in males, reducing miniaturization of the hair. I've been taking 1mg finasteride daily for the past 2 months now to treat my frontal hair loss. A small percentage of people taking finasteride/propecia have side effects ranging from decreased sexual desire or ability to dizziness and allergic reactions. Always consult with your doctor before taking any kind of medication. Also, make sure to keep finasteride/propecia away from females who are pregnant or in the process of getting pregnant as the medication can cause birth defects. Losing any hairs is scary. Sudden, significant hair loss is frightening. But the good news is that many causes of sudden, significant hair loss can be very easily remedied. In this episode of DermTV, Dr. Schultz discusses a type of sudden hair loss called Telegen Effluvium and how to treat it. That's normal and usually not very noticeable. But there's a frightening form of very rapid hair loss that causes 30%, or even up to 50% of your hair to shed in just a few weeks. It's called Telogen effluvium. And today, I'll tell you why it occurs and what you can do. In other DermTV episodes, I've mentioned that most people normally lose 100 to 150 scalp hairs daily. But even if you see some of those hairs in the shower drain, the good news is your hair isn't thinning because you grow the same number of new hairs as you lose, every day, maintaining the status quo. But very rapid frightening hair loss resulting in visible thinning of your scalp hair occurs in a condition called Telegen Effluvium. It results from a resetting of the scalp hair growth cycle. In this case, instead of 5 to 15% of the hair follicles being in the resting phase... and it's the hairs in the resting phase that normally shed... instead of 5 to 15%, 30 to 40, or even 50% of the follicles enter the resting phase at the same time. As a result, all of those hairs are shed synchronously 2 to 3 months later causing massive, rapid hair loss. But don't despair! The follicles aren't damaged. They're just sleeping. And just as 30 to 50% went to sleep at the same time, that same 30 to 50% will wake up and start re-growing at the same time as well. So 6 to 8 months later, there won't be any net hair loss. All the fallen hairs will have regrown! Of course, this takes patience and faith! And I often recommend topical minoxidil for women and propecia pills for men during that time, just to help the regrowth. The most common cause of Telegen Effluvium is childbirth... in which case it's called postpartum alopecia. So predictably in the 3rd or 4th month after giving birth, many women experience this frightening but temporary hair loss. Other causes include high fevers, any surgery involving general anesthesia, very rapid weight loss from severe calorie restriction from a crash diet, emotional disorders, and severe stress. Telogen effluvium can affect hair on all parts of the body, but, generally, only the loss of scalp hair is symptomatic or clinically noticeable. If you experience significant and rapid hair loss 3 to 4 months after one of the causes I just mentioned, shampoo regularly and at least try over the counter topical minoxidil. For reassurance, see your dermatologist. And for the brave of heart, just keep the faith! Male pattern baldness Male pattern baldness is the most common type of hair loss in men. Causes Male pattern baldness is related to your genes and male sex hormones. It usually follows a pattern of receding hairline and hair thinning on the crown, and is caused by hormones and genetic predisposition. Each strand of hair you have sits in a tiny hole (cavity) in the skin called a follicle. Generally, baldness occurs when the hair follicle shrinks over time, resulting in shorter and finer hair. Eventually, the follicle does not grow new hair. The follicles remain alive, which suggests that it is still possible to grow new hair. Symptoms The typical pattern of male baldness begins at the hairline. The hairline gradually moves backward (recedes) and forms an "M" shape. Eventually the hair becomes finer, shorter, and thinner, and creates a U-shaped (or horseshoe) pattern of hair around the sides of the head. Exams and Tests Classic male pattern baldness is usually diagnosed based on the appearance and pattern of the hair loss. Hair loss may be due to other conditions. This may be true if hair loss occurs in patches, you shed a lot of hair, your hair breaks, or you have hair loss along with redness, scaling, pus, or pain. A skin biopsy, blood tests, or other procedures may be needed to diagnose other disorders that cause hair loss. Hair analysis is not accurate for diagnosing hair loss due to nutritional or similar disorders. But it may reveal substances such as arsenic or lead. Treatment Treatment is not necessary if you are comfortable with your appearance. Hair weaving, hairpieces, or change of hairstyle may disguise the hair loss. This is usually the least expensive and safest approach for male baldness. Medicines that treat male pattern baldness include: Minoxidil (Rogaine), a solution that is applied directly to the scalp to stimulate the hair follicles. It slows hair loss for many men, and some men grow new hair. Hair loss returns when you stop using this medicine. Finasteride (Propecia, Proscar), a pill that interferes with the production of a highly active form of testosterone that is linked to baldness. It slows hair loss. It works slightly better than minoxidil. Hair loss returns when you stop using this medicine. Dutasteride is similar to finasteride, but may be more effective. Hair transplants consist of removing tiny plugs of hair from areas where the hair is continuing to grow and placing them in areas that are balding. This can cause minor scarring and possibly, infection. The procedure usually requires multiple sessions and may be expensive. Suturing hair pieces to the scalp is not recommended. It can result in scars, infections, and abscess of the scalp. The use of hair implants made of artificial fibers was banned by the FDA because of the high rate of infection. Outlook (Prognosis) Male pattern baldness does not indicate a medical disorder, but it may affect self-esteem or cause anxiety. The hair loss is usually permanent. When to Contact a Medical Professional Call your health care provider if: Your hair loss occurs in an atypical pattern, including rapid hair loss, widespread shedding, hair loss in patches, or hair breakage. Hair-Growth Drug Linked to Erectile Dysfunction That Lasts for Years

Young men who take drugs to treat hair loss or an enlarged prostate may experience an unwanted side effect — erectile dysfunction — and the problem can persist long after these men stop taking the drugs, a new study finds. Men under age 42 in the study who took one of these drugs for more than 205 days were nearly five times more likely to have long-term erectile dysfunction than men who took the drug for less than 205 days, according to the study. One of the drugs in the study was finasteride, which is sold under the brand names Propecia and Proscar. Finasteride affects how cells make testosterone; it may be taken for hair growth or to shrink the prostate.The other drug, dutasteride, is taken to shrink an enlarged prostate. It is sold under the brand name Avodart and is one of the drugs in the medication Jalyn. The new findings linking these drugs to debilitating sexual dysfunction should be of particular interest to prescribers and patients, senior study author Dr. Steven Belknap, a research assistant professor of dermatology at Northwestern University Feinberg School of Medicine in Chicago, said in a statement. "Our study shows [that] men who take finasteride or dutasteride can get persistent erectile dysfunction, in which they will not be able to have normal erections for months or years after stopping finasteride or dutasteride," Belknap said. In the new study, which was published today (March 9) in the journal PeerJ, the researchers looked at data on more than 12,000 men in the Northwestern Medicine Enterprise Data Warehouse, a database of medical records for patients treated at Northwestern Medicine. They focused on men ages 16 to 89 who had been prescribed one of the drugs at least once from 1992 to 2013. The researchers looked at the both the dosage of the drugs that the men were prescribed and the length of time they took the medications. In addition, they looked for any diagnoses of sexual dysfunction, such as low libido, erectile dysfunction and persistent erectile dysfunction. Results showed that 167 of the men (1.4 percent) who took either finasteride or dutasteride developed erectile dysfunction that lasted for a median of about three and a half years after they stopped taking the drug. In men ages 16 to 42, the length of time that the men took either of the drugs was a better predictor of their risk of persistent erectile dysfunction than other factors, such as the dosage of the drug, the researchers found. That length of time was also a better predictor than other medical conditions that have been linked with erectile dysfunction, such diabetes or hypertension according to the study. Among the men in this age group who developed erectile dysfunction after taking the drugs, the median duration of their ED was about four years, the researchers found. Both finasteride and dutasteride work by blocking the conversion of the hormone testosterone into its more active form, 5-alpha-dihydrotestosterone (DHT). DHT can contribute to male pattern baldness because it dampens another chemical signal in the body that fuels hair-cell growth. In a meta-analysis published in 2015 in the journal JAMA Dermatology, the same group of researchers found there was not enough evidence to establish that finasteride was safe to take for hair-loss treatment in men.
