It is one of the most common types of cancer in women worldwide, but also one of the most preventable, thanks to early detection with Pap tests. The cervix has 2 major cell types: flat squamous cells lining the outer part, and column-shaped glandular cells covering the inside of the cervical canal. Both types can become cancerous but squamous cell carcinomas are MUCH more common. Cancer usually starts in the zone where the two cell types meet, known as the transformation zone. Virtually all cervical cancers are caused by human papillomaviruses, or HPVs. There are over a hundred different types of HPV, some of which pose HIGHER risks than others. About 70% of all cases are caused by just TWO types: HPV-16 and HPV-18. Two proteins produced by HPV, known as E6 and E7, INTERFERE with cell functions that normally PREVENT EXCESSIVE cell division. This causes the cells to grow in an UNcontrolled manner. HPV is sexually transmitted and is VERY common, but in most women, HPV infections resolve on their own and do NOT cause cancers. Factors that may INcrease the risk of PERSISTENT HPV infections include WEAKENED immune system, other sexually transmitted diseases and smoking. Chances of developing cervical cancer also increase with having many children and LONG-term use of birth control pills. Early-stage cervical cancer generally produces NO symptoms. Advanced-stage disease may cause ABnormal or IRregular vaginal bleeding, pelvic pain, or unusual vaginal discharge. EARLY detection is the key to prevent cervical cancer. Cervical cancer screening may include pap tests ALONE, or in combination with HPV DNA tests. In a pap test, cells are scraped from the cervix and examined for PRE-cancerous changes, known as cervical intraepithelial neoplasia, or cervical DYSPLASIA. These morphological changes can range from mild to severe. If the results are ABnormal, the test is repeated again after 6 months or a year to MONITOR the condition. Additional diagnostic tests may also be performed. In most cases, MILD dysplasia resolves on its own and a follow-up pap test is all that is required to confirm. In a small number of cases, these ABnormal cells may develop into cancer, but they usually take YEARS to do so, which allows plenty of time for treatment when detected early. In the US, a pap test is recommended every 3 years, from the age of 21, or every 5 years if combined with an HPV test. Treatment options for cervical cancer include surgery, radiation, chemotherapy or a combination of these. Early-stage cervical cancer is typically treated with surgical removal of the uterus. This option is the most effective in preventing cancer from coming back and is usually preferred when patients do NOT need to maintain fertility.

Condylomata acuminata are anogenital warts caused by the human papillomavirus (HPV). HPV is one of the most commonly transmitted STDs in the world. In 90% of women who have cervical HPV infection, the infection becomes undetectable within two years. A few women have persistent infection, which is a key risk factor for cervical cancer. Regular Pap smears and HPV screening can monitor this risk and provide an early warning that treatment is needed. HPV can be transmitted by vaginal, oral and anal sex. Many individuals who have HPV are not aware of the infection and can spread it to their sexual partners. HPV types 6 and 11 typically cause venereal warts, and along with types 42, 43, and 44 have a low risk of progressing to cancer. HPV types 16, 18, 31, 33, and 36 have a higher risk of progressing to cancer. These and other types commonly infect the cervix where they can cause either harmless changes or dysplasia which is a precancerous change. High-risk HPV infections of the cervix that last for years can result in cervical cancer. HPV infection of the cervix is found by Pap smear or cervical HPV DNA testing. A Pap test showing dysplasia or intraepithelial neoplasia or cervical cancer almost always is a result of HPV infection.

HPV virus (Human Papilloma Virus) is a group of over 100 viruses, that affect skin cells and mucous membranes in humans. There is an association between being infected with these viruses and developing cervical cancer. Some strains of the virus also cause genital warts and may increase the risk of developing oral and anal cancers. HPV was first discovered when researchers noticed that the only group of women that did not develop cervical cancer were celibate nuns. A sexually transmitted cause of cervical cancer was hypothesized, and research discovered the family of viruses. Most women that contract the virus are able to clear it from their body within 1-2 years. If you cannot clear the virus, or have ongoing exposure to it, over time the virus can cause normal cells to turn cancerous, both in the cervix as well as the vagina. Pap smears not only can detect cancerous and precancerous cells, but can also detect the presence of the HPV virus. Gardasil is a vaccine that helps your body to defend against 4 strains of HPV. Types 16 and 18 are associated with cervical cancer. Types 6 and 11 are 2 strains that cause genital warts. The vaccine is over $200 per dose and requires 3 doses. Not all that start the series finish it, either because of cost issues, side effects or non-compliance. The overall effective rate of protection from cancer is probably less than 50%, and may be as low as 17% if all that start the series, but do not complete it are included. Also one must remember that there are other strains that can cause cancer, that are not in the vaccine. It is recommended for girls as young as 11, up to age 26. It is now also recommended for boys, with the theory, that if HPV is decreased in boys and men, that the rate of infection in women will decline. There is some weak data that says that anal and oral cancers in homosexual men may decrease from the vaccine. There is a push in many states to make this a mandatory vaccine. I think the long term data is not known, the actual benefits are probably much less than that reported, and the possibility exists, that if by some way we eliminate types 16 and 18, that another strain may become the new prevalent strain, and could be as bad or worse. What if there were 100 strains of small pox, and our vaccine prevented only 4 strains? Isn't it reasonable that another strain may emerge? We have a wonderful way to prevent cervical cancer, already. The pap smear. Yet, we used to recommend your first pap at age 18, or when you became sexually active. The new recommendation is now age 21, regardless. Why is that? Money. The rationale is, it may take 3-5 years of infection before there are pap smear abnormalities, so lets turn a blind eye when someone is having unprotected sex at age 16. Don't do an exam, don't educate, let them get HPV, and worry about it later. On a similar note (rant to follow)!...very quietly, under president Obama, the starting age for mammogram was moved from age 40 to 50. Why? Because it cost too much money to save the lives of women in their 40s who developed breast cancer. There aren't enough to justify the cost. When there was an outrage over this recommendation, it was rolled back to 40. If you think Obama care and universal healthcare wont have rationing and worse services for the majority, think again. When the government tells you that your 11 year old daughter can't go to school unless she is vaccinated for HPV. Or that you must give your infant the Hepatitis vaccine because the government doesn't know if they will grow up to be an IV drug user, or when a new lung cancer drug is prevented from coming to market, because it is $50,000 more than existing therapy, and "only" improves survival by 6-12 months over existing therapy. Beware.

Cervical cancer may be the most common cancer linked to HPV, but it is not the only one. As it turns out, men are at higher risk to get another form of cancer from the same virus. The human papillomavirus - it's most closely linked to women and cervical cancer. "HPV, there are 140 different types; some are low-risk, some are high-risk. The high-risk types are known to cause almost all cervical cancers," says Dr. Kevin Fleishman, a gynecologist on Lee Memorial Health System's medical staff. Studies show that half of all American men are infected with HPV and it puts them at greater risk of cancer too. "Absolutely, more and more cancers have been shown to be associated with the HPV virus," says Dr. Fleishman. Among them are head and neck cancers. "Many of these cancers are caused by HPV. Prior to 15, 18 years ago, probably 90 to 95 percent of my new cancer patients were smokers. But now about 45 percent are non-smokers and the majority of those are caused by HPV," says Dr. Phillip Andrews, an otolaryngologist on the medical staff of Lee Memorial Health System. Two-thirds of cancers of the tongue and tonsils are caused by HPV and 80 percent of those cases are found in men. If caught early, the prognosis is good. "Some of the smaller cancers in the base of the tongue and tonsils we'll treat with radiation alone, particularly if it's HPV related," says Dr. Andrews. The HPV vaccine was originally promoted as a 'girls' vaccine. Since then, the CDC recommended it for boys as well. And not just to stop the spread of cervical cancer but to keep them safe, too. Diagnosed with Adenocarcinoma (cervical cancer from HPV number 16). Leep was to be my treatment, but pathology found cancer cells in the lesion removed, as well as in the lining. Surgery is scheduled to remove the cervix to remove the HPV invasion from my body. (Stage One Cancer. I was told all along, "The second these low grade cells get aggressive, it's surgery to remove the cervix." I will see an oncologist on the island of Oahu in Honolulu. It wasn't supposed to go this way. We caught abnormal cells early, in the first abnormal pap. Then closely checked ea area with colposcoy, all samples cancer free! Did DnC, samples good. I kept hearing, "Don't worry, the body clears HPV like the common cold! It could take two-years to clear.... You’ll clear it! It's staying Low Grade! You can turn this around!” To be fully honest, (look at my eyes right now) I look sick! I kept telling my doctors, I look in the mirror and don’t look well. And my hair is falling out. (I've been under close watch, every 3months, as this began. Finally, a lesion of cells formed on the lower cervix and my doctor could See It. "Immediately” we went to Leep to remove it. I was thinking, "All better now! "Leep is my treatment! But, NO! That is not how HPV 16 works. Nope! This is a full on hysterectomy for treatment. And something I don't want. (I actually wanted one more child!) I read a report from a Cytotechnologist who studies our cell samples for cancer and reports to a pathologist. She said in the cervical HPV lesions she is finding "50%" are showing pre-cancer and cancer cells. (Huge odds!) I did not know my odds were that bad. Moving ahead, surgery is soon! Nabbing the issue as soon as it reared its ugly head was important. "Early detection with Early treatment" is all we can do for ourselves beyond good diet and exercise. (And finding that "ONE" Special "Monogamous" mate and not holding onto that ONE we keep giving another chance to, calling it the oops!) With Cervical Cancer from an HPV, everything in our lives has to be organic, especially the man we chose to love! 99% of cervical cancer starts from an HPV. HPV is a sexually transmitted disease (STD) treated by SURGERY!!!! It's not treated by a pill or a shot. Kemo and radiation may be needed after surgery, once the pathology report is in to show where else it showed up. After the radical hysterectomy, a pathology report on my tissues takes 5 days. That tells the oncologist if kemo or radiation is needed. Education is Mandatory, and that is the phase I am in. I never heard of HPV. I'm not in the dating game! (If this is how love ends for some of us, where do we go from here?) I did not get HPV (Cervical Cancer-cells) genetically from my mom who died of ovarian cancer. It is two different issues. They are removing the Cervix by the Da Vinci Robotic-assisted surgery. This machine can test the lymph nodes for cancer. That is important!!!! If goes as planned, this surgery will save my life, which means Leep saved my life. There are over 200 strains of HPV and over 15 cause cancer, and that number is rising. You can get several HPV viruses at once! Kissing can transfer HPV, if not cleared it turns into throat cancer. HPV is transferred skin to skin! Easy Peezy! Not by liqiods or ejaculation. I was told 16 and number 18 are not naturally curable. They cause cervical cancer. Guys get cancer from HPV. I explained it to my mate. I had the facts typed out for him with my medical report. I was in grade school till 9th grade, Catholic school. We were not even dating yet. I can't image being in 6th grade and given this news. I married my first boyfriend, that's how it was where I grew up.

One way to reduce the risk of getting the disease is to provide cervical cancer vaccine. Cervical cancer is mostly caused by sexually transmitted virus that is human papillomavirus (HPV). Cervical cancer vaccine is one effort to prevent transmission of the virus. HPV infection in women can cause abnormal growth of cells in the cervix. In some women, the disorder can then develop into cervical cancer. For women, cervical cancer vaccine is recommended three doses in three times. The first vaccine given in adolescence is 11-12 years, then the second vaccine given one or two months after the first vaccine. Then, the third vaccine was given 6 months after the first vaccine. The third dose of the vaccine is believed to be a long-term protection against HPV infection. If the adolescent dose of vaccine is not yet complete, it is better to consult a doctor to complete the dose of the vaccine. Not only women, men can benefit from HPV vaccine. For men, HPV virus can also cause genital warts, anal cancer and throat cancer, apart from cervical cancer. The recommended male group for HPV vaccine is men who have sex with men or who have immune disorders and are 26 years or younger. Types of Vaccines. HPV viruses have various types. New studies show throat and neck cancers are on the rise. But unlike most cancers, doctors say the most common cause isn’t genetics or lifestyle, but a virus that’s typically known for causing cervical cancer in women. Dr. Anthony Anfuso, an oncologist on staff with Lee Health, says it’s becoming a common diagnosis. “We’re seeing a lot of younger people who are non-smokers actually get this type of cancer and the reason in those people often times is a virus called HPV.” The human papillomavirus is the same virus that causes cervical cancer in women. Now, doctors say the sexually transmitted virus is also responsible for throat and neck cancer in men and women. “One difference between HPV induced oral cancer and cervical cancer is that there are screening tests, there are pap smears for women, to try to pick up premalignant lesions. Unfortunately, for oral cancer that has not been developed yet,” said Dr. Anfuso. But there is good news. Patients who are diagnosed with HPV oral cancer have a higher chance of survival. “Someone who has an HPV negative oral cancer, which again is a cancer of the tonsil or the back of the tongue, their 5 year survival rate is 35 to 45 percent. If you have a HPV positive tumor it actually jumps to 90 percent for a patient who is a nonsmoker,” said Dr. Anfuso. Doctors say that’s because HPV has not been found to be an aggressive cancer. Radiation and chemotherapy are common treatments for oral cancer. “The fact of the matter is, probably over 90 percent of the population has been exposed to the HPV virus, even the high risk HPV virus. Most people will clear the infection and never develop symptoms,” said Dr. Anfuso. But those who don’t are at risk for developing oral cancer, making it important to notify your doctor about any changes or pain. The cervix is the lower, narrow end of the uterus. The cervix connects the vagina to the upper part of the uterus. The uterus is where a baby grows when a woman is pregnant. Cervical cancer does not happen overnight. It begins first with abnormal pap smears, that if not treated transforms in stages to cervical cancer. This is why annual checkups and pap smears are the best preventative method. If caught early, there are more options available to you. Causes of Cervical Cancer Weakened immune system: A high level of stress may hamper the body's ability to fight off human papillomavirus (HPV) infection, placing women at a higher risk of cervical cancer, according to a study conducted by researchers from the Fox Chase Cancer Center and published in the Annals of Behavioral Medicine. The researchers found that women who reported more daily stress had a less effective immune system response to infection than women who reported last stress. Sexual history: Studies indicate having sex at an early age, having multiple partners and not suing a condom puts you at greater risk for HPV. Studies also indicate that men who are uncircumcised are at greater risk for carry the human papillomavirus and infecting women. Prolonged use of birth control contraceptives 5 or more years increases chances of cervical cancer. Dr. McDonald discusses abnormal pap smears and what a woman should so if she gets one. Matt McDonald, MD: When a patient presents with an abnormal Pap smear, the initial question that most people have is what does this exactly mean. In a result in an abnormal Pap smear is strictly a screening test to help us know which patients require a further evaluation to detect if cervical dysplasia truly exists. A Pap smear is an excellent screening test. It is not an excellent diagnostic test and what that means is that within the realm of abnormal, meaning abnormal cells on the cervix which is called and physician terms dysplasia, a Pap smear is very good at telling us who has no dysplasia and who does have some dysplasia. But it is not is not a good test in telling us what degree that dysplasia exists and so information that is commonly presented or reported on Pap smears is whether a viral infection called HPV exists. Now HPV is a virus, otherwise known as human papilloma virus, which is incredibly common in the adult population. We don't really understand the true mechanism of why someone women are exposed to HPV and never develop dysplasia, but many do, most do in fact. They are exposed to HPV or the virus and really never develop severe dysplasia of the cervix or cervical cancer. That is an area that much research is going into at this time, but it alarms a of lot of women that they feel like something dramatically different has occurred that they have been exposed to HPV, but the truth of the matter is that the vast majority of us in our lifetime will exposed whether all of these exposures are sexually mediated or not, we really don't know, to be quite honest with you. There is some data and some case reports that HPV can be transmitted through nonsexual means. It is my opinion that there are nonsexual means of transmission of HPV due to the fact that so many of us, almost 90% of us have been exposed by the time we are 50. So an abnormal pap smear truly is a screening test just to let us know which patients require further evaluation with a gynecologist for what is called colposcopy, which is a diagnostic test where we evaluate the cervix with three-dimensional microscopy to see if true dysplasia truly exists. So an abnormal pap smear really is just the very initial beginning of the workup of abnormal cells or dysplasia on the surface.
