Treatment of Bipolar Disorder Treatment for manic depression consists of a lot of trial and error. Patients are usually best treated with between three and five medications. Some patients may respond well to just one medication, but typically, these medications work best when the effects are combined. Mulitple medications can also ensure lower doses of each prescription are needed, thus decreasing the risk of unwanted side effects. There are also what is called atypical antipsychotics. These medications work well to stop the swings, particularly manic episodes. Mood Stabilizers/Anti-Epileptics The benefits of using anti-epileptic medications to treat patients were actually stumbled upon by accident. Neurologists started to notice their epileptic patients becoming less depressed when taking these medications. Eventually, they were approved for treating this disorder in the USA. These prescriptions are usually the foundation of treatment. The most often used prescription medications are Lithium: Lithium has been the most used for manic depression for decades. Depakote: Depakote can cause weight gain and sedation. Lamictal: The least amount of side effects of most of these medications. Highly preferred. Tegretol: Minimal side effects. Atypical Antipsychotics Atypical Antipsychotics should be used in conjunction with mood stabilizing medications like those listed above. This family of medicines can have a paradoxical effect on a patient if not combined with appropriate other pharmacology. If used with a mood stabilizer, this combination can prevent you from becoming as deeply depressed. If used alone, these medications can cause your mania to become worse. The most commonly used Atypical Antipsychotics are: Zyprexa (Olanzapine): Can cause weight gain and sedation. Seroquel (Quetiapine): Weight gain and sedation possible, but not as prevalant as Zyprexa. Latuda (Lurasidone): This one is a new medication. The side effects seem to be less severe. Supplements There are also some over the counter supplements you can add to your daily regimen which helps decrease the intensity of depressed states and shorten the amount of time you will suffer a depressive episode. Your psychiatrist will be able to inform you on the proper therapeutic dose. Some of these are: Fish Oil with EPA L-Folate (Methylfolate) N-acetyl cysteine (NAC) Pharmacology is imperative for manic and depressed patients. It is important to always consult a physician before making medical decisions. My Dad's Depakote story started in 2009 with a Depakote dose of 250mg twice a day, then a Parkinson's diagnosis in 2011. I stayed with my dad this spring while my mom was in the hospital and became more involved with his care. I started to read about his medications and question some of them because of things that I had read. He takes mestinon for MG, which is mandatory, prednisone which I wish he didn't need, and want him to taper off of, and he also took Depakote, Prozac, and simvastatin which are the three meds that I was questioning based on articles on Pub Med and side effects that I had seen on Drugs.com that matched my dad's symptoms. Throughout the last 4 years he has gradually become weaker. He was admitted to the university hospital 3 weeks ago, after an ER visit to another hospital where he was sent home the same night. While at the university I asked them to draw a Valproic acid level and it came back normal. The nurse continued to tell me that the rest of the Valproic Values and the Free Valproic came back at 42. His neurologist checked his Valproic Acid levels often in her clinic and they had always been normal. I looked back through his medical records and noticed that he did have a Free Valproic level drawn only once before, on the same visit to the hospital in 2011 when he was diagnosed with Parkinson's. His free level was also high on that visit. I read about the Free Valproic level, and then I understood. His neurologist had never looked at that level. The neurologist at the university hadn't looked either. No one had, and no one knew what a Free Valproic level was, at least none of the people I was talking to. And they didn't understand what a high free level meant, or what the consequences were. NO ONE KNEW! Including Utah poison control! Utah poison control did understand that a free level was the unbound portion of Depakote but they did not think it was at a toxic level. When I called Abbott Laboratories (the manufacturer of Depakote), they knew the free level was the unbound portion of Depakote but said that they had no information on normal ranges, and that they didn't include that information in their prescribing information! I did find information with normal and toxic ranges on ARUPs site. My dad's Level was toxic! Above 15 is Toxic! My Dads Medications, Prozac, Simvastatin, Prednisone (throughout the years he also took (aspirin {which is a whole other story} ibuprofen and naproxen) are all highly protein bound and fighting for Albumin and increasing Free Depakote levels. It's true that over the years my Dad's Depakote level had been normal, but, if his free level had been checked its probable they would have been high. I explained this to the doctors who said that I was wrong, and one of them said he would read about it. I had to explain this to my mom. I told her to stop the Depakote but she wouldn't listen to me, it had to come from a doctor. After all, doctors are there to protect you. And honestly, I doubted myself. They were "tapering him off" of Depakote by decreasing his dose, but they were actually putting more into his system. My Dad could not move very well anymore, or think or remember anything. Rod helped me be brave, and a phone call to a good friend who firmly said "STOP THE DEPAKOTE," gave me the power to do what I needed to do. I threw a fit, I didn't want to, but I had to, to get people to listen. I told the nurses no more Depakote. Then my mom told my dad to refuse it. The rehab doctor started to read about free levels and protein binding and told my mom that it was ok to refuse it ( I guess he couldn't discontinue because neurology ordered it, I'm not really sure.) This same doctor said, "I know that my practice is forever changed, but I can't speak for others... Thank you." It was nice to hear him say that. I have felt crazy for a while; it was nice to be acknowledged by one of them. There is a lot more to the story, so much more that I am writing a case study hopefully to try and change things and help with awareness of protein bound drug interactions. Apparently this problem is being GREATLY MINIMIZED! And to all the neurologists out there CHECK FREE LEVELS but, the important thing is that my dad is getting better, he is walking and moving and thinking better. It is amazing! I am seeing him do things that I haven't seen him do for over 2 years!!!!!!!! If you get even one prescription of a benzodiazepine medication like klonopin (clonazepam), valium (diazepam), xanax (alprazolam), and ativan (lorazepam), etc. your brain will likely become physically dependent on them after a few weeks use and you could end up like me here or worse. You could end up with countless pill bottles of something that might be helping for a couple weeks, but then it stops working as well and you then up the dose. You get your dose high enough to where your breathing starts to become too depressed. Then you decide you want to come off the meds. But guess what, you can't just stop them. No, you are in for what is going to likely be over a year of trying to taper off your medication because even the smallest cut in dose is going to bring complete hell of withdrawal side effects. These meds change the gaba properties in your brain, the chemical that helps calm your nervous system. When you start cutting the benzo medication out, your brain takes it harshly and takes a long time to adjust back to controlling gaba on its own again. Please trust me when I say do not touch a benzo unless its your last hope. You may end up like me, or countless other people duped by their doctors into getting hooked on something that you are given no warning about its effects both during use and the complete hell of trying to come off of them.

MY STORY: My name is Noah and on May 18 2011, I had a rare reaction to a vaccine called VIVITROL and consequently spiraled into a major, agitated, suicidal depression with depersonalization. I lost 25 lbs in 4 weeks and was in full panic or near panic for 8 weeks straight mixed with the darkest most painful depression I cold have ever imagined. I immediately could not work and had to move in with my parents who along with many siblings and friends had to watch me 24/7 as I was so suicidal. I was eventually hospitalized. Getting through each day seemed truly unbearable and I knew I would surely die. I have been put on many many different SSRI's SNRI's Tricyclics, Mood stabilizers, anti psychotics, holistic meds, acupuncture and even a form of shock therapy called RTMS. I barely saw any improvement in my condition for a full year. It was decided I had treatment resistant depression and I spent nearly every moment in tears. Weeks after starting my newest round of medications (Seroquel & Nortryptaline) as a last ditch effort, I had my blood drawn for possible hormone imbalances and my Testosterone levels came back 200 ng/Dl and 150 ng/Dl. The average 25 year old male has 750 ng/Dl. With this discovery I for the first time had any type of possible explanation as to why I was not getting better and why I might be so so sick. The symptoms of such Low T are very similar to those of major depression. I started Testosterone replacement therapy soon after and have been checking in with the world and documenting my experience with treatment. I am blessed to say that I have slowly over the last 8 months been improving and becoming more stable which I never thought to be possible. My low T manifested itself in the form of Major depression, anxiety, and depersonalization/ derealization for over a year. I am glad to report that I have experienced improvements in all areas and that is a miracle. I do not consider myself to be totally healed yet but I am closer now then ever before and aim to use what I have been through to help or at least offer support to others in need.

A number of other medications were tried, such as lithium, Seroquel and Lamictal. Jacob began to get frustrated at the guessing game of prescribing medications. We found out that we need to work harder to raise awareness of the harms of psychiatric drugs and the soldiers' right to alternative treatment. There is just as much stress on today's soldier as in the past: long hours, grueling missions, extended periods away from loved ones. With that stress comes very real mental and emotional problems.
