Reactive arthritis is classified as an autoimmune condition that develops in response to an infection in another part of the body (cross-reactivity). Coming into contact with bacteria and developing an infection can trigger the disease. By the time the patient presents with symptoms, often the "trigger" infection has been cured or is in remission in chronic cases, thus making determination of the initial cause difficult. The arthritis often is coupled with other characteristic symptoms; this is called Reiter's syndrome, Reiter's disease or Reiter's arthritis. The term "reactive arthritis" is increasingly used as a substitute for this designation because of Hans Conrad Julius Reiter's war crimes with the Nazi Party. The manifestations of reactive arthritis include the following triad of symptoms: an inflammatory arthritis of large joints, inflammation of the eyes in the form of conjunctivitis or uveitis, and urethritis in men or cervicitis in women. Arthritis occurring alone following sexual exposure or enteric infection is also known as reactive arthritis. Patients can also present with mucocutaneous lesions, as well as psoriasis-like skin lesions such as circinate balanitis, and keratoderma blennorrhagicum. Enthesitis can involve the Achilles tendon resulting in heel pain. Not all affected persons have all the manifestations. The clinical pattern of reactive arthritis commonly consists of an inflammation of fewer than five joints which often includes the knee or sacroiliac joint. The arthritis may be "additive" (more joints become inflamed in addition to the primarily affected one) or "migratory" (new joints become inflamed after the initially inflamed site has already improved). Reactive arthritis is an RF-seronegative, HLA-B27-linked arthritis often precipitated by genitourinary or gastrointestinal infections. The most common triggers are intestinal infections (with Salmonella, Shigella or Campylobacter) and sexually transmitted infections (with Chlamydia trachomatis). It most commonly strikes individuals aged 20–40 years of age, is more common in men than in women, and more common in white than in black people. This is owing to the high frequency of the HLA-B27 gene in the white population. It can occur in epidemic form. Patients with HIV have an increased risk of developing reactive arthritis as well. A large number of cases during World Wars I and II focused attention on the triad of arthritis, urethritis, and conjunctivitis (often with additional mucocutaneous lesions), which at that time was also referred to as Fiessenger-Leroy-Reiter syndrome.

Experts: Bacteria in food could lead to chronic arthritis

Salmonella bacteria, typically found in poultry, eggs, and milk can be a source of gastroenteritis, typhoid fever, or even chronic arthritis, experts warned. This type of bacteria can thrive in undercooked meals and improperly filtered beverages. Salmonellosis, an infection caused by the salmonella bacteria can cause abdominal pains, vomiting, fever and diarrhea to its victim, appearing 12-72 hours after the infection. The salmonella bacterium appears in various forms but two of the most common serotypes are the Salmonella enteritidis and Salmonella Typhi. Salmonella enteritidis, which originates from animals and birds, typically thrive in their intestinal tracts and are released through their feces. SE may contaminate eggs once the bacteria found in the feces make its way through cracks in an egg. Similarly, improperly cooked poultry may still be hiding in the bacteria. Another type, Salmonella Typhi, is often found in water. This Salmonella type can cause typhoid fever. According to data by the Department of Health, typhoid fever ranks among the top 5 foodborne outbreaks in the country as of last 2013. Meanwhile, Dr. Windell Rivera, a microbiologist, says individuals infected by Salmonellosis may experience other effects after the usual symptoms. "It can cause joint pain and can lead to reactive arthritis or even chronic arthritis." Reactive arthritis is characterized as joint pains that result from an infection while chronic arthritis is a recurrent type of arthritis inflicting pain on human joints. Joints are known as the bone connectors of the body. Whereas some experts say symptoms of salmonellosis may typically last from 4-6 days without treatment, in some cases, taking in antibiotics might be suggested by doctors if the infection transfers from the intestines to the bloodstream. But again, the public is advised to seek help from medical experts. Meanwhile, experts add that people infected by Salmonella are advised to frequently drink fluids to prevent dehydration. Beyond curing the infection, experts say undertaking necessary precautions could save the public from further illnesses. Because the salmonella bacteria typically thrive in raw foods, experts advise washing and thorough cooking of raw ingredients and utensils to kill the bacteria. Further, experts advise the public to avoid using utensils that have already been used for raw ingredients. This is to avoid cross contamination of the bacteria to cooked food.

The way reactive arthritis develops is the person gets an infection. In the process of fighting off the infection, the body creates an antibody to fight the infection and that antibody actually is misguided and it attacks the person themselves usually in the joints. Reactive arthritis is a type of inflammatory arthritis that usually is self-limiting. So these patients will have swollen, tender joints a few weeks after an infection. Fortunately with conservative therapy, usually some over the counter medications and even some prescription medications, this form of arthritis will go away within about six to nine months, but it can be very painful. There are different types of infections that can lead to reactive arthritis. In 1988, at the age of 25 I suddenly developed arthritis. This was 6 weeks after contracting Shigella food poisoning while camping during a vacation in Michigan. The type of arthritis I was diagnosed with is reactive arthritis, since it was in reaction to a Shigella infection. (Shigella is a bacteria, similar to Salmonella, that cause bacterial dysentery, which is what I had.) Many types of bacteria can trigger reactive arthritis. They include Yersinia, Chlamydia, Salmonella, and Shigella. Reactive arthritis is also known as Reiter's Syndrome. There is an unfortunate stigma attached to the name Reiter's Syndrome because that name is strongly associated with arthritis caused by bacterial infections in the genital area, in other words, venereal diseases. I say unfortunate stigma because it can interfere with being treated with proper respect by the medical community. I assume this is because the idea of self or others being sexual brings up unresolved feelings for many people, and the name Reiter's Syndrome immediately stirs up those issues. However, regardless of exactly why, I've found that I have been treated better within the health care system when I have used the name reactive arthritis instead of Reiter's syndrome, even though they are the same condition. Although my reactive arthritis was painful for me, my doctors initially didn't worry very much about it, because reactive arthritis will often times go away on its own once the infection is gone. So I waited a month, but no improvement. I waited six months, and still no improvement. My doctor then started getting angry at me, because I wasn't getting well. She even refused to continue treating me and accused me of being a slacker. After another year, my next doctor also gave up on me. So I turned to alternative medicine. Turns out I was having an allergic reaction to the starch that was used as an "inert ingredient" in the prescription medication I was being given to calm the inflammation in my joints. The pills I was being prescribed were keeping me sick. I was also having an allergic reaction to several other foods. This is because the dysentery and then the arthritis drugs I was given were so hard on my digestive tract that I started leaking small bits of food into my blood stream, where my immune system started attacking them. I had certainly never had food allergies before this time. Once I cut my problem foods out of my diet, my arthritis went away and my digestive tract healed up. Turns out that this is a problem not only with reactive arthritis, but also with rheumatoid arthritis. In fact, I got the idea from looking at what helps cure rheumatoid arthritis. 1.Fuel Up on Fish. Because certain types of fish are packed with inflammation-fighting omega-3 fatty acids, experts recommend at least 3 to 4 ounces of fish, twice a week. But it’s not the only oil with health benefits. Avocado and safflower oils have shown cholesterol-lowering properties, while walnut oil has 10 times the omega-3s that olive oil has. 4. Cherries. Studies have shown cherries help reduce the frequency of gout attacks. Research has shown that the anthocyanins found in cherries have an anti-inflammatory effect. Anthocyanins can also be found in other red and purple fruits like strawberries, raspberries, blueberries and blackberries. 5. Dairy. Low-fat dairy products, like milk, yogurt and cheese are packed with calcium and vitamin D, both found to increase bone strength. Vitamin D is essential for calcium absorption, and it has been shown to boost the immune system. If dairy doesn't agree with you, aim for other calcium and vitamin D-rich foods like leafy green vegetables. 6. Broccoli. Rich in vitamins K and C, broccoli also contains a compound called sulforaphane, which researchers have found could help prevent or slow the progression of osteoarthritis (OA). Broccoli is also rich in calcium, which is known for its bone-building benefits. 7. Green Tea. Green tea is packed with polyphenols, antioxidants believed to reduce inflammation and slow cartilage destruction. Studies also show that another antioxidant in green tea called epigallocatechin-3-gallate (EGCG) blocks the production of molecules that cause joint damage in people with rheumatoid arthritis (RA). 8. Citrus. Citrus fruits – like oranges, grapefruits and limes – are rich in vitamin C. Research shows that getting the right amount of vitamin aids in preventing inflammatory arthritis and maintaining healthy joints with osteoarthritis (OA). 9. Grain. Whole grains lower levels of C-reactive protein (CRP) in the blood. CRP is a marker of inflammation associated with heart disease, diabetes and rheumatoid arthritis. Foods like oatmeal, brown rice and whole-grain cereals are excellent sources of whole grains. 10. Beans. Beans are packed with fiber, a nutrient that helps lower CRP. Omega-6 in low moderation is good for you, but in the amounts you’d consume with just one serving of fast food Omega-6’s increase inflammation in the body by triggering fibrin build-up in your joints over time. Normally, fibrin is a healing protein – partially by causing inflammation to bring in white blood cells. When fibrin continues to build when it’s not needed (arthritis), pain is inevitable. 4. Barbecued and Blackened Foods Grilling alone doesn’t necessarily cause problems. It’s when you go for that pretty charred look on meats and vegetables that things start to go wrong. Cooking foods (meats in particular) at high temperatures over an open flame releases a byproduct called Advanced Glycation End (AGEs). AGEs are well-known in the medical world to cause inflammation of not only the joints, but of your entire vascular system. 5. Processed Sugar (Especially Soda) Full-sugar soda is laden with more sweeteners and artificial flavoring in one bottle than a person should ingest in a week. Processed sugar triggers your body to release pro-inflammatory cytokines. Cytokines are released by the immune system to send healing cells to sites of injury, infection, or trauma. This is all well and good when you actually have something wrong, but it will cause unnecessary inflammation and pain in your joints if the healing cells have nothing to heal. Dr. Mario Zaiss, the study’s lead researcher, said: “Our findings offer a promising approach for developing innovative therapies for inflammatory joint diseases as well as for treating osteoporosis, which is often suffered by women after the menopause. “We are not able to give any specific recommendations for a bacteria-friendly diet at the moment, but eating muesli every morning, as well as enough fruit and vegetables throughout the day, helps to maintain a rich variety of bacterial species.” Researchers said the molecules in gut bacteria, which aid our metabolism, can affect the entire immune system. That could have an effect on inflammatory autoimmune diseases such as rheumatoid arthritis. So eating a healthy, balanced diet that is rich in fibre and gut-friendly, anti-inflammatory options, could positively change the bacteria in the gut. They are also some of the most popular foods consumed today; they include tomatoes, potatoes, all types of peppers, and eggplant. Not truly a nightshade member but containing the same inflammation-inducing alkaloids are blueberries, huckleberries, goji berries and ashwaganda (Indian Ginseng). Nightshades induce inflammation through a specific chemical known as solanine. Researchers now believe this chemical may actually irritate the gastrointestinal tract. When it’s absorbed into the bloodstream, it can cause destruction of the oxygen-carrying red blood cells. Solanine is known as an aceytlcholinesterase inhibitor – it acts to prevent the breakdown of the neurotransmitter acetylcholine (ACh), leading to excessive build-up of ACh in nerve receptor sites. This action allows for constant over-stimulation of Ach receptors, especially within the nervous system as it’s responsible for stimulating the parasympathetic nervous system. Solanines are not water soluble, are not destroyed by cooking, and are not broken down inside the body but must be excreted as alpha-solanine. Alpha-solanine is classified as a neuro-toxin. Interestingly most “foods” that contain alpha-solanine also contain at least 5 other neurotoxins including atropine and nicotine. One of the major problems attributed to nightshades is arthritis. In fact, one in three arthritics will react badly to nightshades. Some researchers believe that arthritis is often misdiagnosed in people who may in fact only be experiencing the effects of nightshade consumption. Alkaloids appear to affect the metabolism of calcium. Though not yet understood how, nightshade foods may remove calcium from bones and deposit it in soft tissue, setting the stage for arthritis. For this reason, researchers have recommended that all individuals with osteoarthritis, rheumatoid arthritis or other joint problems like gout eliminate nightshade foods from their diet. Norman F. Childers, PhD, is the founder of the Arthritis Nightshades Research Foundation. He has the following to say on this subject: “Diet appears to be a factor in the etiology of arthritis based on surveys of over 1400 volunteers during a 20-year period. Plants in the drug family, Solanaceae (nightshades) are an important causative factor in arthritis in sensitive people.” The primary cause of the reactions in some people is the presence of an alkaloid called tropane which many are very sensitive to. Eliminating nightshades from my diet has had a profound effect on my health. In 2006 I completely eliminated all nightshades from my diet. The first big change I noticed was that I no longer needed an inhaler. I had been diagnosed with reactive airways disease ten years’ earlier and used inhalers on almost a daily basis. My need for inhalers decreased dramatically, until two weeks later, when I noticed I hadn’t used an inhaler at all. Six years later, I have never had the need for an inhaler. Within three months of eliminating nightshades, I noticed that the knee pain and leg weakness I had on a daily basis was also completely gone. For those that suffer from arthritis or an arthritis related disease such as lupus, rheumatism, and other musculoskeletal pain disorders members of the Solanaceae family of flowering plants, more commonly known as nightshades, may be adversely affecting their health.
