Bladder cancer is any of several types of cancer arising from the epithelial lining (i.e., the urothelium) of the urinary bladder. Five-year survival rates in the United States are around 77%. Bladder cancer is the 9th leading cause of cancer with 430,000 new cases[3] and 165,000 deaths occurring in 2012. Signs and symptoms: Bladder cancer characteristically causes blood (redness) in the urine. This blood in the urine may be visible to the naked eye (gross/macroscopic hematuria) or detectable only by microscope (microscopic hematuria). Hematuria is the most common symptom in bladder cancer. It occurs in approximately 80-90% of the patients. Other possible symptoms include pain during urination, frequent urination, or feeling the need to urinate without being able to do so. These signs and symptoms are not specific to bladder cancer, and are also caused by non-cancerous conditions, including prostate infections, over-active bladder and cystitis. There are many other causes of hematuria, such as bladder or ureteric stones, infection, kidney disease, kidney cancers and vascular malformations. Patients with advanced disease refer pelvic or bony pain, lower-extremity edema, or flank pain. Men are four times more likely to get bladder cancer than women. “It’s very treatable. Most cancers of the bladder are treatable without major surgery. The treatment depends on the stage,” said Dr. Benitez. In advanced stages of bladder cancer, patients may undergo chemotherapy or have the bladder removed. “When people die of bladder cancer, they don’t die because it’s in the bladder; they die because it spread, most the time to the liver or the lung,” said Dr. Benitez. Bladder cancer that spreads is called muscle-invasive bladder cancer. Pelvic growths put pressure on other organs, including the intestines, bowels, and prostate gland, causing severe discomfort and pain. 8. Bone Pain. Advanced bladder cancer can spread into the bones, causing bone pain. This pain typically begins close to the urinary tract. It impacts areas like the anus, rectum, and pelvic region. Bone pain can also radiate to the hips and ribs as well, making walking difficult. Patients with bone pain related to cancer often have trouble managing the pain. 9. Anemia. Anemia occurs when there are not enough red blood cells. It is caused by an excess of white blood cells, which are the cells the body produces when cancer is present. Anemia can also develop in people with bladder cancer due to loss of blood during a radical cystectomy, which is surgery to remove the cancerous organ. 10. Weight Loss. Weight loss is often associated with different forms of cancers, and bladder cancer is not the exception. Cancer treatments can also cause nausea, resulting in even more weight loss. The urinary system is composed of two kidneys, two ureters, the bladder, and the urethra. The kidneys remove waste products from the blood and form urine. Urine then travels through the tube-like ureters and is stored in the bladder before it is eliminated from the body via the urethra. The bladder is the hollow-shaped organ that expands and contracts to collect and eliminate urine. As with all organs in the urinary system, the bladder is susceptible to developing cancer, the uncontrolled growth of abnormal cells. The wall of the bladder has several layers of tissue. Bladder cancer type depends on the types of cells and layers of tissue affected. There are 3 types of bladder cancer: • Transitional cell: This type of cancer begins in the transitional cells. These cells form the innermost layer of the bladder, allowing the bladder to stretch when it is full and shrink when it is emptied. It is the most common site of bladder cancer. • Squamous cell: This type of cancer is a slow-growing cancer of the thin, flat cells that line the surface of the bladder. • Adenocarcinoma: Cancer that begins in the glandular or secretory cells of the bladder. Symptoms of bladder cancer can include: • Blood in the urine • Frequent urination, or feeling the need to urinate without being able to do so • Pain during urination Men are more likely to develop bladder cancer than women. Bladder cancer is more common in whites than in blacks. Risk factors for bladder cancer include: • Smoking tobacco • Long-term exposure to certain workplace chemicals, or carcinogens, such as those used in making rubber, textiles, paints, and dyed clothing • A diet high in fats and fried foods • Having a history of recurrent bladder infections • Long-term use of urinary catheters • Being over age 60 If cancer is suspected, diagnostic studies can include: Cystoscopy; an imaging study where a tube with a lens is placed into the bladder through the urethra. Urine culture and cytology; laboratory studies that analyze urine for bacteria and cancer cells. Biopsy; the removal of bladder cells for examination under a microscope. Imaging studies; studies such as MRI, CT scan, and IVU (intravenous urography) that provide a detailed picture of the urinary system. Treatment and prognosis depend on the stage and grade of the cancer and the location of the tumor.
