Sudden Weight Gain or Loss: regardless of whether you have a quick weight gain or a quick weight loss, both can be a sign that something more serious is going on. The first reason is you may not be getting enough sleep, which your body perceives as stress, which can cause weight gain. The second reason could be a hormonal imbalance which should be checked out by your family doctor. The third reason may be from a thyroid gland disorders which causes you to either make too much or too little in the way of hormones. 2. Edema: watch your legs, feet and fingers throughout the day. If they are normal size in the morning and are swollen by the end of the day, this could be an indication that you have edema. You can get edema either from varicose veins which causes veins to protrude out or from high blood pressure and heart problems. If you notice that your face is swollen in the morning, you could also have problems with your kidneys. 3. Bruising: if you keep bruising and don’t have an explanation for it, this may be an indication that you have a blood-related disease or a vitamin deficiency. If you have a lack of vitamin C, this can cause issues with your metabolism and subsequent bruising. Other than this, diseases like thrombocytopenia and vasculitis can cause bruising. 4. The Condition of Your Tongue: take a look at the condition of your tongue and notice whether you have a thin or thick layer of white across the top of it. If it is thin, you are good to go, but if it is thick then you have some issues with digestion. Other than this, if your tongue is yellow and you have a bitter taste in the mouth, then you have issues with your gallbladder and liver. Finally, if you have a grey-yellow layer, it may be an indication that you have a chronic disease. 5. The Condition of Your Gums: if you have swollen, weak, or red gums, this is an indication that you have poor oral hygiene and that you may have gum disease. Pay attention if you have any gum membranes that are inflamed around one single tooth, or whether your gums bleed when you brush them. If you have painful reactions to cold and hot food, this is another indication that something more serious is going on. If you have any gum tissue that detaches itself from the tooth, this is a sign of periodontitis and if you have any cracks in your teeth, this is a sign of periodontal disease. Deficiencies of Thiamine primarily affect 3 different areas; the CNS (Wernicke-Korsakoff Syndrome), PNS (Dry Beri-Beri), and the cardiac system (Wet Beri-Beri). Damage done to the cardiac system and the PNS can be reversed with treatment, but damage done to the CNS is permanent. Thiamine is an important cofactor in multiple processes that generate ATP from glucose sugar. Therefore, deficiencies are worsened by sugar intake (in the absence of thiamine administration) because the little bit of Thiamine left in the system is used up to metabolize the sugar. Wernicke-Korsakoff Syndrome is the collection of CNS damage that results from Thiamine deficiency. It includes amnesia (memory loss), confusion, ataxia (unsteady gait), and visual disturbances due to uncoordinated eye movement. The confusion and memory loss can lead to confabulation in which the patient has bizarre explanations for things like how they got injured. Wernicke-Korsakof syndrome characteristically affects the Mammillary Bodies and the surrounding structures (3rd/4th ventricle & aqueduct). Infarcts or hemorrhages in these areas can be seen on radiologic imaging. Dry BeriBeri is damage to the peripheral nervous system (PNS) that results from Thiamine deficiency. It causes muscle wasting, weakness, and diminished peripheral sensation. Wet BeriBeri is the cardiac effects of Thiamine deficiency. It leads to arrhythmias and/or heart failure by affecting the electrical system of the heart. Edema, dyspnea, increased cardiac output, and cardiac dilation can result. Niacin (Vitamin B3) is part of NAD+ and NADP+ which carry electrons in redox reactions. Vitamin B3 is made from tryptophan using a vitamin B6 cofactor. Therefore, Niacin deficiency can be the result of Vitamin B6 deficiency or tryptophan deficiency (Hartnup Disease or Carcinoid Syndrome). Niacin deficiency is called Pellagra. Pellagra is characterized by the "4 Ds" Diarrhea, Dementia, Dermatitis, and Death. Niacin is one way to treat high cholesterol. Niacin toxicity can result if a patient receives too high of a dose. This causes the side effect of Facial Flushing (red face with the sensation of warmth). Pyridoxine (Vitamin B6) is an essential coenzyme in many metabolic reactions. Deficiencies can cause peripheral neuropathy (numbness & tingling), seizures, anemia & irritability. Deficiencies of Vitamin B6 can also lead to deficiencies of Vitamin B3, because B6 is a cofactor in the pathway that converts Tryptophan to Vitamin B3. The most common cause of Vitamin B3 deficiency is Isoniazid (TB drug) Treatment. Therefore, some patients receiving Isoniazid are given B6 vitamins prophylactically. Vitamin C (Ascorbic Acid) is involved in the hydroxylation of certain amino acids in collagen that are important for crosslinking individual pro-collagen chains into a triple helix. Without this hydroxylation the collagen is very weak. Therefore, deficiencies of Vitamin C cause poor wound healing, gum abnormalities (pain, bleeding & loose/missing teeth) & bruising (purpura, ecchymosis, perifollicular hemorrhages). Scurvy results from not eating enough fruit and other fresh foods.

A wide variety of signs and symptoms may occur including a decreased ability to think and behavioural and emotional changes such as depression, irritability, and psychosis. Abnormal sensations, changes in reflexes, and poor muscle function can also occur as may inflammation of the tongue, decreased taste, low red blood cells, reduced heart function, and decreased fertility. In young children symptoms include poor growth, poor development, and difficulties with movement. Without early treatment some of the changes may be permanent. Common causes include poor absorption from the stomach or intestines, decreased intake, and increased requirements. Decreased absorption may be due to pernicious anemia, surgical removal of the stomach, chronic inflammation of the pancreas, intestinal parasites, certain medications, and some genetic disorders. Decreased intake may occur in those who eat a vegan diet or are malnourished. Increased requirements occur in HIV/AIDS and in those with rapid red blood cell breakdown. Diagnosis is typically based on vitamin B12 blood levels below 120–180 picomol/L (170–250 pg/mL) in adults. Elevated methylmalonic acid levels (values 0.4 micromol/L) may also indicate a deficiency. A type of low red blood cells known as megaloblastic anemia is often but not always present. Supplementation is recommended to prevent deficiency in vegetarians who are pregnant. Once identified it is easily treated with supplementation by mouth or injection. There are no concerns from excess vitamin B12 among those who are otherwise healthy. Some cases may also be helped by treating the underlying cause. Other cases may require ongoing supplementation as the underlying cause is not curable. Vitamin B12 deficiency is common. It is estimated to occur in about 6% of those under the age of 60 and 20% of those over the age of 60. Rates may be as high as 80% in parts of Africa and Asia. Vitamin B12 deficiency can lead to anemia and neurologic dysfunction. A mild deficiency may not cause any discernible symptoms, but as the deficiency becomes more significant, symptoms of anemia may result, such as weakness, fatigue, light-headedness, rapid heartbeat, rapid breathing and pale color to the skin. It may also cause easy bruising or bleeding, including bleeding gums. GI side effects including sore tongue, stomach upset, weight loss, and diarrhea or constipation. If the deficiency is not corrected, nerve cell damage can result. If this happens, vitamin B12 deficiency may result in tingling or numbness to the fingers and toes, difficulty walking, mood changes, depression, memory loss, disorientation and, in severe cases, dementia. The main syndrome of vitamin B12 deficiency is pernicious anemia. It is characterized by a triad of symptoms: 1. Anemia with bone marrow promegaloblastosis (megaloblastic anemia). This is due to the inhibition of DNA synthesis (specifically purines and thymidine) 2. Gastrointestinal symptoms: alteration in bowel motility, such as mild diarrhea or constipation, and loss of bladder or bowel control. These are thought to be due to defective DNA synthesis inhibiting replication in a site with a high turnover of cells. This may also be due to the autoimmune attack on the parietal cells of the stomach in pernicious anemia. There is an association with GAVE syndrome (commonly called watermelon stomach) and pernicious anemia. 3. Neurological symptoms: Sensory or motor deficiencies (absent reflexes, diminished vibration or soft touch sensation), subacute combined degeneration of spinal cord, seizures, or even symptoms of dementia and or other psychiatric symptoms may be present. The presence of peripheral sensory-motor symptoms or subacute combined degeneration of spinal cord strongly suggests the presence of a B12 deficiency instead of folate deficiency. Methylmalonic acid, if not properly handled by B12, remains in the myelin sheath, causing fragility. Dementia and depression have been associated with this deficiency as well, possibly from the under-production of methionine because of the inability to convert homocysteine into this product. Methionine is a necessary cofactor in the production of several neurotransmitters.....
