522 randomized controlled trials involving 21 antidepressants and 116,477 people were analyzed. It appears the most effective antidepressants (at a duration of 8 weeks) include amitriptyline, escitalopram, mirtazapine, paroxetine, and venlafaxine. While reboxetine and trazodone had notably lower efficacy ratings. Some of the most effective drugs had lower tolerability scores, however, indicating they may have a greater side effect burden. The antidepressant literature shows signs of potential bias, so the results have to be considered with that in mind. Escitalopram, sold under the brand name Cipralex among others, is an antidepressant of the selective serotonin reuptake inhibitor class. Escitalopram is mainly used to treat major depressive disorder or generalized anxiety disorder. It is taken by mouth. The Never ending saga that is my search for the right antidepressant may be finally at a close. I recently went to a new psychiatrist and told him I was having serious thoughts of death and blackness beyond normal levels. I was annoyed and fed up because I was now on my fourth antidepressant and I was still feeling like life wasn't worth living. Finally I was given a drug that has changed my outlook on life enormously. It is called Sifrol. Escitalopram (Lexapro) has FDA approval for the treatment of major depressive disorder in adolescents and adults, and generalized anxiety disorder in adults. In European countries and Australia, it is approved for depression (MDD) and anxiety disorders, these include: general anxiety disorder (GAD), social anxiety disorder (SAD), obsessive-compulsive disorder (OCD), and panic disorder with or without agoraphobia. Escitalopram was approved by regulatory authorities for the treatment of major depressive disorder on the basis of four placebo controlled, double-blind trials, three of which demonstrated a statistical superiority over placebo. Controversy existed regarding the effectiveness of escitalopram compared to its predecessor citalopram. The importance of this issue followed from the greater cost of escitalopram relative to the generic mixture of isomers citalopram prior to the expiration of the escitalopram patent in 2012, which led to charges of evergreening. Accordingly, this issue has been examined in at least 10 different systematic reviews and meta analyses. The most recent of these have concluded (with caveats in some cases) that escitalopram is modestly superior to citalopram in efficacy and tolerability. In contrast to these findings, a 2011 review concluded that all second-generation antidepressants are equally effective, although they may differ in onset and side effects. Treatment guidelines issued by the National Institute of Health and Clinical Excellence and by the American Psychiatric Association generally reflect this viewpoint. In 2018, a systematic review and network meta-analysis comparing the efficacy and acceptability of 21 antidepressant drugs showed escitalopram to be one of the most effective antidepressants in head-to-head studies. Pramipexole (Mirapex, Mirapexin, Sifrol) is a dopamine agonist of the non-ergoline class indicated for treating Parkinson's disease and restless legs syndrome. CBD for depression, 13 clinical studies indicate the use of CBD oil for depression. The Chinese were the first to appreciate the medicinal properties of his the cannabis plant. Literature from this period suggests that cannabis was recommended for absent mindedness, rheumatic pains, constipation and disorders of the female reproductive tract. Chinese doctors used every part of the plant, including the seeds, stalks, oil, roots and leaves. The drug was administered in pills and in various porridge style pastes. Cannabis was also used in Ayurvedic medicine which originated in India. Cannabis increases 'gastric fire' or digestion. The first major work to lay out the uses of cannabis in medicine was the Ayurvedic treatise of Sushruta Samhita written in 600 BC. This book described over 700 plant based medicines. In Western medicine basing their knowledge on the Greek and Roman physicians Dioscorides and Galen, medieval herbalists in the West recommended cannabis for a variety of cures including coughs, jaundices, and bed wetting. In 1693 English painter and author William Salmon wrote that the seeds, leaves and juice of cannabis, plus essence and decoctions were all widely available in pharmacists shops. Later William O'Shaughnessy (1809-1889) Assistant Surgeon and Professor of Chemistry at the medical college in Calcutta published a paper entitled 'On the Preparations of the Indian Hemp or Gunjah, their effects on the Animal System of Health and their Utility in the treatment of Tetanus and other convulsive diseases. The US Pharmacopeia would continue to list cannabis until 1941. But whereas up to the 1930s cannabis was seen as a positive, useful medicine, in 1936, under the bombardment of the Federal Bureau of Narcotics, negative campaign, the experts capitulated to ignorance and propaganda, and it was downgraded. The war on drugs and the war on cannabis continues. However through the power of new media, people movements, and intelligent scientists, attitudes are changing quickly. There is a growing body of evidence for cannabis in the treatment of numerous diseases from cancer to depression. Many people suffer from depression and anxiety. They share a common condition with those that have Obsessive Compulsive Disorder (OCD), anorexia nervosa, bulimia, Post Traumatic Stress Disorder (PTSD) and even insomnia. They are all due to a deficiency in Serotonin, a neurotransmitter in the brain. The brain communicates and functions with the help of 3 important neurotransmitters, Serotonin, Norepinephrine and Dopamine. The end of nerve cells release the neurotransmitter, it floats across a gap and is picked up by the next cell, which becomes activated, and it then carries a message to the next nerve cell. If there is not enough neurotransmitter, the cells do not communicate effectively, and brain function can be affected. The same Serotonin deficiency can cause different symptoms in different people. One person may have depression, another may have insomnia or anorexia. Think about hypothyroid, one person may be tired, another dry skin and hair, and yet another may present with constipation. All have low thyroid hormone, but have different symptoms. The medications take 4-6 weeks to be fully effective, may have some manageable side effects the first few days, and should be taken at least 3-6 months, before considering stopping, after talking with your doctor.

I've never told you if I still take antidepressants. If you followed me for long enough, you know that I took antidepressants when I was first diagnosed with Major Depressive Disorder and Generalized Anxiety Disorder back in 2008. I told the community that I came off of of medication but never gave the full story whether or not I ever took them again. The reason I avoided talking about antidepressants (which I explain in the video) is because: 1. Let's pretend I take them now - I didn't want people to think that antidepressants are the only thing that help and that all of the other tips/strategies I talk about mean nothing and aren't effective. Somehow if I did take them, it would mean I am pro-pharma and I'm definitely not on one side or the other 2. Advertising that I do not take antidepressants may have meant that I haven't tried everything and I was suffering needlessly. On this channel I always talk about doing everything in our power to help us feel comfortable in our own skin and to deal with life's challenges. Always telling you I don't take antidepressants may have seemed like I am anti-pharma, and that's not the case either. I've always been on the fence for a reason and never told you either way if I take antidepressants. I hope you appreciate me telling you the truth. Please refrain from talking about medications you may be taking and do not discourage others if they are currently looking for solutions. My name is Matt and 15 years ago I had my first diagnosis of major depression, although now I realize that I have lived with it my whole life. When I was younger I just did what I did and it wasn’t until it really started to affect my life and the lives of those around me that I knew I needed help. I have been in and out of doctors offices, I have done therapies and have spent months in hospital. I have lost jobs and been unable to get out of bed and at times been inconsolable. I have been on many medications including, Zoloft, sertraline, lexapro, escitalopram, endep, amitriptyline, efexor XR, Venlafaxine, mirtazapine, sodium valproate, seroquel, quetiapine and diazepam. (SSRIs). I know that medication is needed at times, but there is also a wealth of other things to learn and put into place in your everyday life. Now, I have a lot of skills to deal with depression in my daily life but I still have times when I all seems too hard. Also anxiety creeps in and without realizing I find myself being a victim of it over and over. Depression, when combined with anxiety becomes more complex but the skills for dealing with depression are just as useful for dealing with anxiety. I want to share what I know, because I would have greatly benefitted from a collective of knowledge 15 years ago. So I am documenting all the things I do every day to keep happy and motivated because I believe we can all grow through shared experience. The peak incidence of mania/bipolar disorder incidence was seen in patients aged between 26 and 35 years. Prior antidepressant treatment was associated with an increased incidence of mania/bipolar disorder ranging from 13.1 to 19.1 per 1000 person-years. Multivariable analysis indicated a significant association with selective serotonin reuptake inhibitors and venlafaxine. Conclusions In people with unipolar depression, antidepressant treatment is associated with an increased risk of subsequent mania/bipolar disorder. These findings highlight the importance of considering risk factors for mania when treating people with depression.

It blocks some adrenergic auto and heteroreceptors, increasing serotonin release, and selectively antagonizes serotonin receptors in the central and peripheral nervous system. It also enhances serotonin neurotransmission and blocks the histaminergic and muscarinic receptors. Mirtazapine is not a serotonin or norepinephrine reuptake inhibitor but may increase serotonin and norepinephrine by other mechanisms of action. Side Effects: Constipation, dry mouth, weight gain, increase appetite, somnolence, sedation, sleepiness, weakness, distrurbance in thinking, peripheral edema, increased blood pressure. When Discontinuing Use: Mirtazapine and other antidepressants may cause a discontinuation syndrome upon cessation. A GRADUAL and SLOW reduction in dose is recommended. Effects of sudden cessation of treatment with mirtazapine may include depression, anxiety, panic attacks, vertigo, restlessness, irritability, decreased appetite, insomnia, diarrhea, nausea, vomiting, flu-like symptoms such as allergies and pruritus, headaches and sometimes hypomania or mania. It can go so far as to cause suicidal thoughts, so please, be careful!! AKA not good!!! PLEASE always been under the care of a great doctor and NEVER stop taking a drug suddenly without guidance from your doctor.
