We should first understand about diabetes. Diabetes is actually a chronic disease where the pancreas in a human body loses its efficiency to produce insulin or the body has less efficiency to utilise the insulin produced by the pancreas. Insulin is a hormone produced by the pancreas which acts as a key which helps the glucose from the food we eat to get absorbed from the blood to the cells to produce energy. Not being able to produce insulin makes glucose levels to be at a higher level in the blood and this is called hyperglycemia. Complications of diabetes can be put into two major parts. It could be acute complications and chronic complications. Acute complications are serious life threatening complications which can arise very quickly. But fortunately at the very same time we can deal with it as quickly as possible if you or people surrounding the patient knows what to do and how to do. These acute complication arise when there is high uncontrolled blood sugar level. Basically it is not meeting the available insulin need. Either you are taking too much of diabetic medication or too little. These acute complications could be a very low blood sugar level, i.e. called a hypoglycaemia or very high blood sugar levels causing conditions like diabetic ketoacidosis or hyperglycaemic hyperosmolar state. These acute complications require immediate attention and prompt and proper treatment at hospital. So when done there is complete recovery of these patients from these conditions. Coming to chronic conditions, a long term exposure to high blood sugars in the blood over years or decades would have already caused damage to the systems in the body before the symptoms appear. These long term exposure to high blood sugar level damage the body and various organ systems and tissues in the body. These complications can affect cardiovascular system, eyes, kidney and nervous system. They can affect the coronary artery disease leading to heart attack and stroke. Hence a regular cardiac check up and keeping your blood glucose, blood pressure, cholesterol levels and your weight at or close to the normal levels help prevent onset of these complications. High sugar in the blood can also damage the small blood vessels in the kidney and can cause the kidney become less deficient to work or fail altogether. People with diabetes are more common to develop kidney dysfunction than with people who are not diabetic. Hence a comprehensive management of keeping the sugar level under control, BP management, cholesterol levels help prevent progression of these kidney disease. Diabetes can affect the nerves throughout the body. The common areas where they affect are the extremities particularly the feet. When the nerves in the feet are affected they are called peripheral neuropathy. Patient can present with tingling, numbness, loss of sensation. Loss of sensation is very important because the injuries can go unnoticed leading to serious infections and amputations. By a comprehensive management of diabetes, blood pressure and cholesterol these amputations can be prevented. The amputations in diabetic patients are much more common than any other patients without diabetes. Even though there is an amputation the remaining part of the leg or patient's life can be saved by a proper check and proper control of blood sugar levels. Patients with diabetes also suffer from some form of the eye disease. It can range from infections to retinopathy. The light sensitive area on the eye are dependent on the small blood vessels for their nourishment. These small blood vessels are affected by the high blood glucose levels. This leads to reduced vision and blindness. They can also present with glaucoma which is increased pressure in the eye. This increased pressure can have a damaging effect on the optic nerve. They also present with cataract which is nothing but the clouding of the lens in the eye. It can develop sooner in diabetic patients than compared to others without diabetics. These are few complications of diabetes, diabetes also have effects on the mental health of the patient. They can have depression, panic disorders, generalised anxiety disorders along with diminished cognitive functions.

Diabetes is an illness which is asymptomatic initially and the symptoms develop only if one develops a complication associated with diabetes. Therefore it is important for us to maintain the symptom free period as long as possible and prevent or postpone grievous complications associated with diabetes. One of the major complications one can have with diabetes are end organ damage. Therefore maintaining a good glucose level throughout is of paramount importance. How do we maintain good glycemic levels? In an established diabetic, the best way to monitor the sugar levels is to keep a check on the HbA1c reading. A1c reading between 6.5 to 7 or below that is considered as good diabetic control and these people will have very less chances of developing diabetes associated complications. Anyone who has very unstable A1c reading, which shuttle between 6 and 9 or 6 and 10 are having a high probability of developing various complications. Now what are the complications that I am talking about? The most common area where a diabetic has to watch out is cardiac problems. Diabetics are more prone to develop heart attacks, angina attacks and various cardiac associated peripheral vascular disease. Therefore by maintaining a very constant glycemic levels by having periodic Diabetics cardiac checkups, by having ECG done periodically, one ensures that cardiac complication are either not occurring or occurring at a later stage. The next common problem are the eye problems. Diabetics are most prone to a condition called as diabetic retinopathy, where they will slowly start getting blind because their retina is damaged by uncontrolled diabetes or progressive diabetes. So visiting an ophthalmologist to have a very good retinal checkup is of paramount importance. We should also not that diabetics also have a higher propensity of developing cataracts compared to the general population. The next organ in question which can be at risk are kidneys. Kidneys can be at risk, what is called as end stage diabetic renal disease which will end up with dialysis and renal failure. So the best thing for us to do is to keep a constant monitor on kidney functions, especially one element called as urinary micro albumin. Close monitoring of urinary microalbumin is an indicator of the trouble to come in future or good kidney health, depending on what the range of that particular reading is. The best organ to be affected are the skin and the soft tissues. Diabetes because if the inherent nature of the illness, develop less sensitivity of the fete because of the peripheral neuropathy and without their knowledge, tend to hurt their feet, what is called as diabetic foot and because of the increased glycemic levels, infections are very rampant on to particular foot and not careful, they may end in amputation of that particular foot or finger in question. So to keep all this in question, please remember to keep your glycemic levels in good control, do a good exercise, have balanced and a good diet and visit your specialist, whenever your doctor asks you to do.

Diabetes affect more than 30 million Americans in the United States. One of the main challenges is many health centers struggle to properly diagnosed patients with diabetes. Join us for the third webinar in the continuation of our Diabetes in Special and Vulnerable Population: A National Learning Series focusing specifically on diabetes and those affected by it. This webinar explores diabetes prevention and care strategies across the lifespan. Presenters discuss standards of care in the screening and prevention for children, adults, and the elderly, sharing experiences from the field. Presenters also engages participants around case examples of successful diabetes prevention programs, adult clinical treatment, and geriatrics care focused on vulnerable populations of children and adults of all ages. Lastly, presenters connect participant with resources to outreach opportunities and tools they can apply and adapt for their own health centers. This series is pending accreditation by the American Academy of Family Physicians (AAFP) through Migrant Clinicians Network (MCN) and is also accredited by the American Nurses Credentialing Center (ANCC) through MCN. Elderly Living with Diabetes in Indonesia

The aim of this article is to describe a socio-demographic profile and perception of elderly with diabetes towards their disease and the actions taken from the first time they are diagnosed with diabetes. Methods: Using a questionnaire for gathering socio-demographic data and focus group discussion for having the perception of elderly prior to attending an educational program for people with diabetes in three local community health centers in Indonesia. Results: Data was collected from 45 elderly patients showed that mean of age 60.78; Sex ; level of education secondary school 46.7%, elementary school 35.6%, 17.8 % diploma/university. Most of the elderly found that they experience losing their weight, feeling fatigue and losing vision, forced them to community health center and were diagnosed with diabetes. Since community health center experienced a lack of facilities and resources for providing diabetes education program, elderly attempted to other alternatives medications : herbal medicine; ion therapy and traditional medication with the hopes it will cure their disease. Conclusion: These finding suggest that there is an urgent need for diabetes education program should be performed and delivered to the elderly as soon as they were diagnosed with diabetes. Properly managing diabetes requires monitoring blood glucose (sugar) levels and adjusting for both highs and lows. According to the American Diabetes Association (ADA), anyone over 80 years of age is considered at risk for hypoglycemia (low blood sugar). But with proper management, seniors and their caregivers can control low blood sugars and their associated dangers. We are joined by Jennifer Greiner, R.N., CDE, who has been with Residential Home Health for over 7 years and currently serves as Residential’s Certified Diabetes Educator. Living with diabetes herself for over 37 years, Jennifer understands the challenges individuals face and the lifestyle adjustments necessary to effectively manage diabetes. Through individualized education, she supports caregivers and helps patients reach their behavioral and treatment goals.
