The bladder is a hollow organ in the lower abdomen (pelvis). It collects and stores urine produced by the kidneys. •The bladder is connected to the kidneys by a tube from each kidney called a ureter. •When the bladder reaches its capacity of urine, the bladder wall contracts, although adults have voluntary control over the timing of this contraction. At the same time, a urinary control muscle (sphincter) in the urethra relaxes. The urine is then expelled from the bladder. •The urine flows through a narrow tube called the urethra and leaves the body. This process is called urination, or micturition. Cancer occurs when normal cells undergo a degenerative, dangerous, or what is called a malignant change or transformation causing them to grow abnormally and multiply without normal controls. A mass of cancerous cells is called a malignant tumor or cancer. The cancerous cells are capable of spreading to other areas of the body through the process of metastasis. A cancer can become destructive locally to the tissues adjacent to where it arises. Cancer cells can also metastasize. Metastasis means that cells spread through the tissue fluid's circulation called the lymphatic system or through the bloodstream where they can then stop off in other tissues or organs where they may grow as metastases or metastatic deposits and can become destructive in these new locations. The term cancer is further described by the tissue in which it has arisen. For example: bladder cancer is a different disease than lung cancer. If a bladder cancer cell metastasizes -- that is, spreads to the lungs through the bloodstream it is still called, and is treated as metastatic bladder cancer, not as lung cancer. Cells which transform in a less dangerous fashion may still multiply and form masses or tumors. These are called benign tumors. They do not metastasize. Of the different types of cells that form the bladder, the cells lining the inside of the bladder wall are those most likely to develop cancer. Any of three different cell types can become cancerous. The resulting cancers are named after the cell types. •Urothelial carcinoma (transitional cell carcinoma): This is by far the most common type of bladder cancer in the United States. The so-called transitional cells are normal cells that form the innermost lining of the bladder wall, the urothelium. In transitional cell carcinoma, these normal lining cells undergo changes that lead to the uncontrolled cell growth characteristic of cancer. •Squamous cell carcinoma: These cancers are comprised of cells that typically form as a result of bladder inflammation or irritation that has taken place for many months or years. These cells grow in flat masses of interconnected cells. •Adenocarcinoma: These cancers form from cells that make up glands. Glands are specialized structures that produce and release fluids such as mucus. •In the United States, urothelial carcinomas account for more than 90% of all bladder cancers. Squamous cell carcinomas make up 3%-8%, and adenocarcinomas make up 1%-2%. •Only transitional cells normally line the rest of the urinary tract. The kidney's internal collecting system, the ureters (narrow tubes that carry urine from the kidneys to the bladder), the bladder, and the urethra are lined with these cells. Bladder cancer is a cancerous growth in the urinary bladder. So how do you know whether somebody has a bladder cancer. It can be silent if it is a small tumor in the bladder, you may not recognize it. But most commonly the patients will present with blood in the urine. That is typically called as painless haematuria, we mean to say that there is no pain, but there is lot of blood in the urine, It can vary from a very small called as microscopic haematuria , that is the blood is detected by the microscopic examination of the urine to a frank haematuria, means a frank blood coming in the urine. This is the most common symptoms of bladder cancer. It can also present with pain abdomen not pain in the lower abdomen area or pain in the region of the kidneys, especially if the cancer into the kidneys. In the advanced stage the bladder cancer can present with loss of appetite, you don’t have appetite or you loose weight or sudden loss of weight, generalised weakness. So the most common thing that you can do is or the most easiest way to prevent bladder cancer is stop smoking. The other things that can cause bladder cancer is the people who work in the painting industry, they work with batteries or petroleum products. These are the things and especially people who work in factories, they can develop these cancers. So to prevent that the companies have to educate their people who are exposed to those chemical then they need to undergo regular screening tests, then if they are identified early, the can be treated early.

This is one of the sly symptoms that men diagnosed with prostate cancer say they remember, now that they look back. If you find yourself waking more than once a night urgently needing to go, a trip to the doctor is in order. Bear in mind, though, that this symptom is far more likely to be caused by BPH or an overactive bladder than by prostate cancer. 9, Leaking or dribbling urine. Here's one almost no one talks about, but we need to. TURBT Transurethral Resection of Bladder Tumor, Female Procedure • PreOp® Patient Education https://preop.com/preop/turbt-transurethral-resection-of-bladder-tumor-female/ Your doctor has recommended that you have a TURBT or Transurethral Resection of a Bladder Tumor. Before we talk about the procedure, let’s review some information about your body and your medical condition. The bladder holds urine until you release it. It stretches like a balloon as it fills with urine. Muscle in the wall of the bladder works to push urine out of your body through your urethra. Your surgeon has recommended a TUR-BT to remove a tumor from the lining of your bladder. About half, or 1 in 2 bladder cancers are found early. That means the tumor is still in the lining of the bladder and hasn’t spread. Bladder Cancer can be diagnosed at any age, but is most common in patients that are over 55 years old, men and are white Bladder Cancer is most often linked to smoking and exposure to certain chemicals in the workplace With a TURBT , (or Transurethral resection of bladder tumor), a surgeon uses a scope to look at the bladder lining and remove the tumor. A scope is an instrument with a light and camera. It has a loop at the tip that can cut with heat energy. The tumor is sent to a pathology lab for examination. Further treatment may be needed in the future, after this surgery for your cancer. These plans are made if needed after the procedure and based on the final lab results. Now let’s talk a little more about what happens during a TUR-BT. To start, you are given anesthesia to keep you free of pain during the procedure. You are positioned carefully. An instrument with a camera, called a scope, is then gently inserted into the urethra to reach the bladder. A numbing gel is used to help the scope glide easily. Once the scope is inside the bladder, your doctor will fill your bladder with water or saline (pronounced say-leen) Using the fiber-optic light and camera lens of the scope to see, your doctor will look carefully at the walls of your bladder. The tumor is cut away from the bladder in small pieces using the loop. Your surgeon uses suction to carefully remove those pieces from the bladder. The loop is also used to stop any bleeding that is seen. After the surgeon makes a final inspection of the bladder the scope is removed. A thin soft tube, called a Foley catheter may be placed in your bladder. This tube can be used to fill your bladder with chemotherapy as part of the procedure if needed. This chemo or medication is used to prevent loose tumor cells from sticking to your bladder lining. Sometimes the foley is left in for a few days to keep your bladder empty for healing. When it is time to be removed, the balloon is deflated and the tube easily slides out. If your surgeon recommends chemo during the procedure, it will stay in your bladder for 1 hour. The decision depends on the size, type and number of tumors removed. This medicine will not make you feel sick. You may have some bladder irritation from this. Ask your doctor if this treatment is planned for you. After surgery, tell someone on your care-team if you have unexpected pain, dizziness or trouble breathing. You will have some discomfort but pain should improve with medication. After you are discharged to home from surgery, you may feel well and have no problems. Some patients will have pain with urination, bladder spasms and frequent urination. You may see blood and small blood clots in your urine for a few days, even in a few weeks as scabs heal in your bladder where the tumor was removed. Risks of the procedure are damage to the bladder, nearby tissues, infection and bleeding. Call your doctor if you cannot urinate, have a fever, worsening pain or bright red bleeding that doesn’t stop Hospital admission, medication or surgery may be needed to fix some complications. You may need to keep the catheter in for a longer time than expected Be sure that you understand why this procedure has been recommended for you. The majority of patients with neuroblastoma will eventually require surgery. These operations tend to be quite difficult because of the nature of these tumors, which often wrap themselves around delicate structures in the body. At The Children's Hospital of Philadelphia, surgical oncologists aim to remove as much of the tumor as possible, while preserving all normal organ functioning. Because such a large number of children receive surgical cancer treatment at CHOP, our surgeons have been able to hone their skills in the most difficult tumor resections. For those patients who require complex surgeries, but are able to receive the rest of their cancer treatment locally, CHOP physicians will partner with the referring oncologist before, during and after surgery so the patients are able to spend most of their treatment time close to home.
