You mentioned that you're doing fertility treatments. You're taking Clomid and you started that mid-September and you're wondering when you can find out if you're pregnant. I can understand why you'd be so anxious and why you'd want to know as soon as possible. Now, as for when you'll be able to find out if you're pregnant or not, there's so many different variables that will affect this and so my recommendation is that you call and talk to your doctor. They know the specifics of your situation, like where you were in your cycle when you started the Clomid, when they would except you to ovulate based on their knowledge of your previous cycle history. They can give you the best information. But generally speaking, you can usually find out if you're pregnant about a week after you've missed a period. So if it was supposed to start on Monday, you can usually find out if you're pregnant by the end of the week, Thursday Friday, Saturday. Over-the-counter pregnancy tests are detecting a hormone called human chorionic gonadotropin, or hCG. Your body releases this hormone early in pregnancy and throughout the first trimester. The levels grow exponentially daily, especially early on in pregnancy. So each day makes a big difference in how much is in your blood. So, if you don't have enough, then the test may not detect it when you take an over-the-counter test too early. But if you give it a few days, like wait a week after your missed period, then usually an over-the-counter test will detect a positive result. But again, I think it would be best for you to talk to your doctor who knows exactly where you were in your cycle and when you could have been expected to ovulate and when you can find out if you're pregnant. In order for a woman to get pregnant, she needs to ovulate, or release an egg that can then be fertilized by sperm. And in order for pregnancy to happen, intercourse needs to be timed just right with ovulation. There are some medical conditions out there that affect a woman's ability to ovulate. For example, one of those is polycystic ovarian syndrome, or PCOS. If you haven't been having irregular cycles, or you're not ovulating, and you know that because you've been testing, then you should talk with your doctor, and they'll be able to recommend a plan - something that will help to stimulate ovulation and hopefully increase your chances of conceiving. One of the treatment methods used commonly for women who aren't ovulating is Clomid. It's a medication that stimulates your body to release an egg. To increase your chances of conceiving, you need to start having intercourse every other day, 5 days after your last dose of Clomid is taken, and continue with this for a week. And if you do this, then hopefully things will be timed just right. In conjunction with Clomid therapy, your doctor will probably recommend checking for a luteinizing hormone surge, or LH surge, and that's associated with ovulation. You can detect this by using ovulation kits. And as soon as you're seeing a positive, that gives you about a 1 to 2 day window. So if you weren't already going to have intercourse during this time, then you need to plan on it. There are some potential side effects associated with Clomid use, and one is the risk of multiple gestation. For some people, that's not a risk, and that would be welcome, especially if they've been trying to have babies for a while. But you should know, when you take this, there's a slightly higher chance that you could end up with twins, or maybe even triplets. The chance of twins is about 7% to 8%, and that's higher than the normal population that isn't taking Clomid. Other side effects include headache, nausea, vomiting, breast tenderness, abdominal pain, bloating, mood swings. And many of these things are actually associated with pregnancy too, so it makes women feel extra hopeful, when they've been taking Clomid, that they're pregnant, but these symptoms, if they're associated with pregnancy won't happen until about 5 to 6 weeks gestation, or about 5 to 6 weeks from your last period. So you can expect to feel things if you've been taking Clomid. Hot flashes are also common. But if you experience anything that seems abnormal to you, call your doctor, and they'll decide if it's a side effect of the medication, or if further investigation is necessary. Now as for how many doses you might take, a lot of women do successfully get pregnant and conceive when they take Clomid, but sometimes it's not effective. And sometimes it takes multiple rounds, even when it does work. So it may not work the first month, or even the second, or even the third. Your doctor may recommend trying for a few months to see if it helps, but after about 6 months, studies have shown that pregnancy rates actually fall significantly for women who have been using Clomid for that amount of time. And at that point, a doctor should look into other reasons for infertility. Maybe in addition to not ovulating, there's fallopian tube blockage. There's so many different factors that go into a woman and a man being able to conceive, so they might want to investigate other possibilities, other underlying conditions that need to be addressed. So I suggest calling your doctor, letting them know that you're trying to have a baby, and based on their knowledge of your circumstances, they'll decide if further investigation or treatment is necessary before you get pregnant. And here's why. In the first place, it can affect your ability to conceive if your thyroid hormone is off, it might make it so you don't ovulate or have regular periods, which would make it difficult. But then if you do get pregnant, if you have abnormal thyroid levels, it can affect the development of the baby. So that's why you want to make sure that your thyroid hormone is in a normal range before you get pregnant. Why do I ovulate and not get pregnant? I’m assuming you are not using birth control methods that would interfere with that. No, I’m trying to get pregnant. If you’ve had social diseases, the inflammation they cause can block the Fallopian tubes, preventing pregnancy. I was treated for that ages ago. You could try getting more iron and micro-nutrients in general, since that might impede conception. I’m taking prenatal vitamins now. There’s the possibility that you’re reading signs like mucus changes that suggest you are ovulating but are not really. I’m using ovulation tests as religiously now as I used to take birth control. Speaking of which, around one percent of pregnancies are reported as virgin births. This would not be a virgin birth. True, but the egg is only good for a day or two, though sperm theoretically lasts five days in her body. The Centers for Disease Control has challenged all women to make a PACT, or a commitment to themselves to do whatever they can to get healthy before trying to get pregnant. PACT is an acronym to help us remember what we can do. P is for Plan ahead. A is Avoid harmful substances. C is Choose a healthy lifestyle. T is Talk with your doctor. Let's start with P, or Planning ahead. One of the first things you can do is start today by taking a prenatal vitamin that has at least 400mg of folic acid. This ensures that there's enough folic acid in your system when you actually get pregnant. It will reduce the chances of your baby having spina bifida. There are different germ layers forming in the first couple of weeks after conception before you even know you're pregnant, and if there's not enough folic acid, that's when this can happen. So you need to plan ahead and take a prenatal vitamin even before you start trying to conceive. A is for Avoiding harmful substances. Avoid alcohol, smoking, illicit street drugs, and think twice about prescription medications, over-the-counter medications, and herbal supplements. There is no safe amount of alcohol at any time during pregnancy, so just avoid it. If you're trying to get pregnant, stop drinking before you even start the process. If you do drink during pregnancy, then it increases the chances of your baby having a host of complications that can last a lifetime for them. These are called fetal alcohol spectrum disorders. Hey guys, I wanted to update you guys on what is going on with our TTC journey. I started my first Clomid cycle of 50mg in March. Unfortunately it didn’t work so my doctor gave me another dose of 100mg. So I basically did 2 cycles of Clomid in one cycle. It has been a week since my last dose and I still haven’t gotten a positive OPK test. I think next steps for me would be to keep restating. If I don’t get a positive my this weekend, my doctor is going to send me to a fertility endocrinologist. Hopefully we can find out what the underlying cause of this. An ovulation prediction kit helps a woman know when she's ovulating. It works by detecting a surge in luteinizing hormone, or LH, and this happens usually about mid-cycle. And if you're testing for it, then you should see this 12 to 36 hours before your body is going to ovulate. Now, home pregnancy tests are detecting human chorionic gonadotropin, a different type of hormone also known as HCG. And a woman's body starts to release this during early pregnancy, and levels rise exponentially until about 8 to 11 weeks gestation. But both of these hormones, LH and HCG, are glycoproteins, basically a protein molecule with a sugar molecule attached to it. And there are some women who have gotten positive ovulation kit test results during early pregnancy, but haven't gotten the positive pregnancy test result yet, because it's too soon to get that, but the ovulation kit is detecting the presence of the glycoprotein. So you can't use ovulation prediction kit tests as pregnancy tests, or vice versa. If you think you're pregnant, basically the best thing to do is to wait and see if your period starts like it should. If you're period never starts, that day has come when it should have, and it doesn't, then it's time to take a home pregnancy test. If that first result is negative, then wait another 5 to 7 days and retest. It's possible to get false negatives if you take the test too early, and for some women, this is even on the day of their missed period, or in the few days following, because they ovulated later than usual. Most women ovulate about mid-cycle, but some women do ovulate a little bit later, and if this is the case with you, then you might not see a positive pregnancy test until at least 2 weeks from that time, which might be a week or so after your missed period. So again, I recommend taking another test in about 5 to 7 days and seeing what that result says, and if it's positive, congratulations! But if it's negative, keep the appointment set that you have with your OB already, and based on their ability to ask you more specific questions and to perform an exam, they'll decide what's going on, and they'll be able to give you the best advice. If you think or know you have fibroids, the most important factor for determining their impact on your fertility is where those fibroids are located. Some fibroids may not cause any problems with conception. Fibroids can cause infertility if they are either located inside the uterine cavity, are causing distortion of the uterine cavity or if they cause the fallopian tubes to become blocked. In any of these cases, surgery should be performed to remove the fibroids. Other treatments for fibroids are not recommended for women who want to attempt pregnancy. They are reserved for women who are done with child bearing. Fibroids that are located entirely inside the uterine cavity can be removed without incisions by using hysteroscopy. Essentially, a thin telescope is placed through the vagina into the uterus. Fibroids can be resected and removed through the scope without making any incisions. Recovery time is quick and pregnancy can be attempted right away. Fibroids that are partly or completely within the muscle of the uterus will require incisions in the abdomen to remove. Previously, many fibroids like this were removed using a telescope that went through an incision in the abdomen called a laparoscope. However, many experts now recommend avoiding this method due to a fear that unseen cancers could be spread when trying to remove the fibroid from the abdomen. Currently, the recommended method to remove fibroids is with a larger single incision. Recover times for this type of surgery are longer and pregnancy should not be attempted until the uterine muscle has had a chance to heal. This usually takes a 2-3 months. The great news is that removal of fibroids can be an excellent way to improve your chances for becoming pregnant. Endometriosis can cause pain and infertility or both. Diagnosing endometriosis can be tricky. Not all women with painful periods have endometriosis. Women with more severe forms of endometriosis will usually have endometriosis cysts in their ovaries that can been on ultrasound. Women with a less severe stage, however, cannot be diagnosed with ultrasound. For these women, surgery to look inside the abdomen is the only way to prove whether endometriosis is present or not. If you have endometriosis, it can be more difficult to conceive. The more severe the endometriosis, the more difficult it may be. There are several options for trying to conceive with endometriosis. Surgery to vaporize endometriosis is an effective option to improve fertility. Word of caution, not every woman will get pregnant simply by vaporizing her endometriosis. In fact, for women with mildest forms of endometriosis, you would have to do surgery on 11 women to get one extra pregnancy. Women with most severe forms of endometriosis, who have endometriosis cysts in their ovary, should not have surgery if their ovarian reserve is poor as it can further lower the number of viable eggs they have remaining. The use of fertility pills such as Clomid or injections such as Follistim with either intercourse or inseminations are commonly to used to treat infertility but their effectiveness in women with endometriosis remains questionable. Finally, IVF has become a go to option for women with endometriosis. Its effectiveness is well proven compared to other treatments but questions remain over whether pregnancy rates are as good as they are compared to couples with other types of fertility problems.
