Ellen Seely, MD, Director of Clinical Research in the Division of Endocrinology, Diabetes and Hypertension at Brigham and Women’s Hospital discusses the health risks associated with gestational diabetes mellitus (GDM) during and after pregnancy. Dr. Seely also describes Balance After Baby, a web-based program designed to educate women on eating healthfully and increasing their physical activity, with the goal of helping them lose their pregnancy weight within a year of giving birth. GDM is a type of diabetes that occurs only in pregnancy. It comes on in the second half of pregnancy and it goes away after delivery. In the US it’s estimated that five percent of all pregnancies are complicated by GDM. The rates of GDM are higher in Hispanic and non-white populations, ranging from ten to 20 percent of all pregnancies. Obesity is one of the main risk factors for GDM. In the United States, most health centers screen all women for gestational diabetes because obesity is becoming so common in the overall population. Gestational diabetes can impact the health of the mother and the fetus, as well as the newborn. Women with GDM have an increased risk of preeclampsia, which is a type of high blood pressure that develops during pregnancy. Preeclampsia is a serious condition that can lead to early delivery. Women who have gestational diabetes may also have larger babies, increasing their risk of cesarean section. While GDM goes away after pregnancy, the health risks persist. Research has found that 50 to 70 percent of women who have gestational diabetes may develop Type 2 diabetes later in life. Research has also found that babies born to mothers with GDM, have a higher birth weight and may become obese during adolescence, increasing their risk of developing diabetes and other conditions such as high blood pressure. The Pregnancy and Diabetes Program at Brigham and Women’s Hospital offers multidisciplinary care to women with GDM. The program includes a high risk obstetrician, two diabetes specialists, a nutritionist, and a nurse practitioner who is certified in diabetes education. The first approach to treatment is to control blood glucose levels with healthy eating and physical activity. Lifestyle changes, healthy eating and physical activity, are effective in controlling blood glucose levels about 75 percent of women with GDM. If the lifestyle changes don't work, insulin therapy is started to control a mother’s blood sugar for the rest of her pregnancy. DAWN PHENOMENON Researchers feel the most common reason for high blood glucose levels in the morning is the dawn phenomenon. The glucose is going up from sources other than digested food. Some of it is produced by the liver from stored starch and fatty acids. Livers that produce too much glucose are one of the main ways diabetes causes high blood glucose levels. Other organs also produce small amounts of glucose. This is called “gluconeogenesis” for those of you who like the technical stuff. Organs produce glucose to keep blood glucose from going too low at night or other times of not eating. From about 2 AM to 8 AM, most people’s bodies produce hormones, including cortisol, glucagon, and epinephrine. All these hormones increase insulin resistance and tell the liver to make more glucose. The idea is to get you enough glucose to get out of bed and start the day. Everyone has a dawn phenomenon. Otherwise they’d be too weak to get breakfast. But in people without diabetes, insulin levels also increase to handle the extra glucose. People with diabetes can’t increase insulin levels that much, so their early morning blood glucose levels can rise dramatically. Experts disagree on how many people have a dawn phenomenon. Estimates range from 3% to 50% of Type 2s and from 25% to 50% of Type 1s. Is dawn phenomenon a serious problem? It can be serious. According to the American Diabetes Association, “Some people with dawn phenomenon find that their glucose continues to rise until they eat in the morning. For others, levels will settle down a few hours after waking, regardless of whether or not they eat.” According to columnist Wil Dubois, the higher your A1C, the more likely you are to have a significant dawn phenomenon. It could be that spending a number of hours each morning out of control is having a significant effect on your overall control. Some people have high glucose levels in the morning because their medicines wear off overnight. This could be true of medicines like insulin, and metformin. If you are taking any long-acting medicine, consider asking your doctor about changing meds, doses, or times THE SOMOGYI EFFECT In some cases, medicine can be too strong. If your glucose goes too low in the night, you could have a rebound high in the morning. This is called the Somogyi effect. If you are waking up high and are suffering pounding headaches, or find your sheets sweat-soaked, the odds are you are having lows in your sleep…You need to visit with your doctor about taking less meds. According to Dubois, the new insulins are much less likely to cause a Somogyi reaction. But because of cost, people are going back to NPH insulin. NPH is cheaper, shorter-acting, and more likely to cause a low, leading to a rebound high in the morning. WANING INSULIN If you take insulin and have been experiencing high blood sugar in the morning, your insulin may simply be wearing off too soon. If this is the case, your doctor can adjust your dosage or change what time you are taking the insulin to prevent high glucose levels. Pinpointing the Cause for Effective Treatment If your blood sugar is fairly even when you go to bed and at 3 a.m. but is higher in the morning, you are probably experiencing dawn phenomenon. If your blood sugar is low at 3 a.m., but high in the morning, you probably suffer from the Somogyi effect. If your blood sugar is elevated at 3 a.m. and then higher still in the morning, you probably have waning insulin. Even if you’ve identified the reason behind your high morning number, never attempt to correct it on your own. Instead, talk with your doctor. Tip Acceptable blood sugar levels are 95 or less upon awakening; 140 or less an hour after meals; and 120 or less two hours after meals. Step 6: Return to your doctor six to 12 weeks after giving birth to have your blood sugar retested. For most women, their blood-sugar levels return to normal after they give birth. Step 7: Stick with your anti-diabetes diet and exercise plan so you won't become part of the approximately 50 percent of women who develop type 2 diabetes within five years of having gestational diabetes. Their body's response to this large amount of sugar is to produce a lot of insulin. This is great and what their body should do while they're inside of you. But once they're born and not getting all of that sugar, they still have a high level of insulin to compensate for that and it causes their blood sugar to rapidly drop. A lot of babies born to mothers with diabetes struggle with their own blood sugar after delivery. So the people who are taking care of your newborn will likely be in tune with this and be watching your newborn for signs of complications. But as for how big your baby is likely to be, who knows? It's hard to say. The chances of your baby being large are reduced by you controlling your blood sugars well during the pregnancy. So if your doctor has recommended a certain diet make sure you follow that and if your doctor has recommended medication to control your blood sugars, make sure you're taking that as prescribed and checking your blood sugars as frequently as your doctor has suggested as well. It's likely that in your third trimester you'll go to see some specialists that will watch you and the baby for signs of complications from diabetes. Usually your doctor will have you go to a specialist called a perinatologist to make sure that your baby is growing well and that the pregnancy overall is going well. They'll probably also have you talk with diabetic educators that will have you keep a log of your blood sugars and have you come in and discuss any complications or issues you're having with them. Your doctor is also likely to have you get frequent non-stress tests where you come in and they put your baby on the monitor, meaning they watch your baby's heart rate and your contraction pattern, for a short period of time to make sure your baby has enough oxygen and is doing well. It can be the happiest time of a woman’s life---but doctors say that joy can be overshadowed by a common medical condition. “It’s very common, unfortunately. As a society we are getting sicker, we are getting obese, and diabetes is on the rise,” said Dr. Ravi Chokshi, an OB/GYN with Lee Health. Diabetes becomes even more concerning when it’s developed during pregnancy. “There are two types of diabetes during pregnancy that we worry about. One is you’re a diabetic before you became pregnant, and that is a very serious problem that we try to get control of. The other is you’re not a diabetic. You had normal blood testing before. What happens in pregnancy is the placenta that the baby relies on releases hormones that increase your blood sugar,” said Dr. Chokshi. If the mother becomes diabetic during pregnancy, both the mother and baby are at risk for health concerns. “It actually affects the development of the baby, and it can cause heart defects and brain defects if the sugars aren’t brought under control quickly,” he said. These concerns are why doctors are screening women for diabetes throughout their pregnancy. “We are actually instituting a policy where almost every patient is going to get some blood sugar test at the start of pregnancy to make sure we’re not missing diabetics who didn’t know they were diabetic,” said Dr. Chokshi. If a woman is diabetic or becomes diabetic during pregnancy, doctors will help them with diet and medication, if necessary to get their blood sugar under control. “If you have diabetes during pregnancy and we don’t treat it adequately, you’re at a two to three time increased the risk for C-section; you’re at a three to four time increased risk for birth injury,” he explained. The goal is to catch diabetes early during pregnancy to prevent health concerns for both the mother and baby—both during and after delivery. If you have a spike in hunger and thirst while pregnant and it isn’t because you’ve been throwing up a lot, you should talk to the doctor about diabetes checks. What symptoms of diabetes during pregnancy should I look out for? Blurred vision. You can get that with preeclampsia and high blood pressure. That’s something you have to see the doctor about too, and in fact, preeclampsia is life threatening, so you have to get to a doctor if you have that symptom. So being hungrier, thirstier or bigger than normal when I’m already experiencing changes that make me hungry, tired, swollen and thirsty are symptoms. That’s why they do the blood tests. You’d think I’d be over the anxiety of blood tests, when I’m about to be peed on, pooped on and spit up on so much it will be cute.

So this is a good sign that you have controlled your sugars well, so the baby will continue to grow well and should not have all the problems that I just mentioned to you. Food cravings during pregnancy can cause some drastic changes to your diet, but health experts say giving in too much to sugary desires can cause health problems for both mom and baby. While gaining weight is expected during pregnancy, Dr. Heitham Ajlouni, an endocrinologist for Lee Memorial Health System, says there are some risks for expecting mothers who gain too much weight. “Five to 15 percent of women who get pregnant develop gestational diabetes.” Gestational diabetes is one of the most common pregnancy complications. It causes high blood sugar, affecting the health of both mother and baby. “When women get pregnant they screen them for diabetes because pregnancy is a high risk for diabetes. We screen them with oral glucose tolerance test between 24 and 28 weeks because that’s the way we catch diabetes,” said Dr. Ajlouni. He says the screening is important because there are typically no symptoms from gestational diabetes. But there are signs that could mean the mother is at risk. If the mother is overweight, has high blood pressure, had gestational diabetes in other pregnancies, or if she has a primary family member with diabetes, she could be at risk for gestational diabetes. If a mother is diagnosed with gestational diabetes, health experts will monitor her blood sugar, weight, diet, and increase her exercise. They will also keep a close eye on the baby’s growth. “After delivery their sugar goes back to normal but after a couple of years 50 to 70 percent of those women will develop diabetes,” said Dr. Ajlouni. He says while the baby is not born diabetic, it is at risk for preterm delivery, low blood sugar and excessive birth weight, making it important to stay active and eat healthy during pregnancy. How to treat gestational diabetes? - Dr. Vijayalakshmi
The first step in the treatment of gestational diabetes is to modify your diet and lifestyle changes. When I say diet changes, it means reduce your carbohydrate intake because gestational diabetes is a condition where the blood sugar levels are high. We would like to keep it in the normal range where it is below 140 mg/dl even after two hours of diet. So by way of diet change, you have to minimize your intake of carbohydrates and at the same time, maintain a balanced nutrition for the growing fetus. So instead of cutting down on carbohydrates totally, I would advice you to take small, frequent meals or you can distribute the meal intake. In between three meals, you can take some fruits and still maintain the blood sugar in the same level.. nnBest advice will be to take more of proteins and vegetables and fruits. To minimize sugars, sweets, candies and pastries. Do not skip meals and go on diet during pregnancy.. nnA little bit of exercise and an active life is mandatory. If there is a contraindication to exercise, maybe you should go for some medication from your doctor. Exercise will be moderate walking for at least half an hour everyday in the morning or evening. Not too brisk walking or not stressful working at office.. nnYou may require frequent visits to the hospital as the doctor would like to monitor your blood sugar. Ideal would be to monitor your blood sugar at home. If you ever feel its more than 140 or 160, then visit the hospital. Maybe ultrasonography every month specially in the second trimester to make sure the baby is doing well. Other tests like NSTs, blood sugar and blood and urine tests will also be checked during your visit. If there are still complications, the doctor would like to put you on medication. With good diet and exercise, oral medication is sufficient. Insulin shots will be given otherwise. If things are not settling down, hospitalization is advised. Pre term delivery might happen if there are more complications..

A day of what I ate yesterday, Sunday, with Gestational Diabetes while trying to keep my glucose levels under control at 34 weeks pregnant. This is based on my drs protocol. Everyone will be different but here are some tips and tricks I've learned so far. Also, it is very similar to what they recommend for the pregnancy diet for everyone. Fasting Blood Sugar: 104 (I think it was high from the Yasso Bar the night before) Breakfast: 12 oz coffee with 1/2 Boost Glucose Control Chocolate Breakfast Blood Sugar: 126 Snack 1: Blueberry Pancake with whipped cream, butter and a very little bit of maple syrup. Small coffee with cream Lunch: Caprese Salad with 2 hershey kisses Lunch Blood Sugar: 112 Snack 2: 2 Yasso Greek Yogurt Bars Dinner: Broccoli with cheese and rice Dinner Blood Sugar: 108 Snack 3: celery with peanut butter, celery with cream cheese, cheese stick Next Day Fasting Blood Sugar: 88 Thanks for watching and I'm not a doctor or nutritionist so be sure to consult with your doctors!
