I think it's really important to know how therapy really works and why therapists ask you the questions they do. Most therapist will ask you at the beginning "what brought you here today" and try to work out what your main goal for therapy is. This helps us better create a plan. From there we begin the assessment process. This could be diagnosis, figuring out what you have worked on before, your school and family history, etc. While doing this, we will also be giving you homework and trying out new tools to see what works for you and what doesn't. The main goal for any therapist is that you don't need to see us anymore. That being said, know that coming back to therapy is normal and most therapists have an open door policy :) So let your process be just that and go back if you need to. Understanding different modalities can be challenging. This session will portray two complex case studies, esophageal and lung, one curative and one palliative, following each from diagnosis through chemotherapy, surgery, and radiation. Quick Diabetes Breakfast Ideas

I personally feel we don’t take the time we should to prepare and eat a good breakfast to get our day off on the right foot. I know, I know, TIME! So while I do not advocate you do this every day, I am going to give you 7 quick and easy breakfast recipes that will be good for your diabetes meal planning. Again, I do NOT advocate you eat these type breakfasts every day, but I do feel eating something is better than nothing at all. With that said, here are my 7 quick and easy breakfast ideas. Ty Mason TriFecta SuperFood Breakfast. My best tip for this video would be what I call a Ty Mason TriFecta SuperFood Breakfast. It is easy and combines 3 of my diabetes superfoods. It is so simple, you can make it in seconds or have it in the fridge ready to go if you are running really late. To a bowl of greek yogurt, add walnuts and strawberries. It is that simple. But what a power packed breakfast this is. You get everything you need to get a great start to your day and it will not affect your blood sugar a great deal at all. Some may put a little sucralose in the yogurt, it is a bit tangy, but many love it just as it is. Fruit Smoothie I know in my breakfast mistakes video I said breakfasts should not be in liquid form. But remember, this is an exception for those days that you must get out of the house quickly and I would rather you drink your breakfast than go out without anything at all. Plus, this has added convenience in that you can truly take this one with you and not have to worry a great deal about getting stains on your tie or dress. As you know, I am an advocate of keeping some type of frozen fruit in your freezer just in case you run out of fresh fruit. To me, fresh fruit should be included in any meal if at all possible. Well, this is a fresh fruit breakfast. It really doesn’t matter what you add to this smoothie as long as it is fruit. I personally like the frozen fruit for a smoothie because it stays colder longer and remains a little thicker. Combine in a blender 3 or 4 ice cubes, one cup of greek yogurt (or 2% milk) a half cup of frozen fruit (strawberries, blueberries, peaches, etc) and a handful of nuts (walnuts, almonds, etc). Blend until smooth and you have a delicious breakfast ready to drink on the way to work. EGGS AND BACON I know, I said quick and easy. Hey, this one really is quick and easy. At almost any grocery store these days you can buy bacon that is already cooked. It is not that bad at all actually. You throw it in the microwave for about 10 seconds per slice (read your package directions) and voila! You have bacon. As for the eggs, boil a few up and keep them in the fridge. They truly are a great snack for any time, but for a quick breakfast you can’t beat it. Throw some bread in the toaster, have the coffee brewing and you have breakfast in just a minute or 2. If you have to eat it on the go, slice the egg in thin sliced, but it on the toast with the bacon and eat it as a sandwich on your way to work. It really is a great breakfast. If you are going to have it as a sandwich, take 30 extra seconds and add a slice of cheese to it and stick in the microwave to melt the cheese and heat up the egg. YUMMY! FRUIT BOWL Easiest breakfast ever. Cut some fruit up the night before if you know you are going to sleep in! Add some strawberries, blueberries, peaches, watermelon, apples, pears, whatever fruit you like. Leave this in a bowl overnight and those juices will mingle and marinate that fruit, I get hungry just thinking about it. FRUIT AND WAFFLE Yes, you read that right. WAFFLE! Look, the GL of an Aunt Jemima frozen waffle is 10. That is a low GL food. I know, the GI is high, but I am a strong proponent of the GL of a food. It tells you exactly how a food is going to affect your blood sugar and a GL of 10 is low. Here is my favorite way to eat a waffle. Of course, toast the waffle. DUH! But in a saucepan, add some fruit, whatever you like with a little splash of orange juice and a couple tablespoons of sucralose. Bring that up to a boil, then simmer for a just a couple of minutes. You won’t believe how good that is over the waffle. Add a glass of milk and you think you will be heading off to school than to work. Childhood memories! OATS (OATMEAL) This is an amazing breakfast, especially on those cold mornings. You can get instant oatmeal if you’d like, but very careful of the ingredients. Many instant oatmeals have lots of sugar. You can make your own oatmeal by using minute oats and adding whatever you like to it. Cook up some oats, add in sucralose, milk, a sliced up apple and cinnamon, that is good stuff. Add fruit or whatever you’d like. It’s quick, easy and actually very good for you! Avoiding malpractice is more than avoiding a lawsuit, it is avoiding the litigation process altogether. Proper documentation is arguably the most important element in avoiding malpractice and litigation for nurses. There are many reasons why nurses spend much of their time charting. However, when it comes to malpractice and the litigation process, documentation may be the only evidence a nurse has to defend his or her position. By the time a lawsuit is filed, chances are the nurses will not remember the particular patient involved, especially if they only cared for the patient for one or two shifts. Even if the nurses do remember the patient, it is their documentation that will be reviewed, presented and relied on, not their memory. All nurses have been taught, "If it's not written, it's not done!" But, what exactly does that mean? Whether you are a student nurse or have been in nursing for thirty years, this question may be difficult to answer. Over the years, documentation has changed a great deal. But, the rationale behind why documentation is important remains the same. Whether you are documenting with a narrative style, using flow sheets or charting by exception, the purpose of documentation is to memorialize what occurred while you took care of your patient and to capture relevant information about the patient's condition and medical history. The more accurately your documentation depicts what actually happened during the time you took care of your patient, the more likely you are to avoid becoming involved in litigation for malpractice. Regardless of the type or style of charting a nurse uses, it is important for documentation to be legible, logical, and complete. Not only to prove what occurred, but to also show what did not occur. For example, if a patient is admitted to a facility with both arms in tact, and claims to have left the facility with a broken arm, the first thing the patient's attorneys and their experts are going to do is review the medical records. They will try to determine from the records when and how this patient's arm could have been broken.

Bad Medicine: The Downfall of Paolo Macchiarini

For several years, Dr. Paolo Macchiarini was known as a scientific pioneer, a supersurgeon and a miracle worker. He was turning the dream of regenerative medicine into a reality. While much of the scientific community was eager to believe he had made breakthroughs, not everyone was convinced. Most of Macchiarini’s patients died within a few years of their surgeries and the experimental procedures actually made their conditions much worse. Investigations revealed that he had actually falsified his data as well as his medical credentials. While at the prestigious Karolinska Institute in Stockholm, Macchiarini invented his technique. Instead of stripping the cells from donor windpipes, he had plastic scaffolds made to order. He gave his “regenerating” windpipes to at least 17 patients worldwide. The results have been disastrous. In 2014, Paolo Macchiarini was hailed as gifted medical pioneer in an NBC special produced by Benita Alexander. Paolo and Benita became romantically involved and planned to marry. But as the wedding day approached, the plans unraveled. Benita realized that Paolo had lied to her about a lot of things. For one thing, he was still married to his wife of 30 years. Why does an intelligent and skilled surgeon create a house of cards in his personal life and perform surgeries that he knows will result in suffering and death for his patients? As one surgeon put it, he would choose to die by firing squad before experiencing a death caused by one of Macchiarini’s tracheal transplants. His experimental surgeries have been compared to the crimes of Nazi doctor Joseph Mengele. Yet he remains free. Nursing standard precautions and isolation precautions (nursing) review for the NCLEX exam. This review will cover standard precautions for infection control and transmission-based precautions (also called isolation precautions) such as contact, droplet, and airborne precautions. In addition, I will give you 3 isolation precaution mnemonics on how to remember droplet, contact, and airborne precautions. Standard precautions are precautions we follow with every patient at all times because we do not know if the person has an infectious disease or not. This is to protect ourselves and other patients. Standard precautions include: hand hygiene, wearing appropriate PPE (personal protective equipment), injection safety, respiratory/cough etiquette, patient room placement, handling of patient laundry and equipment. Isolation precautions are precautions we follow when we know or suspect a patient has an infectious disease. Depending on the type of infectious disease the patient has we will place the patient in the appropriate precautions. For example, if a patient has Pertussis (whooping cough) we will place the patient in droplet precautions. As the nurse, we would continue to follow standard precautions along with always wearing a surgical mask during patient contact. For a patient in airborne precautions, we would wear an N95 mask along with following standard precautions and place the patient in a special AIIR room (which is a negative pressure room). For a patient in contact precautions, we would always wear gloves and a gown during patient contact and follow standard precautions. Acute Kidney Injury (Acute Renal Failure) Nursing NCLEX Review Management, Stages, Pathophysiology

Acute kidney injury (also called acute renal failure) nursing NCLEX review lecture on the nursing management, stages, pathophysiology, and causes (prerenal, intrarenal, postrenal). What is Acute Kidney Injury? It is the SUDDEN decrease in renal function that leads to the build up of waste in the blood, fluid overload, and electrolyte imbalances. What are the causes of Acute Kidney Injury? There are three causes, which are based on location. The first is known as prerenal injury and this is an issue with the perfusion to the kidneys that leads to decreased renal function. A second cause is known as intrarenal injury, and this is due to damage to the nephrons of the kidney. Lastly, postrenal injury is due to a blockage located in the urinary tract after the kidney that can extend to the urethra. This is causing the back flow of urine, which increases the pressure and waste in the kidneys. Stages of Acute Kidney Injury: There are four stage of acute kidney injury, which include initiation, oliguric, diuresis, and recovery stage. The initiation stage starts when a cause creates an injury to the kidney and then signs and symptoms start to appear. This leads to the oliguric stages. The patient will void less than 400 mL/day of urine during this stage and will experience increased BUN/creatinine levels, azotemia, hyperkalemia, hypervolemia, increase phosphate and decreased calcium levels along with metabolic acidosis. After this stage the patient can progress to the diuresis stage and this is where the patient will void 3-6 Liters of urine per day due to osmotic diuresis. The patient is at risk for hypokalemia, dehydration, and hypovolemia. The last stage is recovery and the patient's glomerular filtration rate has returned to normal. Therefore, the kidneys are able to maintain normal BUN and creatinine levels, electrolyte, and water levels.
