As though breast cancer isn't enough to deal with, many patients also experience depression during treatment. Mel Majoros was no exception. She felt no one around her could identify with what she was going through. Mel talked to her doctor and was prescribed a medication to help. The medication she started taking also helped to lessen the side effects from the tamoxifen, also. It's important to remember that depression brought on by breast cancer is very common. How to treat ongoing pain after mastectomy breast operations and other chest surgery

Ongoing pain after mastectomy or other chest surgery is appallingly common - possibly up to 50% of operations. It's particularly distressing to the patient, as every time they get a stab or twinge, they think the original problem is coming back. Surgery is often presented as a very straightforward procedure, like putting a new battery in a torch, say. Very often it’s not that simple, and ongoing pain after surgery is very common. Almost always the operation itself is fine. But surgeons are surgeons and do surgery, and what often gets missed is some simple follow-up so you get the full benefit of that expensive, skilled operation. If you still have pain persisting more than three months after these operations, it’s probably not just a matter of more healing time. Here’s what helps after that three months (not before as repair is still taking place). Usually it works fine and fully fixes the problem, though this can vary, especially with the extent of the surgery. Note that of course if there is any suspicion that the original problem IS in fact coming back, that must always be checked out first. But usually that's not the case. So, after the operation things will repair, with skin and muscle scarring plus some stronger scarring after the scalpel cuts. This is all normal and desirable. BUT scarring also binds down the nerves running through the area, and these often become hypersensitive. Scarring is also relatively non-elastic, so you can get a glued, scarred, tightened patch of muscle and skin with the nerves running through it tethered and hypersensitive. This can mean ongoing pain, every time the area gets strained or tugged on. Medications can dull the pain, but a purely medicinal approach (including pain specialists) misses the point. After a few months it's no longer a healing problem but a tethering one. The obvious way to actually fix it is to sort out the problem at its source. Massage is ideal for quietly making scarring mobile and flexible - not weaker - so that the nerves running through it simply aren't being tethered and bound down as they have been. A good sports massage (a.k.a. deep tissue massage) is fine - maybe take in a friend so they can learn what to do also, and can save you money. You can also work on yourself, as shown in the video. In the video I've talked mostly about POST-MASTECTOMY PAIN, but the same principles apply to persisting pain after any chest surgery. This is especially the case for THORACOTOMY chest operations and especially after STERNAL SPLIT surgery, usually for heart conditions. This usually results in ongoing pain around the back where your ribs join onto your spine, and often also at the other ends of the ribs where they hinge onto your breastbone. This is then called costochondritis (or Tietze's Syndrome if there is also observable swelling) - it is NOT a mysterious inflammation. If you've been through a mastectomy with all that involves, then you don't need ongoing scary pain afterwards. This is what you do about it. It's essentially a physiotherapy problem requiring a physiotherapy solution, not a medicinal answer. Your doctor may have only the medicinal options to offer. CAMBRIDGE, MASSACHUSETTS — New research by the Harvard Medical School has found a possible link between exposure to nighttime light and breast cancer in younger women who smoke. Previous studies have shown that exposure to light at night may lead to decreased levels of melatonin, a hormone made by the body's pineal gland. This can disrupt circadian rhythms and lead to an increased risk of breast cancer. The study tracked data from nearly 110,000 women in the U.S. between 1989 and 2013. The results shows the risk of breast cancer in premenopausal women who smoke or who used to smoke increases with greater exposure to outdoor nighttime light. However, older women and those who had never smoked did not seem to be affected by the lighting. "In our modern industrialized society, artificial lighting is nearly ubiquitous. Our results suggest that this widespread exposure to outdoor lights during nighttime hours could represent a novel risk factor for breast cancer," Peter James, lead author of the study and assistant professor at Harvard Medical School's department of population medicine said in a press release. The study also finds the link was stronger among women who worked night shifts but the authors admit further work is needed to clarify the potential mechanisms. For most breast cancer patients, surgery is the first leg of treatment. "Because not only does it treat the primary cancer, meaning to remove the tumors, but it also gives us answer by looking at the lymph nodes," says Dr. Rie Aihara, surgical oncologist on medical staff at Lee Memorial Health System. "And some of the answers we get from surgery actually dictate what kind of therapy would come next." Once the path becomes clear, the baton is passed to a medical oncologist, radiation oncologist, or both. In best circumstances they work as a team, as is the case within the Regional Cancer Center's multi-disciplinary breast cancer clinic. "We're looking at the same things. We're able to discuss, really ask all the questions, really bring our thoughts together. So not only is it faster, but I think the quality is much better," says Dr. Aihara. Women are empowered with choices when it comes to surgery. Based on the nature of their cancer and personal preference, they may opt to remove a portion of breast, one breast or both if they have genetic risk. "We do see a lot of women who choose a double mastectomy, especially in the younger population because they have so many years to think about recurrences and things like that. And we always emphasize if there's only cancer in one area, certainly if it's treatable by lumpectomy. That's what we would advise." Surgery is often the end of the treatment road. A majority of women chose to undergo reconstruction. Many times it starts during a mastectomy using tissue expanders and is completed once they reach the finish line. "A lot of people come in with this horrible image of a mastectomy that they used to do years ago and once we sit down with them, show them pictures, a lot of people are extremely relieved and really have a positive attitude going forth," Dr. Aihara says. For these reasons, the surgical segment is often the anchor in this relay for life. According to UPI, women receiving a specific type of hormone treatment to ease symptoms of menopause may be at much higher risk for breast cancer. Researchers from the Institute for Cancer Research in England say that risk was previously underestimated. HRT is used to treat menopause symptoms such as hot flashes, migraines, sleep problems, depression and mood swings by increasing levels of estrogen in the body. There are three types of HRT: Estrogen-only, a combination of estrogen and progestogen, or tibolone, which is a steroid that acts like estrogen and progestogen. Hello Everyone, here is a women's health lecture on the use of a mammogram made easy to understand to help aide in your study sessions. I have gathered all of the important information from my Med- Surg Book (Brunners and Suddarth 12th edition) that will prepare you for your nursing test whether it is for school or NCLEX. Here are some Extra information that may help and guide you... Galactography: Galactography is a diagnostic procedure that involves injection of less than 1 mL of radiopaque material through a cannula inserted into a ductal opening on the areola, which is followed by a mammogram. It is performed to evaluate an abnormality within the duct when the patient has bloody nipple discharge on expression, spontaneous nipple discharge, or a solitary dilated duct noted on mammography. Ultrasonography: Ultrasonography (ultrasound) is used as a diagnostic adjunct to mammography to help distinguish fluid-filled cysts from other lesions. A thin coating of lubricating jelly is spread over the area to be imaged. A transducer is then placed on the breast. The transducer transmits high-frequency sound waves through the skin toward the area of concern. The sound waves that are reflected back form a two-dimensional image, which is then displayed on a computer screen. No radiation is emitted during the procedure. Ultrasonography has advantages and disadvantages. Although it can diagnose cysts with great accuracy, it cannot definitively rule out malignant lesions. Microcalcifications, which are detectable on mammography, cannot be identified on ultrasonography. Finally, examination techniques and interpretation criteria are not standardized. Magnetic Resonance Imaging: Magnetic resonance imaging (MRI) of the breast is rapidly gaining in popularity. This highly sensitive test has become a useful diagnostic adjunct to mammography. A magnet is linked to a computer that creates detailed images of the breast without exposure to radiation. An intravenous (IV) injection of gadolinium, a contrast dye, is given to improve visibility. The patient lies face down and the breast is placed through a depression in the table. A coil is placed around the breast, and the patient is placed inside the MRI machine. The entire procedure takes about 30 to 40 minutes. MRI is most useful in patients with proven breast cancer when assessing for multifocal (more than one tumor in the same quadrant of the breast) or multicentric (more than one tumor in different quadrants of the breast) disease, chest wall involvement, tumor recurrence, or response to chemotherapy. The procedure can also identify occult (undetectable) breast cancer and determine the integrity of saline or silicone breast implants. Recent studies have shown that MRI is a highly sensitive screening tool that can be used to identify breast cancer in women at high risk for the disease (Orel, 2008). The ACS now recommends an annual MRI scan in addition to mammography in women at high risk for breast cancer (ie, those with greater than 20% lifetime risk). Candidates include women who have a BRCA1 or BRCA2 mutation, a first-degree relative with either of these mutations, certain rare genetic syndromes, or radiation to the chest between 10 and 30 years of age (Saslow, Boetes, Burke, et al., 2007). MRI should be used in addition to mammography, not instead of it. Some disadvantages of MRI include high cost, variations in technique and interpretation, and the potential for patient claustrophobia. The procedure cannot always accurately distinguish between malignant and benign breast conditions. MRI is contraindicated in patients with implantable metal devices (eg, aneurysm clips, pacemakers, ports of tissue expanders) because of the metallic force. Foilbacked medication patches (eg, nicotine, nitroglycerine, fentanyl) must be removed prior to MRI to avoid burns to the skin.
