Take medications as prescribed To alleviate your congestive heart failure symptoms, your doctor may prescribe a variety of medications, including diuretics, digoxin, vasodilators and other drugs. Never stop taking them and do discuss with your doctor any side effect you may have. Tip number 2. Cut down on salt (sodium) Too much sodium in your diet will raise your blood pressure, make the diuretics less effective and contribute to the swelling in your lower limbs. Do not add salt to your food. Consider seasoning your dishes with natural herbs or lemon. Avoid canned and processed foods, as well as highly salted smoked or cured meats such as bacon, ham and sausages. Tip number 3. Reduce your intake of saturated fats and cholesterol Too much saturated fats and cholesterol can cause plaque deposits to form in the heart arteries resulting in heart attack. Local favourites such as char kway teow and fried hokkien prawn noodles should be avoided as these dishes are high in saturated fats and cholesterol. Tip number 4. Quit smoking Smoking causes plaque buildup and narrowing of heart arteries, resulting in impaired blood flow to the heart muscles. The carbon monoxide in cigarette smoke also restricts the flow of blood in the heart. Tip number 5. Limit alcohol intake If you have heart failure, it is important to abstain from alcohol intake especially if the cause of your heart failure is due to long term alcohol abuse. Tip number 6. Stay active within your limits While it is important to get plenty of rest, some light physical activity may help to improve your well-being and effort tolerance. After checking with your doctor, you should consider engaging in light exercises such as walking, cycling, swimming and yoga. Tip number 7. Go for cardiac rehab Ask your doctor about cardiac rehabilitation. If you are the right candidate, cardiac rehab can maximise your potential for good heart health and function, all under the guidance of a team of health professionals. Tip number 8. Manage negative emotions Receiving a diagnosis of heart failure can be hard to take at first. Talk to your doctor, spouse, family members or close friends about your fears, stress and anxiety. Seek help early if you think you may be at risk of depression. Tip number 9. Travel smart Being diagnosed with heart failure does not mean the end of overseas travel. Make sure you pack more than enough medications in case of delays. Keep your medications with you and not in your checked luggage. Avoid high altitude destinations and wear compression stockings during long flights to prevent blood clots.

Furosemide (Lasix) - Loop Diuretics Generic Name: Furosemide Brand Name: Lasix Furosemide also known as Lasix is a loop diuretic commonly use to treat fluid retention and edema. Which can be associated with Congestive Heart Failure (CHF), cirrhosis of the liver, and many kidney disorders. It is also sometimes use to treat hypertension alone or in combination with other anti-hypertensive drugs. Diuretics are medications that increases the amount of water that passes through the kidneys as urine, thus, it is often called water pill. Lasix belongs to the group known as Loop diuretics. There a few types of diuretics and loop diuretics are just one type. It works by interfering with the sodium, potassium and chloride symporter. A symporter is a protein membrane that manages the transport of molecules across a cell membrane. The symporter that lasix interacts with is found in the thick ascending limb of the loop of Henle. What it does is………. inhibits the reabsorption of salt (Na+) and Chloride (Cl-). Since the Salt and Chloride will be leaving the body system through the loop of henle this means that water will also follow…. causing more water pass through the kidney. Which will ultimately mean, less fluid remaining in the blood stream. Once Lasix is received the system will then compensate the loss by absorbing any fluid accumulated in the tissues, such as in the lungs or legs, back to the blood stream. This will cause more fluid to be in the blood stream. Which once again pass through the kidney, which will also be excreted in the urine, since the loop diuretic inhibits its reabsorption. This mechanism, is what makes loop diuretics a treatment for fluid retention also known as edema. It can therefore, ease symptoms of edema such as breathlessness caused by congestion of fluid in the lungs which patients diagnosed with CHF, cirrhosis of the liver, nephrotic syndrome and other edematous states may encounter. Lasix is available in forms that can be taken orally and intravenously. When taken intravenously, it is considered to be twice as strong. Parenteral or intravenous Furosemide is indicated when a fast acting and an intense diuresis is needed such as in acute pulmonary edema and cerebral edema. It also indicated when oral therapy is not possible because of problem with absorption in the intestine or for other reasons. Parenteral administration should be observed only in hospital or outpatient clinics. However, in cases of emergency that furosemide should be given right away outside hospital setting, recommended doses should be closely adhered to and patient must be closely monitored. Contraindication: Furosemide should not be taken if your patient cannot urinate. Severe Hypokalemia - is a major contraindication. This means low potassium. Lasix can cause a dramatic decrease in potassium level which could cause lethal cardiac arrhythmias Other contraindications include: Hypotension or low blood pressure Lasix should be used with extreme caution for patients who have a diagnosis of Systemic Lupus Erythematosus because it may May

Symptoms of Digitalis poisoning include low pulse rate, nausea, vomiting, and uncoordinated contractions of different parts of the heart, leading to cardiac arrest and finally death. Medicine: Digitalis purpurea is well known as the original source of the heart medicine digoxin (also called digitalis or digitalin). Research and clinical studies prove it to be a very effective drug class for many, as it works by causing the blood vessels to become enlarged or dilated, and thus causing a reduction in the blood pressure. This lowering of the blood pressure makes it easier for the heart to pump blood and it can improve the function of a failing heart. In the United States, 10 ACE inhibitors are in use: Ramipril (Altace) Enalapril (Vasotec, Epaned) Quinapril (Accupril) Captopril (Capoten) Benazepril (Lotensin) Lisinopril (Prinivil, Zestril) Fosinopril (Monopril) Moexipril (Univasc) Trandolapril (Mavik) Perindopril (Aceon) NOTE...that the brand names of all of these above end with the suffix “pril”. NOW.. let’s take a closer look at this drug class, angiotensin-converting enzyme (ACE) inhibitors, these drugs are helpful in treating heart, blood vessel, and kidney problems... Mechanism of Action: The human body is an amazing organism…In order to regulate water and blood pressure, our body follows special mechanisms. For example when the blood pressure drops too low, the kidneys release renin into the bloodstream. Renin then reacts with a protein called angiotensinogen that is produced by the liver. As a result, they form angiotensin I. Then another reaction converts angiotensin I to angiotensin II, with the help of angiotensin-converting enzyme found in the respiratory system. This whole mechanism is known as Renin-angiotensin system (RAS). Angiotensin II is a powerful substance. It circulates around the body in the bloodstream and induces several changes on the cardiovascular system. Such as stimulating adrenal cortex to release adolsterone and Constricting or narrowing down the blood vessels around the body. Aldosterone increases the reabsorption of water and sodium in the tubules of the kidneys. As a result, The volume of the blood and the concentration of salt in the blood increase leading to high blood pressure. The constriction of blood vessels results in high pressure inside the vessels, which also means an increase in blood pressure. Both of these events cause the heart to exert more pressure while pumping blood into the main arteries. Hence, it reverses the low blood pressure but it can also lead to other cardiovascular disorders if it persists for a long time. ACE inhibitors reduce the impact of renin-angiotensin-aldosterone system (RAAS) on the blood pressure by inhibiting the angiotensin converting enzyme. As a result, The conversion of angiotensin I to angiotensin II is slowed down which brings about several changes in the cardiovascular system. The rate of water reabsorption in the kidneys drops and so volume of blood decreases. And the blood vessels relax and become dilated. How to Purify by Recrystallization

We show how to purify aluminum nitrate and strontium nitrate by recrystallization. This is important because those two substances must be extremely pure for our upcoming video on making glow in the dark powder. First make aluminum nitrate from our previous video: http://www.youtube.com/watch?v=u4Ha1SJrazY Dissolve the dry aluminum nitrate in water and filter off any insoluble materials. We could not filter it off before it dried in the previous video because the particles at that stage are much too small. It needs crystallize once to aggregate into particles large that can be filtered. After filtering, dry off the aluminum nitrate. The desiccator bag might be useful here: http://www.youtube.com/watch?v=XJFfS_YbbYI After drying, carefully weigh out the crystals. Then take that mass and add in 20% mass of water. So if you have 50g like me, you add 10g of water (or 10mL since density is 1g/mL). It won't all dissolve, so carefully heat the mixture until it dissolves. Then cover it and let it cool down. Eventually it'll crystallize out purer crystals. After the mixture cools to room temperature, and left for a few hours. The liquid is poured off and discarded, it has the impurities and is not needed. The crystals are again weighed and once again they are recrystallized with 20% water. Once this is done. Dry the final product. I started with 50g and ended up with 17g. But now it's very pure. Better than 99% Strontium nitrate also needs to be purified. But it's solubility doesn't change that much with temperature so we'll have to purify by slow evaporation recrystallization rather than thermal cycle recrystallization. First, make strontium nitrate like our previous video: http://www.youtube.com/watch?v=_rd8b6wNnKA Once again, dissolve it in water and filter it. Then let it crystallize. But before it dries completely, let the liquid reduce to about 20% of it's saturated volume. So I started with a saturated solution of 10mL of strontium nitrate, i then waited until it evaporated down to 2mL of fluid. The crystals are not included in this assessment. Then discard the liquid. Repeat the recrystallization process. Once again, use the desiccator bag to obtain extremely dry chemicals. We are now ready to produce glow-in-the-dark powder.

Dr. Dermish admits she was trained by the Senior Director of Medical Services at Planned Parenthood Federation of America, Dr. Deborah Nucatola. Nucatola described a partial-birth abortion technique to harvest fetal organs in the first Planned Parenthood video released July 14 by CMP. “My aim is usually to get the specimens out pretty intact,” says Dermish, speaking to actors posing as a fetal tissue procurement company. Dermish admits that she will sometimes use ultrasound guidance to convert a 2nd-trimester fetus to a feet-first breech presentation: “Especially the 20-weekers are a lot harder versus the 18-weekers, so at that point I’ll switch to breech.” “That’s what Deb [Nucatola] was telling us, was it really makes a difference for tissue collection at PPLA,” says one of the actors, referencing Nucatola’s technique for organ harvesting. “It’s really nice when it’s, yeah--I trained with her,” replies Dermish. In reply to a question about harvesting fetal brain, Dermish notes, “I haven’t been able to do that yet,” but exclaims with laughter, “This will give me something to strive for!” Dermish does not use the chemical digoxin to kill the fetus before 20 weeks, so her feet-first, intact extraction abortions are done on living fetuses. Using ultrasound guidance to manipulate the fetus from vertex to breech orientation before extracting the yet-living fetus is a hallmark of the illegal partial-birth abortion procedure (18 U.S.C. 1531). Dermish also affirms a fetal body part price of $50 or $60 per specimen. The sale or purchase of human fetal tissue is a federal felony punishable by up to 10 years in prison and a fine of up to $500,000 (42 U.S.C. 289g-2). The state of Texas recently moved to disbar Planned Parenthood from taxpayer funding on account of Medicaid fraud and abortion-related crimes. Governor Greg Abbott recently said in an interview that the state of Texas is now in possession of “hard evidence” that Planned Parenthood is engaged in “violations of federal and state law concerning abortion procedures.” “Less than 4 miles away from the Texas state capitol, Planned Parenthood routinely practices barbaric partial-birth abortions on living, late-term fetuses,” notes CMP Project Lead David Daleiden. “Planned Parenthood’s crimes are not just the result of local bad actors, but are tolerated and even encouraged at the highest levels of the national organization. Muscular Dystrophy for USMLE

This video will cover Muscular Dystrophy (Duchenne and Becker) for USMLE. We will cover Pathophysiology, Physiology, Clinical Signs and Symptoms, Investigations, Treatment and Management of this condition. There is 4 criteria that is required when diagnosing a muscular dystrophy. It must be primary myopathy, genetic basis progressive and there must be degeneration or death of the muscle. PATHOPHYSIOLOGY Most common herediatry Neuromuscular Disease. It is X Linked Recessive Xp21 and codes for the dystrophin gene. Connects the Connective tissue to the muscle therefore it is important in muscle movement. Thirty percent are due to new mutations. Girls experience a milder form due to Lyon hypothesis due to Barr Bodies. Also it is more severe in patients with Turner Syndrome. There will be increase CK in those patients. This pathophysiology is the same in Duchenne Muscular Dystrophy and Becker Muscular Dystrophy except Becker Muscular Dystrophy is much milder. Signs and Symptoms In the first year tehy are normal or there is slight delayed milestones. Normal waliking at 12 months and have hip girdle weakness. THe first sign is poor head cnontrol In Toddler age there is lslightly lodroic to make up for gluteal weakness At 3-5 years they exhibit Gowers sign which makes it apparent that they have the disease. From 7-8 years old they become wheelchair bound and 12 years old with treatment. IN their 20s they can eat write and use a keyboard however after a while there will be respiratory symptoms such as a weak cough causing increase risk of infections and decreased respiratory reserve. Also pharyngeal symptoms will cause aspiration There are other contractures that may occur in ankles, knees, hips, elbows. Scoliosis which becomes more severe after ambuation. Characteristically there is psuedohypertrophy because of hypertrophy of actual fiber and there is fat infiltration and collagen and thigh atrophy due to disuse. Tongue may also hypertorphy. Cardiomyopathy leads to tachy cardia and eventually goes into Cognestive heart failure. Cognestive heart disease doesn't correlate with skeletal disease. Intellect is very low 70 and other times it is much more subtle. There is slight increase in epilepsy and this is due to dystrophin found in the brain. Death is caused by respiratory failure in sleep. Duchenne Muscular dystrophy occurs in mid 20s and Becker Muscular Dystrohpy occurs in late 20s. The investigations to run are the Creatine Kinase which is high early on and they have much more muscle. But becomes lower when muscle mass decreases. Echo, ECG X-ray of the heart. EMG shows no denervation and motor/sensory Nerve conduction is normal. Diagnosis is by PCR of the dystrophin gene. Muscle biopsy can catch 1/3 of the false negative. Take muscle from qaudriceps or gastrocnemius. Treatment there is no cure and there is no way to slow it down. Try to limit complcation and increase quality of life. Cardiac you can use digoxin, pulmonary infections should be treated vigorously. Treat promptly and aggressive. They can also go to physiotherapy due to contractures. SOmetimes these contractures are beneficial in late stages of Duchenne Muscular Dystrophy and Becker muscular Dystrophy.

Also, the number of doses you take each day, the time allowed between doses, and the length of time you take the medicine depend on the medical problem for which you are using the medicine. NCLEX Nursing Precautions Drug information provided by: Micromedex It is very important that your doctor check the progress of you or your child at regular visits. This will allow your doctor to see if the medicine is working properly and to decide if you should continue to take it. Blood tests will be needed to check for unwanted effects. Levothyroxine should not be used for the treatment of obesity or for the purpose of losing weight. This medicine is not effective for weight reduction. If taken in large amounts, levothyroxine may cause serious unwanted effects. Hypothyroidism can sometimes cause infertility in men and women. Levothyroxine should not be used for the treatment of infertility unless it is caused by hypothyroidism. For patients with diabetes: It is very important that you keep track of your blood or urine sugar levels as instructed by your doctor. Check with your doctor right away if you notice any changes in your sugar levels. If you think you have become pregnant while using this medicine, tell your doctor right away. You may need a larger dose of levothyroxine while you are pregnant. Women who use this medicine for a long time may have some bone loss, which could lead to osteoporosis. Talk with your doctor if you have questions or concerns about this. Call your doctor right away if you or your child start to have rapid or irregular heartbeats, chest pain, shortness of breath, leg cramps, headaches, nervousness, irritability, sleeplessness, tremors, a change in appetite, weight gain or loss, vomiting, diarrhea, excessive sweating, heat intolerance, a fever, changes in menstrual periods, hives, or a skin rash. These could be symptoms of too much medicine in your body. Do not suddenly stop taking this medicine without first checking with your doctor. Your doctor may want you or your child to gradually reduce the amount you are using before stopping completely. Make sure any doctor or dentist who treats you knows that you or your child are using this medicine. You or your child may need to stop using this medicine several days before having surgery or medical tests. A temporary loss of hair may occur during the first few months of levothyroxine therapy. Ask your doctor about this if you have any concerns. Do not take other medicines unless they have been discussed with your doctor. This includes prescription or nonprescription (over-the-counter [OTC]) medicines and herbal or vitamin supplements.

Planned Parenthood of the Rocky Mountains (PPRM) is a wealthy, multi-state Planned Parenthood affiliate that does over 10,000 abortions per year. PPRM has a contract to supply aborted fetal tissue to Colorado State University in Fort Collins. In the video, actors posing as representatives from a human biologics company meet with Ginde at the abortion-clinic headquarters of PPRM in Denver to discuss a potential partnership to harvest fetal organs. When the actors request intact fetal specimens, Ginde reveals that in PPRM’s abortion practice, “Sometimes, if we get, if someone delivers before we get to see them for a procedure, then we are intact.” Since PPRM does not use digoxin or other feticide in its 2nd trimester procedures, any intact deliveries before an abortion are potentially born-alive infants under federal law (1 USC 8). “We’d have to do a little bit of training with the providers or something to make sure that they don’t crush” fetal organs during 2nd trimester abortions, says Ginde, brainstorming ways to ensure the abortion doctors at PPRM provide usable fetal organs. When the buyers ask Ginde if “compensation could be specific to the specimen?” Ginde agrees, “Okay.” Later on in the abortion clinic’s pathological laboratory, standing over an aborted fetus, Ginde responds to the buyer’s suggestion of paying per body part harvested, rather than a standard flat fee for the entire case: “I think a per-item thing works a little better, just because we can see how much we can get out of it.” The sale or purchase of human fetal tissue is a federal felony punishable by up to 10 years in prison or a fine of up to $500,000 (42 U.S.C. 289g-2). Federal law also requires that no alteration in the timing or method of abortion be done for the purposes of fetal tissue collection (42 U.S.C. 289g-1). Ginde also suggests ways for Planned Parenthood to cover-up its criminal and public relations liability for the sale of aborted body parts. “Putting it under ‘research’ gives us a little bit of an overhang over the whole thing,” Ginde remarks. “If you have someone in a really anti state who’s going to be doing this for you, they’re probably going to get caught.” Ginde implies that PPRM’s lawyer, Kevin Paul, is helping the affiliate skirt the fetal tissue law: “He’s got it figured out that he knows that even if, because we talked to him in the beginning, you know, we were like, ‘We don’t want to get called on,’ you know, ‘selling fetal parts across states.’” The buyers ask, “And you feel confident that they’re building those layers?” to which Ginde replies, “I’m confident that our Legal will make sure we’re not put in that situation.” As the buyers and Planned Parenthood workers identify body parts from last fetus in the path lab, a Planned Parenthood medical assistant announces: “Another boy!” The video is the latest by The Center for Medical Progress documenting Planned Parenthood’s sale of aborted fetal parts. Project Lead David Daleiden notes: “Elected officials need to listen to the public outcry for an immediate moratorium on Planned Parenthood’s taxpayer funding while the 10 state investigations and 3 Congressional committees determine the full extent of Planned Parenthood’s sale of baby parts.” Daleiden continues, “Planned Parenthood’s recent call for the NIH to convene an expert panel to ‘study’ fetal experimentation is absurd after suggestions from Planned Parenthood’s Dr. Ginde that ‘research’ can be used as a catch-all to cover-up baby parts sales. The biggest problem is bad actors like Planned Parenthood who hold themselves above the law in order to harvest and make money off of aborted fetal brains, hearts, and livers.”
