Topical Patch For Non-Surgical Treatment Of The Cervical Herniated Disc

There are three types of patches that are used for pain related to the herniated disc. One is a local anesthetic patch called "Lidoderm", the second is an anti inflammatory patch called the "Flector Patch" and the third type is a narcotic patch called the "Fentanyl Patch." These can be prescribed by your physician or a pain management specialist. All of these patches treat the symptom of pain and will have no effect on the disc itself. Patches are rarely used for the typical herniated disc.

Learn how to apply and remove a transdermal patch (specifically Fentanyl) to a patient for medication administration. Transdermal patch application requires you know how to properly remove a previous patch along with how to properly apply another one. What are transdermal patches? They are patches that deliver a continuous amount of medication to a patient via the skin. Each patch has an adhesive backing that contains a specific amount of medication ordered for the patient. In this video, I demonstrate how to apply a Fentanyl transdermal patch. It is important to note that many medications are available in the transdermal form as well. Fentayl is used for patients with chronic pain who are opioid-tolerant and are unable to achieve pain management with oral forms of opioids. The transdermal patch allows for continuous amount of the drug to be delivered from the skin to the bloodstream. As the nurse, who is administering this medication, you must always monitor the patient's respiratory and hemodynamic status because this medication can cause respiratory depression and hypotension. In addition, the nurse will want to evaluate the patient's pain rating to determine the effective of the medication. Transdermal patches should never be applied to broken or irritated skin, and the site should always be rotated during new applications. During the removal and application of the patch the nurse should always wear gloves to prevent becoming contaminated with the drug. Fentanyl transdermal patches can be applied to the upper arm, chest, or back. It is VERY important to ALWAYS remove the previous patch before applying the new patch. In addition, after removing the patch it is vitally important to have another nurse witness the disposal of the patch because it is a controlled substance. Always dispose of the transdermal patch according to hospital protocol. NOTE: The patch used in the video is a demo dose and used as stimulation for teaching purposes . How to Properly Apply a Transdermal Patch

The following video is meant to be a general guide only. Always follow the instructions provided by your doctor or pharmacist for your particular patch. How to Properly Apply a Transdermal Patch 1. Select an area of your skin that is clean, dry and free from excessive hair. Be sure to follow your product instructions as to site selection and rotation. If required, hair can be clipped with scissors close to the skin but the area should NOT be shaved. Ensure the area is clean and free from oils, powders and lotions so the patch sticks well to the site. 2. Remove the patch from its package only when you are ready to use it. When removing it, be careful not to cut or damage the patch in any way. If the patch is damaged or altered in any way, do NOT use it. 3. A stiff protective liner covers the sticky side of the patch. This is the side that you will attach to your skin. Remove the protective liner according to your product directions to expose the sticky adhesive layer. Be sure not to touch the sticky side of the patch. Some protective liners are removed in two parts. If so, remove one part of the liner and place the sticky half of the patch on the area of skin. Then, gently peel off the remaining liner, to attach the rest of the patch. 4. Apply the patch to the site or sites in the product-specific instructions or as directed by your doctor or pharmacist. 5. Press down on the patch with the palm of your hand to smooth it down. Make sure the patch is flat to the skin and there are no bumps, folds, or bubbles under it. Then use your finger to press down along the edges of the patch. If the patch does not stick well or loosens after application, you can tape the edges down with first aid tape. Refer to your product information sheet for additional information on how to deal with this problem. 6. Once the patch is applied, dispose of the protective liner and package as directed by your instruction sheet, and wash your hands. Even a used patch may contain enough drug to harm others, especially children and pets. It is important to dispose of used patches and packaging properly. 7. Wear the patch for the amount of time directed by your doctor. With each new patch, remove the old patch and change application sites according to your product’s instruction sheet. If you are not sure when to change the patch, check with your doctor or pharmacist. Some patches have special labels to help you track when to replace the used patch with a new one. If not, it is a good idea to note the time and site of application on a calendar to help track usage. Please check with your doctor or pharmacist if you have any questions.

Patient safety experts have long agreed that medication errors are the single most frequent source of preventable error. Additionally, creating a culture of patient safety requires strong teamwork and effective communication between healthcare providers, yet there is often not enough attention devoted to developing these skills. The NYC Health and Hospitals Corporation (HHC) and CIR's Policy and Education Initiative (PEI) sponsored the one-day conference on November 30, 2011, bringing together 160 healthcare professionals from all 11 hospitals in the HHC system. The conference focused on a team and systems-based approach to reducing medication errors, with emphasis on the use of opioid and effective pain management. ACTINIC KERATOSIS Symptoms, Causes & Treatments OVERVIEW An actinic keratosis is a rough, scaly patch on your skin that develops from years of exposure to the sun. It’s most commonly found on your face, lips, ears, back of your hands, forearms, scalp or neck. Also known as a solar keratosis, an actinic keratosis enlarges slowly and usually causes no signs or symptoms other than a patch or small spot on your skin. These patches take years to develop, usually first appearing in people over 40. A small percentage of actinic keratosis lesions can eventually become skin cancer. You can reduce your risk of actinic keratoses by minimizing your sun exposure and protecting your skin from ultraviolet rays. SYMPTOMS The signs and symptoms of an actinic keratosis include: • Rough, dry or scaly patch of skin, usually less than 1 inch (2.5 centimeters) in diameter • Flat to slightly raised patch or bump on the top layer of skin • In some cases, a hard, wartlike surface • Color as varied as pink, red or brown • Itching or burning in the affected area Actinic keratoses are found primarily on areas exposed to the sun, such as your face, lips, ears, hands, forearms, scalp and neck. CAUSES An actinic keratosis is caused by frequent or intense exposure to UV rays, from the sun or from tanning beds. TREATMENTS An actinic keratosis sometimes disappears on its own, but it typically returns after more sun exposure. Because it's impossible to tell which patches or lesions will develop into skin cancer, actinic keratoses are usually removed as a precaution. Medications If you have several actinic keratoses, you may be better served by treating the entire affected area. Prescription products that can be applied to your skin for this purpose include: • Fluorouracil cream. • Imiquimod cream. • Ingenol mebutate gel. • Diclofenac gel. These creams may cause redness, scaling or a burning sensation for a few weeks. Photodynamic therapy In photodynamic therapy, your doctor applies a chemical solution to the affected skin that makes it sensitive to light. He or she then exposes the area to artificial light to destroy the damaged skin cells. Side effects may include redness, swelling and a burning sensation during therapy. Surgical and other procedures If you have only a few actinic keratoses, your doctor may recommend removing them.

Aphthous Ulcers : Medical Care

Medical Care Identify and correct predisposing factors for recurrent aphthous stomatitis (RAS). Ensure that patients brush atraumatically (eg, with a small-headed, soft toothbrush) and avoid eating particularly hard or sharp foods (eg, toast, potato crisps) and avoid other trauma to the oral mucosa. SLS should be avoided if implicated as a predisposing factor. Any iron or vitamin deficiency should be corrected once the cause of that deficiency has been established. If an obvious relationship to certain foods is established, these should be excluded from the diet. Patch testing may be indicated to reveal allergies. The occasional patient who relates ulcers to her menstrual cycle or to use of an oral contraceptive may benefit from suppression of ovulation with a progestogen or a change in the oral contraceptive. In most cases, the natural history of RAS is one of eventual remission. However, for some patients, remission occurs spontaneously several years later; thus, treatment is indicated in these patients if discomfort is significant. Relief of pain and reduction of ulcer duration are the main goals of therapy. There is a huge range of supposed or possible remedies available, but objective evidence shows the most efficacy from corticosteroids and antimicrobials used topically. [10, 11] Topical corticosteroids (TCs) remain the mainstays of treatment. A spectrum of different TCs can be used. At best, TCs reduce painful symptoms but not the rate of ulcer recurrence. The commonly used preparations are as follows: Hydrocortisone hemisuccinate pellets (Corlan), 2.5 mg used 4 times daily Triamcinolone acetonide in carboxymethyl cellulose paste (Adcortyl in Orabase [withdrawn in some countries], Kenalog), administered 4 times daily Betamethasone sodium phosphate as a 0.5-mg tablet dissolved in 15 mL of water to make a mouth rinse, used 4 times daily for 4 minutes each time Hydrocortisone and triamcinolone preparations are popular because neither causes significant adrenal suppression; however, ulcers still recur. Betamethasone, fluocinonide, fluocinolone, fluticasone, and clobetasol are more potent and effective than hydrocortisone and triamcinolone, but they carry the possibility of some adrenocortical suppression and a predisposition to candidiasis. Other topical medications that can reduce discomfort include the nonsteroidal anti-inflammatory drugs (NSAIDs) diclofenac and amlexanox paste; the latter has been found to shorten the time it takes minor aphthae to heal. Swishing three or four times daily with so-called “magic mouthwash” (MMW) can also offer some pain relief. [1] MMW can be obtained in several formulations, an example being as follows: One part viscous lidocaine 2% One part Maalox (do not substitute Kaopectate) One part diphenhydramine (12.5 mg per 5 mL) Benzydamine hydrochloride mouthwash, though no more beneficial than a placebo, can produce transient pain relief. Chlorhexidine gluconate and bioadhesive (Gelclair) mouth rinses reduce the severity and pain of ulceration but not the frequency. Topical tetracyclines may reduce the severity of ulceration, but they do not alter the recurrence rate. A doxycycline capsule of 100 mg in 10 mL of water administered as a mouth rinse for 3 minutes or tetracycline 500 mg plus nicotinamide 500 mg administered 4 times daily may provide relief and reduce ulcer duration. Avoid tetracyclines in children younger than 12 years who might ingest them and develop tooth staining. If RAS fails to respond to local measures, systemic immunomodulators may be required. A wide spectrum of agents has been suggested as beneficial, but few studies have been performed to assess the efficacy of these drugs (or their adverse effects are significant). Thalidomide 50-100 mg daily is effective against severe RAS, although ulcers tend to recur within 3 weeks. Teratogenicity, neuropathy, and other adverse effects dissuade most physicians from its use. Oral vitamin B-12 may significantly reduce or eliminate RAS recurrences. For example, a randomized, double-blind, placebo-controlled study by Volkov et al found that in patients taking 1000 μg of sublingual vitamin B-12 daily for 6 months, there was a significant decrease in the number of ulcers and level of pain, as well as in the duration of outbreaks, at 5 and 6 months no matter what the patients' initial B-12 blood levels had been. Moreover, during the sixth month of treatment, 74.1% of the patients taking B-12 achieved "no aphthous ulcers status," compared with 32.0% of patients in the control group. [12] Few, if any, of the other medications used for RAS have undergone serious scientific evaluation.

Treating knee osteoarthritis can be broken into two components. There’s reducing the inflammation within the knee, and then there’s getting all of the muscles and biomechanics right to take the pressure off the knee so that the same forces are not going through it so the inflammation does not re-accumulate. A question that doctors who treat knee pain get asked a lot is about supplements. “Are there supplements that are right for me?” is a question that we’ll hear a lot. There are a lot of supplements that are out there, as we all probably know, that are ideally oriented towards treating knee osteoarthritis. The two that have the most research behind them are Glucosamine and Chondroitin. There has been a lot of controversy over whether or not Glucosamine and Chondroitin supplements actually affect knee pain and arthritis in a positive way. There seems to be a lot of conflicting data, the way I look at it, the research is that there seems to be something to it, and yet the original data that came out was a lot more positive, and some of the newer data is a lot less positive. What I generally tell patients is that Glucosamine and Chondroitin are supplements that are worth trying; they will not hurt you. It can be a little bit costly. If you feel like it’s helping you, use it, and maybe at a certain point, stop using it and see if the pain comes back. If the pain comes back, then you know that it was helping you or at least doing something. If the pain does not come back, then maybe it did address it and you don’t have to keep taking it if the pain is not there anymore. Omega-3 fatty acids are another supplement that people take for inflammation in general. And I think that it is nice anti-inflammatory that a lot of people can take. As to whether or not it helps knee pain directly, I think the jury is still out on that. There’s certainly not a lot of great data to suggest that it does. But it does have an anti-inflammatory effect and it may be something to talk to your doctor about. There are many topical medications. Some are over the counter, such as menthol creams like Icy-Hot® and different topical preparations that can help alleviate pain from knee arthritis. Then there are prescription medications that are topical, like Voltaren® gel, Flector® Patch, and Pennsaid®, which are basically topical diclofenac, or topical Advil®, in other words a topical non-steroidal anti-inflammatory drug that diffuse through the skin and can help with some of the pain from different musculoskeletal injuries and to an extent, knee osteoarthritis. It’s probably not going to fix the knee osteoarthritis but it can be a helpful adjunct as one goes through physical therapy.

Herndon, PharmD, BCPS, CPE, Assistant Professor, Southern Illinois University, Edwardsville, IL, discusses how pain management specialists can differentiate between commercially available topical analgesics and anesthetics. Topical opioids delivered via a patch, such as an upcoming oxycodone transdermal patch, and topical nonsteroidal anti-inflammatory drugs (NSAIDs) are among some of the commercially available products. There are certain concerns to consider when dosing with these medications. Since a patch delivers the topical opioids it doesn't matter where the drug is applied since the result is a systemic absorption not a local effect. Topical NSAIDs, on the other hand, need to be applied to the site of the injury or where the pain actually is. They deliver a small amount of the drug through the lipid bilayer, which reaches nerve endings, and potentially reduces the local inflammatory response. Some of these drugs can be absorbed into the blood stream. This response is minimal compared to the oral administration route.

Patellofemoral syndrome is a common cause of knee pain. It generally presents as anterior knee pain, or pain in the front of the knee. Patellofemoral syndrome is a problem with the tracking of the kneecap within the knee. Essentially, here’s a knee, and this the patella of the knee cap. And there’s a groove in which the knee cap is supposed to glide as one goes through life bending, flexing, walking, and sitting. When the knee cap doesn’t track in that groove properly, you can get some friction and some inflammation on the bottom of the knee cap that can lead to a little knee arthritis, chondromalacia patella, some irritation of the knee cap, and that can be painful. Typically, it’s going to present as pain in the front of the knee. It’s usually very painful when you go down the stairs when patients have patellofemoral syndrome. When people sit for a long time, they’ll often feel like they have to straighten their knee because the pressure builds and when you straighten the knee, the pressure relaxes. Patellofemoral syndrome is primarily a biomechanical problem, so that way you go about treating it is by addressing the biomechanics. Typically this means stretching the hip flexor, stretching the iliotibial band, which comes down the outside of the leg and attaches down at the bottom of the knee and can pull the knee cap to the side. Typically when the knee cap is not tracking properly it is tracking laterally, or tracking to the outside of the knee. And then strengthening the quadriceps, ideally strengthening the inside (the VMO, vastus medialis obliquus) in order to get the kneecap back into alignment. Also looking at the feet is important to make sure that if the person has any flat feet or hyperpronation of the feet, that gets addressed as well so that the mechanics get restored throughout the entire kinetic chain. What are the Treatments for Patellofemoral Syndrome? While the hallmark of treating patellofemoral syndrome really is addressing the biomechanics, as you’re going through physical therapy, if the pain is getting in the way of getting better or participating in physical therapy, then there are oral medications that one can take in order to help with the pain of some of the inflammation. There are topical prescription medications like Voltaren® gel, Pennsaid®, Flector® Patch, which are basically topical diclofenac, topical non-steroidal anti-inflammatory drugs like Advil® or Aleve®. These diffuse right through the skin, and that can be somewhat helpful as well with patellofemoral syndrome. It’s not going to fix it, because again, we really need to address the mechanics, but it can help along the way. In a similar vein, one can do different kinds of injections like hyaluronic acid injections, which is basically putting some joint fluid into the joint in order to help lubricate the joint, which again is not going to fix the biomechanics but can help open this window of opportunity during which the patient can participate more fully with physical therapy and break through a plateau in order to really make a full recovery.
