Perimenopausal Depression. Perimenopause, the transition women go through prior to menopause, causes abnormal periods, erratic fluctuations in hormone levels, insomnia and, for many women, unpleasant hot flashes. It has also been linked to depression according to several studies. In a pair of studies published in the Archives of General Psychiatry, researchers found that women in perimenopause are twice as likely to be diagnosed with major depressive disorder (MDD) and four times as likely to develop depressive symptoms than those who haven't yet entered this hormonal transition. The studies found that women with the greatest frequency of hot flashes reported the most significant depressive symptoms. Women who haven't given birth and those who have taken antidepressant medications are at a higher risk for depression as well. Risk Factors for Perimenopausal Depression:. Although some studies show that fluctuating levels of the female hormone estradiol are one predictor of depression, there are several other factors that may account for perimenopausal depression. One interesting finding from a Seattle-based study in 2008 published in the journal Menopause found that women with no prior history of depression were four times as likely to develop depression as a result of perimenopause than previously-depressed women. Hot flashes and their impact on sleep patterns were also implicated in the study. In contrast to prior studies, new research shows that changing levels of the female hormone estrogen have little impact on perimenopausal depression. This may explain why hormone replacement therapies have little impact on overall depression (although it provides some relief for more severe depression, perhaps because it helps with hot flashes). Stressful life events such as divorce, job loss, or the death of a parent are common occurrences for women in this stage of life and may also trigger depression. Several other factors have been linked to perimenopausal depression including:. *a family history of depression. *a prior history of sexual abuse. *negative feelings about aging and menopause. *severe menopausal symptoms. *a sedentary lifestyle. *smoking. *social isolation. *low self-esteem. *disappointment in not being able to have any more children (or any children). Symptoms of Perimenopausal Depression:. Major depressive disorder is a serious condition that requires treatment. Symptoms of the disorder may include fatigue and lack of energy, slowed cognitive function, inattentiveness, a lack of interest in once-enjoyable activities, and feelings of worthlessness, hopelessness or helplessness. Other symptoms related to perimenopausal depression may include:. *mood swings. *irritability. *crying for no reason or tearfulness. *heightened anxiety. *profound despair. *sleep problems related to hot flashes or night sweats. *Depression and Its Effect on Perimenopause. *Perimenopause and depression have a complex relationship. Not only can the effects of perimenopause cause depression, a 2003 study found that depression itself may lead to early-onset perimenopause. The study, published in the Archives of General Psychiatry found that women with "significant symptoms of depression in their late 30s and early 40s" were more likely to enter perimenopause before their 45th birthday than women who hadn't experienced depressive symptoms. Research was inconclusive whether early perimenopause led to early menopause or if it simply resulted in an extended period of perimenopause. Lower estrogen levels during both phases are associated with other health risks including impaired cognitive functioning, heart attack, and stroke. Women using antidepressants were three times as likely to enter perimenopause early than those who weren't according to the study. In adults, this disorder tends to be focused around worry associated with health, finances, and everyday routine life circumstances. In children, this tends to present more as worry about competence (tests and assessments) and the quality of their performance (sports games, musical performances, etc). Although the median age of onset is 30 years old, those who they spoke to stated that they had felt anxious their entire lives. The good news is that there are many treatment options! First is talk therapy and more specifically CBT. This helps us challenge those automatic/faulty thoughts so that we can slowly get back to doing the things we used to do without worry or anxiety. This can take time and lots of therapy homework, but they find CBT to be the best therapy approach. Secondly, medication. I know not everyone is open to trying medication, but they find that CBT with medication gives us the best chance of recovery. Usually, SSRI's or SNRI's (otherwise known as anti-depressants) are used first. If those don't help at certain intense moments, they may prescribe a benzodiazepine to be used only in acute situations due to its habit forming tendencies. So please be careful if you are prescribed these and do not use them every day. Lastly, there are some lifestyle changes you can make. Eating regularly and from a well-balanced diet can help. Also, cutting back on coffee and alcohol can help us sleep more soundly and get rid of any symptoms that may feel like anxiety. Getting exercise and taking time to meditate can help us better manage any of the anxiety symptoms we struggle with. I hope you found this helpful! When anxiety becomes excessive, irrational, or inappropriate, interfering with the normal functioning of a person's life, it is considered an anxiety disorder.What are the different types of Anxiety Disorders? Generalized Anxiety Disorder is marked by "free floating anxiety," with persistent and pervasive feelings of fright. The person experiencing this anxiety constantly feels threatened. Symptoms may include muscle tension, twitchiness, an inability to relax, autonomic hyperactivity (increased heart rate and sweating), an upset stomach, and headaches. Basically, the person suffering from Generalized Anxiety Disorder experiences a chronic state of hypervigilance and tension without any clear source or focus.Panic Disorder is comprised of very intense panic attacks of short duration (minutes to a few hours). The person feels as if they are going to faint and even die. Symptoms may include chest pains, heart palpitations, hot and cold flashes, feelings of unreality, dizziness, or faintness (though it is extremely rare for anyone experiencing a panic attack to actually faint). Another important feature in a Panic Disorder is that the person cannot identify any specific reason for the panic. It feels as though it comes out of nowhere, making it even more terrifying. Phobias are persistent and recurrent fears of a particular situation, object or activity. There are several types of Phobias: Agoraphobia: The most severe of the phobias, it includes a fear of open spaces, public places, and crowds. It is also common for agoraphobics to fear being alone, so they usually stay at home with a friend or relative present. Agoraphobia often begins with a panic attack while the person is away from the house, and the person then stays at home to avoid experiencing that situation again. Social Anxiety Disorder (Social Phobia) is when a person has an excessive and unreasonable fear of interacting with other people in social situations. A person with social phobia constantly fears they will embarrass themselves or make mistakes in front of other people, developing a negative self-consciousness. When placed in a social situation, they become anxious or may have a panic attack. As the illness progresses, persons with social anxiety disorder may have distorted thinking, and may begin to avoid contact with other people, becoming isolated. This may interfere with work, school and personal relationships. Obsessive Compulsive Disorder (OCD) is experienced as obsessive thoughts followed by compulsive behaviors. Those with OCD typically are quite rigid in their thinking and behavior, and seek consistency and avoid spontaneity (which is perceived to be dangerous). Uncontrollable thoughts are usually considered by the person to be socially unacceptable, such as bizarre sexual thoughts, or thoughts of hurting someone. The person then engages in compulsive behaviors (such as counting, touching something repetitiously, and other ritualistic behaviors) in an attempt to control the obsessive thoughts. With severe OCD, the engagement of multiple compulsive behaviors can be extremely debilitating and may result in major disruptions in the person's life. Post-Traumatic Stress Disorder (PTSD) is often considered an anxiety disorder and is frequently associated with other anxiety disorders. PTSD develops in response to actual or threatened extreme danger or trauma, such as wartime experiences, torture, natural disasters, rape, incest, or even psychotic. Symptoms include nightmares, dissociative states, vivid flashback memories of the traumatic event, loss of control, emotional detachment (psychic numbing), hyper-arousal, and survivor guilt. Typically, symptoms of PTSD consist of recurrent re-experiencing of the trauma, avoidance behaviors or phobias, and chronic physical signs related to hyper-arousal, including anxiety, sleep disorders, difficulty concentrating, hypervigilance, memory problems, and fatigue. Anxiety Disorders consist of phobias, social anxiety, panic attacks, obsessive compulsive disorder, agoraphobia, and post-traumatic stress syndrome. Often, when a person with an anxiety disorder seeks help, they are not treated in the most effective manner. Many practitioners will put a person with an anxiety disorder on benzodiazepines, such as Valium or Xanax or Ambien. Holistic therapies round out our treatment system, with yoga, art therapy, mindfulness meditation, massage, and acupuncture all available. Anxiety disorders are often treated with medications. Antidepressants, Beta-blockers, low dose antipsychotic medications, benzodiazepines, and other anti-anxiety medications are commonly prescribed

More women used to be prescribed hormones for hot-flash relief. But ever since the 2002 Women's Health Initiative revealed hormone replacement therapy might be more harmful than helpful, hormone therapy use has dropped off significantly. Now the FDA has approved a new non-hormonal drug. But some critics question its effectiveness. A study showed the off-label use of the antidepressant Lexapro can reduce the frequency of hot flashes. Now the FDA has approved a lower-dose form of another well-known antidepressant specifically for hot flashes. "Brisdelle is actually the same ingredient that's in the FDA-approved medication Paxil, which is an antidepressant," said gynecologist Dr. Steven Rabin. Rabin says research doesn't show it's significantly effective. "The study showed that this particular drug reduced hot flashes, but only a little bit better than sugar pills," said Rabin. Gynecologist Dr. Steven Rabin says labels on antidepressants warn these drugs can increase suicidal thoughts, may interfere with other medications and decrease a woman's libido. But experts say whether or not this reduces side effects remains to be seen. "Nobody can say for sure. Brisdelle seems to have the same warnings in its packaging that its sister, at the full dose, has, so it didn't look like they were ready to say that it had significantly less side effects," said Rabin. Rabin says women should have a discussion with their doctor before deciding on the best treatment approach for them. "Looking at all of the news and the hoopla about this new, non-hormonal, FDA-approved drug, it sounds like we needed it, and I think that what we need is more clarification of the true risks and benefits of hormone-replacement therapy," said Rabin.

A new treatment for hot flashes was just approved by the Food and Drug Administration for use. Local 12's Liz Bonis asks the experts at TriHealth and has what you need to know about it in today's Medical Edge. "It just comes on and you are just burning up for a little bit. Do you start to sweat, start to sweat, turn a little red," says office manager Lisa Runyan. Lisa Runyan is a busy office manager at this women's health practice. She says when hot flashes hit, there are challenges to keeping temperatures down. "Some of us have fans at our desk, and they are running just about all the time." This is also just one of many reasons Doctor Vaishali Bhalani wants women to know that the medication Paroxetine, marketed under the brand name Paxil, might help. "I think that it gives women another option. Up until now the only treatment that was available was menopausal hormone therapy," says Dr. Bhalani. The medication has been in use for quite awhile as an antidepressant. It appears to alter a hormone known as serotonin in the body, but in this case in lower doses. The Food and Drug Administration has just approved it's use for hot flashes and that raises the question of what do you need to know about it, who should be on it and what are the side effects? "The main side effects I think at lower doses are fatigue, being tired, as well as nausea vomiting and headache," says Dr. Bhalani. Doctor Bhalani says those side effects generally diminish in about four few weeks and it's suggested for use in those who say menopausal symptoms really interfere with daily life. "We do see about a good 30-60 percent reduction, depending on the dose, in menopausal hot flashes and symptoms." Lifestyle changes such as yoga, meditation and exercise along with it, are also recommended because she also points out, "Exercise can decrease weight,,and we know weight can lead to an increase in hot flashes."

For women suffering through symptoms of menopause, doctors say the first year is the worst. Some dont suffer any symptoms at all, but 85% of women in North America do. This includes night sweats, fatigue, mood swings, vaginal dryness and hot flashes. Treating hot flashes is what usually brings women in. Says Dr. Deborah Basso of St. Marys Hospital in Troy. An OB/GYN for Seton Health, she says hot flashes can last for 2 to 5 years. They can be treated with estrogen in the form of a pill, spray, patch of cream. Estrogen has always been the gold standard.But staying on estrogen for longer periods of time increases the risk of blood clots and cancer. Dr. Basso says there are alternatives for women who dont or cant take estrogen. Gabapentin which is a Neurontin. It is very effective and not hormonal. It is especially effective for night sweats if you take that at nighttime. Also, some of the new SSRIs which are anti-depressants work well for treating menopausal symptoms.For that, Dr. Basso mentioned Celexa, Effexor and Lexapro among others. She says if vaginal dryness is an issue, women can use local estrogen, moisturizers or lubricants. Some women take herbal remedies like Black Cohosh or Primrose oil to moderate symptoms. Dr. Basso says not a lot of studies have been done on these, but if they prove to be effective there is nothing wrong with using them. If it helps them and doesnt hurt them, then I say okay. Another way to alleviate symptoms is to make lifestyle changes. In particular with hot flashes, try dressing in layers. Also, avoid triggers alcohol, nicotine, and caffeine have been known to cause hot flashes in women.
