Tis is the disease which is limited to the epithelium. If it infiltrates into the lamina propria or muscularis mucosa, it is called as T1a. And T1b, when it infiltrates into the submucosa. Infiltration into muscularis propria is called as T2 disease. Pericolorectal tissue infiltration is called as T3 disease. And infiltration of serosa is called as T4a. If the tumor infiltrates through the wall of colon, to involve the surrounding structures, it is called as T4b. A tumor present in almost any part of the colon may infiltrate into the small intestine. Tumor in the ascending colon may infiltrate into the right kidney. And that in the descending colon may infiltrate into the left kidney. A transverse colon tumor may extend to involve pancreas. And may even extend to involve the stomach. Now we will discuss the N staging or the nodal staging for colon cancer. In this figure, you can see the draining lymph nodes for the colon. It can be called as N1 or N2, depending upon the number of lymph nodes involved. Lastly, we will discuss the M-staging or the metastatic staging of the colon cancer. As you can see in the figure, the venous drainage from most of the part of colon, ultimately pass through the liver. So liver is the most common site for the distant spread of the tumor from colon. As you can see in this figure, the tumor in the right side of colon has spread to the liver. In this figure, a left sided tumor has spread to the liver. Due to the direction of vaccular drainage, after liver, lungs are the second most common organs involved by metastasis. This figure shows the spread of colon cancer to both the lungs, in form of multiple nodular deposits. And sometimes, both lungs and liver maybe involved in the metastatic spread of the disease, as we can see in this figure. The tumor may also spread to the peritoneum, in form of peritoneal deposits. It may rarely spread to one or both the ovaries. Very rarely, the tumor may also spread to the brain or bones. So with this, we finish the TNM staging for colon cancer. The most commonly used colorectal cancer staging system is known as the TNM system, which has been established by the American Joint Committee on Cancer. The TNM staging system looks at three key factors to determine the stage of cancer: Tumor (T) looks at how far the primary tumor has grown into the wall of the colon or rectum, and if it has expanded into nearby areas. Once the T, N and M scores have been assigned, an overall stage is determined, and thus treatment options can be explored. Primary tumor (T) The primary tumor can be categorized as T0 up to T4 for colorectal cancer. Below is a further description of how tumors are defined using the TNM staging system. TX: The main tumor cannot be assessed. T0: There is no evidence of a primary tumor. An update from Robert Wood Johnson hospital after the surgery I had to remove a tumor from my colon. The tumor was cancerous and I'm in stage 2a of the cancer. Will have to get chemo for 6 months for them to aggressively remove all cancer cells from my body. The tumor size was 11 cm by 3 cm by 2 cm. The lymph nodes around the tumor were negative for cancer during the biopsy so it did not spread all over the body as much as they feared.

how to treat colon cancer with surgery |chemotherapy for colon cancer Depending upon the stage of colon cancer you have (stage I, II, III, or IV), you may be best treated with surgery, with chemotherapy, or with a combination of both. It is key to receive treatment for your colon cancer as soon as possible, and to follow up with regular visits with your physician following treatment to monitor your recovery and to check for any possible relapses. 1 Have your colon cancer staged.[1] Before deciding on any course of treatment, the first step is to confirm your diagnosis of colon cancer and to have the cancer, if it is indeed present, staged. Staging is done using additional scanning of the body with CT or PET scanning and a biopsy of the primary cancer lesion that was found. This is important for developing an individualized treatment plan. There are 4 stages of colon cancer: Stage I is limited to the bowel while stage IV has metastasized throughout the body, with stages II and III representing intermediate levels of severity without body-wide metastases. Stages I, II, and III are generally treated with surgery as the first-line option. Stages II and III may require "adjuvant" chemotherapy (chemotherapy to supplement the treatment following surgical excision of the cancer). Stage IV is treated predominantly with chemotherapy, and occasional surgery is used as an adjunct (as an addition) to remove masses causing pain, obstruction (blockage of the bowel), or that are otherwise problematical. 2 Have your cancer removed surgically.[2] Assuming that you fall into stages I, II, or III, you will be booked for a surgery to remove your cancer as soon as possible. The surgery is performed under general anesthesia and normally takes a few hours to complete. The affected section of your bowel (where the cancer is present) will be removed, and the lymph nodes in the surrounding areas will be examined by your surgeon to see if the cancer has spread to them. If the cancer has not spread to the lymph nodes, this is a very good sign. It indicates that your cancer was most likely fully cured by surgery alone. If the cancer has spread to your lymph nodes, however, there is the possibility of "micrometastases" - meaning small cancerous cells that have "escaped" into your bloodstream and pose a heightened risk for recurrence of your cancer down the road. Your surgeon will inform you following the procedure whether or not your lymph nodes contained any traces of cancer. The excised (removed) portion of your bowel will also be examined under a microscope following surgery, by a medical specialist called a pathologist. The pathologist is able to provide additional diagnostic information about the type of cancer you had based on the appearance of the cancerous cells under the microscope. 3 Wear a colostomy bag following surgery.[3] After surgery, the most common procedure to attach the loose end on the proximal (upper) end of your colon to your abdominal wall, so that the opening of your bowel pokes through your skin. This is called a "stoma," and it functions by attaching an "ostomy bag" to the outside of it to collect stool while the rest of your colon heals. For patients who are surgically receptable, meaning they can undergo an operation for their colon cancer, we might not catch small clusters of cancer cells called micrometastases that might be around at the time of surgery, but are not detectable by current methods. At Memorial Sloan Kettering physicians are also increasingly looking to the genetic characteristics of an individual patient's tumor to determine the combination of treatments that is most likely to be effective. {partial transcript} You mention staging being important, Dr. Saltz, talk to us about how you stage a colon cancer. One of the first questions that patients have is what stage cancer do I have? All cancers are now staged on a 1 to 4 scale with one being the earliest stage and four being the most severe. In simple terms, the way that I would think about colorectal cancer is stage 1. It has yet to penetrate through the thickness about the bowel wall. The cancers all start on the inside of the colon or rectum. If it penetrates into the wall, but not through it, that is stage one. If it penetrates through the wall completely, that is stage 2. If the tumor has spread through lymph nodes, which you can think of as like tiny little filters that are right around the colon areas, that tumor is stage 3. And stage 4 means that the tumor has traveled to another site. An example of stage 4 tumors would be a tumor that has spread to the liver or to the lung or to some other place in the body other than where it started. The reason that this matters is that staging is the single best indicator of prognosis. Prognosis is the greek way of hiding that we’re asking the questions, “What does the future hold?” “What do you know about you know about the future prognosis?”

Once the person is diagnosed with colon cancer based on the biopsy result, the next step would be to stage the disease.That is to see how much it has spread whether the disease is limited to the colon, or whether it has spread in and around the colon or whether it has spread far off, to the liver and lungs. Various tests like CT scan helps to determine stage of colon cancer.When the colon cancer is in early stage that is, its limited in the colon and around the colon, it would fall under stage 1 or 2.When it has spread far away like to the liver or lungs, it would fall under stage 4.Totally there would be four stages, stage 1,2,3 and 4. Treatment for colon cancer is based largely on the stage (extent) of the cancer, but other factors can also be important. People with colon cancers that have not spread to distant sites usually have surgery as the main or first treatment. Adjuvant (additional) chemotherapy may also be used. Most adjuvant treatment is given for about 6 months. Since stage 0 colon cancers have not grown beyond the inner lining of the colon, surgery to take out the cancer is typically all that is needed. This can be done in most cases by removing the polyp (polypectomy) or local excision through a colonoscope. Removing part of the colon (partial colectomy) may occasionally be needed if a tumor is too big to be removed by local excision. Stage I colon cancers have grown into the layers of the colon wall, but they have not spread outside the colon wall itself (or into the nearby lymph nodes). Stage I includes cancers that were part of a polyp. If the polyp is removed completely during colonoscopy, with no cancer cells at the edges (margins) of the removed sample, no other treatment may be needed. If the cancer in the polyp is high grade (see Colorectal Cancer Stages) or there are cancer cells at the edges of the polyp, more surgery may be recommended. You may also be advised to have more surgery if the polyp couldn’t be removed completely or if it had to be removed in many pieces, making it hard to see if cancer cells were at the edges. For cancers not in a polyp, partial colectomy ─ surgery to remove the section of colon that has cancer and nearby lymph nodes ─ is the standard treatment. You typically will not need any additional treatment. There are 5 stages of colon cancer, ranging from 0 to 4. Stage 3, Dukes C colon cancer, is one of the more advanced stages. In it, tumors have spread beyond the colon to the lymph nodes. Stage 3 is divided into 3 subgroups. These subgroups range from least to most advanced with a survival rate ranging from 44 to 83 percent. Your doctor and cancer team will explain what this stage means, answer questions, provide treatment options and help you determine which plan will work best for you. 1 Deal with stage 3 colon cancer in steps. Try not to get ahead of yourself by looking at the entire picture. 2 Admit that you are sick. This is the first step in accepting and coping with a diagnosis of stage 3 colon cancer. It takes time to process this information and the new emotions you're feeling. You may become a little overwhelmed, frightened or anxious. These are normal feelings, so allow yourself a little time. 3 Stay positive. It's normal to feel down or depressed following a diagnosis of stage 3 colon cancer, but it's important to keep a positive outlook. When you find you can't fight the depression on your own, see a therapist or a counselor. Your physician can also prescribe medication that can reduce your feelings of depression. 4 Tell family and close friends about your stage 3 colon cancer diagnosis. Presentation: "Overdiagnosis and needless treatment. Liver metastasis resection is such a useless treatment and should be avoided. Listen why. Liver metastases appear shortly after the resection of a colon tumor. When only a few metastases appear it is recommended to remove them surgically, e.g. by lobectomy. For more abundant metastases, medicine recommends to insert a port at the liver entry an treat it locally with chemotherapy. Both recommendation are unnecessary since sooner or later metastases will recur. Some common misconceptions: 1. When observing a single liver metastasis surgeons take it as an isolated phenomenon. According the statistics here they are wrong. For each overt metastasis there are many too small to be detected. In the liver and elsewhere. 2. The second misconception is that metastatic resection cures colon cancer In my breast cancer presentation I show that once a tumor recurs, cancer is incurable. Which applies also to colon cancer. Let's look at some recent data from the SEER data base. 3.. Liver is not the first site of disease recurrence. It is the first site where metastasis was detected. Other micrometastases, which spread all over the body were too small to be detected.. 4. For the same reason it is not the only site of disease recurrence. This so called single metastasis is only a biomarker of a swarm of micrometastases.. All these misconceptions are due to the fact that medicine ignores the role of the host in cancer.
