Rheumatoid Arthritis | Biologic Agents for RA | Third Age
Biologic agents interfere with the harmful process of the Rheumatoid Arthritis disease by mimicking the natural substances your body produces on its own. Your doctor will only prescribe a biologic agent if you experienced serious side effects from Disease-Modifying Anti-Rheumatic Drugs (DMARDs) treatment, or if DMARDs failed to effectively control your RA. New Drug Gives Hope to Rheumatoid Arthritis Patients
Rheumatoid Arthritis often starts during the prime of a person's working life, typically between the age of 35 and 45. Women are 3 times more likely to develop the disease than men. It is an incurable, painful and debilitating auto-immune disease where the body attacks itself, most noticeably in the joints. Methotrexate (MTX) is the current drug prescribed for patients with RA in the UK but if this treatment is not working, patients may also be given a biological drug to help control their disease. However, of the estimated 46,000 RA patients on a biological treatment, 1 in 3 are not taking methotrexate, often because they are intolerant to this treatment. The other most widely prescribed drug is Humira. However a pioneering study has shown fantastic results, among 326 patients, at five trial centres in the UK, which has shown that almost four times as many RA patients achieve complete disease remission taking RoActemra in comparison to Humira for patients who cannot take Methotrexate. Professor Cem Gabey talks about the results of RoActemra. The good news is that at £9,500 per patient per year, RoActemra costs about the same as Humira and The NHS cost-effectiveness watchdog Nice has already approved of RoActemra when combined with methotrexate. http://www.arthritistreatmentcenter.com Surgical Infection Rate Higher with RA Drugs Marianne Wait writing in Arthritis Today reported that people with inflammatory forms of arthritis, such as rheumatoid arthritis, who take more than one traditional disease modifying antirheumatic drug (DMARD), or a type of biologic drug known as a TNF inhibitor, have an increased risk of infection following orthopaedic surgery, according to a new study published in Arthritis Care & Research in December. It is not a new theory that drugs taken for inflammatory arthritis conditions, which in many cases work by suppressing the immune system, might increase post-surgical infections. Over the years, a host of studies has looked at which drugs might elevate the risk and by how much. The results have not been clear. But this study "is one of the best out there regarding this topic," says Tim Bongartz, MD, associate professor of rheumatology at Mayo Clinic in Rochester, Minn. Dr. Bongartz was not involved in the study. Based on their results, senior study author Inès A. Kramers-de Quervain, MD, of the department of rheumatology at the Schulthess Clinic in Zurich, Switzerland, suggests "it may be advisable to consider stopping TNF inhibitors more than one administration interval before surgery, since the risk of postoperative infection appears to be higher if the operation occurs within this period." Good News for Rheumatoid Arthritis Treatment
www.rxwiki.com Anti-TNFs are a class of drugs commonly used to treat inflammatory conditions like rheumatoid arthritis (RA). But when an anti-TNF fails, what's the next step? In two new studies, a team of researchers from France found that RA patients who don’t respond to initial anti-TNF treatment may achieve better outcomes if they are next prescribed biologic therapy instead of another anti-TNF medication. Studying anti-drug antibodies in these patients may also help guide physicians toward the preferred next step in treatment. "This study demonstrated a better effectiveness of [biologic therapy] than a second anti-TNF for patients who did not respond to their first anti-TNF," said lead study author Jacques-Eric Gottenberg, MD, PhD, of the Department of Rheumatology at Strasbourg University Hospital in France, in a press release. "However, at least 40 percent of patients failed to respond to their second-line biologic, which emphasizes the unmet need to continue to increase the number of treatment options and develop personalized medicine for people with RA."

Rheumatoid arthritis patients undergoing surgery who stayed on their antirheumatic medication didn't have a higher risk of early postoperative infection compared with those who temporarily stopped treatment before surgery, according to findings from a large national Veterans Affairs study. Until now many rheumatologists and surgeons have taken a conservative approach, reasoning that the immunosuppressive drugs used in controlling inflammation in rheumatoid arthritis might also increase the risk of surgical wound infection. A common practice has been to have RA patients stop their medication a month ahead of elective surgery, or at least two drug half-lives beforehand, then start treatment again roughly a month after the operation, or when the wound has healed. The new Veterans Affairs (VA) study findings suggest this practice may be unnecessary, Dr. Zaki Abou Zahr said. Dr. Bernard Ng, his senior coinvestigator in the study, added that temporarily stopping antirheumatic agents before surgery may actually be harmful in that it increases the risk of a flare of the RA. But there is a major weakness regarding the VA study: Participation was restricted to RA patients on only a single conventional disease-modifying antirheumatic drug (DMARD) or biologic agent leading up to surgery. This restriction, imposed to make for a more clear-cut analysis, means that the study results can't be extrapolated to patients on multidrug therapy. And multidrug therapy is quite common. A molecule found in the blood is offering a valuable way to predict whether individual rheumatoid arthritis patients are likely to benefit from biologic medications, a Mayo Clinic study shows. Senior author Timothy Niewold, M.D., a Mayo Clinic rheumatologist, says the protein can help physicians prescribe the right medications to each patient, getting their rheumatoid arthritis under control more quickly and sparing them needless side effects and expense. He said, "Patients with rheumatoid arthritis appear to be at higher risk of developing lymphoma -- both Hodgkin and non-Hodgkin lymphoma -- and lung cancer, and at a potentially decreased risk of developing colorectal and breast cancer compared with the general population. The risk of developing lymphoma is associated with disease activity and functional class. Patients treated with TNF inhibitors do not have a significantly higher risk of developing malignancy than patients treated with traditional disease-modifying antirheumatic drugs. Treatment of patients who had prior melanoma with TNF inhibitors has been found in the British Society for Rheumatology Biologics Register to have been associated with more incident malignancy and recurrent metastases. Biologic Therapy | Worrying makes RA worse

Worrying makes RA worse Jennifer Davis writing in Arthritis Today reported, "Patients who have a tendency for more worrying reported slightly more disease activity, more swollen joints and more pain one month later," explains lead author Andrea W. M. Evers, PhD, a clinical psychologist and coordinator at Radboud University Nijmegen Medical Centre in the Netherlands. The researchers were trying to understand how daily stressors and worrying impact symptoms and disease activity in RA, an autoimmune form of arthritis that affects 1.5 million people in the U.S. To find out, they gathered data from 80 RA patients once a month for six months. Patients answered questions about daily stressors (for example, long appointment wait times or losing something valuable), level of worry, and symptoms of pain, fatigue and disease activity. Researchers also took blood samples to measure levels of the stress hormone cortisol as well as inflammatory cytokines (molecules) that are believed to play a key role in RA severity, including TNF-alpha and interleukin-1 beta (IL-1β). (Some biologics, including etanercept (Enbrel) and adalimumab (Humira) are TNF inhibitors, while anakinra (Kineret) is an IL-1 inhibitor.) Researchers found that worrying led to higher self-reported disease activity, more pain and more swollen joints one month later, while daily stressors predicted greater fatigue one month later. They also found that patients with a higher level of certain cytokines -- specifically IL-1β and interferon gamma -- were also likely to experience more fatigue a month later. Shingles vaccine… when to give it… a conundrum Expert advice about shingles vaccine in rheumatoid arthritis Bruca Jancin writing in Rheumatology News reported on a lecture given by Dr. John Cush at the Winter Rheumatology Symposium. While the recommendation is against giving zoster vaccine to patients on biologics, Dr. Cush did a survey of more than 200 patients inadvertently given the vaccine while on biologics. Not one case of shingles occurred. A study conducted in Alabama of 633 patients on biologics given inadvertent zoster vaccine also failed to show an increase in shingles risk and actually showed a 39% reduction in shingles risk. Comment: Reassuring news that shingles vaccine, even though it is live, is not the end of the world if given to patients on biologic therapy. That being said, I still think that a 4 week waiting period before vaccination while holding a biologic is prudent.

Abstract: Biologics, possibly in combination with a conventional disease-modifying antirheumatic drug (DMARD) – preferably methotrexate (MTX), are used in accordance with the recommendations of the international rheumatological societies. However, in clinical practice, this recommendation is often problematic, as many rheumatologists know from personal experience. The quality of life of the patient is affected mainly by drug-induced intolerances (eg, MTX). Thus, the acceptance of the patient to treatment is often so inadequate that a discontinuation of the drug is necessary. In daily practice, approximately 30% of patients with biological therapy receive no concomitant DMARD according to the register data. Heidi Splete writing in Rheumatology News reported on a study from the Canadian Ministry of Health. The study consisted of 4,312 patients. Biologic Therapy | Methotrexate™ Alternative?

Leflunomide (LEF), brand name Arava, offers an alternative with similar effectiveness to Methotrexate (MTX) both by itself as well as in combination with biologic treatment. The addition of Arava to MTX in rheumatoid arthritis patients who have failed MTX by itself has added therapeutic benefit. Safety data on LEF compared to MTX are less conclusive. All patients on LEF, and particularly those on combined LEF+MTX, should be monitored closely for liver toxicity. This last bit needs to be stressed... Arava has been associated with significant liver problems.
