The main joint of the shoulder, commonly referred to as "THE shoulder joint", is the joint between the head of the humerus - the upper arm bone, and the glenoid cavity of the scapula -the shoulder blade. This is a ball-and-socket type of joint. Shoulder arthritis refers to loss of cartilage on the surface of the humeral head and the glenoid of the shoulder joint. The two bones rub against each other and produce pain, stiffness and difficulty moving an arm. Types of shoulder arthritis include: Osteoarthritis - also called degenerative joint disease - is the "wear and tear" condition of the joint, commonly due to old age. Osteoarthritis is characterized by loss of cartilage, bone spurring and NO major inflammation. Rheumatoid arthritis – is a result of inflammation of the synovium - the membrane that encloses the joint. Inflammation brings in the cells of the immune system, whose inflammatory chemicals damage and destroy the joint. It is not clear how rheumatoid arthritis starts but genetic predisposition together with infection of the joint are likely to be among the causes. Rotator cuff arthropathy or cuff tear arthropathy is shoulder arthritis AS A RESULT and IN A SETTING of rotator cuff tears. Rotator cuff muscles hold the ball and the socket together and prevent the humeral head from moving out of the socket when the person raises an arm. In people with torn rotator cuff, the head of the humerus moves upwards and rub onto the acromion. This causes damages to the covering cartilage and eventually arthritis. Combination of cuff tear and arthritis is a devastating condition that seriously impairs functions of the shoulder joint. Post-traumatic arthritis – is arthritis that develops after physical injury to the joint. Chondrolysis - sudden loss of cartilage that happens occasionally after a shoulder surgery, commonly seen in association with infusion of local anesthetics into the joint for pain management.

Dr. Ebraheim’s educational animated video describes examination test of the hand. Adequate evaluation of the hand is necessary for the management of hand and wrist conditions or injury. Knowledge of certain specific tests will help the clinician to reach the correct diagnosis and provide adequate treatment. These are some of the most useful tests that are used frequently for evaluation of wrist and hand conditions. De Quervain's syndrome: inflammation of the sheath or tunnel that surrounds the two tendons that control movement of the thumb. Finklestein’s test: the patient makes a fist with the fingers closed over the thumb and the wrist is bent towards the little finger. The hand is pulled so that the involved tendon is stretched, causing a sharp, local pain if injury and inflammation are present. What is carpal tunnel syndrome? Pressure placed on the median nerve due to thickening of the transverse carpal ligament. Can lead to numbness, tingling or weakness of the hand and fingers. Tinel’s test: the tinel’s test is used to determine symptoms of carpal tunnel syndrome. The test is considered positive if symptoms of tingling worsen while tapping on the median nerve at the wrist. •Pahlen’s test: the Phalen’s maneuver is a diagnostic test performed to determine if the patient has CTS. The back of the hands are pressed together, compressing the nerve, which may cause symptoms of CTS. •Froment’s test: the Froment's test is used to test for palsy of the ulnar nerve which may occur with entrapment of the ulnar nerve within the cutibal tunnel. What is cubital tunnel syndrome? Occurs due to compression of the ulnar nerve at the elbow. Froment’s test: when pinching a piece of paper between the thumb and index finger, the thumb IP joint will flex if the adductor pollicis muscle is weak due to ulnar nerve palsy. •The OK sign is used to check for paralysis of the anterior interosseous nerve due to entrapment or compression injury. This nerve is a branch of the median nerve that innervates the muscles of the deep group of the anterior compartment of the forearm. Patients with paralysis of the anterior interosseous nerve will unable to make the OK sign. •Wrist drop: condition of the hand and wrist that determines the presence of radial nerve injury. •The Allen's test: The examiner applies pressure with the thumbs occluding the ulnar and radial arteries. Patient is asked to make a fist. Patient is then asked to open the hand. When the examiner releases the ulnar artery, the patient;s ulnar artery will cause blood flow to the hand. What is basal thumb arthritis? Arthritis affecting the thumb joint . Grind test: by axial loading, pushing and rotating the thumb metacarpal bone, grinding may be felt within the joint. •Watson’s test: also called the scaphoid shift test, it is diagnostic test used to evaluate scaphoid stability. The examiner places a thumb over the patient’s scaphoid tuberosity which is the distal pole of the scaphoid on the volar surface. The other fingers of the examiner’s same hand are placed dorsally above the radius. The patient’s wrist is placed initially into an ulnar deviation and the examiner's other hand deviates the wrist radially and flexes it slightly. With pressure on the tuberosity of the scaphoid, the scaphoid is pushed dorsally out of the radial fossa. If there is instability of the scaphoid, the test will be positive and produce pain and a “clunk”. •Lumbrical plus finger: caused by laceration of the flexor digitorum profundus distal to the origin of the lumbricals. Lumbrical is tighter than FDP. Most common in the middle finger. With injury of the flexor digitorum profundus tendon, the patient is unable to maintain the grip when the examiner releases the finger. •Testing the integrity of the flexor tendon: keep the PIP of the finger extended and see if the patient can flex the DIP. Keep the other fingers extended and then see if the patient can flex the PIP joint of the involved finger. When multiple slips of the superficialis tendon are cut, identify the tendon properly. The superficial tendon of the long and ring fingers are volar at the wrist. •Bunnell test •Elson’s test: used to determine if the patient has a central slip tear before the deformity is present. •When holding a relaxed cascade, the fingers should normally point towards the region of the scaphoid. Malrotation of the finger will causes the affected finger to deviate from its normal rotational direction. Malrotation is especially important with finger metacarpal fractures. Dr. Ebraheim’s educational animated video about Elbow pain, describing the conditions associated with the area of the elbow. Tennis elbow ( lateral epicondylitis) It is an inflammation, soreness or pain on the outside (lateral) area near the elbow. Injury is usually seen in patients who perform manual labor or sports that require twisting and extension of the wrist against resistance. There is pain with resistant wrist extension. All these symptoms occur because of degeneration, irritation, tears and pain of the extensor tendons. Treatment includes ice, elbow pads, decreased activity, physical therapy, eccentric exercise, steroid injections or surgery as a last resort. Radial tunnel syndrome The pain of radial tunnel syndrome is very similar to the symptoms of tennis elbow. Pain is usually distal to the lateral epicondyle and radiates down the forearm. If the symptoms of tennis elbow are not going away with treatment, rule out the possibility of radial tunnel syndrome. Treatment: ice, elbow pad, decreased activity, physical therapy, eccentric exercise, injections (steroids or PRP), surgery( as a last resort). Anterior elbow pain ( biceps tendon rupture) The biceps muscle aids in flexion and supination of the forearm. Symptoms of biceps tendon rupture include pain, bruising, and swelling located infront of the elbow. It occurs in patients who perform manual labor, body builders, etc. rupture may retract the muscle casuing popeye sign. Rupture affects 40% of supination of the forearm and the tendon needs to be reattached to the tuberosity of the radius. Elbow arthritis Elbow injuries can lead to loss of cartilage and joint degeneration due to extra wear and tear on the surfaces of the joint. Symptoms include global pain, stiffness and loss of motion. This condition is called osteoarthritis, denerative arthritis or posttraumatic arthritis.Treatment includes anti-inflammatory medications, injections, therapy and rarely surgery. Golfer’s elbow ( medial epicondylitis) The flexor muscles of the forearm are responsible for flexion of the wrist. Golfer’s elbow is an inflammation, soreness or pain on the inside area near the elbow. This pain is due to repetitive overuse or overload activities and is aggravated by movement of the wrist. Treatment includes ice, decreased activity, physical therapy, eccentric exercise, steroid injections or surgery as a last resort. Cubital tunnel syndrome It is a condition due to compression of the ulnar nerve at the area of the medial ( inside) elbow resulting in pain, swelling and weakness of the hand. This problem occurs due to compression where the ulnar nerve passes around the elbow through the cubital tunnel. Treatment conservative then surgery if conservative treatment fails. Medial collateral ligament injury Injury typically occurs in throwing sports such as baseball as seen in pitching ( tommy john injury ). It can lead to chronic pain, valgus instability and can be potentially disabling. Treatment includes: stop throwing, physical therapy, evaluation with x rays and MRI. Injury may need reconstruction which is successful but takes a long time to restore function of the elbow. Posterior impingement syndrome Valgus and extension overload can result in injury of the posterior part of the elbow. Athletes present with posterior elbow pain that worsens when the elbow is extended with loss of velocity and control. Athletes may experience catching, locking, clicking and stiffness. With repeated forced extension of the elbow, the top of the olecranon is jammed into the fossa at the back of the elbow causing inflammation, cartilage injury, loose fragments and bony spurs. Treatment is usually conservative and rarely needs surgery. Olecranon bursitis It is inflammation of a small sac of fluid located on the tip of the elbow.this bursa allows the elbow to bend and straighten freely underneath the skin but can become irritated with repetitive movement. Swelling of the joints (or enlarged joints) usually affects the hands, feet and ankles. Patients complain of stiffness and redness of the joints, and report even severe pain. It could get worse when a person is standing or walking, and sometimes it can cause the patient's complete immobility. The most common cause of joint inflammation is arthritis. In addition, infections and injuries to the joints can cause pain and swelling. Rheumatoid arthritis This is an autoimmune disorder, caused when a patient's immune system attacks healthy tissue. It is often found in women between 25 and 55 years old. The exact causes of this disease are unknown. It is suspected that genetic predispositions, hormones and infections may have been associated with the disorder. Patients experience the worst pain when getting out of bed and the attack may last a couple of weeks. Some people have other symptoms, such as fever, loss of appetite, red and inflamed skin and numbness. Patients may also have different eye problems and anemia. It usually affects the joints of the hands and feet. The majority of patients suffering from inflammation of the joints of the fingers suffer from rheumatoid arthritis. The condition must be treated, and usually lasts the entire life of the patients. Osteoarthritis Aging affects the joints and wears them, so arthritis is a very common disease in older patients. This form is arthritis that affects the cartilage of the bone and is known as osteoarthritis. Joint infections Septic arthritis is a bacterial inflammation of the joint that produces inflamed joints. The untreated condition can cause a structural change or complete destruction of the joints, and that is why this condition must be addressed. Thumb pain usually presents as pain at the base of the thumb and wrist. A notable thumb pain condition includes De Quervain Syndrome, also known by many other names such as gamer's thumb, mother's wrist, and mommy thumb. This condition in particular is a degenerative (microscopic injury) condition of the sheath (extensor retinaculum) that surrounds the tendons passing over the thumb side of the wrist. These tendons control the motions of the thumb, most specifically the movement of thumb towards the wrist. When this sheath becomes injured or inflamed, the tendons become entrapped and cause pain during movement of the thumb. Thickening of the tendons from overuse or trauma will also cause problems when the tendons need to slide beneath the sheath. Women are considered at higher risk due to the sharper angles of the bones in the wrist, but the actual cause is somewhat unknown. Many believe overuse is the primary cause leading to inflammation. This hypothesis is backed up by noting that most patients with this pain are new mothers who repeatedly lift their growing babies. Symptoms are most often noted in both wrists and to the same degree of pain, indicating that more overuse activity causes more inflammation leading to pain. When these conditions are present, movement of the thumb is restrained and further perpetuates the inflammatory condition and may create swelling in more severe cases. Pain is noted during thumb and wrist motion along with thickening and hardening of the area at the base of the thumb. Difficulty gripping, tenderness at the base of the thumb, and swelling are common symptoms. Pain is usually slight to moderate, and extreme pain may be an indication of a more serious condition. Treatment of the pain is determined more by noting what has worked in the past rather than scientific data. KT Tape has been shown to provide support during motion of the thumb and wrist, and has markedly reduced pain in many subjects. KT Tape also increases circulation to reduce inflammation and speed the healing process. Corticosteroid injections are thought to be successful, but do need to be weighed against the negative side-effects of increased degenerative effects. Surgery has been well accepted and successful in almost all cases, but conservative therapies are highly preferred prior to making this leap. Ice after activity, rest, and avoidance of activities that incite pain are all important steps to avoid surgery. Carpal tunnel syndrome Pressure placed on the median nerve due to thickening of the transverse carpal ligament. Thenar atrophy is commonly associated with severe carpal tunnel syndrome. Carpal tunnel syndrome can occur during pregnancy since the swelling brought on by pregnancy may compress the median nerve in the hand, causing the fingers to feel numb. These symptoms typically go away after delivery of the baby. Common causes of CTS include: •Obesity •Hypothyroidism •Arthritis •Diabetes •Trauma •Repetitive work (typing, lifting) •Vibration Carpal tunnel syndrome can lead to numbness, tingling or weakness in the hand and fingers. The numbness usually occurs at night because we tend to sleep with our wrists flexed. The Tinel’s test is used to determine symptoms of carpal tunnel syndrome. The test is considered positive if symptoms of tingling worsen while tapping on the median nerve at the wrist. The Phalen's maneuver is a diagnostic test performed to determine if the patient has carpal tunnel syndrome. The back of the hands are pressed together, compressing the nerve, which may cause symptoms of carpal tunnel syndrome. Treatment includes •Anti-inflammatory medication •Splint •Injections •Carpal tunnel release: performed by cutting through the transverse carpal ligament. Carpal tunnel release allows the median nerve to pass freely through the tunnel to receive sensations from the thumb, index, and middle fingers of the hand. This is similar to a tunnel being opened or widened so a large truck can pass through. There is no treatment for it, especially if it is rheumatoid arthritis, which is an autoimmune disease that causes the defense system of the body to attack itself. In this video, we'll let you know how turmeric can be used to treat arthritis. Turmeric has been in use for many centuries now as a cure for several diseases. Turmeric, its rhizome stem, is used as a traditional medicine to cure various ailments like arthritis and inflammation. Here, we have discussed how you can use turmeric along with other ingredients to cure the condition of arthritis. Turmeric contains anti-inflammatory, anti-arthritic, free radical removal and antioxidant properties that help in treating arthritis. Turmeric also contains curcumin that is effective in treating arthritis and its associated pain. It also helps in treating the swelling and tenderness of the joints. Turmeric also helps in blocking certain cytokines and enzymes that result in inflammation and also works as a painkiller that raises the mobility in the joints and decreases the stiffness. So, here's a gist on how to use turmeric for arthritis treatment. Continue watching to know more. 1. Turmeric And Milk : Mix one spoon of turmeric with one cup of warm milk. Drink this milk in the evening before going to sleep to get a sound sleep. Also, make sure to consume turmeric regularly to reduce the joint pain and inflammation caused by arthritis. Mix it thoroughly and boil it for 15 minutes. This will help create 12% of curcumin mixture. You need to drink this solution within four hours. Else the curcumin will gradually reduce to 6% in about 6 hours. 3. Turmeric And Ginger : Ginger has anti-inflammatory properties that help remove the inflammatory chemicals from the painful joints. Take half a spoon of turmeric powder and powdered ginger root. Brew them in boiling water for about 20 to 30 minutes. Drink this tea to get relief from arthritis. 4. Turmeric Powder And Coconut Oil : Mix one spoon of turmeric powder with a sufficient amount of coconut oil. Make a paste and apply it on the affected area. Leave it on for a few hours and rub it off after it dries. Repeat this to get relief from arthritis pain completely. 5. Turmeric Powder, Powdered Lime And Saltpeter (Potassium Nitrate) : Take one spoon of turmeric powder, a little powdered lime and saltpeter. Mix them well to make a paste. Apply this on the painful area and cover it with a cheese cloth. Repeat this frequently to get relief from arthritis and its symptoms. 6. Turmeric Paste : Mix ¼th cup of turmeric powder in ½ a cup of water. Simmer it over medium heat and stir until it becomes a paste. Allow it to cool down and store it in the refrigerator. Apply on the affected area and wash off with plain water. Hence, you can adopt this method to treat arthritis using turmeric. 7. Turmeric, Coconut Oil And Egg Yolk : Beat the egg yolks and melt the coconut oil. Add coconut oil into the mixture along with some turmeric powder. Use a hand blender and emulsify this mixture. Consume this mixture once or twice a day to get relief from arthritis completely. 8. Turmeric, Lemon Grass, Coconut Oil And Cayenne Pepper : Mix 5 spoons of cayenne pepper with 5 spoons of turmeric powder. Add dry organic lemon grass to it and coconut oil to it. Mix all these in an oven pan and preheat it at 150 degrees C for 3-5 hours. Strain these herbs by using a cheesecloth into a glass jar and close the lid tightly. Apply this mixture on the skin to get a good relief from arthritis pain. 9. Turmeric, Fenugreek Powder And Dried Ginger Root : Ginger contains anti-inflammatory phytonutrients called gingerols that help with fighting inflammation and arthritis. Mix one spoon each of turmeric powder, fenugreek powder and dried ginger powder. Mix all these thoroughly along with warm water. Consume this solution twice in a day to get relief. 10. Turmeric And Dimethyl Sulphate Solution : Add two spoons of turmeric powder to 1-2 spoons of dimethyl sulphate. Mix them well to make a paste and rub it over the painful joints. Wrap it with a plastic wrap and cover it with a heating pad for about 25 to 30 minutes. 11. Turmeric And Lemon Juice : Take two spoons of turmeric powder with one spoon of lemon juice. Mix them well and add it to boiling water to make a paste. Apply the paste on the affected area and leave it for a few minutes. Wash it with water and repeat this until it heals.
