I will begin Chemotherapy for Multiple Myeloma next week ~ Blood Cancer Awareness Month

September is Blood Cancer Awareness Month. Sorry for the low quality,, I uploaded it on SD rather than HD :( Update Cancer Journey Video: I will begin Chemotherapy for Multiple Myeloma next week. In the video I said I will be on 10 medication but its actually going to be 11 medications. The reality of this Cancer is kicking in. One of the Chemo drugs gets delivered by CVS Specialty. You can have it delivered to your house or CVS. I choose CVS as I don’t want to waste a day waiting for it to be delivered anytime from 9 am to 6 pm. So picking it up for now is a better option. Originally I thought I would be on 10 drugs but it’s 11. I don’t like the idea of that at all. I don’t know how they will know what is causing side effects when your on so many drugs. For now my Chemo Cycle is 14 days on 7 days off for 4 cycles. A total of 12 weeks with the goal of knocking the Multiple Myeloma back and hopefully into remission. If that doesn’t work then I will have another regimen and eventually a Stem Cell Transplant. (I don’t even want to think about that) I have to make a chart so I don’t mess up which drugs to take on which days. It’s very overwhelming. I bought a weekly pill box to help me set it up. The One chemo drug that was delivered comes with a pill container that’s connected to ATT it makes a sound and lights up alerting me to take it. If I don’t open the bottle then it sends me a text reminder. Pretty cool technology. During the three week cycle I have to go to the Infusion unit twice a week for two weeks. Then off a week. Then back again. On the off week I have to go in for a IV bone drugs that is suppose to make you feel like you have the flu. My Blood work will be monitored a lot. If my counts get to low I will need a transfusion. My ONCO and Nurse said it’s normal to be anxious and feeling fear of the unknown as I have no idea how my body will respond. I’m really scared. This has rocked my world. I have a great team and I’m thankful for that. We are very blessed to have the best Doctors and Nurses at DFCC. I’ll be on a very strong steroid that makes you bounce off the walls and will keep you awake for days. They said they can give me anxiety pills and sleep pills but I don’t want more drugs in me. I guess I’ll have to see how I feel. Three of the chemo drugs cause rapid excessive weight gain. I’ve worked hard losing 130# and have maintained it one year now. The thought of gaining weight it’s very upsetting to me. The whole darn cancer thing is upsetting to me. But I’m going to kicks it’s butt. It won’t define me. It will be rough I know but I’m a fighter. There is not a cure for this Cancer but it is treatable. My Onco said they are making advances in research and the hope is they will find a cure. That’s what’s helping me cope. I have to believe that. They said I will have fatigue and a whole list of other side effects. I’m missing Chris terribly. He will be home Saturday from China. It was awful timing that he had to go the week before Chemo. The poor guy is so worried about me and has bad jet lag. He said we can do whatever I want this weekend to help me get my mind off Chemo. I definitely want to Canoe or Kayak. Water therapy is the best medicine for me. This is just the beginning treatment for Multiple Myeloma. I’m trying my best to stay focused on getting thru the first cycle. If I think to far ahead I will lose it. I can’t even cry. I’ve been numb just going thru the motions. This program discusses the preventive, diagnostic and therapeutic measures indicated for lung cancer. The program notes that pre-cancerous lesions from smoking may be reversible and that periodic chest x-rays should be used as the primary diagnostic tool. Such pathologic conditions as the thickening of the bronchial epithelium basal cells with atypical nuclei are described through microscopic views. Radiographs and drawings of several clinical cases illustrate the inoperability of lesions due to their location. Another clinical patient with alveolar cell carcinoma is presented to illustrate that other pulmonary diseases such as recurrent bronchitis or pneumonia can often obscure the detection of cancer. The program maintains that differential procedures such as sputum cytology, laryngoscopy, bronchoscopy, and mediastinoscopy should be indicated whenever the x-rays reveal a lung shadow persisting longer that two weeks. The program suggests the use of radiotherapy and chemotherapy in patients with inoperable lesions. Produced by the American Cancer Society. Radiation Therapy in the Management of Cancer (American Cancer Society, 1970)

The purpose of this program is to demonstrate the use of radiation therapy in the management of cancer. The program begins with a discussion of the factors which can be used to determine the appropriate treatment for cancer. Next, the film discusses the use of the isodose chart in planning therapy. The program includes in this discussion dose distribution and the indication of radiation therapy in specific types of cancers. The program also provides two case studies to illustrate the use of radiation therapy in the management of cancer. The first case is a woman with a tumor involving the external cervical os which extends into the left fornix. This tumor was a Stage II B tumor which responded to radiation therapy. The program shows the physical examination and treatment sequence. Then the program demonstrates radiation therapy with the goal of palliation in a patient with a six month history of difficulty swallowing food and recent weight loss. The esophogram demonstrated a mid-thoracic obstructive lesion which proved to be a squamous cell carcinoma which was treated by a rotating beam technique. The program then discusses, in less detail, radiation therapy alone and in conjunction with other types of therapy in children with Wilm's tumor, and adults with carcinoma of the larynx, and Hodgkins disease. The iShares Nasdaq Biotechnology ETF is down 13% thus far in 2016, but investors should not throw in the towel on biotech stars Amgen , Gilead , bluebird and Bellicum , said Brad Loncar, CEO of Loncar Investments. Shares of Amgen are up 5.4% thus far in 2016, but fell this week after its Phase III Clarion trial failed to meet its primary endpoint. The trial was testing Amgen's Kyprolis, a drug used to treat patients with relapsed or refractory multiple myeloma, which is a cancer of plasma cells. The drug didn't slow the regrowth of tumors better than rival cancer drug Velcade in combination with chemotherapy. "Everyone is waiting for Amgen to do a deal with the $35 billion in cash they have on hand," said Loncar. "And the Kyprolis trial failure stings, but not too much because it has been approved in other patient subsets already." Loncar Investments is behind the Loncar Cancer Immunotherapy ETF , which is down 8% thus far in 2016. Gilead shares have dropped 20% so far in 2016. The stock was hit this week after Leerink reduced its rating on the stock to "market perform" from "outperform." The firm lowered its price target to $94 from $112 on shares of the company, saying it is "outright bearish" on its Hepatitis C treatments. "Gilead's Hepatitis C business has matured, but the stock is still cheap," said Loncar. "And they have a war chest of $30 billion to do a transformative deal as well." Bluebird bio shares are up a healthy 16% year-to-date, but have soared 43% in the past month as the immunotherapy specialist is rumored to be a target for Japanese drug maker Takeda Pharmaceutical . Loncar said bluebird has been "laying low" this year and will soon unveil a slew of data at the upcoming ASH Conference (American Society of Hematology). Similarly, Loncar expects Bellicum Pharmaceuticals, another immunotherapy outfit, to have a lot to say at the ASH Conference. Bellicum has seen its shares recover to $20 from less than $8 this past March. Originally a five-piece, the band underwent numerous lineup changes while Amphlett and McEntee remained as core members, before its dissolution in 1996. In May 2001, the Australasian Performing Right Association (APRA), as part of its 75th Anniversary celebrations, named "Science Fiction" as one of the Top 30 Australian songs of all time. The band was inducted into the Australian Recording Industry Association (ARIA) Hall of Fame in 2006 and in late 2007 Amphlett and McEntee reconvened to record a new single and begin working on a new album. The band played a short series of live gigs in Australia in late 2007 and early 2008. Divinyls released five studio albums—four placed in the Top 10 Australian chart, while one (Divinyls) reached No. 15 in the United States (US). The song reached No. 4 in the US and No. 10 in the UK. The majority of Divinyls' hits were co-written by Amphlett and McEntee, but in this case they wrote with Tom Kelly and Billy Steinberg. DIVINYLS reached No. 5 on the Australian album charts and No. 15 on Billboard Top 200.[20] The drummer for the diVINYLS sessions was Charley Drayton, who became romantically involved with Amphlett: they married in July 1999, and from 2000, lived together in New York. A disagreement with Virgin Records stifled future development outside Australia where they released popular albums and achieved two more top twenty singles with "I Ain't Gonna Eat Out My Heart Anymore" No. 19 in 1992 and "I'm Jealous" No. 14 in 1995. During the 1980s and 1990s Amphlett collaborated as a songwriter with other artists including Chrissie Hynde and Cyndi Lauper, and both Amphlett and McEntee worked on solo projects. A live album, Divinyls Live, was released in 1991 but Divinyls did not provide another studio album for five years. In the early 1990s they recorded a series of cover songs for various movie soundtracks, including the Young Rascals' "I Ain't Gonna Eat Out My Heart Anymore" for Buffy the Vampire Slayer (1992), the Wild Ones' "Wild Thing" for Reckless Kelly (1993), and Roxy Music's "Love Is the Drug" for Super Mario Brothers (1993). The song "I Touch Myself" caused such a controversy it had trouble getting airplay in many U.S.A. area markets; so much to the point that while performing their song live in Texas at Austin Aqua Fest 1991 the plug was pulled on the band mid set by organizers. This song is also mentioned in The Guide to Getting it On by Paul Joannides. Mesothelioma is a type of cancer that develops from the thin layer of tissue that covers many of the internal organs (known as the mesothelium).[1] The most common area affected is the lining of the lungs and chest wall.[2][3] Less commonly the lining of the abdomen and rarely the sac surrounding the heart,[4] or the sac surrounding the testis may be affected.[2][5] Signs and symptoms of mesothelioma may include shortness of breath due to fluid around the lung, a swollen abdomen, chest wall pain, cough, feeling tired, and weight loss.[2] These symptoms typically come on slowly.[6] Greater than 80% of mesothelioma is caused by exposure to asbestos. The greater the exposure the greater the risk.[3] As of 2013 about 125 million people have been exposed to asbestos at work.[7] High rates of disease occur in people who mine asbestos, produce products from asbestos, work with asbestos products, live with asbestos workers, or work in buildings containing asbestos. Often it takes around 40 years for the disease to begin.[3] Washing the clothing of someone who worked with asbestos also increases the risk.[7] Other risk factors include genetics and infection with the simian virus 40.[3] The diagnosis may be suspected based on chest X-ray and CT scan findings, and is confirmed by either examining fluid produced by the cancer or by a tissue biopsy of the cancer.[6] Prevention centers around reducing exposure to asbestos.[8] Treatment often includes surgery, radiation therapy, and chemotherapy. A procedure known as pleurodesis, which involves using substances such as talc to scar together the pleura, may be used to prevent more fluid from building up around the lungs.[9] Chemotherapy often includes the medications cisplatin and pemetrexed.[6] The percentage of people that survive five years following diagnosis is on average 8% in the United States.[10] In 2013 about 50,000 people had mesothelioma and 34,000 died from the disease.[11][12] Rates of mesothelioma vary in different areas of the world. Rates are higher in Australia and Great Britain and lower in Japan.[3] It occurs in about 3,000 people per year in the United States.[13] It occurs more often in males than females. Rates of disease have increased since the 1950s. Diagnosis typically occurs after the age of 65 and most deaths occur around 70 years old. The disease was rare before the commercial use of asbestos.[3]

Structurally, most bacteria consist of a cell membrane surrounded by a cell wall and, for some bacteria, an additional outer layer. Internal to the cell membrane is the cytoplasm which contains ribosomes, a nuclear region and in some cases granules and/or vesicles. Depending on the bacterial species, a number of different external structures may be found such as a capsule, flagella and pili. In gram negative bacteria, the gap between the cell membrane and the cell wall is known as the periplasmic space. Most gram positive bacteria do not possess a periplasmic space but have only periplasm where metabolic digestion occurs and new cell peptidoglycan is attached. Peptidoglycan, the most important component of the cell wall, is a polymer made of N-acetyl muramic acid alternating with N-acetyl glucosamine which are cross-linked by chains of four amino acids. The function of the bacterial cell wall is to maintain the characteristic shape of the organism and to prevent the bacterium from bursting when fluid flows into the organism by osmosis. Synthesis of peptidoglycan and ultimately the bacterial cell wall occurs in a number of stages. One of the first stages is the addition of 5 amino acids to N-acetyl muramic acid. Next, N-acetyl glucosamine is added to the N-acetyl muramic acid to form a precursor of peptidoglycan. This peptidoglycan precursor is then transported across the cell membrane to a cell wall acceptor in the periplasm. Once in the periplasm, the peptidoglycan precursors bind to cell wall acceptors, and undergo extensive crosslinking. Two major enzymes are involved in crosslinking: transpeptidase and D-alanyl carboxypeptidase. These enzymes are also known as penicillin binding proteins because of their ability to bind penicillins and cephalosporins. Eventually, several layers of peptidoglycan are formed all of which are crosslinked to create the cell wall. Gram positive bacteria have many more layers than gram negative bacteria and thus have a much thicker cell wall. Beta-lactam antibiotics include all penicillins and cephalosporins that contain a chemical structure called a beta-lactam ring. This structure is capable of binding to the enzymes that cross-link peptidoglycans. Beta-lactams interfere with cross-linking by binding to transpeptidase and D-alanyl carboxypeptidase enzymes, thus preventing bacterial cell wall synthesis. By inhibiting cell wall synthesis, the bacterial cell is damaged. Gram positive bacteria have a high internal osmotic pressure. Without a normal, rigid cell wall, these cells burst when subjected to the low osmotic pressure of their surrounding environment. As well, the antibiotic-penicillin binding protein complex stimulates the release of autolysins that are capable of digesting the existing cell wall. Beta-lactam antibiotics are therefore considered bactericidal agents. Bacterial resistance to beta-lactam antibiotics may be acquired by several routes. One of the most important mechanisms is through a process known as transformation. During transformation, chromosomal genes are transferred from one bacterium to another. When a bacterium containing a resistance gene dies, naked DNA is released into the surrounding environment. If a bacterium of sufficient similarity to the dead one is in the vicinity, it will be able to uptake the naked DNA containing the resistance gene. Once inside the bacterium, the resistance gene may be transferred from the naked DNA to the chromosome of the host bacteria by a process known as homologous transformation. Over time, the bacterium may acquire enough of these resistance genes to result in a remodelling of the segment of the host DNA. If this remodelled DNA segment codes for cross-linking enzymes (i.e. penicillin binding proteins), the result is the production of altered penicillin binding proteins. These altered penicillin binding proteins can still cross-link the peptidoglycan layers of the cell wall but have a reduced affinity for beta-lactam antibiotics thus rendering the bacterium resistant to the effects of penicillin and other beta-lactam agents. This transfer process has resulted in penicillin-resistant S. pneumoniae through the acquisition of genes from other naturally occurring penicillin-resistant Streptococcus species. A second important mechanism by which bacteria become resistant to beta-lactam antibiotics is by the production of enzymes capable of inactivating or modifying the drug before it has a chance to exert its effect on the bacteria.
