Kitov Completes KIT-302 Testing for New Drug Application to FDA

TEL AVIV, Israel, December 7, 2016 /PRNewswire/ -- Kitov Pharmaceuticals Holdings Ltd. (NASDAQ/TASE: KTOV), an innovative biopharmaceutical company focused on late stage drug development, announced today that its lead drug candidate KIT-302 met the U.S. Food and Drug Administration's (FDA) standards for establishing six months stability. The study, conducted by Kitov's manufacturing partner, Dexcel Pharma, demonstrated that nine pivotal batches of KIT-302, three in each dosage forms (celecoxib/amlodipine of 200 mg/10 mg, 200 mg/5 mg, and 200 mg/2.5 mg) met the criteria demonstrating stability for 6 months. The successful stability study enables registration batches of KIT-302 to be included in the New Drug Application (NDA) submission which is planned during Q1 2017. The Certificate of Analysis demonstrated adequate stability required for submission, in accordance with FDA guidelines. Kitov expects to submit additional stability data to the FDA at a later time, in order to extend the labeled shelf life. "We are very pleased with the stability study results, which we believe complete the testing required for our New Drug Application to the FDA for KIT-302," stated Dr. J. Paul Waymack, Chairman of Kitov's Board and Chief Medical Officer. About KIT-302 KIT-302 is a fixed dose combination consisting of 200 mg of celecoxib, indicated for osteoarthritis pain, and 2.5, 5 or 10 mg of amlodipine, indicated for high blood pressure. It is intended to treat pain caused by osteoarthritis (OA), as well as simultaneously treat hypertension, which is a common side effect of certain stand-alone drugs that treat osteoarthritis pain, as well as a common concomitant preexisting condition. KIT-302 is comprised of two U.S. Food and Drug Administration (FDA) approved drugs, celecoxib (the active ingredient in Pfizer's Celebrex®), for the treatment of pain caused by osteoarthritis and amlodipine besylate (the active ingredient in Pfizer's Norvasc®), a drug designed to treat hypertension. Kitov expects to submit a New Drug Application for KIT-302, which successfully completed a pivotal Phase III trial, with the U.S. Food and Drug Administration during Q1 2017. FDA strengthens heart risk warning in popular pain relievers

WASHINGTON (AP) -- Federal health regulators are bolstering warning labels for popular pain relievers, adding information about the risk of heart attack and stroke in the short term. The changes announced Thursday apply to prescription non-steroidal anti-inflammatory drugs, or NSAIDs, including arthritis treatments like Celebrex. The agency said it plans similar changes to over-the-counter drugs in the same class, such as Advil and Motrin. Language on the pills currently warns that they can increase the risk of heart-related problems if used long term. However, the Food and Drug Administration's new warning states that heart attacks and strokes can occur in the first few weeks of taking the drugs. The agency also warns that the risks increase with higher doses of the drugs. The updates are based on an FDA review of recent studies and recommendations by outside advisers. "In general, patients with heart disease or risk factors for it have a greater likelihood of heart attack or stroke," the agency notes in the announcement posted to its website. The labeling change is the latest step in the FDA's ongoing safety review of the drugs, which stretches back to 2004. That's when Merck Co Inc. pulled its blockbuster pain reliever Vioxx off the market because of links to heart attack and stroke. Vioxx had been heavily advertised as a new kind of NSAID that was supposed to be easier on the stomach. But its withdrawal shook the medical establishment and ushered in a new era of drug safety at the FDA. In 2005, the agency added boxed warnings to all prescription NSAIDS, including Pfizer's Celebrex and high-dose versions of ibuprofen and naproxen. Celebrex is the only drug from the same class as Vioxx that remains on the market. The prescription NSAIDs are generally used for long-term chronic pain conditions like arthritis. The agency also added similar warnings to lower-dose, over-the-counter NSAIDs like Aleve and Advil. Those drugs currently warn patients to take the lowest dose possible for as short a period as possible. They are not intended to be used for pain longer than 10 days, according to their labels. "These medicines have a long history of safety and efficacy when used as directed," said the Consumer Healthcare Products Association, which represents nonprescription drugmakers, in a statement. The group said it would cooperate with the FDA as it requests updates to the labels. Advil is marketed by Pfizer Inc., while Aleve is sold by Bayer. The FDA's panel of experts reviewed an analysis from last year that looked at the results of over 700 studies involving the use of NSAIDs with around 350 thousand subjects. The Food and Drug Administration has required NSAIDs to be labeled with possible health risks since 2005. Victoria Richards, an assistant professor of medical sciences at the Frank H. Netter MD School of Medicine at Quinnipiac University in Connecticut is quoted as saying: "Research suggests that naproxen has a significantly lower risk of cardiovascular events than ibuprofen, although the dose of ibuprofen may be taken into consideration. This lower risk might be seen with low-dose ibuprofen." In the US, consumers reportedly purchased over 275 million boxes of over the counter pain killers which amounts to about 1 point 7 billion dollars in sales, while prescription NSAIDs like Celebrex reportedly had sales of 2 point 9 billion dollars.

The downside(s) of NSAID use. According to the National Institutes of Health, nearly 50 million American adults have chronic pain or severe pain. The conventional medical model teaches us to reach first for medication to relieve pain, with ibuprofen and other NSAIDs (nonsteroidal anti-inflammatory drugs) at the top of the list. It is after all a pretty realistic expectation. Have Pain. Get rid of Pain. Done. However, with research mounting to show NSAIDs have a notable list of dangers, it makes sense instead to look for the root causes of pain and not just cover up the end result of whatever imbalance is causing your pain. Many people turn to NSAIDs for relief from pain and inflammation. Common NSAIDs are ibuprofen (Motrin, Advil) and naproxen (Aleve). Celecoxib (Celebrex), diclofenac (Cataflam, Voltaren) are prescription NSAIDs. Aspirin is an NSAID, but it does not pose the same risks for stroke and heart attack. Ibuprofen is metabolized by the liver, and can cause lesions, liver failure, or jaundice over time. The FDA has even warned against NSAIDs because they increase the risk of heart attack and stroke. NSAIDs promote leaky gut Another reason to avoid NSAIDs is their tendency to promote leaky gut, as I’ve mentioned before in some of my other blogs. In leaky gut, the lining of the small intestine becomes damaged and overly porous, allowing undigested food, bacteria, yeast, and other pathogens into the bloodstream. This triggers inflammation and pain throughout the body — the same thing people use NSAIDs to relieve, creating a vicious cycle that can be very difficult to get off. So, if the typical go-to medications for pain aren't an option any more, where do we turn? Functional medicine offers solutions. Address inflammation to root out pain When pain is treated with NSAIDs, it typically comes back when the medication runs out. However, in functional medicine the goal is to identify and address the cause of the inflammation and pain instead of simply putting a temporary Band-Aid on it. It’s understandable to want relief so you can feel and function better. The good news is many people find their chronic pain diminishes substantially or disappears completely when they adopt functional medicine strategies. So let me be clear on this. I’m NOT saying that functional medicine will fix all your pain problems. BUT, what I am saying is that the entire body is connected. If you have some sort of chronic pain it often becomes a vicious cycle of pain = inflammation = pain = and on and on. By addressing where that additional or extra inflammation is coming from we can often relieve or at least significantly reduce the amount of chronic pain you are in. It eve applies to acute injuries as well. If you body is balanced and has little inflammation prior to an injury, when you do get injured you will heal much faster. Following are a few ways functional medicine can relieve pain and inflammation: Anti-inflammatory diet. Remove foods that trigger inflammation, such as gluten, dairy, grains, legumes, eggs, sugar, and nightshades. This is typically done as an elimination and reintroduction protocol where you follow the diet strictly for a period of time and then customize it depending on your food sensitivities. This customization is generally done with Food allergy (IGE antibodies) and food sensitivity/intolerance (IGG antibodies) testing. Note: not all tests are created equally. Don’t just pick the cheapest or quickest test. Otherwise your results may not mean too much. Check out the Cyrex Array 10 on our Functional Medicine testing page. I will also put some links in the description below. Check em out! Avoid nightshades. Vegetables in the nightshade family can cause pain and inflammation in the joints. These include eggplant, potatoes (but not sweet potatoes or yams), peppers, tomatoes, tomatillos, hot pepper products and pepper-based spices. Simply removing nightshades from the diet has brought relief from joint pain for many, especially those with rheumatoid arthritis. But keep in mind this doesn’t mean everyone will benefit from this. If you complain of mild to moderate pain a doctor will freely prescribe NSAIDs, or non-steroidal anti-inflammatory drugs. You can even go and just buy them right over the counter at pretty much any store. However, they are not benign as you think, especially if taken long term. I can’t tell you how many patients I see (and kids of patients) that routinely take some sort of NSAID for pain on a daily basis. Many times even multiple times per day. They think it’s Normal! It’s almost like they think that their body has a deficiency in NSAIDs. Maybe so. Maybe there just isn’t enough NSAIDS in the foods or liquids they are eating and drinking. NOT! NSAIDS are not a nutrient. If it wasn’t clear….. I was being facetious. The first step in dealing with a person taking a steady supply or NSAIDS is to evaluate why do they need to take it. NSAIDs are linked to numerous health complaints. Common brands of NSAIDs include ibuprofen (Motrin, Advil) and naproxen (Aleve). Celecoxib (Celebrex), diclofenac (Cataflam, Voltaren) are prescription NSAIDs. Aspirin is also an NSAID, but it doesn’t pose the same heart attack and stroke risks. So…Maybe it’s a slightly lesser of two evils. Or maybe its just evil differently, in it’s own special way ;-) NSAIDs have been linked to many health disorders such as: • 40–60 percent increased risk of cardiovascular problems • 25 percent increased risk of hearing loss • 60 percent increased risk of heart failure • Gastrointestinal problems such as pain, heartburn, nausea, vomiting, bleeding, ulcers, and leaky gut. • Increased risk of asthma and eczema in children NSAIDs increase heart attack and stroke risk The link between NSAID use and heart attack is so well founded the FDA has issued a warning. The risk may begin within a few weeks of taking an NSAID and the longer you take NSAIDs, the higher your risk. So buyer beware! Functional medicine and NSAIDs — leaky gut In functional medicine, we look at another sabotaging factor of NSAIDs —the infamous, notorious, ever elusive and dreaded leaky gut. woooowoooowooo! Leaky gut means the lining of the small intestine has become so damaged that it becomes overly porous, allowing undigested food, bacteria, yeast, and other pathogens into the bloodstream from the gut. This triggers inflammation and pain throughout the body, exactly the sort of thing people use NSAIDs to relieve. Not to mention that leaky gut is very tightly associated with autoimmune issues. The anti-inflammatory drug celecoxib, commonly known under the brand name Celebrex, causes liver cancer cell death by interacting with a protein in a way that triggers those cells to commit suicide, according to research appearing in the journal Cancer Prevention Research. The study authors also found that the combination of celecoxib with two chemotherapy drugs killed more liver cells in culture, making combinations of those drugs more effective than either on their own.

Pfizer Inc. suspended sales of Bextra in the United States and the European Union at the request of the Food and Drug Administration (FDA) and European regulators. The company said that the FDA, in seeking Bextra's withdrawal, cited a risk of serious skin reactions to Bextra on top of the risks shared by other similar drugs. The FDA also wanted other drugs in the same class, such as Celebrex and Vioxx, to carry the strongest possible warnings about increased risk of heart attack and stroke. In addition to the prescription drugs, the FDA asked manufacturers of related over-the-counter painkillers such as Advil and Motrin to revise their labels to include information about the risks of cardiovascular incidents and gastrointestinal bleeding. The FDA has been studying the safety of the so-called Cox-2 inhibitors since Merck & Co. voluntarily pulled Vioxx from the market on September 30, 2004 after heart problems were reported in some users. Once blockbuster sellers, the painkillers were particularly popular among arthritis sufferers. Pfizer said it "respectfully disagrees" with the FDA regarding the overall safety of Bextra and plans further discussions with the government about the possibility of returning Bextra to the market. Pfizer shares dropped 62 cents, or 2.3 percent, to $26.24 in morning trading on the New York Stock Exchange (NYSE). The FDA's move is a significant blow for New York-based Pfizer which had just announced a plan to return to double digit earnings growth by next year, in part by reviving sales from Bextra and Celebrex. Sales of both drugs went into a freefall last year after being linked to heart problems. UF study strengthens concerns about long-term use of certain painkillers

Painkillers such as ibuprofen, naxopren and celecoxib provide needed relief for many patients who have chronic pain. But an ongoing source of contention is whether those drugs and others in their class known as nonsteroidal anti-inflammatory drugs, or NSAIDs, are linked to harmful health effects. Now a new study from the University of Florida raises the concern about potential risks to a higher degree than before, finding a doubling of deaths from heart attack, stroke and related events among people who have both hypertension and coronary artery disease and use the drugs long term. The findings, based on data from the international INVEST clinical study of hypertension therapies, are published in the current issue of The American Journal of Medicine. "It does strengthen our practice recommendations," said lead author Anthony A. Bavry, M.D., M.P.H., an assistant professor of cardiovascular medicine in the UF College of Medicine's department of medicine. Physicians already discourage the use of NSAIDs among the elderly and after heart attacks, on the basis of several studies showing that the drugs are linked with a higher risk of stroke and heart attack. But the UF researchers, including senior author Carl J. Pepine, M.D., a professor of cardiovascular medicine in the UF College of Medicine, advise patients to talk to their doctors before stopping use of prescribed treatments. "It's a tricky issue, because NSAIDs are useful for relieving pain, and that is much of what we do in medicine — alleviate pain and suffering," said Deepak L. Bhatt, M.D., M.P.H., an associate professor of medicine at Harvard Medical School and chief of cardiology at the VA Boston Healthcare System, who recently published findings that NSAIDs are linked to a higher risk of stroke. "Unfortunately, most medications have some potential side effects, and it's important to know what those might be." Bhatt was not involved in the UF study. Patients who have both high blood pressure and coronary artery disease are generally put on aspirin, a unique type of NSAID, to reduce their risk of a heart attack. Physicians are concerned that giving those patients other NSAIDs for pain relief could cancel out aspirin's beneficial effects and raise the risk of negative cardiovascular effects. The UF research team took advantage of the availability of INVEST study data from 882 chronic NSAID users and almost 22,000 intermittent or nonusers to try to settle the question. They looked at patients who reported using NSAIDs over an average of about three years, to see whether there was an increase in adverse events or cardiovascular-related death compared with patients who did not use those pain medicines long term. The risk of death from cardiovascular causes was 2.3 times higher among patients who chronically used the drugs than among other patients. NSAIDs are thought to act in a variety of ways to increase cardiovascular risk. They are thought to prevent aspirin's protective anti-clotting effect by preventing the aspirin from binding properly to platelets in the blood. Some NSAIDs might also increase bleeding risk. In addition, NSAIDs raise blood pressure, thus potentially raising the risk of heart attack and stroke. Some NSAIDs have already been removed from the market because of concerns about an elevated risk of heart attack and stroke.

Researchers from the University of Milan say the use of prescription-strength ibuprofen, naproxen and other commonly used pain relievers may be tied to a higher risk of heart failure. According to UPI, medicines like these fall into a category of painkillers known as non-steroidal anti-inflammatory drugs--or NSAIDs. After analyzing the medical records of millions of patients across Europe, researchers found these medications may raise a person's relative risk of heart failure by nearly 20 percent. In fact, the greater the amount of NSAIDs the person is taking, the greater the risk of heart failure.
