My sick toddler refuses her antibiotics, and barely eats or drinks. What can I do?
You mentioned that you have a two year old little girl who has been sick lately and needs some medication. First of all, I'm so sorry, I can sympathize from mother to mother; when you have sick kids it's not fun. And it can add to everything when you can't get them to take their medication, that's really stressful because you know they need it. You said you've tried many things. You've tried adding it to milk, you've tried adding it to food, and nothing seems to be working. You even tried blowing on her face. This is advice coming from me as a mother not necessarily me as a nurse. It's come to the point with our kids where it's required two of us, one of us has to hold the child in our arms and hold their hands down so they can't bat the syringe away, and the other person kind of squeezes their cheeks and injects the medication. What this does is it makes it so they can't spit it out. They have no choice but to swallow it. It definitely doesn't seem nice but it gets the job done. Make sure to give her a drink afterwards to wash it down and hopefully with time she'll just learn that this is going to happen and probably about the time you're out of the medication she will have succumbed to it. You did mention something that's also a little concerning to me, you said she's having a hard time eating and drinking. This alone is a reason to call the pediatrician. If this is ongoing and you feel like she's not getting enough to drink and signs of this might include less wet diapers than she usually has, specifically less than six a day. If she doesn't have tears when she cries, or she has sunken eyes, or a dry, parched mouth, these are all signs of dehydration and she should be seen by the doctor as soon as possible. Be aware that medication usually works after about 24 to 48 hours. If her symptoms aren't getting better the doctor should know about it. I hope that she's feeling better though, and good luck with it. When used properly, they either kill or inhibit the growth of bacteria that cause a particular health problem. This in turn helps your natural immune system fight the infection. But antibiotics often have side effects that cause other temporary health issues. Some common side effects include diarrhea, nausea, vomiting, vaginal yeast infections, allergic reactions and inner ear problems. 1. Eat Probiotic Yogurt When taking antibiotics, it is highly recommended to include yogurt with ‘good bacteria’ in your diet. Eating yogurt that contains ‘live cultures’ (also labeled as probiotic yogurt) helps your body recover from diarrhea, one of the common side effects of antibiotics. These live cultures are gut-friendly bacteria that provide a layer of protection in the intestines and help generate lactic acid to get bacterial toxins out of the body. 2. Eat Fermented Foods or Take a Probiotic Supplement. Apart from yogurt, many fermented foods naturally contain a high amount of probiotics. So, in addition to yogurt, include kombucha, kefir, sauerkraut, pickles, pickled beets, pickled turnips, pickled carrots and kimchi in your diet. 3. Eat Garlic. Garlic is a natural antibiotic that helps kill harmful microbes without the side effects of antibiotic medicines. Also, it contains an important compound known as allicin that protects your kidneys and liver from the potential damage of antibiotics. 4. Take Milk Thistle Supplements. Taking traditional antibiotics can be hard on your liver, which is responsible for breaking down the medications you ingest. To protect your liver from any damage due to a course of antibiotics, the herb milk thistle is very effective. The rich antioxidants in this herb help protect the liver. 5. Sip Ginger Tea Whether you suffer from diarrhea, nausea or vomiting due to prescribed antibiotics, ginger is effective in dealing with these side effects. Drink Apple Cider Vinegar Tonic. Apple cider vinegar has antibacterial properties that help fight harmful bacteria in your body. Also, it is a good remedy for controlling diarrhea, one of the common side effects of antibiotic medicine. 8. Drink Enough Fluids. To counter dry mouth caused by medicines and a mild loss of fluids from diarrhea or vomiting, drink more water. Drinking an ample amount of water throughout the day will keep your body hydrated. For a more-severe loss of water, drink fluids that contain water, sugar and salt to help replace electrolytes that the body needs. 9. Stay Healthy. One way to deal with side effects of antibiotics is to stay healthy so you don’t have to take them for long. Take steps to support your health in the following way. 10. Take Antibiotics as Directed. Last but not least, you must always take your antibiotic medicine as directed by your physician. For instance, some antibiotics should be taken only with water. Others need to be taken with food in order to increase their absorption in the body and ward off an upset stomach, a common side effect of antibiotics. They may resist it, refuse it or spit it up once you get it into them. Here are some tips to make things easier. Babies are more likely to take their medicine when they're hungry, so give it to them before you feed them. They're also less likely to spit it up that way. We recommend, unless you have to, you don't mix medicine with food or drinks. Mixing it makes it harder to know if the full dose has been taken. If that's the only way your child will take the medicine, check with your pharmacist first, because certain foods, like yoghurt or applesauce, can stop the medication from working and others, like honey or syrup, work only if your child is not on a restricted diet. If you do use food or drink, use a very small amount, like this, so that the child will finish it all. Now, let's go over how to prepare the medicine. First, measure the dose with the spoon or syringe you got from the pharmacy. Now give the medicine to your child in small amounts and watch for them to swallow before you give them more. If you're using a syringe, try putting the medicine inside their cheek pouch. That will prevent them from gagging. If you're still having trouble, try giving your baby a popsicle or a freezie before giving them medicine. The sweet taste, and slight numbing sensation will "help the medicine go down". When giving medicine to any child, it's important that you double check the amount and the dose with the instructions that are written on the bottle. If you use a log or a medicine diary, that can help to prevent either giving too many doses, or missing some doses. If your child still can't swallow the medicine or keep it down, or if they're showing any side effects, get in touch with your doctor or pharmacist. Some oral medicines can be made to be given in other ways. Talk with your pharmacist about this to see if it's possible. - Give medicine according to instructions (Remember to give medicine to your child according to the instructions.) - Check about mixing foods or other medicines (Ask your pharmacist about foods or other medicines that may stop it from working properly.) - Check dosing amounts (If you have any concerns about the amount of medicine, or if your child is refusing to take it, speak with your pharmacist.) We paid less than a dollar for one onion and all is well in less than 24 hours! I'm a believer... (For those who say, BUT YOU GAVE HER TYLENOL!) you are right, I did, but we all know that Tylenol only covers the symptoms! I would have had to keep giving her Tylenol if I hadn't used the onion. Do I always need to give my child antibiotics when he/she has an ear infection?
Often times ear infections are treated with antibiotics. It's very common in children under the age of 2. However, once you get above the age of 2, the American Academy of Pediatrics recommends a watch and wait approach. If they are over the age of 2 and can communicate, a lot of times they will clear their ear infections on their own without antibiotics. If you can control pain with something such as acetaminophen and ibuprofen, and watch for 48 hours, most kids will clear their ear infections. If after watching them for 48 hours, their pain is still there or whereupon having their ear be checked, antibiotic can be started. In this era of antibiotics resistance, this watch and wait approach can be very good for your child.

UTI in Kids Urinary tract infection refers to an infection of the urine and lining of the organs of the urinary tract which include the bladder, ureters, which are the tubes connecting bladder to kidneys and the kidneys. Mostly the infection is bacterial of which, Escherichia Coli, what we call as E Coli, is the most common bacterial causative agent. It is more frequently seen in girls than in boys. In terms of causes and risk factors in UTI, we know that structural abnormality in the genitourinary pathway such as, what we call as VUR that is, vesicoureteral reflux. This is a condition where the valves between the bladder and ureters are dysfunctional, so they allow for backward flow of urine towards kidney, predisposing the child for kidney infections. The other causes can include constipation, withholding urine for long periods of time regularly, an inadequate fluid intake thereby lesser urine output, poor toilet cleaning habits where fecal contamination of the genetalia can lead to a UTI, History of previous UTI puts you at high risk. While some children are just prone for UTI without any underlying cause. Now, in terms of symptoms, older kids can complain about pain or burning upon urination. There can be frequent urination and the urine may be foul smelling or appear cloudy sometimes with a little bit of blood present. There can be fever, nausea or vomiting, loss of appetite, an abdominal pain. There can be something we call as flank pain that is pain on the sides of the back. Now, younger children cannot get specific with their complaints but can be presented with fever with no other obvious cause for this fever like runny nose or cough or anything. There can be vomiting, there can be diarrhea, there can be like a poor appetite. There can be irritability, there can be urine leakage into their underpants and a new onset bedwetting may be present. Now, in infants that are less than 3 months of age, sometimes fever maybe the only presenting symptom. However, there may be prolonged jaundice, there can be some lethargy, and a failure to thrive. In terms of diagnosis, in younger children and infants a high index of suspicion needs to be present especially when they have any of the above said symptoms, we just discussed. Now, in babies less than 3 months, any fever should warranty investigation for UTI as it can cause septicemia where the infection can spill into the bloodstream and infect the other organs. Now, urine analysis and urine culture should be done in every child suspected of having a UTI. Now, this urine sample needs to be collected in a sterile container after the genitalia has been washed with warm water. It should be midstream catch. The sample should to be dropped off into the laboratory for plating as soon as possible, preferably in less than an hour. In case of infants, the sample needs to be obtained ideally by urethral catheterization. It is a sterile and non-painful procedure performed by a trained personnel. Now the urine analysis reveals pus cells , positive nitrites and positive leucoesterase , which will imply the presence of a UTI. However, urine culture results wherein a bacteria in a specified amount is grown over a period of 48 hours, is what confirms a UTI. Any child between the ages of 2months to 2years with a UTI, will be required to also have an ultrasound of the bladder and kidney to check for the structure. In other investigations such as VCUG to look for reflux, or a DMSA scan where you look for scarring in the kidney may be recommended by your pediatrician or a pediatric urologist, for children who have recurrent UTI. In terms of management, UTIs once diagnosed are usually treated with appropriate antibiotics for 10 to 14 days. Infants less than 3 months or older babies who appear unwell will require hospitalization with additional blood work or a spinal tap in suspicion of sepsis and antibiotics may be given intravenously. Now, fever will require paracetamol. Keep your child hydrated with adequate fluid intake and if not able to retain fluids orally, your child may require hospitalization to prevent dehydration. Now, in case of VUR, which is urinary reflux, when diagnosed will require the child to be on prophylactic antibiotics for long periods of time and regular follow ups with pediatric urologist. In terms of prevention, make sure your child gets into the habit of drinking plenty of fluids. Ensure the children do not control their urinary urge and empty their bladder at least 3-4 times a day. Do not wash the genitalia with soap or other chemical irritants but instead with just plain water. Teach children to clean properly after bowel movements especially girls needing to clean from front to back direction only. Give kids a high fiber diet to prevent constipation, which is one of the risk factors for UTI. Team Ovum Hospitals
