NCLEX Diabetes Mellitus Practice Question on Insulin | Pharmacology Review

NCLEX pharmacology practice question on Insulin for the patient with diabetes mellitus. This NCLEX-style question will test your knowledge about Insulin Lispro (Humalog) and wants to know when the patient is at most risk for hypoglycemia based on the time you administered the medication. On the NCLEX exam, it is inevitable you will receive pharmacology type questions. This particular NCLEX practice question will require you to determine what type of insulin Lispro is (rapid, short, intermediate, or long-acting) and when a patient is at most risk for hypoglycemia (onset, peak, duration). This video is part of a weekly NCLEX review series where I will be going over NCLEX-style questions with you. I will be helping you analyze and breakdown each question, and walk you through how to select the correct option. NCLEX questions require critical thinking and you must know how to use your nursing knowledge to gather the facts and analyze what the question is asking. NCLEX Pharmacology Practice Question on Insulin: Your patient's blood glucose level is 215 mg/dL. The patient is about to eat lunch. Per sliding scale, you administer 4 units of Insulin Lispro (Humalog) subcutaneously at 1130. As the nurse, you know the patient is most at risk for hypoglycemia at what time? Delegation nursing review for the NCLEX exam. What is delegation in nursing? It’s where a licensed nurse (RN) transfers responsibility to a person, who is competent, to perform a certain task for the nurse. Purpose of delegation in nursing: It frees up the RN to care for more critical patients and complete other necessary tasks. However, in delegation the RN is still ACCOUNTABLE (liable or responsible) for that delegated task getting done even though they are not the one doing it. Therefore, accountability is NOT transferred to that person performing the task. It's important to remember not all tasks can be delegated to other nursing staff such as the LPN/LVN or UAP (nursing assistant). This is because some tasks are only within the RN’s scope of practice to perform. This is where the RN should know when, how, and what tasks should be delegated. In addition, it's very important the RN is always familiar with their facilities and state's protocol (ex: who can do what...ex: some LPN can give IV medications in some states if they have training, while other states don't allow it). This NCLEX-style question requires you to correctly put in order the steps on how to correctly mix a short-acting and intermediate insulin in one syringe for administration. On the NCLEX exam, you may receive drag and drop type questions where you will be required to put the options in the correct order. These questions can be about nursing procedures or nursing priorities. This particular NCLEX practice question will require you to determine which insulin in drawn up in the syringe first and how you would perform these steps correctly. This video is part of a weekly NCLEX review series where I will be going over NCLEX-style questions with you. I will be helping you analyze and breakdown each question, and walk you through how to select the correct option. NCLEX questions require critical thinking and you must know how to use your nursing knowledge to gather the facts and analyze what the question is asking. NCLEX Drag and Drop Practice Question on Nursing Procedure: The physician orders 10 units of Humulin-R and 12 units of Humulin-N subcutaneously before breakfast daily. You are required to mix these two medications in one syringe for administration. Correctly put in order the steps for how you will perform this procedure: Withdraw 10 units of Humulin-R Clean off each vial top with alcohol prep Don gloves Inject 10 units of air into the vial of Humulin-R Withdraw 12 units of Humulin-N to equal a total of 22 units in the syringe Roll the vial of Humulin-N in between the palms of the hands Inject 12 units of air into the vial of Humulin-N Wash hands Watch the video for the correct answer and rationale. Nursing Cast Care NCLEX Practice Question for Fractures

Nursing cast care NCLEX review question on how to care for a patient with a bone fracture. This NCLEX practice question will test your nursing knowledge on how to properly care for a new cast placed after a bone fracture. To answer this question correctly, you must know the complications associated with casts placement (compartment syndrome), how to prevent swelling, monitoring the neurovascular status (6 P's), how to promote even drying, etc. Why are casts placed after a bone fracture? To help with immobilizing the fracture to help promote the natural healing process. A cast is usually placed after a closed bone reduction, which is where the bone is manually put back into its original state. There are various types of casts, such as plaster or fiberglass. Nursing Interventions for a Cast: -Monitor for compartment syndrome: 6 P’s -Monitor for infection: hotspots in the case, severe pain, fever -Keep the cast and extremity elevated above the heart level (decreases swelling). -Apply ice packs to the case for the first 2 days to decrease swelling. -Turn the cast every 2 hours to promote even drying. -Use palms of hands to handle (not fingertips) when new with wet plaster: WHY? Prevents dent formation in cast by handling with the palms of hand. These dents can cause ulcer formation overtime. -Maintain skin integrity: petal the cast and apply a soft tape called moleskin around the edge to prevent skin breakdown. -Keep cast dry and never stick anything inside it to itch a scratch. This is an NCLEX hotspot practice question example about a nursing assessment for the drug Digoxin (Lanoxin). For this particular NCLEX practice question, you will have to identify what area on the diagram you will assess prior to administering Digoxin. Before a nurse administers Lanoxin, it is important to assess the apical pulse for 1 full minute. The apical pulse must be 60 beats per minute or greater. Therefore, to answer this question correctly you must identify on the diagram where the apical pulse is located on an adult patient. The apical pulse is located at the apex of the heart and is found at the 5th5th intercostal space midclavicular line. This location is also the point of maximum impulse. The client newly diagnosed with Diabetes type 2 asks the nurse the meaning of the insulin’s peak level. The nurse correctly explains that it is: A. the time the insulin will be working in your bloodstream B. the time it takes for the insulin to reach the bloodstream C. the time the insulin is working hardest to lower your blood sugar D. none of the above Ans.: C. Peak hours of the insulin is described as the time the insulin is working its hardest to lower the blood sugar levels. I like this question a lot, since there are so many different types of insulin it is difficult to keep track of them. It is always really important though to keep in mind which insulin you are giving to your patient and also when that medication is going to be working the hardest. I have had several episodes working as a nurse, especially on night shift, where my patients blood sugar crashed in the middle of the night from lantus or intermediate acting insulin. Although my patient had received these medications hours before.... the effects had not reached their peak. Mix this with not eating a sufficient dinner and also missing their evening snack and your patients blood sugar can really go down. If my patient has a lot of insulin that they take on a normal basis. However, I notice that they are not eating that much - I usually communicate with them… they know their bodies better than anyone else --- especially if they have had diabetes and been taking insulin for a while. I will say to them: Your blood sugar is: 126, we have an order to give 10 units of humalog with your meal…. is this how much you usually eat? And if you were home, would you take this much insulin. A lot of times they will tell me that their endocrinologist suggest a smaller dose when they do not eat that much. So with this information I will page the doctor and see what they would like me to do. When it comes to insulin, you can never be too safe and giving too much insulin, which can induce a hypoglycemic event…. even coma can be extremely dangerous….
